연골하 낭종을 동반한 거골의 골연골 병변의 관절경적 치료 후 임상적 결과 및 방사선학적 변화 (Clinical and Radiologic Results of Arthroscopic Treatment for Osteochondral Lesion with Subchondral Cyst on Talus)

  • Purpose: The purpose of this study is to review clinical and radiologic changes after arthroscopic operation without bone graft for osteochondral lesion with subchondral cyst on the talus. And we compared the results with those without cyst. Radiologic changes were also observed. Materials and Methods: Arthroscopic microfrature or abrasion arthroplasty was performed on 10 caeses of osteochondral lesions with subchondral cyst and 20 cases without cysts. Preoperative and postoperative symptoms were evaluated by Ankle-Hind foot scale of AOFAS and the score of two groups were compared(t-Test : Paired Two Test for Means). Radiologic evaluation was performed after operation on patients with osteochondral lesion with cyst. Results: On patients of osteochondral lesion with subchondral cyst on talus, there was clinical improvement compared to the preoperative status and compared to patients without cysts(P=0.01) after arthroscopic operation. We could get increase of density and decrease of size of cystic lesion on plain film with time passage. Conclusion: We report 10 cases with osteochondral lesion with subchondral cyst on talus which resulted in clinical and radiological improvement after arthroscopic microfracture or abrasion arthroplasty without bone graft.

전부피개관의 물성과 시멘트의 물성이 시멘트 내부의 응력에 미치는 영향 (Characterization of the Stress in the Luting Cement layer Influenced by Material Properties of Full Veneer Crown)

  • 이 논문의 목적은 시멘트 층 내의 응력분포에 대해 금관의 물성과 시멘트의 물성 및 교합력의 방향이 미치는 영향을 이차원 유한요소분석으로 조사하는 것이다. Chamfer 변연을 가지는 23개의 하악 제1대구치 모델을 상정하였다. 금관의 재질은 제 3형 금합금, Ni-Cr 합금, 세라믹, 복합레진 등 4가지이며, 시멘트의 재질은 인산 아연 시멘트와 글라스 아이오노머 시멘트 등 2가지이다. 치축과 평행하게 또는 경사지게 하여 1N 단위로 금관에 하중을 가했다. 시멘트층의 응력은 금관 변연이 다른 곳보다 더 높았다. 치축에 평행한 하중이 치축에 경사진 하중보다 더 높은 응력을 발생시켰다. 경도가 더 큰 금관은 시멘트 층 내에서 더 높은 응력을 나타냈으며, 더 높은 응력은 더 큰 Young's modulus을 나타냈다.

Er:YAG 레이저 조사가 산화 티타늄 블라스팅 임플란트 표면 미세 구조 및 거칠기에 미치는 영향 (The effect of Er:YAG laser irradiation on the surface microstructure and roughness of $TiO_2$ implant)

  • Purpose: The aim of this study was to evaluate the effect of Er:YAG laser on microstructure and roughness of $TiO_2$ blasting implant surface. Materials and Methods: Ten $TiO_2$ blasting implant were used in this experiment. One implant was control group, and nine $TiO_2$ blasting implant surfaces were irradiated with Er:YAG laser under 100 mJ/pulse, 140 mJ/pulse, and 180 mJ/pulse condition for 1 min, 1.5 min, and 2 min respectively. Optical interferometer and scanning electron microscopy was utilized to measure roughness and microstructure of specimens. Results: The surface roughness was decreased after Er:YAG laser irradiation in all groups, but there was no significant difference. 100 mJ/pulse and 140 mJ/pulse group did not alter the $TiO_2$ blasting implant surface in SEM study while 180 mJ/pulse group altered the $TiO_2$ blasting implant surface. Implant surfaces showed melting, microfracture and smooth surface in 180 mJ/pulse group. Conclusion: Detoxification of implant surface using Er:YAG laser must be irradiated with proper energy output and irradiation time to prevent implant surface alteration.

