Application of Monitored Anesthesia Care Using Dexmedetomidine to Common Oral and Maxillofacial Trauma Cases

  • Oral and maxillofacial surgery (OMFS) trauma cases are commonly treated under general anesthesia. The purpose of this case report is to introduce an alternative method of anesthesia in patients who refuse general anesthesia. A combination of dexmedetomidine and ketamine for sedation anesthesia in 3 frequent fracture types in the field of OMFS-Le Fort I fracture, mandibular fracture, and alveolar bone fracture-was used. Dexmedetomidine as the single agent has not shown stable success rates for invasive procedures. To overcome some of the pitfalls with dexmedetomidine, combination sedation using ketamine was performed. Visual analogue scale scores were recorded postoperatively. Dexmedetomidine combined with ketamine administration provided safe and effective sedation and anxiolysis for surgical reduction and internal fixation of OMFS fractures. It showed advantages of decreased admission time, reduced expenses, minimal pain, and reduced anesthetic burden for the patient thus ultimately increasing overall satisfaction.

  • The postoperative respiratory complications such as pneumonia and ARDS, are poor prognostic factors after esophagectomy in patients with esophageal cancer. To avoid these complications, there have been attempts to use minimally invasive approach. Recently introduced daVinciTM S surgical system is used in esophagectomy because of its advantages of minimal invasiveness, clear 3-dimensional imaging and precise handling of robotic arms. We report a 83-year-old esophageal cancer patient who underwent daVinciTMS robot-assisted esophagectomy, laparoscopic stomach mobilization followed by cervical esophagogastrostomy.

  • 기관지늑막강루는 드문 질환이지만, 일단 생기면 높은 이완율과 치명률을 보이며 입원 기간과 비용부담이 초래되는 합병증의 하나이다. 일반적으로 수술적 치료가 우선이지만, 최근 들어 비침습적 방법으로 기관지 내시경을 이용한 치료가 보고되고 있다. 괴사성 폐렴에 합병된 기관지 늑막강루를 코일과 글루를 사용하여 기관지내시경을 이용, 성공적으로 치료하였기에 그 결과를 보고한다.

  • As intervention techniques have been developed recently, minimal invasive treatment for uterine myoma using uterine artery embolization is receiving attention increasingly. Uterine rupture occurs rarely, but once it occurs, it may cause fatal results in both mother and fetus. Although the relationship between uterine artery embolization for treatment of uterine myoma and uterine rupture has not been clearly revealed yet, a case implying that the embolization for treatment of uterine myoma can be a risk factor of uterine rupture like previous caesarian delivery or myomectomy.

Gastric Ulceration and Bleeding with Hemodynamic Instability Caused by an Intragastric Balloon for Weight Loss

  • Obesity in the United States is a medical crisis with many people attempting to lose weight with caloric restriction. Some patients choose minimally invasive weight loss solutions, such as intragastric balloon systems. These balloon systems were approved by the Federal Drug Administration (FDA) in 2015-2016 and have been considered safe, with minimal side effects. We report a patient with a two-day history of melena, abdominal pain, hypotension, and syncope which developed five months after placement of an intragastric balloon. Esophagogastroduodenoscopy with balloon removal revealed a small 8-mm gastric ulcer in the incisura. This gastric ulcer probably developed secondary to mechanical compression of the stomach mucosa by the gastric balloon which contained 900 mL of saline. The FDA is now investigating five deaths since 2016 associated with these second-generation balloons. Clinicians should be aware of these complications when evaluating patients with gastrointestinal complications, such as bleeding.

The Vestibuloplasty Using Non-eugenol-based Non-zinc Oxide Oral Dressing Material

  • This study describes a patient with insufficient vestibular depth who was operated with a vestibuloplasty using a non-eugenol-based non-zinc oxide oral dressing material. Partial thickness flap was elevated on recipient. After stabilization of apical positioned flap, the dressing material was applied on the recipient site with additional fixation by suture. The patient presented minimal discomfort such as in pain, food impaction and management of oral hygiene. Adequate vestibular depth without relapse was observed until 4 weeks after surgery. The vestibuloplasty with the oral dressing material may be an option for obtaining proper vestibular depth without complications.

