안동 농촌지역 중년 및 노인 주민의 대사증후군 유병율과 관련 위험요인 분석 2. 생화학 측정결과와 영양소 섭취를 중심으로 (Prevalence of Metabolic Syndrome and Related Risk Factors of Elderly Residents in Andong Rural Area 2. Based on the Biochemical Measurements and Nutrient Intakes)

  • 본 연구는 농촌지역 주민들의 대사증후군 예방을 위한 영양 사업에 기초 자료를 제공하고자 안동시 읍면 지역 농촌의 45세 이상 1,431명의 주민을 대상으로 대사증후군 집단과 정상 집단의 생화학적 특성 및 영양소 섭취 상태를 조사하고 대사증후군 발생 위험도와의 관련성을 다항로지스틱회귀모델을 사용하여 분석하였다. 대사증후군 집단과 정상 집단간에 연령 차이는 없었으며, 혈액 AST, ALT, $\gamma$-GT 및 과산화지질은 대사증후군 집단이 정상 집단에 비해 유의적으로 높았다. 대사증후군 위험도 분석에서 여자가 남자에 비해 위험도가 2.953배 높았으며, 연령에 따른 차이는 나타나지 않았다. 혈액 AST, ALT 및 $\gamma$-GT의 경우, 30 U/L 이상 집단에서 각각 1.839배, 2.302배 및 2.143배 위험도가 높았으며, 혈액 과산화지질 농도도 5.7 nmole/mL 이상 집단에서 위험도가 1.874배 높은 것으로 나타났다. 각 진단요소별 위험도 분석에서, 혈액 AST, ALT 및 $\gamma$-GT가 각각 30 U/L 이상에서 AST는 복부비만, 고혈압, 고 중성지방혈증 위험도가 각각 1.394배, 1.514배, 1.528배 높으며, ALT는 고 중성지방혈증과 높은 공복혈당이 각각 2.138배와 2.310배, $\gamma$-GT는 복부비만, 고혈압, 고 중성지방혈증 및 높은 공복혈당이 각각 1.513배, 1.594배, 2.354배 및 1.858배 높은 것으로 나타났다. 혈액 과산화지질은 5.7 nmole/mL 이상에서 복부비만, 고 중성지방혈증 및 높은 공복혈당이 각각 1.607배, 3.095배 및 1.757배 높았다. 대사증후군 집단과 정상 집단의 영양소 섭취 상태는 유의적인 차이가 없었으며, 대사증후군 위험도 분석에서 에너지의 70% 이상을 탄수화물로 섭취하는 경우 대사증후군 위험도가 0.781로 낮은 경우를 제외하고는 영양소와 대사증후군 간에 유의한 관련성이 나타나지 않았다. 영양소 섭취와 대사증후군 진단요소별 위험도 분석에서 열량 섭취가 EER 미만인 집단의 복부비만 위험도가 유의적으로 낮았으며(OR 0.696, p<0.05), 열량에 대한 탄수화물 비율이 55% 미만인 경우 저 HDL-콜레스테롤 위험도가 1.630배 증가하였고, 70%를 초과한 경우 복부비만과 고혈압 위험도가 각각 0.724배와 0.733배 낮았으며, 지질 섭취가 25%를 초과한 경우 저 HDL-콜레스테롤 위험도가 1.864배 높았다. 이상의 결과로부터 대사증후군 집단의 혈액 트랜스아미나제 활성과 과산화지질 농도가 정상 집단에 비해 유의하게 높으므로 이 지역 주민들의 대사증후군 예방을 위해서는 정기적으로 혈액 트랜스아미나제와 과산화지질 농도의 측정을 통한 관리가 필요하다고 생각되며, 영양소 섭취가 대사증후군 발생에 미치는 영향에 대해서는 더 많은 연구가 필요한 것으로 사료된다.

Diabetes, Overweight and Risk of Postmenopausal Breast Cancer: A Case-Control Study in Uruguay

