Development of Korean Version of Acute Concussion Evaluation using Cross-cultural Translation Methodology: Pilot Study (Acute Concussion Evaluation의 한국어 번역 및 문화적 개작: 예비 연구)

  • Objectives The purpose of this study is to provide Korean version of mild traumatic brain injury assessment tool. Methods The original version of acute concussion evaluation (ACE) was translated into Korean, and it was then back-translated into English without any prior knowledge of ACE. Finally, the pre-final version of Korean version of acute concussion evaluation (K-ACE) was derived. 49 Korean patients who had been diagnosed with mild traumatic brain injury participated in the study and completed K-ACE. Overall, 44 data were used to analyze findings. Validity of the study was assessed based on Concurrent validity. Reliability was also evaluated using Cronbach's ${\alpha}$ and the intraclass correlation coefficient. Results The Cronbach's ${\alpha}$ value for each item presented a proper level of internal consistency with results of 0.711 to 0.893 in two evaluations, respectively. The intraclass correlation coefficient of the retest reliability was marked as 0.892 (95% CI 0.840~0.933). Concurrent validity demonstrated positive correlations between K-ACE and Korean version of postconcussional syndrome questionnaire. Conclusions The K-ACE is concluded as a valid and reliable tool for measuring mild traumatic brain injury and post-concussion symptoms. Upon completion of the follow-up study, the K-ACE will be well-utilized by both clinicians and researchers.

Emotional Labor, Social Support, and Depressive Symptoms of Clinical Nurses in a Province, Korea (임상간호사의 감정노동, 사회적 지지 및 우울 간의 관계)

  • Purpose: This study was conducted to identify the level of emotional labor, social support, and depressive symptoms of nurses, and the relationships among them. Methods: The data were collected from 314 nurses. Surface and deep-acting scales of the Emotional Labor Scale (ELS), the personal resource questionnaire, the organizational support scale, and the Korean version of the Center for Epidemiologic Studies Rating Scale for Depression (CES-D) were used. Collected data were analyzed by descriptive statistics, t-test, ANOVA, and Scheffe test. Results: Fifty six percent of total subjects had mild or major depressive symptoms. As the level of surface-acting of ELS was higher, so was the level of depressive symptoms, while as the level of individual and organizational support was higher, the level of depressive symptoms was lower. Conclusion: It is necessary to provide organizational-level support and to manage surface-acting in order to prevent the nurses' depressive symptoms. In particular, for the nurses who show relatively higher level of depressive symptoms; who are in 20s and have short employed years or not married, it is urgently required to be provided depressive symptoms management programs that targeted for those nurses.

  • Pericardial cyst and diverticula are not uncommon. Rarely do they cause symptoms and the diag-nosis has been made primarily at operation. In Korea, there is no reported case of pericardial coelomic cyst, and one case of pericardial coelomic cyst, middle aged female with symptoms of general malaise, weight loss and mild discomfort on her anterior chest, is presented with related references. Pathological diagnosis after removal of the mass revealed pericardial coelomic cyst.

Study of Premenstration Syndrome of Female College Students According to Sasang Constitution (사상체질에 따른 여대생의 월경전 증후군 양상에 관한 연구)

  • Objectives The purpose of this study was to investigate the premenstrual symptoms according to Sasang constitution and to utilize for understanding the characteristics of Sasang constitution. Methods Thirty one female university students participated in this study and Sasang Constitution and patterns of premenstrual symptoms was determined by Questionnaire for Sasang Constitution Classification (QSCC) II and premenstrual symptoms screening tool, respectively. For statistical analysis, chi-square test, analysis of variance and correlation test has been used. Results There existed significant differences in menstrual symptoms between Sasang constitutions. Taeumin had higher body mass index in severe PMS(Premenstrual Symptoms) group than normal/mild PMS group, and Soyang in and Soeumin are vice versa. Taeumin showed depressed mood and hopelessness, decreased interest in school activities, Soyangin showed decreased interest in home activities, and Soeumin showed decreased interest in social activities. In terms of interfered functional activities, Soyangin had increased feeling overwhelmed or out of control and Taeumin and Soeumin had increased hypersomnia. Conclusion Menstrual symptoms are different among Sasang constitutional types based on PSST(Premenstrual Symptoms Screening Tool). These results may serve as the basis in identifying and treating premenstrual syndromes related to Sasang constitution.

