Polyunsaturated Fatty Acids in Children

  • Polyunsaturated fatty acids (PUFAs) are the major components of brain and retina, and are the essential fatty acids with important physiologically active functions. Thus, PUFAs should be provided to children, and are very important in the brain growth and development for fetuses, newborn infants, and children. Omega-3 fatty acids decrease coronary artery disease and improve blood flow. PUFAs have been known to have anti-inflammatory action and improved the chronic inflammation such as auto-immune diseases or degenerative neurologic diseases. PUFAs are used for metabolic syndrome related with obesity or diabetes. However, there are several considerations related with intake of PUFAs. Obsession with the intake of unsaturated fatty acids could bring about the shortage of essential fatty acids that are crucial for our body, weaken the immune system, and increase the risk of heart disease, arrhythmia, and stroke. In this review, we discuss types, physiologic mechanism of action of PUFAs, intake of PUFAs for children, recommended intake of PUFAs, and considerations for the intake of PUFAs.

건강기능식품의 체중 감량 효과 (The Effect of Health Functional Food on Body Weight Reduction)

  • Obesity is a serious health concern, which has been linked to an increased risk for cardiovascular diseases and some cancers. The traditional obesity control program is expensive. Moreover, it is difficult to maintain a healthy body weight as well as reduce body fat. The long-term use of effective and tolerable medication is carefully recommended to control body weight. In addition to obesity control medications, health functional foods, related to body weight control, have become popular in the commercial market. Known mechanisms include lipolysis, appetite control, inflammation reduction, and lean body mass maintenance. Previous clinical trials have documented the efficacy of some health functional foods; however, there are limitations. Studies on the potential roles and efficacy of some health functional foods, including caffeine, green tea, protein supplement, probiotics, and arginine, were reviewed. More large-scale and randomized placebo-controlled trials should be conducted eventually.

  • The prevalence of obesity is continually increasing in South Korea; about 1/3 of adults are diagnosed with obesity and 1/2 of adults are overweight in 2016. Abnormal body fat mass increased the risk factors of metabolic syndrome (including hypertension, type II diabetes, dyslipidemia), chronic kidney failure, osteoarthritis, and cardiovascular disease. Ministry of Food and Drug Safety (MFDS) in Korea established the validation and approval system for "functional food" based on related laws and regulations. According to the guideline of MFDS, the biomarkers for obesity study in vitro, in vivo, and clinical trial are well summarized. The analysis of physical phenotypes is necessary condition to study further molecular phenotypes and pathway analysis in vivo study. Thus, we will review currently available physical phenotype analysis; dual energy X-ray absorptiometry (DEXA) and Oxylet gas analysis will be examined in-depth.

한국 건강불평등의 현황과 문제점 (Health Inequalities in Korea: Current Conditions and Implications)

  • Objectives : The aim of this study is to summarize the current conditions and implications of health inequalities in South Korea. Methods : Through a literature review of empirical studies and supplementary analysis of the data presented in the 1998, 2001, and 2005 KNHANEs, we evaluated the extent and trends of socioeconomic inequalities in both health risk factors, such as smoking, physical activity, and obesity, and outcomes, such as total mortality, subjective poor health status by self-reports and metabolic syndrome. Relative risks and odds ratios were used to measure differences across socioeconomic groups, and the relative index of inequality was used to evaluate the changes in inequalities over time. Results : We found clear inequalities to various degrees?in most health indicators. While little change was observed in mortality differences over time, the socioeconomic gaps in risk factors and morbidity have been widening, with much larger differences among the younger population. Conclusions : Socioeconomic inequalities are pervasive across various health indicators, and some of them are increasing. The trends in socioeconomic inequalities in health should be carefully monitored, and comprehensive measures to alleviate health inequalities are needed, especially for young populations.

