Center Array-Sequencing 위상펼침 기법의 MR 온도영상 적용에 관한 기초연구 (Preliminary Study on the MR Temperature Mapping using Center Array-Sequencing Phase Unwrapping Algorithm)

  • 목적: 물체 내부의 온도를 비침습적으로 측정할 수 있는 양성자 공명 주파수 이동에 의한 MR 온도영상의 재구성에 center array-sequencing 위상펼침(PU) 기법을 적용시켜 그 성능 및 유용성을 평가하고자 하였다. 대상 및 방법: MR 온도 영상에 앞서 잡음 수준이 다른 타원형 팬텀들을 컴퓨터 모의 실험으로 제작하고 제안된 PU방 법을 적용시켜 잡음에 대한 성능을 평가하였다. MR 실험은 PU 실험과 이를 이용한 온도분포영상획득 실험으로 구분하여 수행되었다. 1.5T MR 영상장치에서 무릎코일과 $T2^*$ 경사자장에코 펄스열을 이용하여 MR 영상을 얻었다. 물통, 오렌지, 아가젤 등의 팬텀을 실험 대상으로 하였고 자체 제작된 온수펌프 장치로 팬텀의 온도를 조절하였다. T 형 열전쌍 온도측정장치로 팬텀 온도를 측정하고 MR 온도영상 결과와 비교하였다. 획득된 MR영상의 위상분포는 제안된 PU방법으로 위상을 편 후 온도분포 영상을 재구성하였다. 가열 전 후의 온도변화와 MR 영상의 위상변화 관계를 이용하여 아가젤 팬텀 내의 MR온도분포 영상을 구하였다. 결과: 제안된 center array-sequencing PU 알고리즘을 이용하여 여러 팬텀에 대한 MR 위상영상의 접힘 현상을 기존 방법보다 간편하고 빠르게 제거할 수 있었고 이를 이용하여 MR 온도영상을 획득할 수 있었다. 결론: 본 연구는 제안된 center array-sequencing 위상펼침 방법이 잡음에 강하고 처리 속도가 빠를 뿐만 아니라 양성자 공명 주파수 이동의 성질을 이용한 MR 온도 영상 획득에 성공적으로 적용될 수 있음을 보였다.

우리나라 성인에서 Haemophilus influenzae type b에 대한 항체 평가 (Evaluation of Antibodies Against Haemophilus influenzae Type b in Korean Adults)

  • 목적: 1995년 한국에서 b형 헤모필루스 인플루엔자(Haemophilus influenzae type b [Hib]) 백신이 도입된 이후, 2013년 국가예방접종에 포함되었다. Hib 백신 도입 시기 이후 다른 나라의 일부 연구에 따르면, 침습적 Hib 질환은 소아보다 성인, 특히 노인 및 면역 저하 환자에서 더 자주 발생한다고 보고하였다. 따라서, 본 연구는 20-85세의 한국 성인에서 항-polyribosylribitol phosphate (PRP) 항체의 양적 및 질적 측정을 통하여 질병 감수성을 평가하고자 하였다. 방법: 혈청은 39명의 건강한 성인(20-50세)과 면역 저하 상태가 아닌 30명의 노인(75-85세)에서 수집하였다. 모든 피험자는 7일 이내에 항생제를 복용하지 않았다. 항-PRP 면역글로불린 G (immunoglobulin G [IgG]) 항체가 및 혈청 살상능 역가(serum bactericidal index [SBI])는 효소결합면역흡착측정법과 혈청 살상능 평가분석을 통해 측정하였다. 결과: 항-PRP IgG 항체가의 기하평균 및 SBI의 기하평균은 성인군에서 각각 $0.88{\mu}g/mL$ (95% 신뢰 구간, 0.17-3.85) 및 354 (95% 신뢰구간, 50-2,499)이었고, 노인군에서는 각각 $1.67{\mu}g/mL$ (95% 신뢰구간, 0.53-5.24) 및 449 (95% 신뢰구간, 146-1,376)이었다. 소아에서 Hib 감염에 대한 방어 농도로 인정하고 있는 0.15 및 $1.0{\mu}g/mL$을 적용하였을 때, 항체 양성률은 성인군에서 87.2%, 노인군에서 100%였다. SBI의 양성률($SBI{\geq}4$)은 성인군에서 82.1%, 노인군에서 100%였다. 결론: 대부분의 성인군과 노인군에서 양적으로나 기능적으로 Hib 질환에 대한 방어 면역력을 가지고 있었으나 모두는 아니었다. 높은 면역력과 낮은 Hib 순환율은 인구에서 Hib에 대한 자연면역을 감소시킬 수 있으므로, Hib 면역과 질병에 대한 지속적인 감시가 필요하다.