  • Obese postmenopausal women increase their risk of developing breast cancer (BC), in particular if they display an android-type pattern of adiposity, which is also associated to increased risks of diabetes mellitus, hypertension and cardiovascular disease. In order to explore the associations among anthropometry (body mass index, body composition, somatotype), some specific items of medical history (diabetes, hypertension, dislypidemias, hyperuricemia) and the risk of BC in Uruguayan women, a case-control study was carried out between 2004-2009 at our Oncology Unit. 912 women of ages between 23-69 years (367 new BC cases and 545 non hospitalized, age-matched controls with a normal mammography) were interviewed. Twenty body measurements were taken in order to calculate body composition and somatotype. Patients were queried on socio-demographics, reproductive history, family history of cancer, a brief food frequency questionnaire and on personal history of diabetes, dislypidemias, hyperuricemia, hypertension and gallbladder stones. Uni- and multivariate analyses were done, generating odds ratios (ORs) as an expression of relative risks. A personal history of diabetes was positively associated to BC risk (OR=1.64, 95% CI 1.00-2.69), being higher among postmenopausal women (OR=1.92, 95% CI 1.04-3.52). The risks of BC for diabetes in postmenopausal women with overweight combined with dislypidemia (OR=9.33, 95% CI 2.10-41.5) and high fat/muscle ratio (OR=7.81, 95% CI 2.01-30.3) were significantly high. As a conclusion, a personal history of diabetes and overweight was strongly associated to BC. The studied sample had a subset of high-risk of BC featured by postmenopausal overweight and diabetic women, who also had a personal history of hypertension and/or dyslipidemia. The present results could contribute to define new high risk groups and individuals for primary as well as for secondary prevention, since this pattern linked to the metabolic syndrome is usually not considered for BC prevention.

학령기 아동을 위한 신체활동-행동수정 비만관리 프로그램이 대사증후군 위험인자에 미치는 효과 분석 (The Effects of a Physical Activity-Behavior Modification Combined Intervention(PABM-intervention) on Metabolic Risk Factors in Overweight and Obese Elementary School Children)

  • Purpose: The purpose of this study was to identify the effects of a physical activity-behavior modification combined intervention(PABM-intervention) on metabolic risk factors in overweight and obese elementary school children. Methods: Thirty-two participants ($BMI\geq85$ percentile or relative $obesity\geq10$) were allocated to the PABM-intervention group and behavior modification only intervention group. The PABM -intervention was composed of exercise intervention consisting of 50 minutes of physical activity(Hip-hop dance & gym-based exercises) twice a week and the behavior modification intervention consisted of 50 minutes of instruction for modifying lifestyle habits(diet & exercise) once a week. Effectiveness of intervention was based on waist circumference, BP, HDL-cholesterol, TG, and fasting glucose before and after the intervention. Results: The proportion of subjects with 1, 2, 3 or more metabolic risk factors were 28.1, 43.8, and 15.6%, respectively. After the 8-week intervention, waist circumference, systolic BP, diastolic BP, and HDL-cholesterol changed significantly(p<.01) in the PABM group. Conclusion: This provides evidence that a PABM-intervention is effective in changing metabolic risk factors such as waist circumference, systolic BP, diastolic BP, and HDL-cholesterol in overweight and obese elementary school children.

정상체중 대사비만 (Metabolically Obese Normal Weight (MONW))

  • Metabolically obese but normal weight(MONW) syndrome is characterized, with potentially increased risks for development of the insulin resistance or metabolic syndrome despite their normal body mass index(BMI) < 25 kg/m2. Such characteristics could confer upon MONW individuals a type 2 diabetes mellitus and cardiovascular diseases(CVD) risk however, research on MONW is scarce. MONW individuals have metabolic disturbances typical of obese persons and are identified by having a high amount of visceral fat, a low BMI, a high fat mass, a low lean body mass, low insulin sensitivity, and high triglyceride concentrations. The purpose of this study is to review several markers as potential modulators in individuals displaying the "MONW". Body fat appears to be functionally comparable with a dynamic endocrine organ, producing and secreting various adipocy tokines, such as leptin, adiponectin, CRP, tumor necrosis factor(TNF-), interleukin(IL)-6, all of which play an important role in the onset of cardiovascular disease, and insulin resistance. Otherwise, physical activity and a lower inflammation state might be helped to reduce the number of persons at risk of diabetes, CVD complications, or premature mortality. We should provide a method to optimal treatments resolving the emerging public health problem to prevention of MONW by providing guideline for physical activity as an optimal treatment for the MONW Korean. Furthermore we expect to develop a new strategy to manage MONW Korean in this society in terms of reducing medical costs and enhancing public health care for uprising population with MONW.

