We need to understand the outcomes into adulthood for survivors born either extremely low birthweight (ELBW; <1,000 g) or extremely preterm (EP; <28 weeks' gestational age), particularly their blood pressure and cardiovascular metabolic status,respiratory function, growth, psychological and mental health performance, and functional outcomes. Blood pressure is higher in late adolescence and early adulthood in ELBW/EP survivors compared with controls. In some studies, expreterm survivors have higher insulin and blood lipid concentrations than controls, which may also increase their risk for later cardiovascular disease. ELBW/EP survivors have more expiratory airflow obstruction than do controls. Those who had bronchopulmonary dysplasia (BPD) in the newborn period have even worse lung function than those who did not have BPD. As a group, they are unlikely to achieve their full lung growth potential, which means that more of them are likely to develop chronic obstructive airway disease in later life. Although they are smaller than term born controls, their weight gradually rises and ultimately reaches a mean z-score close to zero in late adolescence, and they ultimately attain a height z-score close to their mid-parental height z-score. On average, ex-preterm survivors have intelligence quotient (IQ) scores and performance on tests of academic achievement approximately 2/3 SD lower than do controls, and they also perform less well on tests of attention and executive function. They have similar high rates of anxiety and depression symptoms in late adolescence as do controls. They are, however, over-represented in population registries for rarer disorders such as schizophrenia and Autism Spectrum Disorder. In cohort studies, ex-preterm survivors mostly report good quality of life and participation in daily activities, and they report good levels of self-esteem. In population studies, they require higher levels of economic assistance, such as disability pensions, they do not achieve education levels as high as controls, fewer are married, and their rates of reproduction are lower, at least in early adulthood. Survivors born ELBW/EP will present more and more to health carers in adulthood, as they survive in larger numbers.
Objectives: This review aimed to investigate the effectiveness of Chai-Hu-Shu-Gan-San (CHSGS) for treatment of insomnia. Methods: 9 databases were comprehensively searched from their inception dates until Jan 2020 using keywords like Insomnia, Chai-Hu-Shu-Gan-San and Sihosogan-san. We included randomized controlled trials (RCTs) using CHSGS as the main intervention for insomnia patients. We assessed the quality of RCTs using the Cochrane risk of bias tool and Jadad scale. Results: 5 RCTs were included. The Chinese classification of mental disorders -3 (CCMD-3) was the most frequently used diagnostic criteria. The effective rate was the most frequently used as outcome. All the 5 RCTs that reported CHSGS and CHSGS adjunctive therapy on insomnia were more effective compared to western medication. Only one study reported adverse effects. The methodological quality of the included RCTs were generally poor. Conclusions: This review found that CHSGS may be effective to treating insomnia. However, because of the small number, heterogeneity and very low methodological quality of included studies, we could not draw any definitive conclusions. Therefore, more rigorous randomized control studies are warranted.
Objectives: Although increased reactive oxygen species (ROS) is caused by stress accelerates collagen degradation, there was no data on the relationship between stress and urinary hydroxyproline (Hyp) and proline (Pro), a good marker of collagen degradation. The purpose of this study was to evaluate the relationship between depression, anxiety, and stress (DAS) and concentrations of urinary Hyp and Pro. Methods: 97 hospital employees aged 20 to 58 were asked to fill out comprehensive self-administrated questionnaires containing information about their medical history, lifestyle, length of the work year, shit-work and DAS. Depression Anxiety Stress Scale (DASS) was applied to evaluate chronic mental disorders. Urine samples were analyzed using High Performance Liquid Chromatography (HPLC) with double derivatization for the assay of hydroxyproline and proline. Results: The mean value of Hyp and Pro concenturation in all subjects was $194.1{\pm}113.4\;{\mu}mol/g$ and $568.2{\pm}310.7\;{\mu}mol/g$. DASS values and urinary Pro concentrations were differentiated by sex (female > male, p < 0.05) and type of job (nurse > others, p < 0.05). In the stepwise multiple linear regressions, urinary Hyp and Pro concentrations were influenced by stress (Adjusted $r^2$ = 0.051) and anxiety and job (Adjusted $r^2$ = 0.199), respectively. Conclusions: We found that stress and anxiety were correlated with urinary Hyp and Pro concentrations. To identifying a definite correlation, further study in large populations will be needed.