유방암 방사선치료에 있어 치료도중 및 분할치료 간 위치오차에 대한 전자포탈영상의 컴퓨터를 이용한 자동 분석 (Computer Assisted EPID Analysis of Breast Intrafractional and Interfractional Positioning Error)

  • 유방암 환자의 방사선치료에 있어 치료도중(intrafractional) 및 분할 치료 간(intefractional)에 발생되는 오차를 측정하는 자동분석소프트웨어를 개발하였다. 오차 분석 결과는 3차원 입체조형 방사선치료를 임상에 적용하기에 앞서 적절한 치료계획용적(Planning Target Volume, PTV)을 설정하는 데 있어 매우 중요하다. 본 연구에서는 전자포탈영상장치(Electrical Portal imaging Device, EPID)로써 Portal Vision LC250 액체 충전형 이온화 검출기를 사용하였다(fast frame-averaging 모드, 초당 1.4 프레임, 256X256 픽셀). 12명의 환자에 대해 최소 7일 이상씩 영상을 획득하였다. 매 치료마다 평균 8 내지 9개의 영상을 각 빔에 대해 얻었다(분당 400 MU 선량률). 총 2,931 (720 측정을 포함하는)개의 영상을 정량적으로 분석할 수 있는 자동화 영상 분석 소프트웨어를 개발하였다. 이를 통해 호흡으로 인해 발생하는 치료도중 오차와 분할 치료간 발생하는 분할치료오차의 표준편차($\sigma$)들을 계산하였다. 신뢰 구간 95%로 임상표적체적(Clinical Target Volume, CTV)을 포함할 수 있는 PTV 마진은 $2\;(1.96\;{\sigma})$으로 계산되었다. 주로 호흡으로 인해 유발되는 치료도중오차를 보상하기 위해 필요한 PTV 마진은 2 mm에서 4 mm이었다. 반면에 분할 치료간 오차를 보상하기위해 필요한 PTV 마진은 7 mm에서 31 mm이었다. 12명의 환자에 대한 전체 평균오차는 17 mm이었다. 분할치료 간 오차는 호흡에 의해 유발되는 치료도중 오차에 비해 2배에서 15배까지 더 크게 나타났다. 유방암 치료에 있어 3차원 입체정형조사나 세기조절방사선치료(Intensity Modulated Radiation Therapy, IMRT)를 적용하기에 앞서 반드시 셋업 오차의 크기를 측정하여 PTV에 적절히 반영되어야 한다. 유방에 대한 3차원 입체정형조사나 세기조절방사선치료를 위해 반드시 필요한 것은 아니지만, 큰 PTV 마진을 줄여주기 위해서는 영상유도방사선치료(Image Guided Radiation Therapy, IGRT)가 매우 유용하게 이용될 수 있다. 전자포탈영상장치 들은 본 보고서에서 기술한 바와 같은 자동분석소프트웨어를 반드시 포함하여야 한다. 이를 통해 수많은 EPID 영상들을 자동화 처리하고 오차분석을 시행함으로써 각 병원의 임상적용 방법 및 환경에 따라 상이하게 나타날 수 있는 오차의 크기를 감안한 적절한 PTV마진을 구하는데 도움을 얻을 수 있다. 이러한 장치들은 또한 최소의 노력으로 환자 치료를 관찰할 수 있는 귀중한 정보를 제공해 준다.

대기오염에 의한 폐암 및 만성폐색성호흡기질환 -개인 흡연력을 보정한 만성건강영향평가- (Lung cancer, chronic obstructive pulmonary disease and air pollution)