복부종괴로 인한 체중증가를 비만으로 오인한 2개의 증례 (Weight Gain Due to Abdominal Mass Misdiagnosed as Obesity: Case Report)

  • Early evaluation of obesity is important. Obesity is defined as an excessive accumulation of fat that causes harm to health. Among patients who visit a hospital for weight-related issues, diseases other than body fat gain may be present; however, people often cannot distinguish between these issues and concerns. Therefore, among patients who visit the hospital with weight gain concerns, it is necessary to determine whether the weight gain is actually the result of excessive fat accumulation. After being diagnosed with obesity, the cause and degree of obesity, the amount and distribution of body fat, and the degree of risk should be evaluated. Additionally, obesity-related complications should be identified and, even if there are no complications, all related risk factors should be evaluated and managed. In all these processes, diagnostic methods such as history taking, physical examination, body fat measurement, blood tests, and imaging tests are necessary, but history taking and physical examination, which can provide a lot of information from the beginning, should not be overlooked.

가족성 고콜레스테롤혈증은 언제 의심해야 할까?: 관상동맥 질환이 없는 젊은 이상지질혈증 환자 증례 (When Should Familial Hypercholesterolemia Be Suspected?: A Case of Dyslipidemia in Young Patient without Coronary Artery Disease)

  • Familial hypercholesterolemia (FH) is a genetic disease that is not well known or diagnosed in Korea. This disease is associated with persistently high levels of low-density lipoprotein cholesterol (LDL-C), which increase the risk of coronary artery disease at a young age. Therefore, early diagnosis and treatment are important; however, there are no global consensus diagnostic criteria. In Korea, the Dutch Lipid Clinic Network diagnostic criteria, and the Simon Broome diagnostic criteria were used for diagnosis of FH according to the agreement announced at the Korean Society of Lipid and Atherosclerosis (KSoLA) in 2022. Recently, the absence of coronary artery calcification has been considered a good prognostic factor, even among patients with very high LDL-C levels who are considered to be at high risk for atherosclerotic cardiovascular disease. We describe throughout this paper the diagnosis and treatment of FH in a young male without coronary artery calcification.

소아청소년 비만의 임상적 이해 (Clinical Perspectives on Obesity in Children and Adolescents)

  • The prevalence of obesity in children and adolescents is increasing worldwide. Obesity in children and adolescents not only increases the risk of transitioning to obesity in adulthood but also increases the risk of cardiometabolic diseases such as high blood glucose, high blood pressure, dyslipidemia, fatty liver, and hyperinsulinemia during childhood. The goal of treating obesity in children and adolescents is not to focus on weight loss but to help children reach a healthy weight while maintaining normal growth appropriate for their age and sex. To achieve this goal, regular physical activity and exercise, dietary modification, improvement of obesity-prone environmental factors, and behavioral changes are required for a healthy lifestyle. If appropriate weight control is not achieved through lifestyle modifications, pharmacotherapy may be considered for adolescents with severe obesity aged 12 and above. Recent clinical trials have reported the efficacy and safety of pharmacotherapy in severely obese adolescents. Currently, two medications can be prescribed in Korea for patients with obesity aged 12 and above: Orlistat and Liraglutide. However, despite effective weight control through drug treatment, body weight may increase again after treatment discontinuation. Therefore, it is crucial to evaluate adherence to health behaviors during visits and continue to educate on lifestyle modifications, even during pharmacotherapy, to minimize weight regain.

젊은 성인에서 비만이 심혈관대사질환 위험인자 군집에 미치는 영향 (Impact of Obesity on Cardiometabolic Disease Risk Factor Clusters among Korean Young Adults)