Studies have shown that both the psychomotor stimulant effects and rewarding properties of addictive drugs, including morphine, are sensitized by repeated drug administration and it is suggested that both of these effects are mediated by the same or closely overlapping dopamine systems. Specifically, the mesolimbic dopamine system has been implicated in the reinforcing and sensitizing properties of morphine. In oriental medicine, Shenmen (HT7) point on the heart channel has been used to treat mental and psychosomatic disorders. This study was designed to investigate the effect of acupuncture on acute and repeated morphine-induced changes in extracellular dopamine levels using in vivo microdialysis and morphine-induced behavioral changes. In the morphine sensitization experiment, male Sprague-Dawley rats were treated twice a day for three days with increasing doses of morphine (10, 20 and 40 mg/kg, s.c.) or with saline. After 15 days of withdrawal, rats were challenged with morphine hydrochloride (5mg/kg, s.c.). Acupuncture was applied at bilateral Shenmen (HT7) points for 1 min after the morphine challenge. In the acute experiment, rats also received acupuncture for 1 min after an injection of morphine hydrochloride (5 mg/kg, s.c.). Results showed that acupuncture at the specific acupoint HT7, but not at control points (tail) significantly decreased both dopamine release and behavior induced by a systemic morphine challenge or a single sc morphine injection in the acute animals. These results suggest that reduction in sensitization may be one mechanism whereby acupuncture alleviates morphine craving in addicts.
Objectives : Poststroke depression is a frequent and specific entity that impaires the rehabilliation and functional recovery of patients with hemiplegia. The author evaluated the effect of Banhahubak-tang(Banxiahoupotang) in patients with poststroke depression. Methods : 38 patients suffering from poststroke depression(determined by Diagnostic and Statistical Manual of Mental Disorders, revised. 3rd edition. and Beck Depression Inventory[BDI] cutoff $point{\geqq}10$) in Kyunghee Oriental hospital were randomized into two groups; treatment group(n=19) and control group(n=19). The treatment group was prescribed with Banhahubak-tang(Banxiahoupotang) three times a day fur a week. Control troop was prescribed with other herbal medicines used for stroke Patients three times a day for a week. Patients were evaluated by use of BDI scale, Modified Barthel Index, Depression of Ki score, Yin syndrome score, and Yang syndrome score. Among 38 patients, 24 patients got BDI scores above 21, which is the cut-off score for depression in Korean. The same procedures and assessments described above were applied. Results : Treatment group did not significantly improve compared with control group. Results yielded only slight significance (P=0.086). Especially. patients with poststroke depression as yin syndrome improved more significantly on BDI than those classified as yang syndrome. When BDI cutoff point for depression was defined as being ${\geq}\;21$, treatment group did not significantly improve compared with control group(P=0.114). However, patients with poststroke depression classified as yin syndrome were also significantly improved on BDI than those classified as yang syndrome. Conclusions : This study suggests that Banhahubak-tang(Banxiahoupotang) is significantly effective in patients with poststroke depression classified as yin syndrome.
Social security is concerned with ensuring all citizens maintain basic needs, community health nursing maintains and promotes health for all community members. Lately, This new area of community health nursing, concern social welfare has increased. The objectives of this study are, first, to analysis the activities of nurses at community social welfare institutes, second, to estimate nursing demand for social welfare areas. The study methods used were as literature review, an analysis of statistical data and case study etc. The analytical framework also included a demand analysis of nursing manpower in community social welfare areas. The major results are as follows; 1. Employees which work at social welfare institutes number 55,464, nursing manpower (including nurse aids) number 1,458 and this is 3% of the total employees. Within nursing manpower, nurses number 780, nurse aids number 670. 2. The rates of nurses among total employees were high in institutes for mental disorders and institutes for the age. 3. The salary level of nurses was lower than average and the rates of retirement showed a roughly middle level in welfare institutes. 4. The satisfaction level of nursing services was high, and it is the trend that nurses substitute for nurse aids which retire at social welfare institutes. 5. Nurse demand that follows legal criteria is 2,221, but only 35% are working. It is therefore insufficient from the minimum of 733 to a maximum 1433. 6. The sufficiency rates by institution were high at institutes for vagrants, aged and the handicapped. In conclusion, the conditions are of nurses which are working in with the social welfare institutes are poor. Also the number of nurses compared rates of demand were in surplus. But, the basic direction of welfare policy is universal-preventive and provision of the family and of community centered service, and nursing service demand in the social welfare institute will increase continuously, we predict. Therefore, we will need a positive plan such as the development of an inservice education program and the construction of an information collection system etc.
Air pollution is a severe risk factor in public health. It is not only a cause of a variety of physical disorders but also attributable to mental health problems. This study aimed to investigate the differences on emotions, physical symptoms, future perspectives by perceived air pollution. Sixty-one university students were randomly allocated either to air pollution group (n = 30) or clean air group (n = 31). Participants watched a news video clip of a polluted city or a clean city according to their condition and were asked to imagine as if they were living in the city. Results showed that air pollution group reported increased depression, anxiety, anger and physical symptoms after the exposure. Meanwhile, clean air group showed no significant changes or decrease of them. In addition, air pollution group reported restrained future time perspectives and decreased plan of outdoor activities, while clean air group did not report significant changes after the exposure.

