Objective: To compare inferior capsular redundancy by using magnetic resonance arthrography (MRA) images in patients with multidirectional instability (MDI) of the shoulder and control subjects without instability and thereby develop a screening method to identify the presence of shoulder MDI. Materials and Methods: The MRA images of patients with MDI of the shoulder (n = 65, 57 men, 8 women; mean age, 24.5 years; age range, 18-42 years) treated over an eight-year period were retrospectively reviewed; a control group (n = 65, 57 men, 8 women; mean age, 27.4 years; age range, 18-45 years) without instability was also selected. The inferior capsular redundancy was measured using a new method we named the glenocapsular (GC) ratio method. MRA images of both groups were randomly mixed together, and two orthopedic surgeon reviewers measured the cross-sectional areas (CSAs) and sagittal capsule-head ratios on oblique sagittal images, as well as the axial capsule-head ratios on axial images and GC ratios on oblique coronal images. Results: The CSAs and GC ratios were significantly higher in patients than in controls (both, p < 0.001); however, the sagittal capsule-head ratios and axial capsule-head ratios were not significantly different (p = 0.317, p = 0.053, respectively). In addition, GC ratios determined the presence of MDI more sensitively and specifically than did CSAs. A GC ratio of > 1.42 was found to be most suggestive of MDI of the shoulder, owing to its high sensitivity (92.3%) and specificity (89.2%). Conclusion: GC ratio can be easily measured and used to accurately screen for MDI of the shoulder.
Background: The purpose of the study was to evaluate the relationship and route of dental Social Media marketing by age group and support effective dental marketingy by age group. Methods: A study was conducted on 265 people, aged 20 to 64 years, who lived in Seoul, Gyeonggi area and regularly used one or more of the social media platforms, Naver Band, Facebook, Instagram, KakaoStory, Twitter, or YouTube more than once a day. A 27-question questionnaire survey of approximately 10 minutes was conducted, and the collected data was statistically analyzed using the PASW program, with the significane level set to 0.05. Results: "Introduction of acquaintances" was the most common route to visit the dentist. Regarding the use of social media platforms based on age group, 'Instagram' had the highest frequency among people belonging to the age groups of 20 to 29 years and 30 to 39 years; 'YouTube' had the highest frequency among those aged 40 to 49 years; and 'Naver Band' had the highest frequency among those aged 50 to 65 years. Conclusion: The most frequently used social media by consumers according to age included Facebook, YouTube, and Instagram. However, social media was found to have no significant impact on the choice of dental institutions, as the number of people who visited the dentist through "Introduction of acquaintances" was the highest, and "Introduction of acquaintances" did not have experience accessing the dentist site after dental marketing. If this study could provide customized marketing information for each age group through social media, it is expected that the marketing effect of dental institutions through social media would be maximized in the future.

Objective: Clostridium difficile Infection (CDI) is one of the common nosocomial infections. As elderly population increases, the proper treatment has been emphasized. We investigated the risk factors associated with CDI unimprovement in elderly patients. Furthermore, we performed drug use evaluation of old CDI patients and oldest-old CDI patients. Methods: It was a retrospective study using electronic medical record at Kangbuk Samsung Medical Center (KBSMC) from January 2016 to December 2017. Seventy three patients aged 65 years or older, diagnosed with CDI by Clostridium difficile Toxin B Gene [Xpert] were screened and they were assessed for risk factors regarding unimprovement status. We also evaluated drug use evaluation in old patients ($65{\leq}age$<80) and oldest-old patients ($80{\leq}age$) by assessing the use of initial therapy, severity, dose, route, treatment course, days of use, total days of use and treatment outcome of initial therapy. Results: Out of 73 patients aged over 65 years, four patients were excluded because they did not receive any treatment. There were 31 improved patients and 38 unimproved patients after initial therapy. We were able to find out patients with surgical comorbidity or endocrine comorbidity (especially, diabetes mellitus) had 2.885 more risk of becoming unimproved than those patients without surgical comorbidity or endocrine comorbidity. Drug use evaluation for CDI was generally fair, but vancomycin as initial therapy is more recommended than metronidazole. Conclusion: Although age, antibiotics exposure, use of antacids are all important risk factors for CDI, our result did not show statistical significance for these risk factors. However, the study is meaningful because the number of elderly population keeps increasing and recently updated guideline suggests the use of vancomycin as drug of choice for CDI.