  • Immediate implant placement (IIP) in fresh extraction sockets exhibits similar survival and success rates to delayed implant placement in healed sockets. Several advantages of IIP involve shortened total treatment time, reduction of the number of invasive surgeries, and subsequent reduction of patient discomfort due to lack of additional surgeries. The major shortcomings in IIP, however, include the inability to obtain early bony support, presence of a gap between the extraction socket and fixture, and the inability to cover the fixture with soft tissue, leading to increased risk of infection and implant loss. When IIP is performed, atraumatic or minimally traumatic extractions, conservation of the septal bone in molars, minimal flap elevation or flapless surgery, bone grafting the gap between the fixture and the extraction socket, and coverage with soft tissue or a membrane must be considered.

  • Well-differentiated papillary mesothelial tumor (WDPMT) is an uncommon tumor, formerly named well-differentiated papillary mesothelioma in the 2015 World Health Organization classification. It has a characteristic papillary architecture, bland cytologic features, a tendency toward superficial spread without invasion, and a good prognosis due to its clinically indolent behavior with prolonged survival. Rare cases with superficial invasion are termed WDPMT with invasive foci. WDPMT occurs primarily in the peritoneum of reproductive-age women, but also rarely in the pleura. We report a case of a 60-year-old woman who developed WDPMT with minimal invasion in the pleura with atypical radiological features and a family history of mesothelioma and indirect asbestos exposure.

A case of laparoscopic excision of choledochal cyst, hepaticojejunostomy, and Roux-en-Y anastomosis using Artisential®

  • Choledochal cyst is a condition involving an abnormal dilation of the bile ducts, which can lead to various symptoms and comorbidities, including cancer. The treatment of choice for choledochal cyst is surgical correction including choledochal cyst excision and Roux-en-y hepaticoenterostomy. Minimal invasive methods like laparoscopic methods or robotic methods are used for surgical correction of choledochal cysts; however, it is still controversial which method is superior. A Korean company, LIVESMED, developed Artisential®, a laparoscopic surgical instrument that can overcome the drawbacks of laparoscopic methods. This article presents a case of the first Artisential®-performed surgical excision of a choledochal cyst and hepaticojejunostomy.

밀리미터파를 이용한 무혈 혈당 측정에 관한 기초 연구 (Basic Investigation for the Won-invasive Measurement of Blood Glucose Concentrations by Millimeter Waves)

  • 본 논문은 밀리미터파를 이용한 무혈혈당측정기 개발의 기초연구로서, 높은 유전손실을 지닌 유전체의 반사 유전특성 측정법을 제시하고, 이에 의해 10~90 GHz의 밀리미터파 대역에서 글루코오스 농도에 따른 글루코오스 수용액 및 글루코오스 -0.9% NaCl 용액의 유전특성 변화를 조사하였다. 제시된 측정법은 측정 유전제의 앞단에 평행평면판을 배치하여 측정 주파수대역 내에서 전력반사계수가 최소가 되는 최소반사조건이 형성되도록 하고, 이 조건에서 측정된 최소 전력반사계수와 주파수로부터 측정 유전체의 유전특성을 결정할 수 있는 방법이다. 순수의 유전특성에 대한 측정 결과들은 제시된 측정법의 타당성을 입증하였다. 또한 10~90 GHz 대역에서 글루코오스 농도 변화에 따른 글루코오스 용액 및 글루코오스 -0.9% NaCl 용액들의 유전특성 변화에 대한 실험을 통해, 타 대역에 비해 30~45 GHz 범위에서 글루코오스 농도 변화에 의한 각 용액들의 유전특성의 변화가 최대임을 알 수 있었다. 이를 통해 본 측정법에서 전력반사계수와 주파수의 측정 정밀도가 각각 ±0.1 dB와 ±0.01 GHz일 경우, 대략 3 mole/L 정도의 분해능으로 용액 내 글루코오스 농도 변화를 측정할 수 있음을 보였다.