Chest CT Findings of COVID-19 Patients with Mild Clinical Symptoms at a Single Hospital in Korea (경증의 임상 소견을 보이는 COVID-19 환자들의 흉부 CT 소견)

  • Purpose To retrospectively evaluate the chest computed tomography (CT) findings of coronavirus disease 2019 (COVID-19) in patients with mild clinical symptoms at a single hospital in South Korea. Materials and Methods CT scans of 87 COVID-19 patients [43 men and 44 women; median age: 41 years (interquartile range: 26.1-51.0 years)] with mild clinical symptoms (fever < 38℃ and no dyspnea) were evaluated. Results CT findings were normal in 39 (44.8%) and abnormal in 48 (55.2%) patients. Among the 48 patients with lung opacities, 17 (35.4%) had unilateral disease and 31 (64.6%) had bilateral disease. One (2.1%) patient showed subpleural distribution, 9 (18.8%) showed peribronchovascular distribution, and 38 (79.2%) showed subpleural and peribronchovascular distributions. Twenty-two (45.8%) patients had pure ground-glass opacities (GGOs) with no consolidation, 17 (35.4%) had mixed opacities dominated by GGOs, and 9 (18.8%) had mixed opacities dominated by consolidation. No patients demonstrated consolidation without GGOs. Conclusion The most common CT finding of COVID-19 in patients with mild clinical symptoms was bilateral multiple GGO-dominant lesions with subpleural and peribronchovascular distribution and lower lung predilection. The initial chest CT of almost half of COVID-19 patients with mild clinical symptoms showed no lung parenchymal lesions. Compared to relatively severe cases, mild cases were more likely to manifest as unilateral disease with pure GGOs or GGO-dominant mixed opacities and less likely to show air bronchogram.

Benign Prostatic Hyperplasia in Community-Dwelling Elderly in Korea

  • Background. This study was done to identify the prevalence of benign prostatic hyperplasia(BPH) and BPH-related symptoms among community-dwelling elderly men in Korea. In addition, quality of life and health care-seeking behavior were explored. Methods. A total of 417 elderly men were surveyed using the IPSS (International Prostatic Symptom Score) and a structured questionnaire on health care-seeking behaviors. Results. The prevalence of BPH was $19.7\%$. Of those with BPH, $80.3\%$ reported mild symptoms, $13.2\%$ moderate symptoms, and $6.5\%$ severe symptoms. The severity of BPH-related symptoms was significantly correlated with quality of life. Among those with BPH, $42.7\%$ had never consulted with anyone about their symptoms. Conclusions. BPH has emerged as a serious public health problem in elderly men. Elderly people who experience worse symptoms of BPH have a lower quality of life. Many elderly with BPH do not seek health care, mainly due to misconceptions about BPH. The provision of educational programs for BPH may significantly improve the quality of life of elderly men.

  • The effectiveness, the safety and the usefulness of HPS-A(An Jung San with Chyag Yag Kam Cho Tang), herbal digestive, was examined on 32 patients complained digestive symptoms. This medicine was administered 1 package per 1 time 3 times per 1 day, during 2 weeks. From the examination of the improvemnt and the safety mainly by subjective symptoms, the usefulness was gained. The results were as follows ; 1. In the improvement rate of susbjective symptoms, high rate symptoms was gastric distension, anorexia, heart burn, gastric discomport, dyspepsia, acrid eructation, indigestion, nausea. 2. The improvment over moderate effective was 85.7% after 1 week 96.4% after 2 weeks. The 4 cases breaked medicine during experimental period were all over moderate offectiveness. 3. There weren't any cases complaind particular problem about safety except for 1 case complained mild pushing up of heat. There weren't also any side effect of medicine on common blood count, blood chemistry and urinalysis. 4. In the dicision of usefulness, 19cases(59.4%) were usefulnesss, 12cases(37.5%) were moderate usefulness, 1 case was uselessness(3.1%). So the cases over moderate usefulness were 31 and rate was 96.9%. 5. The usefulness rates according to physical features were all over 95.5% in sturdiness, middle feature and infirmity. So the rates were no relations with the features. 6. The usefulness rates over moderate usefulness according to total seriousness were all over 90% in high, middle, and mild cases. So the rates were no relations with the seriousness. Totally HPS-A was a medicine that was highly effective, safe and useful to patients complained various digestive symptomes.

