Association Assessment among Risk Factors and Breast Cancer in a Low Income Country: Bangladesh

  • Background: In the low incoming country Bangladesh, breast cancer is second most common neoplasm and is increasing at an alarming rate among females. Lack of awareness and illiteracy are contributory factors for late presentation and therefore mortality. Purpose: To examine associations of different factors with breast cancer mortality and to raise awareness among the women of society in Bangladesh. Materials and Methods: This descriptive case-control study was conducted on 160 participants from April 2011 till July 2014. Through a valid questionnaire covering personal and family history, data were collected by face to face interview. For analyzing correlations among factors with breast cancer data, binary logistic regression, Pearson's ${\chi}^2$-value, odd ratios and p-value tests were conducted with SPSS version 20. Results: The mean age of the patients was 43.0 ($SD={\pm}11.12$). In ascending order the leading significant factors were hormone therapy (p<0.0000, OR=4.897), abortion (p<0.0001, OR=3.452), early start menarche (p<0.0002, OR=3.500), family history (p<0.0022, OR=3.235), and late menopause (p<0.0093, OR=3.674) with both ${\chi}^2$ test and logistic regression analyses. Non-significant factors were cancer experience, fatty food habits, marital status and taking alcohol. Conclusions: Regarding the investigation of this study, significant and insignificant factor's correlation visualization with breast cancer will be helpful to increase awareness among Bangladeshi women as well as all over the world.

  • Pubertal onset is known to result from reactivation of the hypothalamic-pituitary-gonadal (HPG) axis, which is controlled by complex interactions of genetic and nongenetic factors. Most cases of precocious puberty (PP) are diagnosed as central PP (CPP), defined as premature activation of the HPG axis. The cause of CPP in most girls is not identifiable and, thus, referred to as idiopathic CPP (ICPP), whereas boys are more likely to have an organic lesion in the brain. ICPP has a genetic background, as supported by studies showing that maternal age at menarche is associated with pubertal timing in their offspring. A gain of expression in the kisspeptin gene (KISS1), gain-of-function mutation in the kisspeptin receptor gene (KISS1R), loss-of-function mutation in makorin ring finger protein 3 (MKRN3), and loss-of-function mutations in the delta-like homolog 1 gene (DLK1) have been associated with ICPP. Other genes, such as gamma-aminobutyric acid receptor subunit alpha-1 (GABRA1), lin-28 homolog B (LIN28B), neuropeptide Y (NPYR), tachykinin 3 (TAC3), and tachykinin receptor 3 (TACR3), have been implicated in the progression of ICPP, although their relationships require elucidation. Environmental and socioeconomic factors may also be correlated with ICPP. In the progression of CPP, epigenetic factors such as DNA methylation, histone posttranslational modifications, and non-coding ribonucleic acids may mediate the relationship between genetic and environmental factors. CPP is correlated with short- and long-term adverse health outcomes, which forms the rationale for research focusing on understanding its genetic and nongenetic factors.

폐경 전 유방암 환자의 치료 전.후 골밀도 변화 분석 (An Analysis of BMD Changes With Preoperative and Postoperative Premenopausal Breast Cancer Patient)