Objectives The purpose of this study was to investigate the pattern characteristics of symptoms for ADHD, the relationship between pattern identifications in TKM and TCM and three presentations of ADHD defined in the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5). Methods The pattern characteristics of symptoms for ADHD from the 13 Korean and Chinese literatures were analyzed. ADHD symptoms of each pattern were classified into 3 categories; symptoms of inattention, hyperactivity-impulsivity and others. The characteristics of patterns for ADHD in Traditional Medicine were compared with three presentations of ADHD defined in DSM-5. Results 1. The pattern of dual deficiency of the heart and spleen (心脾兩虛) showed predominant symptoms of inattention and rarely showed those of severe hyperactivity and impulsivity. This characteristic of ADHD can be related to the predominantly inattentive presentation in DSM-5. 2. The pattern of phlegm-fire harassing the heart (痰火擾心), and the pattern of deficiency of liver-yin and kidney-yin (肝腎陰虛), frequently showed symptoms of hyperactivity-impulsivity compared with those of inattention. This characteristic of ADHD can be related to the predominantly hyperactive-impulsive presentation in DSM-5. 3. The pattern of spleen weakness and liver energy preponderance (脾虛肝旺) frequently showed both symptoms of hyperactivity, and this characteristic of ADHD can be related to the combined presentation in DSM-5. Conclusions This study showed the relationship between three presentations of ADHD defined in DSM-5 and the pattern identifications of ADHD in TKM and TCM.
Objectives The current study investigated the putative relationship between chronotype and suicidality or bipolarity in patients with major depressive disorder (MDD). Method Nineteen outpatients who met the criteria for MDD according to the fourth edition of the Diagnostic and Statistical Manual of Mental Disorders-text revision were recruited for the current study. The subjects were divided into two subgroups based on their Basic Language Morningness (BALM) scores (dichotomized according to the median BALM score). The Loudness Dependence of Auditory Evoked Potentials (LDAEP) was evaluated by measuring the auditory event-related potentials before beginning medication with serotonergic agents. In addition, K-Mood Disorder Questionaire (K-MDQ), Beck Scale for Suicidal Ideation (BSS), Beck Hopelessness Scale (BHS), Barratt Impulsiveness Scale (BIS) were applied. Results The K-MDQ, BSS, BHS, BIS score was higher for the eveningness group than for the morningness group. However, the LDAEP, Hamilton Depression Rating Scale, Hamilton Anxiety Scale scores did not differ significantly between them. There were negative correlations between the total BALM score and the total K-MDQ, BSS, and BHS scores (r=-0.64 and p=0.0033, r=-0.61 and p=0.0055, and r=-0.72 and p=0.00056, respectively). Conclusions Depressed patients with eveningness are more vulnerable to the suicidality than those with morningness. Eveningness is also associated with bipolarity.

We have previously demonstrated that repeated injections of nicotine produced an increase in locomotor activity, dopamine(DA), release and c-Fos expression in the nucleus accumbens, one of the major projection areas of the central DA system. Acupuncture as a therapeutic intervention is widely used for the treatment of many functional disorders such as substance abuse and mental dysfunction. And many studies have shown that Coptidis Rhizoma has a suppressive effect on the central nervous system (CNS) and can affect the neurotransmitter systems in the CNS. In order to investigate whether acupuncture and Coptidis Rhizoma have an influence on nicotine-induced reinforcing and behavioral effects, we examined the effect of zusanli(ST36) and Coptidis Rhizoma on repeated nicotine-induced locomotor activity, and zusanli(ST36) on c-Fos expression as an important maker of postsynaptic neuronal activity in nucleus accumbens. Male SD rats received Coptidis Rhizoma (100mg/kg, p.o.) 30 min before injections of nicotine (0.4 mg/kg, s.c.) for 7 days. Rats were followed withdrawal for 3 days and one challenge for 1 day. Systemic challenge with nicotine produced a much larger increase in locomotor activity. Pretreatment with acupuncture at zusanli(ST36, 100Hz) and Coptidis Rhizoma decreased in nicotine-induced locomotor activity. These results demonstrated that reduction in locomotor activity by acupuncture at zusanli(ST36, 100Hz) and Coptidis Rhizoma may be mediated by reduction of dopamine release. Our results suggest that acupuncture at zusanli(ST36, 100Hz) and Coptidis Rhizoma may have therapeutic effect on nicotine addiction.
