군병원건축의 시설개선을 위한 실태조사 및 개선방향에 관한 연구 (A Study on the Improvements of Planning Issues for Korean Military Hospitals)

  • This study is a part of the architectural planning to meet the military healthcare development project according to the Korea national defense reform 2020. At this time, it is intended to identify the overall issues and to set the improvement directions of military hospital planning. Literature reviews, such as military healthcare development project, Korea national defense reform 2020, etc., and case studies for the advanced foreign military hospitals and several Korean private hospitals is analyzed. And also, field surveys, such as questionnaire collections for patients and workers, interviews with surgeons and department officials, and observation survey were conducted in order to be realistic improvement suggestions for military hospital planning issues.

군 간부 건강검진 결과를 통한 검사항목 품질향상에 관한 연구 (A Study on the Quality Improvement of Test Items through the Results of Military Personnel Health Examination)

  • 본 연구의 목적은 군 간부건강검진결과를 분석하여 군 간부의 건강상태를 파악하고자 하는 것이다. 연구결과는 다음과 같다. 첫째, 연구대상자의 일반적 특성은 전체 120명의 연구대상자 중, 40세 미만이 60명, 40세 이상이 60명인 것으로 나타났다. 둘째, 연구대상자의 연령 및 체질량 지수와 검사결과간의 정준상관분석 결과, 3개의 정준함수가 도출되었다. 이 중, 2개의 정준함수가 각각 통계적으로 유의한 것으로 나타났다. 본 연구에서 군 간부에게는 건강에 대한 총체적 진단과 검진제도가 보완되어야 한다는 의미 있는 결과가 도출되었고, 향후 군 간부에 대한 건강검진은 확대된 건강검진 품질에 대한 추가 검사항목을 받아야 하며, 이를 위한 예산반영은 정책 관련기관에서 판단하여야 할 것으로 사료된다.

군부대 내 부적응 병사에서 충동성과 혈청 지질과의 상관성 (The Relationship between Serum Cholesterol and Impulsiveness in the Division of the Republic of Korean Army)

  • Objective : Previous studies reported a correlation between the low serum cholesterol level and impulsive behaviors. In this study, we investigate an association between the serum lipid levels and psychological parameters in maladaptive soldiers in the Korean Army. Methods : A total of ninety-six maladaptive subjects and thirty-six normal controls in the Korean army were evaluated with the Korean version of Barratt Impulsiveness Scale (K-BIS), Korean version of Beck Suicidal Ideation Scale (K- BSIS), Korean version of Beck Depression Inventory (K-BDI) and Korean version of Beck Anxiety Inventory (K-BAI). Serum total cholesterol (TC), triglyceride (TG), low density lipoprotein (LDL) and high density lipoprotein (HDL) level were measured by overnight fasting blood sampling. Results : There were no significant differences between the groups in demographic characteristics. Serum total cholesterol levels (t=-2.209, p=0.032), triglyceride levels (t=-4.593, p<0.001), and LDL levels (t=-3.753, p=0.001) of maladaptive subjects were significantly lower than those of normal controls, and maladaptive subjects had higher K-BIS scores than normal controls (t=7.542, p<0.001). Negative correlation was found between LDL levels and non-planning impulsiveness in the maladaptive subjects (r=-0.253, p=0.013). LDL levels (${\beta}=-0.258$, p=0.008) and K-BDI scores (${\beta}=0.266$, p=0.043) emerged as significant predictors for non-planning impulsiveness. Conclusion : These results suggested that LDL level was associated with non-planning impulsiveness. These findings suggested that serum cholesterol levels might be available as a biological marker of impulsiveness. However, more large samples, longitudinal biological study and psychiatric evaluations should be needed to develop a preventive intervention for maladaptive male conscripts in the Korean army.

Comparison of 2-Dimensional and 3-Dimensional Conformal Treatment Plans in Gastric Cancer Radiotherapy

  • Background: Postoperative chemoradiotherapy is accepted as standard treatment for stage IB-IV, M0 gastric cancer. Radiotherapy (RT) planning of gastric cancer is important because of the low radiation tolerance of surrounding critical organs. The purpose of this study was to compare the dosimetric aspects of 2-dimensional (2D) and 3-dimensional (3D) treatment plans, with the twin aims of evaluating the adequacy of 2D planning fields on coverage of planning target volume (PTV) and 3D conformal plans for both covering PTV and reducing the normal tissue doses. Materials and Methods: Thirty-six patients with stage II-IV gastric adenocarcinoma were treated with adjuvant chemoradiotherapy using 3DRT. For each patient, a second 2D treatment plan was generated. The two techniques were compared for target volume coverage and dose to normal tissues using dose volume histogram (DVH) analysis. Results: 3DRT provides more adequate coverage of the target volume. Comparative DVHs for the left kidney and spinal cord demonstrate lower radiation doses with the 3D technique. Conclusions: 3DRT produced better dose distributions and reduced radiation doses to left kidney and spinal cord compared to the 2D technique. For this reason it can be predicted that 3DRT will result in better tumor control and less normal tissue complications.