  • 유방암 환자의 외과적 수술과 방사선 및 화학적 치료 전 후의 골밀도 수치를 비교함으로써 암 치료에 의한 골소실 문제의 심각성을 도출하여 고 위험군선별 및 골다공증예방을 위한 기초자료를 제공하고자 하였다. 2007년 3월부터 2013년 9월까지 유방암으로 진단을 받고 수술 및 치료를 받은 후 골밀도 검사를 시행한 환자 254명을 대상으로 하였다. 이중 폐경에 이르렀거나 자궁적출술을 받아 폐경된 84명을 제외한 171명을 최종 분석의 대상으로 하였다. 방사선흡수법에 의한 골밀도 영상(요추부위와 대퇴골부위)에서 치료 전 후 측정된 골밀도 수치를 분석하였다. 치료 전 후의 전체적인 골밀도 변화량과 치료유형별 변화량을 비교하였으며, 환자의 결혼 유무, 자녀수, 수유유무, 초경나이, 유방암 치료형태 등을 변수로 하여 세부적인 차이점 및 연관성을 분석하였다. 측정된 자료는 SPSS for Windows Program(Version 18.0)을 사용하여 통계 분석하였다. 요추에서는 치료 후 평균 7.1% 감소하였고, 대퇴골에서는 평균 3.1% 감소하였으며, 통계적으로 매우 유의한 차이를 보였다(p<.01). 또한 화학적 치료를 시행한 환자군의 대퇴골 부위에서 치료 및 수술 후 $0.067g/cm^2$의 상대적으로 큰 골밀도 수치의 감소량을 보였으며 통계적으로 유의하였다(p<.05). 그 외 결혼 유무, 자녀수, 수유 유무, 초경 나이 등에 따라서는 감소량의 차이가 있었지만 통계적 수준에서 유의성은 없었다. 폐경 전 유방암 환자의 치료 후 골밀도 수치 감소를 보임에 따라 감소량이 상대적으로 큰 환자를 고 위험군에 포함시켜야 할 것이며, 이를 토대로 적극적인 예방 정책이 필요하리라 사료된다.

대장 선종 이형성 및 대장암과 임신, 출산, 월경 요인의 관련성에 관한 환자-대조군 연구 (A Case-control Study of the Relationships between Reproductive Factors and Degree of Dysplasia of the Colorectal Adenoma and Cancer)

  • Objectives : Evidence for an effect of rerroductive factors on colorectal carcinogenesis is not yet consistent. Little research has been conducted to investigate whether reproductive factors were associated with colorectal adenomas that are the precursors of colorecta1 cancer, We evaluated the relationships between reproductive factors and the degree of dysplasia of the colorectal adenoma and cancer as colorectal adenoma-carcinoma sequence. Methods : For this study, 241 adenoma cases with histopathologically confirmed incident colorectal adenoma, 76 cancer cases with colorectal cancer and 1677 controls were collected from Our Lady of Mercy Hospital, The Catholic University of Korea, during 1994-1999. Before colonoscopy, information on demographic characteristics, reproductive factors, life style habits and dietary intake were obtained by interviewed questionnaire. Adjusted OR and 95% CI were estimated by using polytomous logistic regression model, Potential confounders that were selected based on the goodness of fit Statistics and interaction between risk factors were considered in this adjustment. The Wald statistic was calculated to test the heterogeneity of the odds ratios for each case. Results .: Postmenopausal women with natural menopause were found to be positively associated with the risk of mild dysplasia adenoma (multivariate-adjusted OR : 2.59, 95% CI=1.1-0.2). Parity was found to be negatively associated with the risk of colorectal lancer (age-adjusted OR : 0.40, 95% CI=0.2-0.9), but did not significantly decrease the risk of colorectal cancer (multivariate-adjusted OR : 0.95, 95% CI=0.3-2.9). Me associations were seen between a9e at menarche, breast feeding, induced abortion, oral contraceptive use, menopausal types, menopausal age or hormone replacement therapy (HRT and the degree of dysplasia of the colorectal adenoma and cancer. However, none of these associations differed' significantly between the degree of dysplasia of the colorectal adenoma and cancer. Conclusions : These findings suggest that postmenopausal women with natural menopause may experience increased risk of mild dysplasia adenorna among colorectal adenoma-carcinoma sequence.

