Body Adiposity Index and Metabolic Syndrome Risk Factors in Korean Adults: A Comparison with Body Mass Index and Other Parameters

  • A new body adiposity index (BAI) has been proposed that is expected to replace body mass index (BMI). We evaluated the correlations between metabolic syndrome risk factors and BAI, BMI, and other adiposity indices, such as waist circumference (WC), waist-to-hip ratio (WHR), and waist-to-height ratio (WHtR), by sex in the Korean population. We also evaluated whether BAI would be useful to diagnose metabolic syndrome. A total of 20,961 Korean adults who underwent health examinations were included in this study. The metabolic syndrome diagnostic criteria used in this study were those set by the American Heart Association/National Heart, Lung, and Blood Institute (AHA/NHLBI). In men (12,719), BMI and WHtR were more strongly correlated to metabolic syndrome risk than BAI, and in women (8,242), WHtR showed the strongest association with metabolic syndrome risk. BAI (area under the curve [AUC] = 0.678) presented lower discriminatory capacity than that of BMI (AUC = 0.836) for diagnosing metabolic syndrome. Moreover, BAI underestimated fat levels in men and women when considering the ability to discriminate overweight and obese individuals. In conclusion, WHtR and BMI in men, and WHtR in women may be better candidates than BAI to evaluate metabolic risk factors in Korean adults.

  • Background: Metabolic syndrome has been known as a risk of cardiovascular disease. Meanwhile, high sensitivity C-reactive protein (hs-CRP) is used as a predictor of cardiovascular disease. In this paper, we aimed to investigate the association between hs-CRP and metabolic syndrome. Method: A total of 7,633 were chosen as the study population from the 7th Korea National Health and Nutrition Examination Survey dataset (2016-2017). Our dependent variable was whether an individual had metabolic syndrome or not, and the independent variable of interest was hs-CRP which was categorized into three groups. The chi-square tests and hierarchical logistic regression analyses reflecting survey characteristics were conducted. All analyses were stratified by gender. Results: According to the adjusted model with all covariates, compared to individuals having the low risk of hs-CRP, those having its average risk were more likely to have metabolic syndrome in men (odds ratio [OR], 1.41; 95% confidence interval [CI], 1.12-1.76) and women (OR, 1.69; 95% CI, 1.33-2.16). Individuals having the high risk was not significantly different in men; however, they were more likely to have metabolic syndrome in women (OR, 2.03; 95% CI, 1.28-3.23). Conclusion: In an upcoming aging society, it is important to reduce the risk of metabolic syndrome to improve population health. This study suggests that hs-CRP may be used as a marker of the risk of metabolic syndrome in a gender-specific way, thereby contributing to enhancing awareness of the risk of metabolic syndrome among the general public.

Association between Metabolic Syndrome and Physical Fitness in Postmenopausal Women

  • The research is performed in order to know the relationship between the metabolic syndrome and the physical fitness targeted for menopausal women (over 45 years). All subjects were divided into 4 groups; group without risk factors of metabolic syndrome (MS-0: n=74), group having one risk factor of metabolic syndrome (MS-1: n=68), group having two risk factors of metabolic syndrome (MS-2: n=44), and group having more than three risk factors of metabolic syndrome (MS ${\geq}$ 3: n=30). All groups' height, weight, body mass index and percent of body fat were measured. High density lipoprotein cholesterol (HDL), triglyceride, glucose and blood pressure (BP) levels were measured. Their cardiorespiratory ($VO_2max$) endurance, muscular strength, muscle endurance, and flexibility were measured. HDL, triglyceride, glucose and BP levels in MS-1, MS-2, and MS ${\geq}$ 3 group were significantly greater than those of MS-0 group. The endurance ($VO_2max$) in MS ${\geq}$ 3 group was higher than that of MS-0 group. Multiple regression with the risk factors of metabolic syndrome and the physical fitness showed a statistical significance in only $VO_2max$. We found that the risk factors of the metabolic syndrome adversely affect postmenopausal women's $VO_2max$ and that a decreased $VO_2max$ may have prognostic value for the prediction of metabolic syndrome.

여성 노인의 대사증후군과 골밀도의 관련성 조사연구 (Metabolic Syndrome and Bone Mineral Density among Elderly Korean Women)

  • Purpose: Although the risk factors of metabolic syndrome have been extensively studied, the association between osteoporosis and metabolic syndrome has remained unclear in Korean elderly women. Yet to be determined are the effect of risk factors of metabolic syndrome on osteoporosis in these subjects. The purpose of this study was to investigate how the risk factors of metabolic syndrome affect the bone mineral density in Korean elderly women. Methods: One hundred twenty one elderly women from a community center in Seoul elderly welfare center participated in this study. A structured questionnare was used to assess their demographics and lifestyles. Participants' anthropometric information was also obtained by measuring heights, weights, and waist circumferences. The blood samples were also obtained to measure blood glucoses and blood lipids. Bone mineral density was measured with the use of ultra sono. Results: The prevalence of metabolic syndrome in our subjects was 58%. In multivariate regression analysis, fasting plasma glucose level (p=.036) and triglycerides (p=.006) were significant factors predicting bone mineral density after adjusting age and other factors of metabolic syndrome. In age-adjusted analysis, women with metabolic syndrome had significantly higher bone mineral density as compared to those without metabolic syndrome (p=.026). Conclusion: Bone mineral density among elderly Korean women is associated with the level of blood glucose and triglycerides.

