Impact of conversion at time of minimally invasive pancreaticoduodenectomy on perioperative and long-term outcomes: Review of the National Cancer Database

  • Backgrounds/Aims: Current literature presents limited data regarding outcomes following conversion at the time of minimally invasive pancreaticoduodenectomy (MI-PD). Methods: The National Cancer Database was queried for patients who underwent pancreaticoduodenectomy. Patients were stratified into three groups: MI-PD, converted to open pancreaticoduodenectomy (CO-PD), and open pancreaticoduodenectomy (O-PD). Multivariable modeling was applied to compare outcomes of MI-PD and CO-PD to those of O-PD. Results: Of 17,570 patients identified, 12.5%, 4.2%, and 83.4% underwent MI-PD, CO-PD, and O-PD, respectively. Robotic pancreaticoduodenectomy (R-PD) resulted in a higher lymph node yield (n = 23.2 ± 12.2) even when requiring conversion (n = 22.4 ± 13.2, p < 0.001). Margin positivity was higher in the CO-PD group (26.6%) than in the MI-PD group (21.3%) and the O-PD (22.6%) group (p = 0.017). Length of stay was shorter in the MI-PD group (laparoscopic pancreaticoduodenectomy 10.4 ± 8.6, R-PD 10.6 ± 8.8) and the robotic converted to open group (10.7 ± 6.4) than in the laparoscopic converted to open group (11.2 ± 9) and the O-PD group (11.5 ± 8.9) (p < 0.001). After adjusting for patient and tumor characteristics, both MI-PD (odds ratio = 1.40; p < 0.001) and CO-PD (odds ratio = 1.24; p = 0.020) were significantly associated with an increased likelihood of long-term survival. Conclusions: CO-PD does not negatively impact perioperative or oncologic outcomes.

Design Errors and Cryptanalysis of Shin's Robust Authentication Scheme based Dynamic ID for TMIS

  • In this paper, we analyze Shin's proposed dynamic ID-based user authentication scheme for TMIS(Telecare Medicine Information System), and Shin's authentication scheme is vulnerable to smart card loss attacks, allowing attackers to acquire user IDs, which enables user impersonation attack. In 2019, Shin's proposed authentication scheme attempted to generate a strong random number using ECC, claiming that it is safe to lose a smart card because it is impossible to calculate random number r'i due to the difficulty of the ECC algorithm without knowing random number ri. However, after analyzing Shin's authentication scheme in this paper, the use of transmission messages and smart cards makes it easy to calculate random numbers r'i, which also enables attackers to generate session keys. In addition, Shin's authentication scheme were analyzed to have significantly greater overhead than other authentication scheme, including vulnerabilities to safety analysis, the lack of a way to pass the server's ID to users, and the lack of biometric characteristics with slightly different templates.

A study on the Job Activity Analysis of the Hemodialysis Nurses (혈액투석 간호업무 분석)

  • The purpose of this study is to analyse the job activities of the H/D nurses andto develop their job activities. This study surveyed 222 H/D nurses' reports using a questionnaire which contained 101 questions about the nursing care contents of the H/D nursing and 7 questions about the characteristics of the H/D nurses who were working in the H/D rooms of the 80 hospitals and local clinics. H/D nurses checked for 6 days the frequency of the H/D job activities and the amount of time spent in each individual nursing content by the stop watch which had been provided. The data was gathered from Jan. 21, 1999 to Oct. 30, 1999 and analysed by descriptive statistics, F-test and Pearson's correlation r with SPSS program. The results are as follows:

Simultaneous Determination of Baicalin and Glycyrrhizin in Eul-Ja-Tang by HPLC/DAD

  • A high performance liquid chromatographic (HPLC) method for the simultaneous determination of marker constituents, baicalin and glycyrrhizin was established for the quality control of traditional herbal medicinal preparation, Eul-Ja-Tang (EJT). Separation and quantification were successfully achieved with a Waters XTerra RP18 column ($5{\mu}m$, 4.6 mm I.D. ${\times}$ 150 mm) by gradient elution of a mixture of acetonitrile and water containing 0.03% phosphoric acid (pH 2.03) at a flow rate of 1.0 mL/min. The diode-array UV/VIS detector (DAD) was used for the detection and the wavelength for quantification was set at 250 nm. The presence of baicalin and glycyrrhizin in this decoction was ascertained by retention time, spiking with each authentic standard and UV spectrum. Both baicalin and glycyrrhizin showed good linearity ($r^2$ > 0.999) in a relatively wide concentration ranges. The R.S.D. for intra-day and inter-day precision was less than 5% and the limits of detection (LOD) were about 30 ng. The mean recovery of each compound was 99.5 - 101.2% with R.S.D. values less than 4.0%. This method was successfully applied to the determination of contents of baicalin and glycyrrhizin in three commercial products of EJT, which resulted in the difference in the contents of these compounds. These results suggest that the developed HPLC method is simple, effective and could be readily utilized as a quality control method for commercial EJT products.