  • Background : Although there are growing concerns about the adverse health effect of air pollution, not much evidence on health effect of current air pollution level had been accumulated yet in Korea. This study was designed to evaluate the chronic health effect of ai. pollution using Korean Medical Insurance Corporation (KMIC) data and air quality data. Medical insurance data in Korea have some drawback in accuracy, but they do have some strength especially in their national coverage, in having unified ID system and individual information which enables various data linkage and chronic health effect study. Method : This study utilized the data of Korean Environmental Surveillance System Study (Surveillance Study), which consist of asthma, acute bronchitis, chronic obstructive pulmonary diseases (COPD), cardiovascular diseases (congestive heart failure and ischemic heart disease), all cancers, accidents and congenital anomaly, i. e., mainly potential environmental diseases. We reconstructed a nested case-control study wit5h Surveillance Study data and air pollution data in Korea. Among 1,037,210 insured who completed? questionnaire and physical examination in 1992, disease free (for chronic respiratory disease and cancer) persons, between the age of 35-64 with smoking status information were selected to reconstruct cohort of 564,991 persons. The cohort was followed-up to 1995 (1992-5) and the subjects who had the diseases in Surveillance Study were selected. Finally, the patients, with address information and available air pollution data, left to be 'final subjects' Cases were defined to all lung cancer cases (424) and COPD admission cases (89), while control groups are determined to all other patients than two case groups among 'final subjects'. That is, cases are putative chronic environmental diseases, while controls are mainly acute environmental diseases. for exposure, Air quality data in 73 monitoring sites between 1991 - 1993 were analyzed to surrogate air pollution exposure level of located areas (58 areas). Five major air pollutants data, TSP, $O_3,\;SO_2$, CO, NOx was available and the area means were applied to the residents of the local area. 3-year arithmetic mean value, the counts of days violating both long-term and shot-term standards during the period were used as indices of exposure. Multiple logistic regression model was applied. All analyses were performed adjusting for current and past smoking history, age, gender. Results : Plain arithmetic means of pollutants level did not succeed in revealing any relation to the risk of lung cancer or COPD, while the cumulative counts of non-at-tainment days did. All pollutants indices failed to show significant positive findings with COPD excess. Lung cancer risks were significantly and consistently associated with the increase of $O_3$ and CO exceedance counts (to corrected error level -0.017) and less strongly and consistently with $SO_2$ and TSP. $SO_2$ and TSP showed weaker and less consistent relationship. $O_3$ and CO were estimated to increase the risks of lung cancer by 2.04 and 1.46 respectively, the maximal probable risks, derived from comparing more polluted area (95%) with cleaner area (5%). Conclusions : Although not decisive due to potential misclassication of exposure, these results wert drawn by relatively conservative interpretation, and could be used as an evidence of chronic health effect especially for lung cancer. $O_3$ might be a candidate for promoter of lung cancer, while CO should be considered as surrogated measure of motor vehicle emissions. The control selection in this study could have been less appropriate for COPD, and further evaluation with another setting might be necessary.

클로자핀을 투여한 조현병 환자에서 혈소판 활성 증가에 관한 후향적 연구 (Clozapine Administration Potentiate Platelet Activation in Patients with Schizophrenia : Retrospective Study)

  • 연구목적 클로자핀은 조현병 치료에 널리 처방되는 항정신병약물로, 대사성 부작용 등으로 인한 심뇌혈관질환의 위험을 증가시키는 것으로 알려져 있다. 하지만, 클로자핀이 심뇌혈관질환의 발생에 중요한 영향을 미치는 또 다른 요소인 혈소판 활성 정도에 어떤 영향을 미치는지에 대해서는 알려진 바가 거의 없다. 본 연구에서는 클로자핀 투여 전후 평균혈소판요소 (mean platelet component, MPC) 값 비교를 통하여 조현병 환자에서 클로자핀이 혈소판 활성에 미치는 영향을 알아보고자 하였다. 방 법 2003년 9월 1일부터 2007년 4월 30일까지의 기간 동안 건양대학교 병원 정신건강의학과에서 클로자핀을 새롭게 투여 받기 시작한 환자들을 대상으로 의무기록을 후향적으로 검토 하였다. 최종 통계 분석에는 14명이 포함되었다. 평균혈소판요소는 Bayer ADVIA $120^{(R)}$ system를 이용하여 측정하였다. 결 과 14명의 연구 대상자 중, 남성은 4명(28.60%), 여성은 10명(71.40%)이었으며, 평균 나이는 $37.50{\pm}11.64$세였다. 유병 기간은 평균 $91.00{\pm}93.96$개월 이었으며 피험자들이 마지막 혈액 검사를 한 시점에 복용하였던 클로자핀 용량의 평균은 $337.50{\pm}109.52mg$이었다. 클로자핀 투여 전과 투여 후의 평균혈소판요소 값은 각각 $26.12{\pm}2.22g/dL$$25.14{\pm}2.08g/dL$ 이었다. 윌콕슨 부호-순위 검정에서 클로자핀 투여 후 MPC 값이 유의미하게 감소하였다(V=16, p=0.024). 결 론 본 연구의 결과는 클로자핀 투여가 혈소판을 활성화하며, 이로 인해 혈전색전성 질환 등의 발생에 부정적인 영향을 미칠 수 있음을 시사한다. 또한 클로자핀 투여 시 혈소판 활성에 대한 평가를 포함하여 뇌혈관질환의 위험성에 대한 주의 깊은 모니터링이 필요함을 시사한다.