We observed 79 patients, who visited the Department of Oph. & Otorhinolaryngology in Oriental medicine of Kyung Hee University Medical Center from July 1995 to June. 1996, with the complaint of decreased visual acuity. The results were as follows. 1. In the incidence of decreased visual acuity, men's was $46.84\%$(37 cases) and women's was $53.16\%$(42 cases), which showed that more women were suffering decreased visual acuity than men. In the incidence of myopic ametropia and astigmatic ametropia among the total patients, men's was $44.62\%$(29 cases) and $37.50\%$(9 cases) each, and women's was $55.38\%$(36 cases) and $62.50\%$( 15 cases) each, which also showed that more women were suffering myopia and astigmatism than men. 2. The average age of patients was 11.08 years at the first visit. The most were the patients from 6 to 15 years old, with 63 cases($79.74\%$). 3. The age of onset in the decreased visual acuity was mainly 6∼10 years with 45 cases($56.96\%$). In the case of myopic ametropia and astigmatic ametropia, the age of onset was also mainly 6∼10 years with 65 cases($60.00\%$), and with 12 cases($50.00\%$) each. 4. In ABO blood type, the frequency was, A type, O type, B type and AB type in order. In men, O type was the most, while in women, A type. 5. In the liking for cool or warm food or tepidity, the liking for cool food was the most in both men and women. 6. The type of decreased visual acuity was mainly myopic ametropia with 65 cases($82.28\%$). Astigmatic ametropia was $30.38\%$ with 24 cases, hyperopic ametropia $2.53\%$ with 4 cases, and the decreased visual acuity accompanied by amblyopia $7.59\%$ with 4 cases. 7. At the first visit, the average visual acuity of O.D. was 0.29 and that of O.S.. 0.24, which showed that O.S.. is worse than O.D.. The visual acuity below 0.2 was the most, $63.29\%\;in\;O.D..\;72.15\%$ in O.S.. 8. In the treatment period, 4∼7 weeks occupied $35.44\%$ with 28 cases, 8∼11 weeks $30.38\%$ with 24 cases, so the treatment period was mainly these two periods with 52 cases($65.82\%$). 9. The average frequency of acupuncture treatment per week was mainly 2.1∼3.0 times with 45 cases($56.96\%$). In this case, men was 24 cases($53.33\%$) and women 21 cases($46.67\%$), so men was more than women. 10. The frequency of herbal prescription was mainly Gamijungjitang and Gamijingjibogansan with 76 cases($85.39\%$).
Purpose: Malnutrition affects all age groups, but older adults are particularly more vulnerable to nutritional deficiencies. This study evaluated the age-specific factors affecting malnutrition in hospitalized older adults. Methods: A retrospective study was conducted on inpatient elderly people who received artificial nutrition from 2010 to 2017. Data of demographics, diagnosis, type of nutrition therapy, number of comorbidity, fall risk assessment, Acute Physiologic Assessment and Chronic Health Evaluation (APACHE) score, and intensive care unit admission were collected. Malnutrition was defined as a body mass index (BMI) of less than 18.5 kg/m2. Patients were classified as the young-old (65~74 years old), the old-old (75~84 years old), or the oldest-old (85 years old or older). Results: A total of 7,130 older adults were included, and 4,028 patients were classified as the young-old, 2,506 into the old-old, and 596 into the oldest-old. Proportion of malnutrition was higher in the oldest-old compared to the other groups. In multivariate analysis, parenteral nutrition, alcohol, and high risk of falls were factors affecting malnutrition in all groups. Parenteral nutrition and alcohol in the young-old, high risk of falls in the old-old, and male sex in the oldest-old were the factors affecting malnutrition by the age group. Conclusion: Older age was the most significant factor affecting malnutrition. Specific strategies by age are needed to improve nutritional status in hospitalized older adults as influencing factors for malnutrition vary among different age groups.
Purpose: This study was to investigate two-point discrimination(TPD) and related factors in rural community residents. Methods: The sample consisted of 68 people who have been living in rural community in June 2005 studied. TPD was measured from the tips of the thumb, index, middle, ring, and little finger of each hand with the TPD esthesiometer. The research was designed to be a cross-sectional measured study. SAS statistical software was used for the analysis. The characteristics of the study sample were described by mean and standard deviation(SD) for continuous variables and by frequency and percentage for categorical variables. The Student's t-test and analysis of variance (ANOVA) were used to compare TPD in gender and diabetes mellitus. A Pearson's correlation analysis was conducted for relationship among values of 5 fingers. Multiple regression analysis was performed to determine the factors associated with TPD. Results: A total of 68 residents were measured, their average $age{\pm}SD$ was $54.2{\pm}23.2$ years(range: $12{\sim}88\;yr$). The mean TPD for 5 fingers tips in dominated side was 4.76mm(thumb 3.98mm, index 4.22mm, middle 4.79mm, ring 5.17mm, and little finger 5.65mm, respectively). 4.91mm for thumb finger of women was significantly higher than 3.54mm that of men's in difference between gender(p=.01l4), also women(4.39mm) was higher than men(3.71mm) in median nerver area(p=.0318). There was a statistically significant difference in age(p=.0022), which were under age of 20(3.36mm), 30 years(3.61mm), 40 years(5.38mm), 50 years(4.84mm), 60 years(5.28mm), 70 years(5.18mm), and over age of 80(5.29mm). Factors related to TPD of fiver fingers by multiple regressions were gender(${\beta}=.03$, p=.0001). Conclusions: Taken together, gender and age were significantly associated with TPD. We recommend that further research should measure TPD by using larger sample sizes and more sensitive measurement instruments.