Some Factors Affecting Bone Mineral Status of Premenopausal Women

  • This study was conducted to investigate dietary and other factors affecting bone mineral density (BMD) in Korean premenopausal women. Seventy-eight premenopausal women who visited health promotion center for health examinations volunteered to participate in this study and they were divided into two groups according to the bone status by T-score : normal or osteopenic group and osteoporotic group. The demographic and general characteristics, and dietary intake were surveyed using the questionnaire. BMDs of the lumbar spine and femoral neck of subjects were measured by dual energy X-ray absorptiometry. Serum samples were measured for Lipid concentration, and calcium, phosphorus, alkaline phosphatase as bone formation indicators. Urine was analysed for creatinine as bone resolution indicators. The results are as follows:The mean BMDs of the lumbar spine and femoral neck were 1.21 0.02g/cm$^2$ and 0.97 0.04g/cm$^2$, respectively and the BMD levels of osteoporotic group were significantly lower than that of normal or osteopenic group (p<0.001, respectively). Height in osteoporotic group was significantly lower than that of normal or osteopenic group (p<0.01), and bodyweight did not show any significant difference but had a lower tendency. Mean daily intake of energy was 1720 52ka1. When nutrient intake was compared with recommended dietary allowances (RDA) of subjects, calcium, Fe, vitamin A and riboflavin intakes showed means lower than RDA. The nutrient intake did not show any significant difference between normal or osteopenic group and osteoporotic group except intakes of protein fat and niacin. Serum and urine levels did not show any significant differences between normal or osteopenic group and osteoporotic group and all were within normal range, however, serum alkaline phosphatase level of osteoporotic group was significantly higher than that of normal or osteopenic group (p<0.001). Height showed positive correlations with lumbar spine bone mineral density (LBMD, r=0.332, p<0.01), no correlation was found with femoral neck bone mineral density (NBMD). Age, age at menarche, bodyweight, body mass index (BMI) and obesity showed no correlation with BMD. The BMD of the lumbar spine was significantly and positively related to the intake of niacin and vitamin C (r=0.236, p<0.05; r=0.274, p<0.05). Serum levels of calcium and phosphorus showed a negative correlation with LBMD (r=-0.698, p=0.0001; r=-0.503, p=0.0001, respectively). The results suggested that the BMDs of the lumbar spine was positively related to the intake of niacin and vitamin C in premenopausal women. Therefore, this study confirmed that one of the most effective way to minimize bone loss would be higher intake of niacin and vitamin C rich foods and habitual physical activity may have a beneficial effect on BMD in premenopausal period.

여대생의 골밀도와 식이 및 비식이 요인들 간의 상관성 (Dietary and Non-dietary Factors Related to Bone Mineral Density in Female College Students)

  • The purpose of this study was to investigate correlations between bone mineral density (BMD) and dietary and non-dietary factors in female college students. The BMD of the subjects (n = 38) was measured using DEXA (Dual Energy X-ray Absorptiometry) at lumbar spine and three femoral sites including femoral neck, ward's triangle, and femoral trochanter. Three-day 24-hour dietary recall data were collected from each subject to assess consumption levels of nutrients and food groups. The mean (${\pm}SD$) values of age, age of menarche, height, weight, body mass index (BMI), waist-to-hip ratio (WHR), fat mass, and % body fat of the subjects were 21.34 (${\pm}1.73$) years, 13.1 (${\pm}1.2$) years, 161.3 (${\pm}5.0$) cm, 53.7 (${\pm}7.2$) kg, 20.6 (${\pm}2.6$) $kg/m^2$, 0.80 (${\pm}0.04$), 15.4 (${\pm}4.4$) kg, and 28.2 (${\pm}4.7$), respectively. The BMD values of lumbar spine, femoral neck, ward's triangle, and femoral trochanter as T-value were $1.150{\pm}0.13\;g/cm^2$, $0.932{\pm}0.11\;g/cm^2$, $0.850{\pm}0.13\;g/cm^2$, and $0.721{\pm}0.10\;g/cm^2$, respectively. The daily mean energy intake of the subjects was 1660.6 kcal. The intake levels of carbohydrate, calcium, iron, vitamin C, and folic acid were lower than the KDRIs, while those of fat, phosphorus, sodium, vitamin A, and vitamin $B_6$ were higher than the KDRIs. Significantly negative correlation were detected between consumption of fat and oils and the BMD of all sites measured (p < 0.05). Potato and starch intake was negatively correlated to the BMD of femoral neck and word's triangle (p < 0.05). The intake of cereals was found to be negatively correlated to the BMD of ward's triangle (p v 0.05). There was also negative correlation of intake of soybeans and products with the BMD of lumbar spine (p < 0.05). Weight and muscle mass were positively correlated to the BMD of both lumbar spine and femurs (p < 0.01). Significantly positive correlations between BMI, and fat mass and the BMD of lumbar spine were observed (p < 0.001). Additionally fat mass was positively correlated to the BMD of femoral neck. WHR and % body fat were positively correlated to the BMD of lumbar spine (p < 0.05). Negative correlation was found between a serum calcium level and the BMD of femoral neck (p < 0.05). In summary non-dietary factors generally showed significant correlations with the BMD of lumbar spine, while several dietary factors showed significant correlations with the BMD of femurs.

