A Study on the Variation in Meical Service Utilization of The Dead by Cerebrovascular Diseases Patients in Korea (뇌혈관질환 사망자 의료이용 변이의 분석)

  • This study focused on finding the variation of medical service utilization, paths of medical service utilization and medical payments of the patients died by cerebrovascular diseases. For this study, data of the one-year episodes of the health insurance subscribers died in 2004 were selected. The frequency of medical visits, the lengths of stays, the days of outpatient visits, the total period of medical services and the total medical payments were compared by the characteristics of the suppliers and utilizers. This study is useful in reviewing the equity of medical service utilization because it analyzed variance in utilization by episodes. In oder to collect accurate data of the patients died by cerebrovascular diseases in 2004 the 2004 reimbursement data of all medical institutions were matched to the data of funeral fee payment by the National Health Insurance Corporation from January 2004 to May 2005. The major results of the study are as follows. The variation of medical service utilization of cerebrovascular diseases was influenced by supplier factors suppliers, such as types and locations of medical institutions and user factors such as sex and age. It was suspected that the reimbursement by fee-for-service contributed to the variation quite a lot, but we could not compare the variation between the different reimbursement systems in Korea. On the basis of analyzing results this study suggests that the factors of suppliers and utilizers should be reviewed to reduce the under use and over use expressed by variations of medical service. The processes of care, effective communication and management system should be investigated for the equity of medical service utilization and also. alternative medical services would be recommended to reduce the high medical payment. Additionally to find other causes of variation further in depth study controling the severity of diseases, socio-economic status of the users and the system factors is required.

  • In this study, the Commission proposed the mitigation of remedies by improving the role of medical disputes and preventing medical disputes. Medical disputes include a comprehensive description of medical malpractice, medical negligence, medical malpractice, and medical malpractice. Medical negligence refers to the neglect of medical care due to careless medical care in the treatment of patients, leading to patient injury and death. An inappropriate response in the process of international treatment could result in international trials and a decline in international credibility. In cases where medical disputes arise, health care is strictly necessary to determine the truth or absence of medical malpractice, and these expertise and experience are usually provided by emotion. With the neutral and objective emotions provided fairly and impartially, medical care expertise and experience can be fair, and the medical disputes can be resolved peacefully if the parties are trustworthy. The Health Care Dispute Mediation Committee should focus on enhancing the professionalism, objectivity, and reliability of medical care.

Trends of Studies on Journal of Korean Medical History in Korea (한국(韓國) 의사학회지(醫史學會誌) 연구(硏究) 동향(動向))

  • Objectives : Most academic societies have their own journals. Those journals represent the trends of academic societies' studies. Therefore, it is important to review articles in journals. Korean Society of Medical History (KSMH) has Journal of Korean Society of Medical History (J. of KSMH). We analyze articles in J. of KSMH and try to recognize the trends. Methods : From Vol. 12 No. 2 in 1999 to Vol. 27 No. 1 in 2014 of the J. of KSMH, we retrieved article's headings and analyzed those subjects. Results : We got 342 research articles about ; book 97, person 44, theory 65 (medical theory 48, thought 8, origin of word and story 9), clinical research 42 (disease 19, method 16, treatment 7), system of medicine and medical treatment 22 (system of medicine 10, system of medical treatment 12), mutual and comparative study in medicine 18 (mutual study 14, comparative study 4), etc. 56 (materia medica and prescription 23, trends study and essay 18, traditional medicine informatics 10, community medicine 5). Conclusions : J. of KSMH contained several subjects based on medical history ; book, person, theory, thought, origin of word and story, clinical research, mutual and comparative study in medicine, etc. However, the subjects were biased to books and persons.