텍스트 마이닝과 의미 네트워크 분석을 활용한 뉴스 의제 분석: 코로나 19 관련 감정을 중심으로 (Analysis of News Agenda Using Text mining and Semantic Network Analysis: Focused on COVID-19 Emotions)

  • 전 세계적으로 퍼진 코로나 19 상황은 우리의 일상생활의 많은 부분에 영향을 끼쳤을 뿐만 아니라, 경제·사회 등 많은 부분에 걸쳐 막대한 영향력을 미치고 있다. 확진자와 사망자 수가 증가함에 따라 의료진과 대중은 불안, 우울, 스트레스 등 심리적인 문제를 겪고 있다고 한다. 장기적인 부정적인 감정은 사람들의 면역력을 감소시키고 신체적인 균형을 파괴할 수도 있으므로 코로나 19로 인한 심리적인 상태를 이해하는 것이 필수적인 상황이다. 본 연구에서는 코로나 19 감정과 관련된 뉴스 데이터를 수집하여, 텍스트 마이닝을 통해 키워드를 분류하고, 키워드 사이의 의미 네트워크 분석을 통해 단어들의 관계를 시각화하였다. 코로나 감정과 관련된 기사의 키워드에 나타난 단어들의 빈도수를 확인하고 이를 워드 클라우드로 분석하였다. 키워드 빈도 분석 결과 코로나 19 감정과 관련하여 '중국', '불안', '상황', '마음', '사회', '건강'과 같은 단어의 빈도가 높게 나타난 것을 확인할 수 있었다. 각 데이터 간 연결 중심성을 분석한 결과 키워드 중심성 네트워크에서 가장 중심적인 핵심어는 '심리'와 '코로나 19', '블루', '불안'이라는 단어가 높은 연결 중심성을 가지는 것을 확인할 수 있었다. 기사의 헤드라인에 나타난 주요 핵심어 사이의 동시 출현 빈도 네트워크를 그래프로 시각화한 결과, '코로나-블루' 쌍이 가장 굵게 표시되었고, '코로나-감정', '코로나-불안' 쌍이 비교적 굵은 선으로 표시된 것을 알 수 있었다. 코로나와 관련된 '블루'는 우울증을 의미하는 단어로, 코로나와 우울증은 이제 관심을 가져야 할 키워드임을 확인할 수 있었다. 본 연구에서는 장기화한 코로나 19 상황에서 신체적인 방역뿐만 아니라 심리적인 방역에도 힘써야 할 이 시기에 보건 정책담당자가 빠르고 복잡한 의사결정 과정에 도움이 되고자 미디어 뉴스를 모니터링 함으로써, 더욱더 쉬운 소셜 미디어 네트워크 분석 방법을 제시하고자 한다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. 소아 천식에서 호기산화질소와 폐기능 검사의 관계 (Relationship between exhaled nitric oxide and pulmonary function test in children with asthma)

