Prevalence, Risk Factors and Disease Knowledge of Breast Cancer in Pakistan

  • Breast cancer is the most common cancer in females all over the world with approximately one million new cases each year as well as one of second leading causes of death among females. In Pakistan, the most frequently diagnosed cancer among females is also breast cancer, accounting for nearly one in nine female patients. Its incidence in Pakistan is 2.5 times higher than that in neighboring countries like Iran and India. The risk factors associated with breast cancer are age, family history, early menarche, intake of combined estrogen and progestin menopausal hormones, alcohol consumption, physical inactivity, low socioeconomic status and lack of awareness regarding the disease. This mini-review article aims to provide awareness about breast cancer as well as an updated knowledge about the prevalence, risk factors and disease knowledge of breast cancer in Pakistan.

여고생의 비만도에 따른 식습관과 식품섭취에 관한 연구 (A Study on Eating Habits and Food consumption pattern among High school girls)

  • This study was undertaken to assess eating habits and food consumption pattern of 365 adolescent girls in Kwangju area. Subjects were divided into 3 groups based on relative body weight as obesity index. Anthropometric data showed that mean height and weight were $161.9{\pm}19.0cm$ and $53.3{\pm}7.2kg$ respectively which are similar to those in the Korean Standard Growth data. Mean BMI and relative body weight were 20.50 and 97.1%. Age of menarche in the subjects significantly influenced the obesity index. Food habit score in the underweight group was significantly lower than that in the normal group at ${\alpha}=0.2$ level by multiple range test. Obesity was significantly associated with more frequent and irregular eating. It seems that students in the obese group were concerned on their body weight and tended to consume much vegetable. Obese group consumed more fruits, less butter and fruit juice compared to other groups. It might be suggested that more effective nutrition program might be developed and implemented to ensure good food habit of adolescent girls including obese as well as underweight group.

일 대학 여대생의 골밀도와 골밀도에 영향을 주는 요인 (Bone Mineral Density and Factors influencing Bone Mineral Density in College Women)

  • Purpose: The purpose of this study was to identify the levels of bone mineral density (BMD) and to explain the factors influencing BMD among female college students in Korea. Methods: A cross-sectional study was conducted with 144 college women. Lifestyle factors were determined by self-report questionnaire. Body composition was measured by body composition analyzer and bone mineral density was measured by ultrasound bone densitometry. Data were analyzed using SPSS for windows, version 19.0. Results: The mean BMD at calcaneus site was 0.56$g/cm^2$ (mean T-score=-0.22). The incidence of osteopenia was 21.5%. Factors predicting BMD were menarche age (r=-.22, p=.009) and height (r=-.18, p=.030) with 7% of explained variance. Conclusion: These results suggest that health care professionals need to provide young women with program that is intended to affect their intention toward osteoporosis preventive behavior change.

Reproductive Risk Factors for Thyroid Cancer: A Prospective Cohort Study in Khon Kaen, Thailand

  • Background: Because of the gender disparity in the incidence of thyroid cancer, this study aimed to determine the association between reproductive factors and thyroid cancer. Methods: A total of 10,767 eligible women from the Khon Kaen Cohort, recruited and interviewed between 1990 and 2001, were followed up until 2011. The data were linked to the Khon Kaen Population-Based Cancer Registry to detect thyroid cancer cases. Results: There was 17 thyroid cancer cases detected, an incidence of 11.2 per 100,000 person-years, of which 70.6 % were papillary tumors. The incidence was apparently greater among those with an early age of menarche, nulligravida women, and oral contraceptive users. Conlusions: There was a trend for thyroid cancer to develop in relation to longer estrogen exposure. This evidence is inconclusive but warrants further investigation.