일 지역 중년여성의 폐경증상과 폐경관리와의 관계에 대한 연구 (An Analysis of the Relationship between Climacteric Symptoms and Management of Menopause in Middle-aged Women)

  • The purpose of this study was to investigate the relationship between climacteric symptoms and management of menopause of middle -aged women. The subjects of this study were 261 women(40 to 60 years old). Data were collected from Jun. 1 to Jul. 15, 2001 by a structured questionnaire. The instruments employed were : 1) The Climacteric Symptoms Scale developed by Aeri Song and Eun soon Chung(1998). 2) The Management of Menopause Scale developed by Aeri Song(1997). The data were analyzed by the SPSS p.c. program using t-test, ANOVA and Pearson correlation coefficient. The results of the study were as follows : 1. Mean score of climacteric symptoms was $2.18{\pm}0.39$(Maximum 4, Minimum 1). The mean scores among the categories of climacteric symptoms, in descending order, were : a) physical and physiological reactions ($2.62{\pm}0.53$), b) social and family relationships ($2.23{\pm}0.50$), c) psychiatric and psychological reactions ($2.08{\pm}0.49$), d) relationship with sexual partner($1.73{\pm}0.54$), e) genitourinary reactions ($1.72{\pm}0.55$). 2. Mean score of management of menopause was $1.79{\pm}0.45$ (Maximum 4, Minimum 1). The mean scores among the categories of management of menopause, in descending order, were : a) dietary management($2.57{\pm}0.52$), b) self control ($2.24{\pm}0.57$), c) management of exercise and physical activity($2.14{\pm}0.75$), d) management of sex life($1.71{\pm}0.47$), e) management of professional health maintenance($1.61{\pm}0.59$). 3. There were statistically significant differences in the score of middle-aged women's self reported climacteric symptoms according to : a) occupation (t=-2.79, p<0.001) b) marriage state (t=-2.29, p<0.05) c) age of menarche (F=4.66, p<0.001) d) method of Sanhujori (post natal care & treatment) (F=4.22, p<0.001) e) hormone replacement therapy (t=-3.09, p<0.05). From the above statistics, several significant findings were noted : a) There were more climacteric symptoms from those who were unemployed, those who had no partner or were divorced and those who started a menarche earlier. b) There were less climacteric symptoms reported from those on hormone replacement therapy and those who followed their parents or parents-in-law advice regarding Sanhujori (postnatal care) 4. There were statistically significant differences in the score of middle-aged women's self reported management of menopause according to : The educational background (F=7.63, p<0.001), religion (F=3.74, p<0.001), income (F=3.65, p<0.001), number of parity (F=4.87, p<0.001), method of Sanhujori(postnatal care) (F=5.73, p<0.001), period of Sanhujori (postnatal care) (F=2.81, p<0.05), hormone replacement therapy (t=3.81, p<0.001). Women with higher educational background, strong religion, higher income, large number of parity, managed their post natal care well, were on HRT, managed their menopause significantly better than the others who took part in the survey. 5. It will be noted from the above that women's degree of climacteric symptoms showed a negative correlation to the management of menopause(r=-0.2146, p<0.001). The findings shown above suggest the need to develop a variable management of menopause, in order to improve climacteric symptoms of middle-aged women. It is hoped that the above findings will stimulate more detailed research into this matter, and thereby enable guidance to be given to women going through the menopause to cope with it in a less stressful way.

