뇌성마비 소아청소년의 발달재활서비스 운동발달재활 영역 이용 및 지원금 현황: 보호자 조사연구 (A Study of Current Use and Subsidy Adequacy of Motor Development Rehabilitation Part of Development Rehabilitation Service in Children and Adolescents with Cerebral Palsy: A Parental Survey)

  • Purpose : Accumulation of accurate data regarding the use and adequacy of subsidies is important to provide optimal development rehabilitation services. However, no reports have discussed the status regarding the use and adequacy of subsidies available for motor development rehabilitation as a part of development rehabilitation services in children and adolescents with cerebral palsy. In this study, we investigated the current use and subsidy adequacy of motor development rehabilitation as an essential part of development rehabilitation services in children and adolescents with cerebral palsy. Methods : The study included parents of children and adolescents with cerebral palsy, who underwent motor development rehabilitation of development rehabilitation services (n=148). The participants were administered a questionnaire to investigate the current use and subsidy adequacy of the motor development rehabilitation part of development rehabilitation services. Results : Most respondents indicated that 310,000-410,000 /month was an appropriate subsidy and agreed that the subsidy should be extended to adults with cerebral palsy. We observed a significant difference in the appropriate subsidy based on age groups (p=.029), as well as type of development rehabilitation service most needed (p=.005) and whether or not agree to extend the subsidy to adults according to gross motor function classification system level (p=.015). There were significant relations of appropriate subsidy (p<.001) and appropriateness of copay (p=.004) according to degree of transportation cost burden. Moreover, there were significant relations of appropriateness of current subsidy (p=.015) and appropriate subsidy (p<.001) according to degree of inconvenience of using transportation. Conclusion : This study highlights the need to increase subsidies for motor development rehabilitation of development rehabilitation services and that the subsidy should be determined based on the burden of transportation costs and the inconvenience of using transportation. Development rehabilitation service for adults with cerebral palsy should also be supported.

발달지연 아동의 인지발달과 운동발달의 양태분석 (Characteristic Analysis of Cognitive and Motor Development in Children With Developmental Delay)

  • The purpose of this study was to investigate the characteristics of cognitive development and motor development in children with developmental delays. Subjects were 43 children with a mean corrected age of 19.19 month (SD=7.48). The Gross Motor Function Measure (GMFM) and the Bayley Scale of Infant Development-second edition (BSID-II) were administered to the total children. Statistical analysis was done by paired t-test and Pearson correlation, using SPSS software. The results were as follows: 1) Most of the children with developmental delays showed statistical delays on the mental scale rather than on the motor scale. 2) The correlation coefficients of cognitive performance and motor performance were ranged between r=.38 and r=.83 in children with developmental delays. 3) Lower motor performance showed significant association with lower cognitive score. The findings suggest that cognitive and motor performance in children with developmental delays was significantly correlated.

수전동 휠체어용 모터 개발 (Development of In-wheel Motor for Power Add-on Drive Wheelchair)

  • The recent power add-on drive wheelchairs (PADWs) provide greater physical activity, are easier to transport, and may be an excellent alternative for the typical manual or electric wheelchairs. The development of in-wheel motor for a PADW is the principal issues. In this paper, design, implementation, and testing of the permanent magnet synchronous motor (PMSM) for a PADW are presented. To design output power and torque of the motor, the equation of motion has been investigated. The design parameters were calculated and the dimension and shape of the motor which was limited by the In-wheel mechanism of the PADW were done by applying FEM and optimal design technique. The prototype of the motor mentioned above was fabricated with precise machining and assembling. Then the motor tested on dynamometer and the measured results of the motor were verified by comparing the design results. The fabricated motor was 80 mm in length with a diameter of 110 mm and small enough to be attached the driving unit of the PADW.

  • This review highlights the ontogenesis and the differentiation of motor neuron in spinal cord, and introduce the survival motor neuron(SMN) which is associated with growth and survival of motor neurons. The differentiation of floor plate cells and motor neurons in the vertebrate neural tube appears to be induced by signals from the notochord. This signal is Sonic hedgehog(Shh). The early development of motor neurons involves the inductive action of Shh. The SMN gene is essential for embryonic viability. SMN mRNA is also expressed in virtually all cell types in spinal cord, including large motor neurons. The SMN protein is involved in RNA processing and during early embryonic development is necessary fer cell survival. Two SMN genes are present in 5q 13 in humans: the telomeric gene(SMNt), which is the SMA-determining gene, and the centromeric analog gene(SMNc). The majority of transcripts from the SMNt gene are full length but, major transcripts of the SMNc gene have a high degrees of alternative splicing and tend to have little or no exon 7. The SMN is involved in the RNA processing(the biogenesis of snRNPs and pre-mRNA splicing), the anti-apoptotic effects, and regulating gene expression.