여자 청소년의 비뇨생식기 관련 개인위생과 월경기 위생 관리 (Personal Hygiene Practices related to Genito-urinary Tract and Menstrual Hygiene Management in Female Adolescents)

  • Purpose: The study was to survey personal hygiene practice related to genito-urinary tract and menstrual hygiene management in female adolescents in order to obtain basic information for health education. Methods: With a descriptive survey design, 389 adolescents were recruited via convenience sampling in Korea. Survey instrument was the feminine and menstrual hygiene practice and perception of vaginal douching. Data were collected from a self-administered structured questionnaire. Results: Mean age of adolescents was 16.09 and menarche was at 13.21. While washing hands after urination/defecation was highly performed, wipe front to back and wash with soap and water were reported as being not well done. Twenty-eight percent reported douching habit. More positive beliefs about douching were reported by adolescents who practiced douching. Menstrual hygiene management was very appropriate with changing sanitary pads regularly with hand washing; but less performed for limiting bathing activity during menstrual periods and washing hands after activity of genito-urinary area. Conclusion: Some adolescents practiced inadequate hygiene practices especially for body cleansing during menstrual period and vaginal douching. It is important to develop and implement school health education programs on feminine and personal hygiene for adolescents to help them perform adequate health behaviors.

여자 중학생의 월경불편감과 수면장애와의 관계 (Relationship between Menstrual Distress and Sleep Disturbance in Middle-school Girls)

  • Purpose: To identify factors associated with menstrual distress and characterize the relationship between menstrual distress and sleep disturbance in middle-school girls. Methods: Participants in this correlational study were 117 middle-school girls who were recruited through convenience sampling. Data were collected from March 2018 to April 2018 using self-reported structured questionnaires and analyzed using IBM SPSS Statistics 23.0. Factors associated with menstrual distress included physiological, psychological, and situational factors based on the theory of unpleasant symptoms. Results: Age of menarche (${\beta}=-.28$, p<.001), amount of menstruation (${\beta}=.23$, p=.004), lifestyle-related exposure to endocrine-disrupting chemicals (${\beta}=.21$, p=.027), and academic and peer-relationship stress (${\beta}=.19$, p=.025) influenced menstrual distress, explaining 47.4% of the variance in this regression model. The relationship between menstrual distress and sleep disturbance was statistically significant. Sleep disturbance was increased 1.26 folds when dysmenorrhea score increased by one unit (OR=1.26, 95% CI: 1.01~1.58). However, parental support was not a significant moderating factor of sleep disturbance. Conclusion: This study provides basis to develop an intervention strategy to alleviate menstrual discomfort in adolescents and improve their quality of sleep.

여성 골다공증의 위험요인에 관한 환자-대조군 연구 - 서울지역 1개 병원 내원환자를 중심으로 - (A Case-control Study on Risk Factors of Osteoporosis in Some Korean Outpatient Women of One General Hospital of Seoul)

  • Until now there are few available epidemiologic data of osteoporosis in Korea, and the severity of osteoporosis-related health problem has not been widely recognized yet. But the numbers of the old people are increasing in Korea, and in 2000, the proportion of people over 65 will be up to about 6.8% of total population. Therefore, osteoporosis, one of the most common metabolic bone disease among the old people, will be one of the most important public health problem. On this background this study was performed to find out risk factors of the development of osteoporosis in Korean women through case-control approach. The subject of this study were selected among the women who visited menopause clinic in one general hospital in Seoul and were checked bone density from Sep. 1988 to Sep. 1993. Those who were diagnosed to have hypertension, diabetes mellitus, thyroid disease, breast disease, or liver disease, which are thought to influnce bone density, were excluded. Also excluded those who are age-unknown. Finally the subjects were 2,139 women aged between 18 and 79. We operatively defined patient group as those whose bone density is below $1.039/cm^2$, 90% of average bone density of women of 4th decade who visited the same hospital. And we defined control group as whose bone dendity is above $1.15g/cm^2$. We randomly selected 201 women from the patient group and 202 from the control. As independent variables we chose age, menarche age, menopause age, menopause type, the number of siblings, the number of pregnancies, body mass index, taking oral pill or not, feeding type, and educational state. Multiple logistic regression analysis was done to see the influence of these variables on the risk of osteoporosis. Results are as follows; 1. menopausal status was statistically significant risk factor to all women irrespective of her age, while obesity and later menopause age were found to be statistically significant protective factors. 2. The more siblings and pregnancies, the greater tile risk of osteoporosis, but these factors were not statistically significant. This result is not consistent with other studies. Further studies are strongly needed.