  • 목 적 : 천식의 특징은 기도염증의 결과인 가역적인 기류제한과 기관지과반응성이다. 최근에 호기산화질소(exhaled nitric oxide; FeNO)이 비침습적으로 기도염증을 알 수 있는 지표로서 이에 대한 많은 연구가 있었다. 또한 FeNO는 천식의 중증도를 나타내는 데 이용되는 객담내 호산구수나 메타콜린 기도유발시험과 관련이 있다고 보고되었다. 이에 저자들은 간헐성 천식환자에서 폐기능검사와 호기산화질소와의 관계를 알아보고자 본 연구를 시행하였다. 방 법 : 2005년 4월부터 8월까지 경희의대부속병원 소아과에서 천식으로 진단되어 폐기능검사와 호기산화질소 측정이 가능하였던 5세부터 15세의 80명의 환자를 대상으로 하였다. 최근 4주내에 상기도 감염이 없었고, 부신피질스테로이드를 포함한 조절약물을 사용하지 않은 경우에 연구 대상에 포함하였다. 폐기능검사는 Microspirometer를 사용하였다. Eco Medics사의 Chemiluminescence NO-analyzer(CLD 88 sp, Duernten, Switzerland)를 사용하여 FeNO를 측정하였으며 폐기능 검사와 FeNO와의 상관관계 분석은 Spearman correlation coefficient를 사용 하였다. 결 과 : 1초간 강제호기량(forced expiratory volume in 1 second, $FEV_1$)의 평균은 $0.890{\pm}0.455L$이었으며 강제폐활량 (forced vital capacity; FVC)의 평균은 $1.071{\pm}0.630L$이었다. $FEV_1%pred$의 평균은 $98.39{\pm}34.27%$였으며 $FEV_1/FVC$의 평균은 $88.53{\pm}19.49$이었다. FeNO의 평균치는 16.88 parts per billion(ppb)이었다. FeNO 측정치는 $FEV_1$(r=0.345, P<0.01), FVC(r=0.244, P<0.05)와 유의한 상관관계를 보였으나 $FEV_1%pred$$FEV_1/FVC$는 FeNO와 유의한 상관관계를 나타내지 않았다. 결 론 : FeNO의 측정은 폐기능 검사와 함께 소아천식 환자의 관리에 유용하게 사용될 수 있는 신빙성있는 검사로 생각되며 측정 방법이나 측정치의 참고치 설정에 앞으로 많은 연구가 필요할 것으로 생각된다.

중환자의 욕창 예방 연구 : 욕창 예방 QI팀을 중심으로 (CQI Action Team Approach to Prevent Pressure Sores in Intensive Care Unit of an Acute Hospital Korea)

  • Background : A pressure sore was defined as any skin lesion caused by unrelieved pressure and resulting in damage to underlying tissue. The health care institutions in the United States were reported the incident rate of pressure sores ranging from 6 to 14 %. Intensive Care Unit needed highest quality of care has been found over 40% incidence rate of pressure sore. Also, Annual expenditures for the care of pressure sores in patients in the United States have been estimated to be $7.5 billion; furthermore, 50 percent more nursing time is required to care for patients with pressure sore in comparison to the time needed to implement preventive measures against pressure sore formation. However, In Korea, there were little reliable reports, or researches, about incidence rates of pressure sore in health care institution including intensive care unit and about the integrated approach like CQI action team for risk assessment, prevention and treatment of pressure ulcers. Therefore, this study was to develop pressure sore risk assessment tool and the protocol for prevention of pressure sore formation through CQI action team activities, to monitor incident rate of pressure sore and the length of sore formation for patients at high risk, and to approximately estimate nursing time for sore dressing during research period as the effect of CQI action team. Method : CQI action team in intensive care unit, launched since early 1996, reviewed the literature for the standardized risk assessment tool, developed the pressure sore assessment tool based on the Braden Scale, tested its validity, compared on statistics including incidence rate of pressure sore for patients at high risk. Throughout these activities, CQI action team was developed the protocol, called as St. Marys hospital Intensive Care Unit Pressure Sore Protocol, shifted the emphasis from wound treatment to wound prevention. After applied the protocol to patients at high risk, the incident rate and the period of prevention against pressure development were tested with those for patients who received care before implementation of protocol by Chi-square and Kaplan-Meier Method of Survival Analysis. Result : The CQI action team found that these was significant difference of in incidence rate of pressure sores between patients at high risk (control group) who received care before implementation of protocol and those (experimental group) who received it after implementation of protocol (p<.05). 25% possibility of pressure sore formation was shown for the patients with 6th hospital day in ICU in control group. In experimental group, the patients with 10th hospital day had 10% possibility of pressure sore. Therefore, there was significant difference(p<.05) in survival rate between two groups. Also, nursing time for dressing on pressure sore in experimental group was decreased as much as 50% of it in control group. Conclusion : The collaborative team effort led to reduced incidence, increased the length of prevention against pressure sore, and declined nursing care times for sore dressing. However, there have had several suggestions for future study. The preventive care system for pressure sore should be applied to patients at moderate, or low risk throughout continuous CQI team activities based on Bed Sore Indicator Fact Sheet. Hospital-wide supports, such as incentives, would be offered to participants for keeping strong commitment to CQI team. Also, Quality Information System monitoring incidents and estimating cost of poor quality, like workload (full time equivalence) or financial loss, regularly in a hospital has to be developed first for supporting CQI team activities as well as empowering hospital-wide QI implementation. Being several limitations, this study would be one of the report cards for the CQI team activities in intensive care unit of an acute hospital and a trial of quality improvement of health care in Korea.