Treatment and Follow-up of Human Papillomavirus Infected Women in a Municipality in Southern Brazil

  • Background: This study aimed toanalyze the risk behavior for cervical cancer (CC) and the human papillomavirus (HPV) prevalence and resolution among women who received care through the private healthcare network of a municipality in southern Brazil. Materials and Methods: This descriptive and retrospective study was conducted with 25 women aged 20 to 59 years who received care through the private healthcare network and were treated at a specialty clinic in the period from January to December 2012 in a municipality in Northwest Parana, Southern Brazil. Data from medical records with cytological and HPV results were used. Following treatment, these women were followed-up and reassessed after 6 months. Data were statistically analyzed using the t-test and chi-squared test at a 5% significance level. Results: The mean age of the studied women was $27.8{\pm}7.75$ years old, and the majority were married, with paid employment and were non-smokers. The mean age at menarche was $13.0{\pm}0.50$ years old, and the mean age at first intercourse was $17.5{\pm}1.78$ years, with only 8.0% (2) initiating sexual activity at an age ${\leq}15$ years old. The majority had 1 to 2 children (60.0%), while 88.0% reported having had one sexual partner in their lifetime, and all the women were sexually active. A total of 68.0% used a hormonal contraceptive method. All the women had leukorrhea and pain and were infected by a single HPV type. Regarding the lesion grade, 80.0% showed high risk and 20.0% low risk. The most prevalent high-risk HPV strain was 16. Conclusions: These findings provide relevant information on HPV risk factors and infection, as well as the treatment and 6-month follow-up results, in economically and socially advantaged women with no traditional risk factors, corroborating previous reports that different risk factors may be described in different populations. Thus, this study reinforces the fact that even women without the traditional risk factors should undergo HPVmonitoring and assessment to determine the persistence of infection, promoting early diagnosis of the lesions presented and appropriate treatment to thus prevent the occurrence of CC.

서울지역 일 여자 상업고등학교의 성에 대한 지식 및 태도에 관한 연구 (A Study on the Knowledge and Attitude about Sex in One Commercial Girls' High school in Seoul)

  • This study was conducted in order to grasp the condition of the student's knowledge and attitude about sex. And to offer the basic materials for sex education of school health service. The objects were comprised of 464 volunteered students who study in commercial girls' high school. The material of this study was the questionaire suited to the purposed of this research which has been made through studying references. All the questionaire written by students. The self reported questionaires were collected immediately without explanation on supervision of school nurse. The data was collected from 28th to 30th of June on 1990. Analysis of the data was done utilizing SPSS for percentage, mean, ANOVA and Pearson Correlation Coefficients. The Results are as follows; 1. General features of the objects of study School grade distribution was similar. Fathers of 41-50 years were the highest(58.3%), mothers of 41-50 years were the highest(64.3%), family of living together were the highest(87.5%), fathers of graduated high school were highest(60.7%), mothers of graduated middle school were the highest(43.0%) and neuclear families were highest(91.5%). 2. The Conditions of Knowledge about sex When 5 Point was given to "Well known" and 1 point was given to "Never known", the total average was 2.97, Especially the mean of Female physiology was shown 3.93, 73.4% of students have known. But the mean of male physiology was shown 2.23, 17.2% of students only. And Family planning item was 3.54, hymen item was 3.38, female genitalia item was 3.35, abortion item was 3.25, Intercourse and pregnancy item was 3.24, Ovulation item was 3.02, Contraception item was 2.97, Veneral disease and masturbation item was 2.82, maintenance of pregnancy item was 2.76, Anatomical differences between male and female item was 2.59, male genitalia item was 2.31, ejaculation item was 2.27. 3. The conditions of attitude about Sex When 5 point was given to "Very affirming" and 1 point was given to "Very deny" the total average was 3.20. Especially the mean of social intercourse between other sex was shown 3.92, 73.4% of students have affirmed. But the mean of psychic response on menstruation was shown 2.24, 8.8% of students only. And baby birth item was 3.72, the fact that I am a woman item was 3.53, marriage item was 3.49. Secondary body change item was 3.38, puberty item was 3.31, delivery and sexual intercourse item was 3.05, pregenancy item was 3.02, psychic condition on menarche item was 2.50. Also present counsellors about sex were teachers in charge (44.9%), friends(21.6%), mothers(20.6%), elder sisters (10.6%), mass-communications (1.5%), fathers (0.4%), school nurses and elder brother(0.2%).