Weight Gain and Alcohol Drinking Associations with Breast Cancer Risk in Japanese Postmenopausal Women - Results from the Japan Collaborative Cohort (JACC) Study

  • Background: We investigated four factors, height, weight gain since age 20, physical activity, and alcohol drinking, for associations with risk of breast cancer (BC) according to menopausal status, using the latest data of the Japan Collaborative Cohort Study (JACC Study). Materials and Methods: We confined the analysis to 24 areas available of cancer incidence information, excluding women with a previous diagnosis of BC. Baseline data were collected from 38,610 (9,367 premenopausal, and 29,243 postmenopausal) women during 1988 and 1990. The study subjects were followed-up at the end of 2009, and 273 (84 premenopausal, and 189 postmenopausal) cases of BC were newly diagnosed in 501,907 person-years. The Cox model was used to estimate a hazards ratio (HR) and its 95% confidence interval (CI) of BC risk. Results: As a result of the multivariate analysis adjusting for age at baseline survey, age at menarche, number of live births, and, age at first delivery, weight gain since age 20 of 6.7 kg-9.9 kg, and ${\geq}10.0kg$ were significantly associated with increased risk for postmenopausal BC (HR=2.48, 95% CI 1.40-4.41, and, HR=2.94, 95% CI 1.84-4.70, respectively). Significantly increased trend of BC risk was also observed in weight gain since age 20 (p for trend, p<0.001). Amount of ethanol intake per day${\geq}15.0g$ was significantly associated with increased risk for postmenopausal BC in the multivariable-adjusted analysis (HR=2.74, 95% CI 1.32-5.70). Conclusions: Higher weight gain in adulthood and larger amounts of ethanol intake were significantly associated with increased risk of BC in Japanese postmenopausal women. None of the investigated factors were significantly associated with BC risk in Japanese premenopausal women.

Breast Cancer Risk Factors in Women Participating in a Breast Screening Program: a Study on 11,850 Iranian Females

  • The incidence rate of breast cancer in developed countries is almost three-fold higher than in developing countries. Iran has had one of the lowest incidence rates for breast cancer in the world, but during the recent decades a marked increase has been seen. The purpose of this study was to investigate some established risk factors of breast cancer in Iranian women. A study of 11,850 women participating in abreast screening program was conducted. The 197 women diagnosed with breast cancer and 11,653 healthy women were compared. Logistic regression was performed to investigate associations of reproductive and anthropometric factors with breast cancer risk. Family history of breast cancer (OR=1.94, 95%CI=1.35-2.78), occupation (OR= 1.65,95%CI=1.20-2.25), education level (OR=0.50,95%CI=0.28-0.91), parity (OR=0.27, 95%CI=0.12-0.59), menopausal status (OR=3.15, 95%CI=2.35-4.21), age at menarche (OR=0.33, 95%CI=0.15-0.70), and age at the first pregnancy (OR=4.10, 95%CI=1.13-14.77) were related to the risk of breast cancer. Decrease in parity may to some extent explain the rising trend of incidence of breast cancer incidence in Iranian women.

기온 변화는 폐경후 안면홍조의 침 치료 효과에 영향을 미치는가? (Can Ambient Temperature Change Affect the Efficacy of Acupuncture on the Postmenopausal Hot Flash?)

  • Purpose: To find the correlation between ambient temperature change and the efficacy of acupuncture on the postmenopausal hot flash. Methods: 52 patients entered a randomized clinical trial which was to verify the efficacy of acupuncture on postmenopausal hot flash. Participants were treated with optimal/minimal acupuncture in the course of 13 weeks. 34 of 52 visited for the temperature-rising period(Group 1), while 18 of 52 for the temperature-falling period(Group 2), 100mm hot flash VAS and average temperature were taken 18 times from each person. Linear mixed effects model was used to find the correlation between temperature and hot flash VAS score(p-value<0.05). Group1/2, optimal/minimal acupuncture, age, age of menarche, height, weight, percent body fat, BMI, waist-hip ratio, past history of hysterectomy and HRT were also examined. Results: Period of treatment had influence on the hot flash VAS. As time went by, hot flash decreased(p<.0000). Participants who underwent minimal acupuncture showed higher hot flash VAS(p=0.0479) and as waist-hip ratio rose, hot flash increased(p<.0000). As the temperature rose, hot flash VAS score became lower(p=0.0143) and it was because 34 of 52 participants were treated for the temperature-rising period. Group 1/2 had no interrelation with hot flash(p=0.5741). Conclusion: Hot flash diminished as participants were treated with acupuncture, independently of whether the ambient temperature rises or falls.