Determining the Awareness of and Compliance with Breast Cancer Screening among Turkish Residential Women

  • Background: Breast cancer is the leading cause of cancer-related deaths in women. Despite being associated with high morbidity and mortality, breast cancer is a disease that can be diagnosed and treated early. Materials and Methods: In this cross-sectional study of 321 women, data were collected by Questionnaire, Breast Cancer Risk Assessment Form and Champion's Health Belief Model Scale. Mann-Whitney U, Kruskal-Wallis, Chisquared tests and logistic regression were used in the statistical analysis. Results: It was found that only 2.2% of women have high and very high risk levels of breast cancer risk. There is a positive correlation between early diagnosis techniques and Health Belief Model Sub-Dimension scores which are sensibility, health motivation, BSE (Breast self-examination) self-efficient perception and negative correlation between mammography barrier score and BSE barrier score (p 0.05). When factors for not having BSE were examined, it was determined that the women who do not have information about breast cancer and the women who smoke have a higher risk of not having BSE. Conclusions: It is important to determine health beliefs and breast cancer risk levels of women to increase the frequency of early diagnosis. Women's health beliefs are thought to be a good guide for planning health education programs for nurses working in this area.

재난대응 이동형 병원의 역사적 배경 및 배치 특성 연구 - 군 이동병원을 중심으로 (A Study on the Historical Background and Deployment Characteristics of Mobile Hospital for Disaster - Focused on Military Mobile Hospitals)

  • Purpose: The purpose of this study is to examine the deployment types and characteristics of mobile hospitals with it's historical backgrounds. Methods: Since the disaster can not be classified by country, the scope of the study is to include both domestic and foreign mobile hospitals. In order to minimize the casualties from the field hospitals used at the time of the First World War, which is the mother of mobile hospitals, we analyzed the mobile hospitals which are more compact and changed to the target areas and analyzed the mobile hospitals. Results: Historically, mobile hospitals have been transformed in a way that they are close to the target area and rapidly inject essential elements, and the deployment of wards has evolved to be able to combine in any form with center corridor. In the case of hospitals that can deal with infectious diseases, each treatment room was installed separately to thoroughly separate the copper wire. Implications: As disaster damage increases and incidence increases, field response should be quick. However, research on mobile hospitals, which are indispensable for field response in Korea, has not been conducted in various ways. From the origins of mobile hospitals, the development process is reviewed, and research is carried out to clarify the grounds and backgrounds for the planning of mobile hospitals that are quick and appropriate to the situation in Korea and abroad.

Could the Breast Prognostic Biomarker Status Change During Disease Progression? An Immunohistochemical Comparison between Primary Tumors and Synchronous Nodal Metastasis

  • Background: Prognostic biomarkers in breast cancer are routinely investigated in the primary tumors to guide further management. However, it is proposed that the expression may change during the disease progression, and may result in a different immune profile in the metastatic nodes. This work aimed to investigate the expression of breast prognostic biomarkers in primary tumors and in its axillary nodal metastasis, to estimate the possible discordant expression. Materials and Methods: 60 paired primary and axillary nodal metastasis samples were collected from patients with primary breast cancer with positive nodal deposits, diagnosed at the Maadi Military Hospital, Cairo, Egypt, during the year 2013. ER, PR and HER2 expression was assessed by immunohistochemistry in all samples Results: 48.3% of the included cases showed concordant results for both ER and PR receptors between the primary tumor and its nodal metastasis while 51.7% showed discordant results and the discordance level was statistically significant. On the other hand, 70% of the cases showed concordant Her2 results between the primary tumors and the nodal deposits, 30% showed discordant results and the difference was significant. Conclusions: The study indicated that the discordance in ER and PR receptor expression between the primary breast tumor and their nodal metastasis may be significant. The possible switch in the biomarker status during the disease progression is worth noting and may change the patient therapeutic planning. So, whether the treatment selection should be based on biomarkers in the lymph node is a topic for further studies and future clinical trials.