  • Background: This study investigated the association between obesity and cardiometabolic disease clusters in Korean adults aged 19-39 years. Methods: We included 3,785 participants (1,767 men and 2,018 women) aged 19-39 years who participated in the Korea National Health and Nutrition Examination Survey (2020-2022). The risk factors for cardiometabolic disease were defined as follows: high blood pressure; hypertriglyceridemia; hyperuricemia; and high low-density lipoprotein cholesterol, low high-density lipoprotein cholesterol, high fasting plasma glucose, elevated alanine aminotransferase levels. Results: The prevalence of participants with ≥ 1, ≥ 2, and ≥ 3 cardiometabolic disease risk factors was 95.2%, 84.2%, and 65.6% in men and 90.0%, 70.5%, and 43.0%, in women with class 2-3 obesity, respectively. Compared to those for young adults with underweight/normal weight, the odds ratios (ORs) and 95% confidence intervals (CIs) for the clustering of cardiometabolic risk factors were 17.26 (8.32-35.80) for ≥ 1; 19.43 (12.18-31.00) for ≥ 2; and 22.93 (14.15-37.15) for ≥ 3 factors in men with class 2-3 obesity and 14.67 (8.00-26.91) for ≥ 1; 20.88 (12.15-35.88) for ≥ 2; and 36.26 (20.20-65.09) for ≥ 3 factors in women with class 2-3 obesity. Conclusion: Although the patients were young, the prevalence and ORs for cardiometabolic disease risk factor clusters in young adults with obesity were prominently high. Prevention and management of obesity in young Korean adults are urgently needed at both individual and public levels.

A neonate with hyperornithinemia-hyperammonemia-homocitrullinuria syndrome from a consanguineous Pakistani family

  • Hyperornithinemia-hyperammonemia-homocitrullinuria (HHH) syndrome is a rare autosomal recessive urea cycle disorder. HHH is caused by a deficiency of the mitochondrial ornithine transporter protein, which is encoded by the solute carrier family 25, member 15 (SLC25A15) gene. Recently, government supported Korean newborn screening has been expanded to include a tandem mass spectrometry (MS/MS) measurement of ornithine level. We report a case of a neonate with HHH syndrome showing a normal MS/MS measurement of ornithine level. A female newborn was admitted to neonatal intensive unit due to familial history of HHH syndrome. Her parents were consanguineous Parkistani couple. The subject's older sister was diagnosed with HHH syndrome at age of 30 months based on altered mental status and liver dysfunction. Even though the subject displayed normal ammonia and ornithine levels based on MS/MS analysis, a molecular test confirmed the diagnosis of HHH syndrome. At 1 month of age, amino acid analysis of blood and urine showed high levels of ornithine and homocitrulline. After 11 months of follow up, she showed normal growth and development, whereas affected sister showed progressive cognitive impairment despite no further hyperammonemia after protein restriction and standard therapy. Our report is in agreement with a previous Canadian study, which showed that neonatal samples from HHH syndrome patients demonstrate normal ornithine levels despite having known mutations. Considering the delayed rise of ornithine in affected patients, genetic testing, and repetitive metabolic testing is needed to prevent patient loss in high risk patients.

What is the meaning of hs-CRP and HbA1c in patients with dry eye syndrome in diabetes?

  • 본 연구는 2016년 6월 16일부터 2016년 12월 31일까지 건강검진센터를 방문한 726명을 대상으로 Schirmer's 검사와 대사증후군 검사를 통해 전신적 만성 염증성 상태가 국소적 만성 염증성 상태로서의 안구건조증에 미치는 영향을 알아보고자 시행하였다. 대상자중 hs-CRP, HbA1c를 동시 측정한 214명을 대상으로 연구를 진행하였다. 연구대상자 중 고 연령일수록 (p<0.001), 성별로는 여성에서 유의하게 안구건조증이 증가하였으며(p=0.020), 공복혈당과 hs-CRP와는 연관이 없었다. 또한 HbA1c가 증가할수록 안구건조증에 대한 위험도가 증가하였다(β=1.960, p=0.025). 따라서 안구건조증은 만성 염증성 상태로 인한 눈물샘의 미세혈관 변화에 의한 것이 아니며 당뇨환자의 경우 단기간의 혈당 조절보다 장기간의 혈당 조절상태가 더욱 중요하게 작용할 수 있음을 알 수 있으며 당뇨병 환자에서 안구 건조증의 높은 유병률은 만성 염증으로 인한 미세 혈관 변화보다는 자율 기능 장애로 인한 것으로 생각된다.