Relationship between occlusion analysis using the T-scan III system and oral behavior checklist according to temporomandibular joint disorder in female college students (여대생의 측두하악장애에 따른 T-scan III System을 이용한 교합분석 및 구강행동유형과의 관련성)

  • Objectives: This study aimed to evaluate the effects of occlusion patterns using the T-scan $III^{(R)}$ system and oral parafunctional behaviors on temporomandibular joint disorder (TMD) in female college students. Methods: A questionnaire survey assessed TMD symptoms and oral behavioral checklist (OBC) scores in 120 female college students in their early twenties. Occlusion was evaluated using the T-scan $III^{(R)}$ system. Logistic regression analysis was performed to investigate the relationship between occlusion and OBC scores based on patients' symptoms of TMD and to determine whether OBC scores were a predictor of TMD. Results: A total of 101 participants with an average age of 22 (${\pm}2.17$) years were included in the analysis. The reported symptoms of TMD were categorized as moderate (28.8%), slight (27.7%), severe (25.7%), and asymptomatic (17.8%). Occlusion was analyzed using the T-scan $III^{(R)}$ system, and the mean occlusion time was 0.42(${\pm}0.5$) seconds. The results of logistic regression analysis showed that the group that would 'clench or grind teeth' during sleep showed an odds ratio for TMD that was 8.9 times higher than that in the group without this behavior. The group that would 'hold, tighten, or tense muscles without clenching' while awake showed an odds ratio for TMD that was 21.3 times higher than that in the group without this behavior. Conclusions: We confirmed that oral parafunctional behavior affects TMD. Therefore, we would like to continue studying the diagnosis and treatment of TMD by evaluating occlusion patterns and related OBC scores.

Mediating Effect of Therapeutic Relationship with Mental Health Professionals in the Relation between Empowerment and Recovery of Community Dwelling Patients with a Mental Illnesst (지역사회 정신장애인의 임파워먼트와 회복 : 치료적 관계의 매개효과)

  • This study was conducted to identify the mediating effects of the therapeutic relationship with mental health professionals between empowerment and recovery in mental illness. The participants were 168 adults in two community mental health centers and three rehabilitation centers in Jeonbuk. The data were collected by structured questionnaires from April 15 to April 30, 2015 and analyzed using descriptive statistics, Pearson's correlation coefficient, and multiple regression analysis with the SPSS 19.0 statistical program. The mean score of empowerment for mental illness, recovery and therapeutic relationship were $60.4{\pm}10.85$, $101.8{\pm}20.70$ and $74.5{\pm}14.30$, respectively. There were significantly positive correlations between recovery in empowerment and therapeutic relationship (r=0.33, p<0.001), as well as empowerment and recovery (r=0.51, p<0.001), while there were significantly positive correlations between therapeutic relationship and recovery (r=0.55, p<0.001). Therapeutic relationship had a partial mediated effect between empowerment and recovery, which was found to be significant by the Sobel test (z=3.706, p<0.001). The results of this study can be used as basic data for development of a practical recovery-enhancing program to promote therapeutic relationships and improve empowerment in relation to therapist of the mentally disabled.

  • Cholesteryl methyl ether$(CH_3OC_{27}H_{45})$ crystallizes in the monoclinic space group $P2_1$with a = 11.740 (8), b = 7.576 (5), c = 15.492 (10)${\AA},\; {\beta}\;= 110.39 (5)^{\circ}$, Z = 2, Dc = 1.03 g/$cm^3$ and Do = 0.96 g/$cm^3$. The intensities were collected on a Nonius CAD-4 diffractometer with Nb-filtered Mo-$K_{\alpha}$ radiation. The structure was solved by direct methods and refined by full matrix least-squares methods. The final R factor was 0.085 for 1479 observed reflections. Compared with other cholesterol derivatives, the cholesteryl ring and tail region of the molecule are normal. The molecular long axes are parallel to the [101] axis and molecules are packed together in a similar way to those in cholesteryl cliloride and bromide.