선천성 근육 사경 환아에 대한 운동 발달적 치료 접근: 증례보고 (A Therapeutic Approach Based on Motor Development in Congenital Muscular Torticollis: A Case Report)

  • Although conservative management of congenital muscular torticollis (CMT) has been well documented, relatively little is known about the response to the treatment. The purposes of this case report were to describe the use of a therapeutic approach based on motor development in physical therapy intervention for an infant with CMT and to report the result of the treatment. The patient was a 20-day-old baby boy with left CMT presenting muscular mass in the left sternocleidomastoid muscle. The angle of the lateral head tilt was 20 degrees. The size of muscular mass was 5.3 mm in ultrasonography. Intervention included ultrasonic therapy, soft tissue massage, passive and active range of motion exercises, motor developmental therapy, and parent instruction. The procedures of motor developmental therapy and changes in the amount of lateral head tilt were documented using photography. The size of the mass was decreased to .3 mm before the 5-month follow-up. The patient also maintained a midline head position in the supine position and a midline head alignment during all functional activities. A therapeutic approach based on motor development is a beneficial method for reducing an asymmetrical head and neck position, and facilitating normal development as a component of physical therapy intervention.

뇌성마비 아동 운동발달 예후 지표로 대동작 기능 분류법 활용에 관한 연구 (The Usability Study for Gross Motor Function Classification System as Motor Development Prognosis in Children With Cerebral Palsy)

  • Purpose: Lack of a valid prognosis of gross motor development in children with cerebral palsy (CP) and the absence of longitudinal data on which to base an opinion in Korea have made it difficult to plan treatment and counsel prognosis issues accurately. The purposes of this study were to examine whether the Gross Motor Function Classification System (GMFCS) is valuable to prognostication about gross motor progress in children with CP in Korea. Methods: Medical records of 61 patients were retrospectively reviewed that visited outpatient department and were diagnosed as CP. Various information was surveyed including CP type, visual acuity, cognitive function, motor acquisition age, ambulatory status, development curves of Gross Motor Function Measure (GMFM) according to each of the 5 level of GMFCS. All of them were compared with other studies. Also the gross motor development curves and the maximum GMFM score derived from this study were compared with the Palisano's report and the Rosenbaum's report. Results: Based on a total of 494 GMFM assessments provided by this study, the 5 distinct motor development curves and the maximum GMFM score were created. These observations is corresponding with the Palisano's and the Rosenbaum`s Development curves. Conclusion: The 5 distinct motor development curves (GMFCS) that were created by Palisano's and Rosenbaum's study is useful in Korea, providing parents and clinicians with a means to plan interventions and to judge progress over time.

국립재활원 외골격 로봇(NREX)의 손 모듈 개발 (Development of Robotic Hand Module of NRC Exoskeleton Robot (NREX))

  • This paper describes the development of a hand module of NREX (National Rehabilitation Center Robotic Exoskeleton) designed to assist individuals with sustained neurological impairments such as stroke and spinal cord injuries. To construct a simple and lightweight hand module, the robotic hand adopts a mechanism driven by a motor and moved by two four-bar linkages. The motor facilitates the flexion-extension movements of the thumb and the other four fingers simultaneously. Thus, an individual using the robotic hand module can effectively grip and release objects related to daily life activities. The robotic hand module has been designed to cover the range of motion with respect to its link distance. This hand module can be used in therapeutic rehabilitation as well as for daily life assistance. In addition, this hand module can either be mounted on an NREX or used as a standalone module.

The purposes of this study were to describe motor development concept and rolling task patterns and to approach the therapeutic exercises.

The driving system consists of a motor and a motor driver is the most important component of the PADW In this paper, design, implementation, and testing of a driving system for a PADW are presented. To design the output power and torque for the driving system, the equation of motion has been investigated. The motor and driver were fabricated with precise machining and assembled to implement our prototype driving system. The dynamometer test has been carried out using the prototype in order to examine the torque of the system. The experimental results demonstrates that the designed driving system can provide enough output power and efficiency for utilization in a PADW.
