Improvement of Atmospheric PM2.5 Levels and Related Premature Deaths in Seoul, Korea (서울시 대기 중 PM2.5 농도 개선과 조기사망 감소 효과)

  • In recent studies, $PM_{2.5}$ has been reported to be more harmful to human health than $PM_{10}$ because it penetrates more deeply into the lung. We estimated $PM_{2.5}$ related health benefits in Seoul from implementing the World Health Organization (WHO)'s guidelines (annual average $10{\mu}g/m^3$, 24-hour average $25{\mu}g/m^3$) and U.S. Environmental Protection Agency (EPA)'s National Ambient Air Quality Standard (annual average $15{\mu}g/m^3$, 24-hour average $35{\mu}g/m^3$). U.S. EPA's Environmental Benefits Mapping and Analysis Program was utilized for the analysis. It was predicted that the attainment of the WHO annual guideline and U.S. EPA's annual standard, relative to the concentration in 2006, would result in reduction of 2,333~2,895 premature deaths and 1,703~2,121 premature deaths, respectively. If the WHO and EPA's daily standard for $PM_{2.5}$ are attained, 1,211~1,394 and 1,012~1,165 premature deaths could be avoided, respectively. Sensitivity analyses indicated that the estimates were robust regardless of air quality simulation methods for attaining the $PM_{2.5}$ goals. This study provides a quantitative approach to evaluate health risks from air pollution as well as to assess the potential health benefits of improving atmospheric $PM_{2.5}$ concentration. Even considering the intrinsic limitations and uncertainties of the analysis, it is an important information to rationalize the enforcement of $PM_{2.5}$ management policies and measures in Seoul, Korea.

The Clinical Comparisons of 10% Pentastarch and 20% Albumin in Priming Solutions for Cardiopulmonary Bypass in Cardiac Operations (개심술중 심폐우회 충전용객속에서의 10% 펜타스타치와 20% 알부민의 임상 비교)

  • Pentastarch is a new synthetic hydroxyethyl starch similar to hetastarch. We report on the clinical comparisons the clinical efficacy and safety of 10% pentastarch in prime solutions for CPB in cardiac operations with that of 20% serum albumin. During CPB, group P [n = 20] received 500ml of 10% pentastarch and group A [n = 20] received 100ml of 20% albumin in prime solutions The postoperative time of ICU stay in group P and the day and amount of chest drain, hospital stay in group A were longer [p<0.05]. Fresh whole blood and PRBC were added only in group A and a higher amount of hartman solution was added in group A during CPB [p<0.05]. Prothrombin time was prolonged preoperatively and 2 days postoperatively in group A and 7 days postoperatively in group P [p<0.05] but there were no significant differences in bleeding time or fibrinogen level. Platelet count was higher immediately postoperatively in group A and preoperatively and 1, 2, and 7 days postoperatively in group P [p<0.05].Total protein and albumin level were higher 1 day postoperatively in group A and 2 and 7 days postoperatively in group P [p<0.05]. BUN was increased 2 days postoperatively in group A and Cr was increased 1 day postoperatively in group P [p<0.05]. CPK was higher preoperatively and 1, 2, and 7 days postoperatively in group A and plasma hemoglobin level was also higher 2 and 7 days postoperatively in group A [p<0.05]. There were no significant differences in arterial blood gas analysis but higher pO2 and lower pCO2 levels were maintained in group P and ejection fraction was higher 7 days postoperatively in group P [p<0.05]. Both groups were improved postoperatively in NYHA class and the hemodynamic parameters such as MAP, CO, CI, SV, LVSWI were well maintained in group P [p<0.05]. The amount of blood products used was higher in group A and urine output was higher immediately postoperatively in group A and 1, 2 days postoperatively in group P and the chest output was higher in group A. The complications were developed in 7 patients in group A and 5 patients in group P and mortality was not present in both groups.In conclusion, 10% pentastarch is as safe and effective as 20% albumin in prime solutions for cardiopulmonary bypass in cardiac operations.

The Phenomenological Study on Self-actualization of Middle-aged Single Mothers - Application of Guided Imagery and Music (GIM) - (한 부모 중년 여성가장의 자기실현과정에 관한 현상학적 연구 -심상유도 음악치료(GIM) 적용-)

  • The number of single-parent families in South Korea increased since 2000, related to a sharp rise in the divorce rate of 50s and an increase in male mortality rates among those aged 40s-50s. Middle-aged single mothers experience a critical period realizing self-actualization needs, while being in the middle adulthood from the lifespan developmental perspective. In this respect, it is significant to study self-actualization of middle-aged single mothers through guided imagery and music (GIM) in order to provide them with psychological support. This study was conducted from September 2018 to June 2020, and the GIM sessions were conducted at least 10 times. Four participants were selected among the middle-aged single mothers. The imagery experiences of participants in the GIM sessions were classified into four sub-elements: physicalness, emotion, memory, and sense. Within those sub-elements, eight semantic units were categorized into 46 elements. Finally, 152 semantic units were derived. Moreover, the self-actualization which participants experienced through GIM presented three archetypal images: shadow, persona, and the self. In the GIM sessions, experiences of putting their negative emotions associated with family into words and changing passive self-imagery into active one enabled participants to bring the shadow into their consciousness, there by recognizing their positive and bright internal self. Furthermore, participants could map that their current status as people marginalized by siblings and parents, enraged and holding double standards for others, was suppressed by their 'good daughter' and 'religious' personas. This realization lead them to realize and restore their persona. The use of GIM in the study allowed participants to elicit re-experiences of the negative events, while experiencing various imagery and music. This process helped participants achieve self-actualization.