  • Today, there is considerable evidence to support a cause-effect relationship between microbial colonization and the pathogenesis of implant failures. The presence of bacteria on implant surfaces may result in an inflammation of the peri-implant mucosa, and, if left untreated, it may lead to a progressive destruction of alveolar bone supporting the implant, which has been named as peri-impantitis. Several maintenance regimens and treatment strategies for failing implants have been suggested. Recently, in addition to these conventional tools, the use of different laser systems has also been proposed for treatment of peri-implant infections. As lasers can perform excellent tissue ablation with high bactericidal and detoxification effects, they are expected to be one of the most promising new technical modalities for treatment of failing implants. It is introduced that Er,Cr:YSGG laser, operating at 2780nm, ablates tissue by a hydrokinetic process that prevents temperature rise. We studied the change of the titanium implant surface under scanning electron microscopy after using Er,Cr:YSGG laser at various energies, irradiation time. In this study, Er,Cr:YSGG laser irradiation of implant fixture showed different effects according to implant surface. Er,Cr:YSGG laser in TPS surface with RBM not alter the implant surface under power setting of 4 Watt(W) and irradiation time of 30sec. But in TPS surface with $Ca_3P$ coating alter above power setting of 2W and irradiation time of 10sec. TPS surface with RBM showed microfracture in 4W, 30sec and TPS surface with $Ca_3P$ coating showed destruction of fine crystalline structure, melting in excess of 2W, 10sec. We concluded that proper power setting, air, water of each implant surface must be investigated and implant surface must be irradiated under the damaged extent.

열-음향방출기법을 이용한 복합재료의 미세손상 검출 및 평가 (Detection and Evaluation of Microdamages in Composite Materials Using a Thermo-Acoustic Emission Technique)

  • 열-음향방출(thermo-acoustic emission)기법을 이용함으로써, 직교형 복합재료적층판의 저온냉각 시에 형성되는 미세손상을 검출하고 평가하여 그 유효성을 검토하였다. 액체질소에 의한 저온냉각($-191^{\circ}C$)으로 생성된 미세손상은 가열-냉각 열부하사이클 시에 발생하는 음향방출(AE)거동의 해석을 통해 평가되었다. 저온냉각에 따른 섬유파단과 모재파손은 초음파 C스캔, 광학현미경, 주사형 전자현미경을 통해 관찰되었으며, 이들 미세파손 모드는 AE신호의 퓨리에 변환(FFT)처리해석, 단시간 퓨리에 변환(STFT)처리해석으로 분류되는 3종류의 서로 다른 특징을 갖는 AE신호로 검출될 수 있었다. 이들 AE신호특성을 시간 단계별로 검출하여 저온냉각시에 형성되는 복합재료 미세파괴의 과정을 실시간으로 평가할 수 있었으며, 또한 열부하 사이클시의 AE신호해석을 통해서 저온 냉각으로 생성된 미세파손의 정도를 추정할 수 있었다.

철계 반비정질 합금 분말과 VC 분말을 고에너지 전자빔으로 투사하여 제조된 표면복합재료의 미세조직, 경도, 파괴인성 (Microstructure, Hardness, and Fracture Toughness of Surface Composites Fabricated by High-Energy Electron-Beam Irradiation of Fe-Based Metamorphic Alloy Powders and VC Powders)

  • In this study, surface composites were fabricated with Fe-based amorphous alloy powders and VC powders by high-energy electron beam irradiation, and the correlation of their microstructure with hardness and fracture toughness was investigated. Mixture of Fe-based metamorphic powders and VC powders were deposited on a plain carbon steel substrate, and then electron beam was irradiated on these powders without flux to fabricate surface composites. The composite layers of 1.3~1.8 mm in thickness were homogeneously formed without defects and contained a large amount (up to 47 vol.%) of hard $Cr_2B$ and $V_8C_7$ crystalline particles precipitated in the solidification cell region and austenite matrix, respectively. The hardness of the surface composites was directly influenced by hard $Cr_2B$ and $V_8C_7$ particles, and thus was about 2 to 4 times greater than that of the steel substrate. Observation of the microfracture process and measurement of fracture toughness of the surface composites indicated that the fracture toughness increased with increasing additional volume fraction of $V_8C_7$ particles because $V_8C_7$ particles effectively played a role in blocking the crack propagation along the solidification cell region heavily populated with $Cr_2B$ particles. Particularly in the surface composite fabricated with Fe-based metamorphic powders and 30 % of VC powders, the hardness and fracture toughness were twice higher than those of the surface composite fabricated without mixing of VC powders.