PTSD Symptoms in Elementary School Children After Typhoon Rusa

  • Purpose. A natural disaster negatively affects children's emotional and behavioral adjustment. The purpose of this paper was to examine the prevalence, symptoms, and correlates of PTSD after the occurrence of Typhoon Rusa. Method. 261 elementary school children living in Kimcheon, which was a devastated rural area in South Korea by Typhoon Rusa, were selected. Data were collected 4 months after the disaster using the PTSD Reaction Index categories recommended by Frederick, severity of PTSD. Results. 12.3% of the children had either moderate or severe PTSD symptoms; 22.7% reported mild symptoms; and the remaining 65% had sub-clinical symptoms of PTSD. The most frequent symptom was recurrenct fear(67.0%). 13% to 17.2% of children exhibited difficulty in concentration, sleep disturbance, and guilt feeling. The regression model of severity of PTSD was composed of the level of exposure to traumatic experiences, grade in school, gender, negative coping style, and social support, and explained 34.3% for PTSD symptoms. Exposure to traumatic experiences was the strongest factor of all predictors. Conclusion. Emotional support from friends and coping style were correlated with PTSD severity. School-based interventions that emphasizes coping with disaster related problems and problem-solving may prove to be useful, and may aid in building close and supportive ties with teachers, classmates, and friends.

Prostatic Hypertrophic Symptoms, Activities of Daily Living, Satisfaction of Sexual Activities and Well-Being between Subjects with Prostatic Hypertrophic Symptoms and General Persons (전립선 비대 증상 자각군과 일반 남성군의 전립선 비대 증상, 일상생활 활동, 성생활 만족 및 안녕감 비교)

  • Purpose: This study was designed to explore and compare the prostatic hypertrophoc symptoms, activities of daily living, satisfaction of sexual activities and well-being among subjects with prostatic hypertrophic symptoms and the general population. Method: One hundred subjects with prostatic hypertrophic symptoms and one hundred general persons were recruited in K medical center. The data were collected from October 20, 2001 to March 30, 2002 by structured questionnaire. Result: The results were the following; 1. The percentage of mild(0~7), moderate(8~19) and severe(20~35)symptoms between subjects with prostatic hypertrophic symptoms and general persons were 6% : 60%, 54% : 39%, 40% : 1%. 2. There was a significant difference in prostatic hypertrophic symptoms(t=12.82, p<.001), in activities of daily living(t=-7.77, p<.0001), in satisfaction of sexual activities (t=-4.80, p<.0001), in well-being(t=-4.80, p<.0001) between subjects with prostatic hypertrophic symptoms and general persons 3. There was a significant difference in activities of daily living(F=16.28, p<.0001), satisfaction of sexual activities(F=3.98, p<.05) according to prostatic hypertrophic symptoms in subjects with prostatic hypertrophic symptoms. Conclusion: According to the above findings, prostatic hypertrophic symptoms influence activities of daily living, satisfaction of sexual activities, well-being of subjects with prostatic hypertrophic symptoms negatively.

The Function and Symptoms of Ankle Joint in the Distal Tibial Fractures Treated by Nailing (족관절 기능과 증상을 중심으로 평가한 경골 원위부 골절에서의 금속정 치료 결과)

  • Purpose: To evaluate the safety and prognostic factors of intramedullary nailing of distal tibia fractures in terms of function and symptoms of the ankle joint. Materials and Methods: We retrospectively analyzed 22 distal tibial fractures with intramedullary nailing. The mean duration of follow-up was 43 months. We reviewed medical records to describe each case. We measured radiographic parameters such as fracture configuration, arthritic change of the ankle joint and status of reduction. We also assessed clinical results by AOFAS ankle hind foot scoring system, degree of pain by VAS and range of motion to find out prognostic factors for functional result of the ankle joint. Results: Bone healing was obtained in all cases without any wound complications. Mean AOFAS ankle score was 94. There were 4 cases with mild (VAS<3/10) ankle pain and 2 cases with mild limitation of ankle motion. The comminution of fracture had a significant relationship with delayed angular deformity of ankle joint (p=0.032). There was no other significant parameter affecting ankle joint function except the location of nail-end. Conclusion: Intramedullary nailing in distal tibia fracture is a safe and effective procedure. But further study may need to evaluate the relationship between the position of nail-end and the function of ankle joint.