  • Catalytic subunit of pyruvate dehydrogenase phosphatase (PDPc) has been suggested to have three major funational domains such as dihydrplipoamide adetyltransferase(E2)-binding domain, regulatory subunit of PDP(PDP)r-binding domain, and calcium-binding domain. In order to identify functional domains, recombinant catalytic subunit of pyruvate dehydrogenase phosphatase(rPDPc) was expressed in E. coli JM101 and purified to near homogeneity using the unique property of PDPc: PDPc binds to the inner lipoyl domain (L2) of E2 of ppyruvate dehydrogenase complex (PDC) in the presence of Ca+2, not under EGTA. PDPc was limited-proteolysed by typsin, chymotypsin, Arg-C, and elastase at pH 7.0 and 30C and N-terminal analysis of the fragments was done. Chymotrypsin, trypsin, and elastase made two major fragments: N-terminal large fragment, approx. 50kD and C-terminal small fragment, approx.10 kDa. Arg-C made three major fragments: N-terminal fragment, approx. 35kD, and central fragment, approx. 15 kD, and C-terminal fragment, approx. 10 kD. This study strongly suggest that PDPc consists of three major functional domains. However, further study should be necessary to identify the functional role.

A Study on the Relationship Between the Sasang Constitution and the Minnesota Multiphasic Personality Inventory-2 Questionnaire of Korean Medical Students (한의대생의 사상체질별 MMPI-2 특성 연구)

  • Objectives: The purpose of this study is to investigate how the psychological and mental tendencies manifested in the Sasang Constitution are reflected in an objective personality test, MMPI-2. Methods: The survey was conducted on 101 Korean medical students to investigate the relationship between the Sasang Constitution and MMPI-2. Results: Soeumin showed higher scores in the Hs, Pt, RC1 and RC3 than Taeumin and showed higher scores in the D, Si, RC2, R, MAC-R and INTR than Soyangin. Soeumin showed higher scores in the ANX, OBS, and HEA and showed lower scores in the Es and GM than other Constitutions. Soyangin showed the higher scores in the AGGR than Soeumin and showed the higher scores in the RC9 than other Constitutions. Soyangin showed the lower scores in the SOD and Taeumin showed the lower scores in the ASP than other Constitution (p<.05). Conclusions: MMPI-2 contains items that distinguish the characteristics of Soeumin, Soyangin, and Taeumin and it is therefore useful to employ it to obtain basic data for analyzing oriental psychopathology.

Structural and Functional Relationship of the Catalytical Subunit of Recombinant Pyruvate Dehydrogenase Phosphatase (rPDPc): Limited Proteolysis (Pyruvate dehydrogenase phosphatase의 catalytical subunit의 구조와 활성에 대한 연구)

  • Pyruvate dehydrogenase phosphatase (PDP)와 kinase는 당대사시 해당과정에서의 대사 산물인 pyruvate를 acetyl CoA로 만들어 구연산 회로로 진입시켜 주는 효소인 pyruvate dehydrogenase complex (PDC)의 활성을 조절하는 중요한 효소이다. PDP의 catalytic subunit는 PDC의 dihydrolipoamide acetyltransferase (E2), PDP regulatory subunit (PDPr), 그리고 칼슘 결합 도메인 등으로 구성되어 있는 것으로 추측되어지고 있다. 본 연구에서는 그 구조와 기능과의 상관관계를 알아보기 위해 PDPc를 E. coli JM101에서 발현시켜 순수 정제 후 단백분해 효소를 이용한 제한적 가수분해 방법을 이용해 그 구조와 기능과의 상관관계에 대해 연구하고자 하였다 정제된 PDPc는 trypsin, chymotrypsin, Arg-C 그리고 elastase를 이용하여 3$0^{\circ}C$ 그리고 pH 7.0에서 제한적으로 분해시켰으며 각 분해산물의 아미노 말단의 아미노산 배열을 분석하였다. 그 결과 PDPc는 trypsin, chymotrypsin, elastase에 의해 N-terminal의 50 kD과 C-terminal의 10 kD의 두개의 분해산물을 만들었으며, Arg-C에 의해 50kD의 분해산물은 약 35kD와 15kD으로 더 가수분해가 되었다. 이러한 결과로 볼 때 PDPc는 앞에서 추측한데로 세개의 주요한 기능적 도메인으로 이루어져 있음을 알 수 있었다 또한 C-terminal의 10kD은 PDPc의 활성에는 영향을 주지 않는 것으로 밝혀졌으나 다른 도메인의 기능은 더 연구가 되어져야 할 것으로 생각된다.