Comparison of Echocardiogram and Clinical Profile between Metabolically Healthy Obese (MHO) and Non Metabolically Healthy Obese (Non-MHO) Subjects

  • Obesity is associated with cardiovascular risk factors, such as dyslipidemia, hypertension and diabetes. However the presence of the obesity related deranged metabolic profiles varies widely among obese individuals. These individuals, known as 'metabolically healthy obese phenotype (MHO)', despite having excessive body fatness, display favorable metabolic profiles characterized by insulin sensitivity, no hypertension, as well as less dyslipidemia, less inflammation. The purpose of this study was to compare cardiac characterization and clinical profile of MHO and Non-MHO (nonmetabolically healthy obese) subjects in men. We measured treadmill exercise capacity (METs) and maximum blood pressure (BP) in 210 subjects through a medical checkup at J General Hospital. Metabolic syndrome was defined according to the modified Adult Treatment Panel III definition criteria. Both MHO and Non-MHO subjects showed statistically significant changes in the left ventricular mass index (P<.001, P<.01, respectively), A-velocity (P<.01, P<.001, respectively), E/A ratio (P<.01, P<.001, respectively), E'-velocity (P<.001, P<.001, respectively), HOMA-IR (P<.01, P<.001, respectively) and maximum systolic BP (P<.01, respectively) compared with the MH-NO (metabolically healthy non obese) subjects. In conclusion, MHO participants were at increased risk of cardiovascular disease and partly metabolic disorder.

  • Obesity increases the risk of developing metabolic diseases such as hypertension, type 2 diabetes, hyperlipidemia, and cardiovascular diseases, as well as some cancers. To prevent the occurrence of these diseases and death, it is essential to manage obesity. Though there are several treatments for obesity, lifestyle interventions, such as diet and exercise, and drug therapy are most widely used in clinical practice. Among the anti-obesity drugs available, the weight loss effect of naltrexone/bupropion has been well-proven. We present a case study in which naltrexone/bupropion, a glucagon-like peptide-1 agonist, and a sodium-glucose transporter 2 inhibitor showed significant weight loss and improved metabolic parameters. Additionally, the management of type 2 diabetes and hypertension, which are common diseases in patients with obesity, was also included.

일개 대학병원 건강검진 수진자의 대사증후군 호전과 관련요인 (Metabolic Syndromes Improvement and Its Related Factors among Health Checkup Examinees in a University Hospital)

  • 본 연구는 대사증후군 환자의 건강관리를 위한 기초자료를 제공하고자 대사증후군 환자의 호전과 그 관련된 요인을 파악하였다. 2013년 1월부터 2014년 12월까지 대학병원 종합검진센터에서 2년 연속 검진 받은 만 20세 이상 성인 중 2013년 검진에서 대사증후군 진단을 받은 280명을 대상으로 하였다. 자료 분석은 SPSS WIN18.0을 이용하여 평균과 표준편차, t-검정, ${\chi}^2$ 검정, 로지스틱 회귀분석을 하였다. 연구결과 호전군의 대사증후군 구성요소는 기준년도(2013년)의 3개에서, 추적년도(2014년)에 2개로 감소된 비율이 60.6%로 가장 높았다. 호전군은 기준년도에 비해 추적년도에 허리둘레, 수축기 혈압, 중성지방의 감소 및 고밀도 지단백 콜레스테롤 증가가 유의한 차이를 보였다. 대사증후군 호전군과 유지군 간에는 음주, 운동, 열량, 지방, 당질 섭취 및 지질저하제 복용에서 유의한 차이를 보였다. 로지스틱 회귀분석에서 대사증후군 호전에는 운동, 열량섭취, 당뇨병 복용 유지가 유의한 영향력을 보였다. 본 연구의 결과 대사증후군 관리를 위해서는 운동증가, 식생활개선을 위한 중재프로그램 제공이 필요하며, 이를 지속적으로 관리하기 위한 보건교육 강화가 요구되었다.

남성 건강검진 수진자들에서 알코올성 및 비알코올성 지방간과 대사증후군과의 관련성 (Association of Alcoholic & Nonalcoholic Fatty Liver Disease with Metabolic Syndrome in Health Screen Examinees of Men)