한국 성인의 식생활평가지수에 기반한 전체 식사의 질과 대사증후군 구성요소 및 대사증후군 발생의 연관성 (Relation between the Total Diet Quality based on Korean Healthy Eating Index and the Incidence of Metabolic Syndrome Constituents and Metabolic Syndrome among a Prospective Cohort of Korean Adults)

  • Objectives: This study examined the association of the total diet quality with the incidence risk of metabolic syndrome constituents and metabolic syndrome among Korean adults. Methods: Based on a community-based cohort of the Korean Genome and Epidemiology Study (KoGES) from 2001 to 2014, data from a total of 5,549 subjects (2,805 men & 2,744 women) aged 40~69 years at the baseline with a total follow-up period of 38,166 person-years were analyzed. The criteria of the National Cholesterol Education Program Adult Treatment Panel was employed to define metabolic syndrome. The total diet quality was estimated using the Korean Healthy Eating Index (KHEI). Hazard ratios (HR) and 95% confidence intervals (CI) for risk of metabolic syndrome constituents and metabolic syndrome in relation to KHEI quintile groups was calculated by multivariate Cox proportional hazards regression model. Results: After adjusting for age, energy intake, income, education, physical activity, smoking, and drinking, the incidence of abdominal obesity and high blood pressure was significantly lower, by approximately 29.7% (P < 0.01) and 25.2% (P < 0.01), respectively, in the fifth KHEI quintile compared to the first quintile in men. A significant decreasing trend of the metabolic syndrome incidence was observed across the improving levels of KHEI (HRq5vs.q1: 0.775, 95% CIq5vs.q1: 0.619~0.971, P for trend < 0.01). In women, the incidence of abdominal obesity and metabolic syndrome was significantly lower, by approximately 29.8% (P < 0.01) and 22.5% (P < 0.05), respectively, in the fifth KHEI quintile compared to the first quintile adjusting for multiple covariates. On the other hand, the linear trend of metabolic syndrome risk across the KHEI levels did not reach the significance level. Conclusions: A better diet quality can prevent future metabolic syndrome and its certain risk factors among Korean men and women.

청소년의 대사증후군 진단개수에 따른 영향요인 분석; 국민건강영양조사(2016) 자료 이용 (Influencing Factors for the Development of Metabolic Syndrome by the Number of Metabolic Syndrome Diagnostic Components in Korean Adolescents)

  • Objectives: The purpose of this study was to investigate the prevalence of metabolic syndrome and to find related factors according to the number of metabolic syndrome diagnostic components in Korean adolescents. Methods: The subjects of this study were 469 Korean adolescents aged from 12 to 18 enrolled in the 2016 Korea National Health and Nutrition Examination Survey. Statistical package R 3.4.2 was used for programming to apply diagnostic criterion of adolescent metabolic syndrome and for the analysis of the data such as weighted frequent analysis, weighted mean analysis and complex sampling design logistic regression analysis. Results: For adolescents 12 to 18 years of age, 2.55% had more than 2(${\geq_-}3$), 9.88% had more than 1(${\geq_-}2$) and 33.17% had more than 0(${\geq_-}1$) metabolic syndrome diagnostic components. It has been found that risk factors for no less than 2 metabolic syndrome diagnostic components were higher body mass index and higher stress, and risk factors for no less than 1 were higher body mass index, younger teenager and female. Conclusion: Obesity is the primary risk factor for the development of adolescent metabolic syndrome. Female or younger teenager are more likely to have one or more metabolic syndrome diagnostic components, and higher stress develop to the risk level of having two or more metabolic syndrome diagnostic components. Therefore, it is important to focus on obesity and stress management for the prevention and control of Korean adolescent metabolic syndrome.

사상체질에 따른 대사증후군의 유병률과 위험인자에 대한 연구 (A Study on the Prevalence and Risk Factors of the Metabolic Syndrome according to Sasang Constitution)