보조기구를 이용한 DR System 활용 방법에 대한 연구 (A Study on how to Utilize the DR System using Assistive Devices)

  • 엉덩관절 고정기구는 골밀도 검사 및 미세골절, 관절염의 진단 시 내회전을 위한 중요 고정기구입니다. 이러한 고정기구는 벨크로를 사용하여 재현성 유지에 한계가 있습니다. 이에 본 연구에서는 개선된 엉덩관절 고정기구를 개발하여 유용성을 평가하였습니다. 방사선과 재학생 30 명을 대상으로 기존과 개선된 고정기구를 적용하여 자세잡이 소요시간과 의료영상을 통한 재현성을 평가하였습니다. 또한, 시간경과에 따른 변화도 평가하였습니다. 그 결과 기존 고정기구에 비해 개선된 고정기구는 자세잡이 시간은 약 32%의 감소를 보였으며, 재현성은 약 6.2 배 향상되었습니다. 시간경과에 따른 변화량은 자세잡이 시간은 기존 고정기구는 증가된 반면 개선된 고정기구는 감소되었으며, 재현성은 개선된 고정기구보다 기존 고정기구가 더 저하되었습니다. 이를 통해 개선된 고정기구의 사용을 통해 영상의 질 향상과 방사선사의 업무부담 감소가 나타났다고 할 수 있습니다.

Energy-dispersive X-ray spectroscopic investigation of a fractured non-submerged dental implant associated with abutment fracture

  • The biocompatibility and durability of implant fixtures are major concerns for dentists and patients. Mechanical complications of the implant include abutment screw loosening, screw fracture, loss of implant prostheses, and implant fracture. This case report aims to describe management of a case of fixture damage that occurred after screw fracture in a tissue level, internal connection implant and microscopic evaluation of the fractured fixture. A trephine bur was used to remove the fixture, and the socket was grafted using allogeneic bone material. The failed implant was examined by scanning electron microscopy (SEM) and energy dispersive X-ray spectroscopy (EDS), which revealed a fractured fixture with both normal and irregular bone patterns. The SEM and EDS results give an enlightenment of the failed fixture surface micromorphology with microfracture and contaminated chemical compositions. Noticeably, the significantly high level of gold (Au) on the implant surface and the trace amounts of Au and titanium (Ti) in the bone tissue were recorded, which might have resulted from instability and micro-movement of the implant-abutment connection over an extended period of time. Further study with larger number of patient and different types of implants is needed for further conclusion.

골관절염에서 줄기세포를 이용한 연골 재생의 최신 지견 (Current Update of Cartilage Regeneration Using Stem Cells in Osteoarthritis)

  • 골관절염은 관절연골 침식의 진행을 특징적으로 보이는 질환으로 관절운동 중에 통증을 증가시키고 기계적 스트레스를 견디는 능력을 감소시켜 결과적으로 관절의 가동성과 기능을 저하시킨다. 외상 또는 퇴행성으로 인한 관절연골의 손상이 일반적 관절염의 주요 원인으로 생각되며 이러한 관절연골 손상의 재생에 관한 수많은 연구와 시도들이 현재까지 진행되어 오고 있다. 현재까지 연골 손상의 경우 미세골절술과 자가연골세포 이식술이 일반적인 수술적 치료방법으로 제시되어 왔으나 비교적 양호한 임상 결과에도 불구하고 정상 유리연골의 생성이 미흡하여 시간이 경과하면서 결과가 악화되는 등 단점이 있다. 이를 보완하기 위하여 줄기세포 기반 치료법이 개발되었다. 이 종설에서는 현재 사용되는 다양한 연골 재생 방법들의 장단점 및 결과에 대해 요약하고 특히 중간엽 줄기세포(mesenchymal stem cells) 기반 연골 재생 치료법을 논하고 나아가 이상적인 미래 연골 재생 치료법에 대해서도 고민해보고자 한다.

골수흡인물 농축액 및 기질유래연골형성 수술 이후 관찰된 거골의 골연골병변의 골유합: 증례 보고 (Bony Union of Osteochondral Lesion of the Talus after Bone Marrow Aspirate Concentrate and Matrix-Induced Chondrogenesis: A Case Report)

  • Traditionally, arthroscopic microfracture and autologous osteochondral autograft transplantation have been the primary surgical treatments for osteochondral lesions of the talus. On the other hand, recent advancements have introduced alternative approaches, such as autologous chondrocyte transplantation, matrix-derived autologous chondrocyte transplantation, intra-articular injection of concentrated bone marrow aspirate concentrate, and the use of fibrin glue to address chondral defects. Furthermore, some studies have explored a combination of bone marrow aspirate and matrix-derived chondrogenesis. In light of these developments, this report presents a case study of a young male patient in his early twenties with a relatively large osteochondral lesion exceeding 1.5 cm2 on the medial talar dome. Instead of removing the osteochondral lesion, a surgical approach was employed to retain the lesion while addressing the unstable cartilage in the affected area. This approach involved a combination of bone marrow aspirate concentrate and matrix-derived chondrogenesis. The treatment yielded favorable clinical outcomes and ultimately successfully induced the bony union of osteochondral lesions. This paper reports the author's experience with this innovative approach with a review of the relevant literature.
