  • Standard open procedures for resection of the first rib in thoracic outlet syndrome can prove to be quite difficult with extensive incisions. A minimal invasive procedure can also be painstaking, but provides an attractive alternative to the more radical open procedures. We report the details of the technique with direct video footage of the procedure performed in a 41-year-old man with thoracic outlet syndrome done entirely by thoracoscopic methods.

  • 관상동맥 우회수술은 일반적으로 저체온하에 인공심폐기를 사용하여 체외순환을 하면서 심정지상태에서 시행하는 것이 일반적 인 방법이다. 그러나 선택적 인 병변에 한 하여는 체 외순환을 하지 않고 정상적인 심박동 상태에서 관상동맥 우회 술을 사용하는 방법이 시도되고 있다. 저자들은 우관상동맥에 100%협착을 보이고 경미한 운동시 심한 흉통을 호소한 환자에서 체외순환 없이 관상동맥 우회 술을 성공적으로 시행하였기 에 문헌고찰과함께 보고하는 바이다.

Endoscopic treatment of vesicoureteral reflux in pediatric patients

  • Endoscopic treatment is a minimally invasive treatment for managing patients with vesicoureteral reflux (VUR). Although several bulking agents have been used for endoscopic treatment, dextranomer/hyaluronic acid is the only bulking agent currently approved by the U.S. Food and Drug Administration for treating VUR. Endoscopic treatment of VUR has gained great popularity owing to several obvious benefits, including short operative time, short hospital stay, minimal invasiveness, high efficacy, low complication rate, and reduced cost. Initially, the success rates of endoscopic treatment have been lower than that of open antireflux surgery. However, because injection techniques have been developed, a recent study showed higher success rates of endoscopic treatment than open surgery in the treatment of patients with intermediate- and high-grade VUR. Despite the controversy surrounding its effectiveness, endoscopic treatment is considered a valuable treatment option and viable alternative to long-term antibiotic prophylaxis.

  • Meckel's diverticulum is one of the common causes of gastrointestinal bleeding in the pediatric patient requiring laparotomy. Two children with Meckel's diverticulum have been successfully treated by laparoscopic excision. Both patients recovered without incident and were discharged at 3 and 5 days after surgery. The authors believe that laparoscopic diverticulectomy is a safe, effective. and minimal invasive treatment of Meckel's diverticulum in children.

  • Neuroendoscopic surgery is performed because it causes minimal damage to normal structures, carries a lower rate of complications, and achieves excellent outcomes. Surgeons using an endoscope and related instruments can perform complex operations through very small incisions, which is especially useful for minimally invasive procedures for the brain and spine. Neuroendoscopic surgery is now performed in cases of obstructive hydrocephalus, various intraventricular lesions, hypothalamic hamartomas, craniosynostosis, skull base tumors, and spinal lesions. This review discusses the brief history of neuroendoscopy and the current state and future perspectives of endoscopic surgery.

Subcision Using a Spinal Needle Cannula and a Thread for Prominent Nasolabial Fold Correction

  • Deepening of the nasolabial crease is an esthetically unpleasing aging phenomenon occurring in the midface. Various treatment modalities have been introduced to improve the appearance of prominent nasolabial folds, all of which have pros and cons. Currently, a minimally invasive technique using synthetic dermal fillers is most commonly used. A simple and easy subcision procedure using a wire scalpel has also been used and reported to be effective for prominent nasolabial fold correction, with minimal complications. As an alternative to the wire scalpel, we used a 20-gauge metal type spinal needle cannula (Hakko Co.) and 4-0 Vicryl suture (Ethicon Inc.) for subcision of nasolabial folds. This technique is less expensive than the use of a wire scalpel and easily available when needed. Therefore, on the basis of favorable results, our modified subcision technique may be considered effective for prominent nasolabial fold correction.

광배근 피판의 공여부에 대한 문제점의 분석 (Complications of Donor Site in Latissimus Dorsi Muscle Flap)

  • Purpose : To investigate the complication of donor site in latissimus dorsi muscle flap. Materials and Methods : From April 1983 to March 1999, forty patients with latissimus dorsi muscle flap for reconstruction of extremity and with a follow-up period of more than 12 months were included. We analysed the complication, shoulder function, degree of shoulder muscle weakness, skin scar width after operation. Results After 1 year, skin scar widening in 12 cases(30%), limitation of shoulder motion in 9 cases(12.5%), muscle weakness in 14 cases(17.5%) were found. Conclusion. The rate of complication at donor site after latissimus dorsi flap operation is around 10%. To minimize the complication, avoiding axillary skin incision, minimal invasive harvesting by endoscopy, meticulous suturing of subcutaneous layer are needed.

  • Hypopharyngeal cancer have shown poor prognosis though various treatment modalities were developed for several decades. Therefore current trend in managing hypopharyngeal cancer is organ preservation therapy to improve patient's quality of life. Recently, surgery via robotic surgical system in genitourinary surgery improved minimal invasive technique and reduced morbidity dramatically. Hypopharyngeal cancer treatment using Transoral robotic surgery (TORS) in treating lesions of hypopharynx could reduce the morbidity and achieve organ preservation.

  • 폐렴의 합병증으로 생긴 기관지 늑막강루의 치료로 최근 들어 비침습적 방법으로 기관지 내시경을 이용한 치료가 보고되고 있지만 그 사용 물질 및 방법은 다양하다. 본 증례에서는 혈관 폐쇄용 코일과 피브린 글루를 사용하여 굴곡형 기관지내시경을 이용하여 치료하였다. 기관지 늑막강루의 크기가 크지 않고 말초 기관지에 위치한다면 본 증례의 치료 방법이 고려될 수 있다.

  • Cyst is a closed sac, having a distinct membrane and division compared to the nearby tissue. Most cysts in the oro-maxillofacial area are odontogenic origin and found most commonly by oral and maxillofacial surgeons. Enucleation is considered as a definitive therapy in the management of these cysts. However, to select the treatment method for large cyst, consideration of position of the cyst, size and accessibility to main structure are needed. Marsupialization is a minimal invasive technique to treat large odontogenic cyst and tumor without damage to adjacent important structures. This study was undertaken to describe three cases of odontogenic cysts and tumors of jaws that were successfully treated with enucleation after marsupialization.