Analysis of the characteristics of medical service depending on the latent classification of medical information (의료정보 이용의 잠재적 유형에 따른 의료서비스 특성분석)

  • The primary purpose of this study is to examine consumers'probing actions to see what information sources consumers search for medical information when there are diverse medical service information channels, and classify consumers by information source. Its secondary purpose is to understand trust of information and attitude toward information by consumer type, value of medical service, satisfaction with medical service, and word-of-mouth intention. This study will concretely identify information utilization patterns of medical consumers, and explain the unique characteristics and behavior of segmented types of medical consumers. The significance of this study lies in the search for ways to establish information channels trusted by consumers for building an efficient medical service market in the future. The results of this study show that consumers were classified by the latent class analysis(LCA) into 5 types: low-level information seekers, word-of-mouth information seekers, mass media information seekers, digital information seekers and diverse information seekers. The reliability of information sources by type of medical consumer was statistically significant, and in the analysis of differences in consumer attitude, there was a statistically significant difference in cognitive responses. The value of medical service was statistically significant in health recovery and medical service word-of-mouth intention.

  • Accoring to the astonlshing progress of medical science, the medical roles of the radiologic technologist are increasing gradually and specializing highly. However, there are the wide disagreements the actual roles of the radiologic technologists at clinics and the relating rules of the medical law. Therefore, it is required that the medical law should be corresponded with the actual state. To solve these problems. this study has proceeded to make the survey of the present medical law and has tried to offer the most suitable theories to the actual state. This study includes the survey of relevant professional literatures. The major contents of this study are as follows. First, medical technician is written "技士" (in Chinese character) at the present medical technician law, and that word is written wrong. So, it should be replaced with "技師". Therefore, radiologic technologist should be written "放射線師". Second, the relations between the doctor and the radiologic tecnologist should be written the "request or other words" instead of "direction". Third, in spite of the rules of the present medical law, the medical act of radiologic technologist at clinics should be belonging to the boundary of medical practice. Forth, to present the appropriate medical service to the patients, legal status of radiologic technologist as a member of medical team should be established. Fifth, it is desired that Magnetic Resonance Imaging Technology as a business of radiologic technologist should be provided for in the medical law.

  • An account book of medical treatment is a form of collection materials for diagnostic standardization, and it is a basis of standardization, standardization of medical records is a preconsideration of each standardization. But an account book of medical treatment is only a kind of form for recording medical treatment, therefore standardization of medical treatment eventually holds the key to the standardization of recording charts. However until now we have gradually reformed medical records in accordance with individual characters of medical treatment, and didn't have even standard sheme of medical records, also medical terms for medical records had an inconsistency of redescription and reiterative representation for an identical terms in all parts of the East learning, medical terms for medical records didn't unity. To make better this realities, standardization study used orginated system in the process of existing study, it can get ready the basis of discussion between O.M.D and O.M.D. it can make analysis of diagnostic course and can clearly understand usable information by diagnostic course. for that reason we hope that the basis of standardization is accomplished. And in advance of study for this standardization we have to analysis the course of medical treatment with demonstration of roof, first of all we have to study term definition by diagnostic course and prepare basis by diagnostic course. because this study have limits of indivisual study, it needs to long and synthetic investigation in Association levels. Although we cann't completely alternate with methods of measurement which relyed on individual mastery, if we exclude erroes of individual measurement through mechanization and verify results of diagnosis through keynotes, we can realize standardization of medical treatment with demonstration of proof and in this process we can use medical records as a tool collecting exact data, also we can realize standardization of drawing up medical records.

A Study of Pattern Change Medical Advertiging (의료법개정 전.후에 따른 의료광고의 변화 -주요 3대 일간지를 중심으로-)

  • This study is aims finding out characteristics and change levels of medical advertizing between before and after revised medical laws. We investigated to medical advertizing of 3 major newspaper(Josen, Joongang, Donga) to achieve study purpose. After revised medical laws, major change is seen below. 1. Increase rates of medical advertizing was 65.5%. Specially, increase rates of medical advertizing by each department were differently each department; Dermatology was 450%, dentist's clinic was 342%, orthopedic surgery was 171%, and plastic surgery was 133%. In spite of increasing trends of most departments, urology decreased to 50% than before revised medical laws. 2. Of types of medical advertizing, Question and Answer type increased to 230% than before. Illegal level of medical advertizing was 10.4%. 3. The contents of medical advertizing were hospital location, photograph before and after treatments, and carrier, name and introduction of medical staffs. 4. The size of medical advertizing increased 1.73 column then before. The size of medical advertizing in clinics was shown statistically increase than hospital level. With above results, it is cleared that increase rates and trends of medical advertizing are more increased and complicated before revised medical laws. We try to be better situations for patients to get more exactly information and facts.