  • 최근 성인 남성에서 비만 인구가 늘어나며, 심혈관질환을 야기하는 대사증후군 유병률이 증가하고 있고, 증상이 없지만 지방간으로 진단되는 환자가 늘고 있다. 본 연구는 알코올성 또는 비알코올성 지방간 질환과 대사증후군과의 관련성이 있는지 알아보고자 하였다. 799명의 남성을 대상으로 평소 음주량을 조사하였고, 초음파 검사를 통해 지방간 유무를 확인 한 결과 297명은 정상군이었고, 206명은 알코올성 지방간, 296명은 비알코올성 지방간이었다. IDF(international diabetes federation) 기준을 적용한 대사증후군과 구성요소들을 독립변수로, AFLD(alcoholic fatty liver disease) 및 NAFLD(nonalcoholic fatty liver disease)을 종속변수로 하여 로지스틱 회귀분석을 실시하였다. 정상군과 AFLD 또는 NAFLD에서 대사증후군의 분포는 각각 7.4%, 48.8%, 34.9%이었고, 대사증후군 구성요소 중 고혈압, 고혈당, 고중성지방, 비만은 AFLD 또는 NAFLD의 위험인자였고, AFLD에선 고중성지방과 복부비만이, NAFLD에서는 복부비만이 영향력이 가장 큰 것으로 나타났다. 또한 AFLD군의 경우 대사증후군과 비만, 저HDL-콜레스테롤의 이상범위에서 지방간 중증의 교차비가 높았고, NAFLD군에서는 대사증후군과 고중성지방군의 이상범위에서 지방간 중증의 교차비가 높았다. 결론적으로 AFLD 및 NAFLD는 대사증후군 구성요소 각각의 기준과 밀접한 관련성이 있었고, 지방간 관리를 위해 꾸준하고 포괄적인 접근이 필요하다.

Low HDL cholesterol is associated with increased atherogenic lipoproteins and insulin resistance in women classified with metabolic syndrome

  • Both metabolic syndrome (MetS) and elevated LDL cholesterol (LDL-C) increase the risk for cardiovascular disease (CVD). We hypothesized that low HDL cholesterol (HDL-C) would further increase CVD risk in women having both conditions. To assess this, we recruited 89 women with MetS (25-72 y) and LDL-C ${\geq}$ 2.6 mmol/L. To determine whether plasma HDL-C concentrations were associated with dietary components, circulating atherogenic particles, and other risk factors for CVD, we divided the subjects into two groups: high HDL-C (H-HDL) (${\geq}$ 1.3 mmol/L, n=32) and low HDL-C (L-HDL) (< 1.3 mmol/L, n=57). Plasma lipids, insulin, adiponectin, apolipoproteins, oxidized LDL, Lipoprotein(a), and lipoprotein size and subfractions were measured, and 3-d dietary records were used to assess macronutrient intake. Women with L-HDL had higher sugar intake and glycemic load (P< 0.05), higher plasma insulin (P< 0.01), lower adiponectin (P< 0.05), and higher numbers of atherogenic lipoproteins such as large VLDL (P < 0.01) and small LDL (P<0.001) than the H-HDL group. Women with L-HDL also had larger VLDL and both smaller LDL and HDL particle diameters (P<0.001). HDL-C was positively correlated with LDL size (r=0.691, P<0.0001) and HDL size (r=0.606, P<0.001), and inversely correlated with VLDL size (r=-0.327, P<0.01). We concluded that L-HDL could be used as a marker for increased numbers of circulating atherogenic lipoproteins as well as increased insulin resistance in women who are already at risk for CVD.

대학병원 종합검진센터 수진자의 대사증후군 발생과 관련요인 (Incidence and Related Factors of the Metabolic Syndrome in a University Hospital)

  • Objectives: The purpose of this research was to investigate incidence and related factors of the MS in a university hospital Methods: This survey was conducted from January in 1997 to March in 2008. This study is a cohort study. The baseline subjects were 529 men and 609 women without metabolic syndrome(MS) who visited the health promotion center of Chungnam National University Hospital for general health check-up. The MS was diagnosed according to the definition by the NCEP ATP III. The abdominal obesity guidelines for waist circumference applied the Korean Society for the Study of Obesity Guideline Results: The survey exhibited that baseline risk factors that mainly affect odds ratio(OR) of the MS at follow-up were sex, ages, heavy drinking, uric acid and transpeptidase(GGT) in women. Incidence of the MS was 10.6% in total cases, 18.1% in men and 4.0% in women. The risk of the MS was significantly lower in women than men(OR=0.4, 95% confidence interval(CI: 0.2-0.9), significantly higher 50-59 Ages than < 40 ages(OR=3.4, 95% CI: 1.9-6.4), significantly high in women with heavy drinkers(OR=14.3, 95% CI: 1.2-177.5), significantly high in whole, men and women with high uric acid group(whole, OR=2.6, 95% CI: 1.6-4.2. men, OR=2.1, 95% CI: 1.3-3.5. women, OR=14.2, 95% CI: 3.0-68.3) and significantly high in women with high GGT(OR=1.8, 95% CI: 2.4-12.6). Conclusion: Risk factors of incident MS were baseline sex, ages, heavy drinking in women, uric acid and GGT in women. These can be utilized as an important index to determine risk factors of incident MS and will be basic data in part of management, education, countermeasure and selection test of the MS.