Purpose: This study aims to explore the association between unemployment and depression in people from different age groups ranging from 18 to 65 years old. Methods: This study used a cross-sectional design. We performed bivariate analysis and multivariable logistic regression on the 2010 Behavioral Risk Factor Surveillance System (BRFSS) data from 12 states in the United States. Results: On a sample comprised of n=53,406 individuals, of whom 2,546 (7.8%) were identified as being depressed and 3,448 (10.6%) as unemployed, we found that individuals aged 61~65 years have a lower depression risk compared to those aged 18-25 after adjusting for other variables including employment status. However, people from 61~65 have higher increased risk of depression when unemployed compared to other age groups in all three models tested (3.95 times higher in unemployed people in model 1, and 2.81 times higher in model 2 and model 3). Conclusion: Our findings indicate that there may need to be more focus on older adults who are unemployed, with associated support services for their mental health. The results of our study indicate that although older adults are less likely to be unemployed, there are more likely to experience depression if they are unemployed (once other confounding factors are taken into account) than younger adults. Policies and interventions can be developed to address not only the physical difficulties but also the mental challenges with which older adults can be at risk facing in case of unemployment.

Nutrition factors play an important role in the pathogenesis of osteoporosis. The purpose of this study is to investigate the relationship between nutritional status and bone mineral density of elderly women. Three hundred thirty five elderly women (over 65 years) in Asan were divided into three age groups (group 1, less than 70 y ; group 2, from 70 to 75 y ; group 3, 75 y or more). Total alkaline phosphatase and serum calcium (Ca) were analyzed using spectrophotometric procedure. Data for food and nutrient intakes were obtained by a 24-hour recall method. Bone density was measured by broadband ultrasound attenuation (BUA) using QUS-2. Age differences were tested with the X 2 test for categorical variables and with ANOVA and Tukey's test for continuous variables. Correlation was conducted to test the association between bone density and nutrient intake. The subjects in age groups 1,2 and 3 were $36.7\%$, $32.8\%$ and $30.4\%$, respectively. Height and body weight of the subjects were significantly decreased with age. Average bone density of the subjects in group 3 was lower than the other age groups. Osteoporosis determined by t-score is $17.9\%$ for group 1, $24.5\%$ for group 2 and $55.9\%$ for group 3 (p < 0.001). The serum Ca level of the subjects in group 2 was significantly lower than that of group 1 although mean values in all age groups are within the normal range. Dietary Ca intake, nutrient adequacy ratio (NAR) and index of nutrient quality (INQ) were decreased with age. Bone density was negatively correlated with age (p < 0.001), while body mass index (p < 0.01) was positively related with bone density. Although partial correlation did not reveal the significant correlation of BMD and dietary calcium after controlling for age, since calcium intake was very poor compared to sodium and phosphorous intakes, recommendation of more calcium intake for elderly women especially those over 75 years must be continuously emphasized.

Background: The clinical significance of bilateral breast cancer is unclear and its influence on prognosis is controversial. Materials and Methods: Between 2005 and 2009 we identified 110 cases of bilateral breast cancer (BBC) ; 49 patients had synchronous (duration between the occurrence of carcinoma in both breasts was less than 12 months) and 61 had metachronous (duration was more than one year with no ipsilateral local recurrence). We compared the patient characteristics including age, menopausal status, clinical stage, tumor size, histological classification, lymph node status, and hormone receptor and Her-2 status. We also compared the treatment given and overall and disease free survival (DFS) of both groups. Results: Synchronous cases tend to present more aggressively than metachronous cases and age at first presentation adversely affects survival. The 5 year overall survival was 78.7% for metachronous and 60% for synchronous. Patients with positive hormonal status had better five year disease free survival in metachronous compared to synchronous cases, at 76% and 63%, respectively. Age at first presentation >45years had better DFS (65%) compared to those with age ${\leq}45$ years (52%) at 5 years follow up. Conclusions: Patients with synchronous breast cancer may have worse prognosis. Young age and hormone receptor negative were risk factors in our study. Close follow up and early detection of contralateral breast cancer is mandatory.
Purpose: The purpose of this study is to investigate the association between unmet healthcare needs due to financial reasons and catastrophic health expenditures. Methods: This study used secondary data from the 2014~2015 Korean Health Panel survey. The subjects of this study were 21,495 people aged 20 or older, and of them, there were 16,227 people aged 20 to 64 and 5,268 people aged 65 or older, which were surveyed between 2014 and 2015. The association between unmet healthcare needs due to financial reasons and catastrophic health expenditures was analyzed through logistic regression. Results: In 2015, 1.7% of people aged 20~64 years and 7.9% of those aged 65 or older experienced unmet healthcare needs due to financial reasons. In the 20~64 age group, people who repeatedly experienced catastrophic health expenditures (=10%,=20%) were less likely to experience unmet healthcare needs due to financial reasons than those who did not experience catastrophic health expenditures for two years (OR=0.50, OR=0.41). However, in the 65-or-older group, people who repeatedly experienced catastrophic health expenditures (=20%) were more likely to experience unmet healthcare needs due to financial reasons than those who did not experience catastrophic health expenditures for two years (OR=1.68). Conclusion: A greater percentage of the elderly repeatedly faced both catastrophic health expenditures and unmet healthcare needs due to financial reasons compared to the non-elderly.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