A Empirical Study on the Factors of Taxation Cognition for Medical Institutions in Korea (우리나라 의료기관에 대한 과세인식에 영향을 미치는 요인에 관한 실증연구)

  • Toe objective of this study is to verify the factors influencing the taxation cognition of medical institutions and to verify the difference of taxation cognition among the interest groups in medical services. The factors that influence taxation cognition are supposed to be five: cognition of public benefits on medical services, cognition of profits from medical institutions, cognition of self-responsibility of medical institutions, cognition of distinction of medical institutions, and cognition of the importance of medical services. The interest groups are divided into four: medical institution employees, medical treatment consumers, taxation experts, and tax officials. As a result of this study, first, cognition of public benefits, cognition of profits, and cognition of distinction are verified to have statistical significance as factors for taxation cognition. It means that cognition of the public benefits of medical services is low, while cognition of profits is high, and taxation cognition such as tax supports and tax exemption appears low in accordance with lowness of cognition of distinction of medical institutions. Second, taxation cognition of the interest groups about medical service shows statistical significance between the group of medical institution employees and the group of tax officials, and between the group of medical institution employees and the group of taxes experts. This study is expected to contribute to tax policy, which can support medical institutions to provide medical consumers with good medical services, by analyzing the factors that influence taxation cognition on medical institutions.

Analysis of factors affecting Korean Medicine utilization of multicultural family members (다문화가정의 한의의료이용에 미치는 요인 분석 연구)

  • Objectives: This study aimed to analyze the Korean medical utilization and Korean medical expenses by characteristics of multicultural family members to investigate whether it affects Korean medical utilization. Methods: This study utilized The Korea Health Panel data in 2018, with 238 final participants. We performed a t-test and ANOVA on the difference between Korean medical utilization and Korean medical expenses according to Anderson's Model of Health Service Utilization variables. Logistic regression analysis and generalized linear model analysis were conducted to analyze Korean medical utilization factors. Results: The Korean Medical utilization was 12.61% among the multicultural family members. As a result of regression analysis, the female had high Korean medical utilization(p=.008), and rural area residents had low utilization(p=.017). Korean medical expenses were high when they were female or married. Including the utilization of western medical services by outpatients, Korean medical expenses were high when they were female or outpatients who received western medical services. Conclusions: As a result of this study, the factors influencing Korean medical utilization were gender and residence area. There were differences in Korean medical expenses depending on western medical services use or gender. Therefore, it is necessary to use these factors to expand the Korean Medical utilization by multicultural family members, and research of the Korean medical utilization by disease is needed.

Development of a Medical Humanities Course Based on Design Thinking and Medical Students' Perceptions (디자인사고 기반 의료인문학 수업 개발과 의과대학생의 인식)

  • Amid the increasing interest in medical humanities education, this study developed a medical humanities course that utilized design thinking to foster creative thinking, problem-solving, and collaboration skills that pre-medical students should possess. The course's efficacy was assessed by evaluating improvements in core design thinking skills. The present study was conducted among 83 first-year medical students after planning and implementing a design thinking course. The reflection journals written by students along the course of the class were examined using the template analysis technique to evaluate the effectiveness of the class. The study's primary findings showed the successful development of step-by-step medical humanities education content utilizing design thinking and its practical implementation in a class. Moreover, the course improved students' core design thinking skills effectively, and in a balanced way. These results illustrate the effective application of design thinking in medical school through a medical humanities course. These findings indicate that a medical humanities course can help medical students showcase their abilities to collaborate and solve problems in the real world. This paper suggests the need for further research to develop a curriculum that integrates design thinking and investigate the relationship between medical students' core competencies and design thinking-based courses.