  • Objectives : The purpose of this study was to find the prevalences and risk factors of the metabolic syndrome according to Sasang constitution. Methods : The medical records of 364 subjects who had taken health examinations and diagnosis of Sasang constitution from January to June of 2003 at a health examination center of a hospital in Seoul were reviewed. The prevalences and the risk factors of the metabolic syndrome according to Sasang constitution were compared and analyzed. Results : 1) Among the 364 subjects, 88 (24.2%) were Soeumin, 101 (27.7%) were Soyangin, and 175 (48.1%) were Taeumin. 2) The prevalences of high WC, high TG, low HDL-c, high BP and high FBS of Taeumin were significantly higher than those of the other constitutions (p-value < 0.05). 3) The prevalence of the metabolic syndrome of Taeumin, Soyangin, and Soeumin were 46.3%, 16.8% and 9.1% respectively. The rates were significantly different according to Sasang constitution (p-value < 0.001). 4) Taeumin had higher risk of high WC than Soeumin (adjusted OR : 3.83, 95% CI : 1.19-12.29) and higher risk of high FBS than Soeumin (adjusted OR: 5.93,95% CI : 1.11-31.77). 5) Taeumin had higher risk of the metabolic syndrome than Soeumin (adjusted OR : 3.40, 95% CI : 1.25-9.23). Conclusions : There were significant differences in the prevalence of metabolic syndrome according to Sasang constitution. Sasang constitution was identified as an independent risk factor of metabolic syndrome.

Gender specific effect of major dietary patterns on the metabolic syndrome risk in Korean pre-pubertal children

  • There is a lack of data on metabolic risk factors during pre-puberty, which is important for identifying the subgroups of youth, at whom early interventions should be targeted. In this study, we evaluated the prevalence of metabolic risk factors and its subsequent relations with dietary patterns in Korean pre-pubertal children through a cross-sectional sample (n = 1,008; boys = 513) of pre-pubertal children (aged 8-9 years) from a sub-study of the Korea Metabolic Syndrome Research Initiatives (KMSRI) in Seoul, Korea. Measures of anthropometry and blood pressure as well as fasting blood samples were used in the analysis. A three-day food records were collected. The metabolic syndrome was defined according to the age-adjusted National Cholesterol Education Program Adult Treatment Panel III guidelines. An added metabolic risk score was calculated for each subject by summing the quintile values of the five individual risk factors. Among the 5 risk components of metabolic syndrome, high waist circumference (WC) was the major factor (P < 0.001). A significant increasing trend of the added metabolic syndrome risk score was observed with the increase of WC (P (trend) < 0.001) among both genders. The cutoff point for high WC for pre-pubertal children was 61.3 cm for boys and 59.9 cm for girls. The prevalence of high triglyceride (TG) values was significantly higher in girls than it was in boys (P < 0.01). Girls in the highest quintile of balanced dietary pattern scores had lower TG values (P (trend) = 0.032) than did those in the lowest quintile. Moreover, girls in the highest quintile of western dietary pattern scores showed increasing trend for the added metabolic risk score (P (trend) = 0.026) compared with those in the lowest quintile. Adverse associations exist between western dietary patterns and the accumulation of metabolic risks among girls, not in boys, even during pre-puberty.

교대근무와 수면시간이 우리나라 임금근로자의 대사증후군에 미치는 영향 (The Effects of Shift Work and Hours of Sleep on Metabolic Syndrome in Korean Workers)

  • Purpose: The purpose of this study was to investigate the effects of shift work and hours of sleep on metabolic syndrome in Korean workers. Methods: This study used the Korean National Health and Nutrition Examination Survey data of 2014. The study included 1,579 Korean workers aged over 20. $x^2$ test, t-test, and hierarchical multiple logistic regression were used for statistical analysis of data. Results: From the univariate analysis, hours of sleep, regular work, occupational group, sex, age, marital status, education, smoking, and self-rated health were significantly related to metabolic syndrome. After adjusting demographic, occupational, and health-related variables, workers with under 6 hours/day of sleep showed higher risk for metabolic syndrome (AOR: 1.56, 95% CI: 1.01~2.44), and shift work was not significantly related to metabolic syndrome. Male laborers and workers aged 40 or older also showed higher risk for metabolic syndrome. Conclusion: Results of this study suggest that workers with under 6 hours of sleep, male laborers and workers aged 40 or older are the risk groups of metabolic syndrome. Therefore, prevention and management program for metabolic syndrome should be implemented for this population.

  • Purpose: The purpose of this study was to identify factors affecting health promotion behavior among workers with high risk of metabolic syndrome. This study was based on the planned behavior theory. Methods: The participants were 167 workers at high risk of metabolic syndrome. Data were collected using a structured questionnaire. Surveyed variables were attitude, subjective norm, perceived behavioral control, intention, and health promotion behavior. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficients, and hierarchical regression analysis with SPSS/WIN 22.0. Results: Perceived behavioral control affected the intention of health promotion behavior among the workers with high risk of metabolic syndrome. It explained 62% of variance in the intention of health promotion behavior (F=40.09, p<.001). Perceived behavioral control and occupation affected health promotion behavior among the risk workers with high risk of metabolic syndrome. The two factors explained 16% of variance in health promotion behavior (F=4.95, p<.001). Conclusion: The findings of this study suggest that perceived behavioral control is the only factor affecting health promotion behavior when the theory of planned behavior was applied. Therefore, intervention programs for improving health promotion behavior should be focused on strengthening perceived behavioral control.
