Group Performance and the Team Learning Climate as Perceived by Hospital Nurse (임상간호사가 인지한 팀학습분위기와 집단성과)

  • Purpose: To investigate the influence of a team learning climate on group performance of hospital nurses. Method: The subjects were 386 nurses who have been working in six hospitals. The data were collected by a structured questionnaire from January 20 to April 30 of 2006. The data were analyzed by SAS version 8.2, including descriptive statistics, Pearson correlation coefficient, and stepwise multiple regression. Results: The mean score of group performance was 3.38 and team learning climate was 4.89. The group performance was positively correlated with team learning climate(r=.40, p<.0001). The team learning climate explained 15% of the variance in group performance. Conclusion: The findings showed that team learning climate was an important factor in enhancing group performance in nursing organization. Therefore, the nurse manager will establish the strategies to improve the team learning climate of the nurses in order to promote organizational performance.

A research on the management condition of group homes for the elderly and their living environment (고령자 그룹홈의 운영과 거주자의 생활 실태)

  • To investigate the actual management condition of group homes for the elderly and their living environment, four group homes and 26 elderly people living in the group home were surveyed. As the results, the relationships between the administrator and the elderly were acceptable, but the group homes seem suffer from insufficient administrative supports. The most required in the group homes was a conversational partner service. Moreover, the most satisfactory point for the elderly living in group homes was the settlement of food, clothing, and housing. The unsatisfactory things were the service related to medical, leisure, and house work and an invasion of their private life. The resident elderly in group homes were fully satisfied with their living environment including the relationship with the manager and the people in the facility. They, however, do not have much chance to make an exchange with their neighbor. Overall, the elderly want to stay continuously in the group homes where they are living current.

Efficacy of probiotics for managing infantile colic due to their anti-inflammatory properties: a meta-analysis and systematic review

  • Background: Infantile colic (IC) is excessive crying in otherwise healthy children. Despite vast research efforts, its etiology remains unknown. Purpose: Most treatments for IC carry various side effects. The collection of evidence may inform researchers of new strategies for the management and treatment of IC as well as new clues for understanding its pathogenesis. This review and meta-analysis aimed to evaluate the efficacy and possible mechanisms of probiotics for mananaging IC. Methods: Ten papers met the study inclusion and exclusion criteria, and the meta-analysis was conducted using Review Manager (RevMan) software and a random-effects model. Results: This meta-analysis revealed that probiotics are effective for treating infantile colic, while the review showed that this efficacy may be due to their anti-inflammatory effects. Conclusion: Probiotics may be an important treatment option for managing infantile colic due to their anti-inflammatory properties.

A Study on Health Care Activities of Some Industrial Nurses and their Related Factors in Kyungnam Area (경남지역 일부 산업간호사의 보건관리 업무 및 관련요인에 관한 연구)

  • The purpose of this study was to assess the performance of the role and function of some industrial nurses and to characterize the factors affecting the performance of their activities. Thus the results could be used to suggest the direction in the performance of industrials nurses' activities effectively. During a period from January 10 to March 31, 1994, the data were collected from 87 industrial nurses, who were working as health managers in the plants, in Ulsan city and the vicinity in Kyungnam province, using a structured questionnaire. The results were as follows : 1. The general characteristics of industrial nurses in this study were 82.8% being 30 years old or less, 60.9%, being not married, and 93.1% having eduction levels above junior college. 2. With respect to general work conditions, 94.3% were working in a separate room provided for health care division, 40.2% working under the safety and health department, and 98.9% working as common-level staffs. And 60.9% were working less than 44 hours a week, 70.1% had work experiences less than 5 years, and 50.6% had annual incomes ranging 10 to 14 million wons. 3. As work conditions related to health care activities, 49.4% performed the activities not related to health care as always or occasionally, and 87.4% answered that occupational physicians were appointed in their plant and among them, however, only 6.9% worked on full-time basis and 52.8% perform little activities as occupational physicians. For a decision related to health care activity, 69.0% discussed the problems with the supervisors, and 19.5% made decisions by themselves. 4. As for attitude and perception to their activities as health managers, 66.7% moderately recognized the importance of health manager in the workplace, with 63.2% being satisfied their wages and treatment from the company, 57.5% being satisfied with their job positions and 51.7% having positive attitudes as being health managers. 5. The degree of performance at least in one of health related activities were very high in activities such as general medical care(100%), general health examination(98.0%) and specific health examination(100%), and relatively high in health education(72%), new employee health examination(60.9%), document handling(79.3%) and activity for work environment(70.1%). However, the performance rate was very low in preparing protective equipment (20.8%). 6. The levels of activities related to health care were significantly high when making decisions by themselves, when occupational physicians not being full-time, and when satisfying their job positions, and, on the other hand, significantly decreased as work hours increased. 7. In addition to some kinds of periodic education asked by all of the nurses, 89.7% wanted a specialized licensing system for industrial nurse, and 97.4% wanted to apply for the license test. As a conclusion, it is suggested that industrial nurses should be given more authority and placed in more self-controlled system to perform health care and other activities more efficiently, and the role and function of the occupational physician should be clearly distinguished from that of the industrial nurse as a health manager to avoid an unnecessary overlapping.

Effectiveness of Medical Aid Case Management in Excessive Healthcare User by Interventions based on the Number of Accesss (의료급여 과다이용자의 중재방법별 접근횟수에 따른 사례관리 효과)

  • This study analyzed medical aid case management effects based on the number of access by visits, phone calls, letters, internal investigation, resource links, and requests, in medical aid case management business that is carried out to improve the quality of life of medical care beneficiaries and reduce financial cost. Secondary analysis study using data of 564 high-risk group in the medical aid case management and their healthcare utilization data. Letters had positive correlations with the scores of all case management domains. The higher visits was, the lower the score of self-health care ability was, and the higher the number of phone calls was, the higher the score for reasonable medical use was. While there was no significant difference in medical cost according to aid management by interventions, the higher visits and resource links were, the lower the subjects' total number of payment days was. There is a difference in the various areas of the quality of health -related life and medical use depending on case management by intervention method and its number. It is necessary to carry out the efficient number of access to case management by intervention method.

Transplantation of human umbilical cord mesenchymal stem cells optimized with IFN-γ is a potential procedure for modification of motor impairment in multiple sclerosis cases: a preclinical systematic review and meta-analysis study

  • Stem cells transplantation (SCT) is known as a newfound strategy for multiple sclerosis (MS) treatment. Human umbilical cord mesenchymal stem cells (hUCMSCs) contain various regenerative features. Experimental autoimmune encephalomyelitis (EAE) is a laboratory model of MS. This meta-analysis study was conducted to assess the overall therapeutic effects of hUCMSCs on reduction of clinical score (CS) and restoration of active movement in EAE-induced animals. For comprehensive searching (in various English and Persian databases until May 1, 2024), the main keywords of "Experimental Autoimmune Encephalomyelitis", "Multiple Sclerosis", "Human", "Umbilical Cord", "Mesenchymal", and "Stem Cell" were hired. Collected data were transferred to the citation manager software (EndNote x8) and duplicate papers were merged. Primary and secondary screenings were applied (according to the inclusion and exclusion criteria) and eligible studies were prepared for data collection. CS of two phases of peak and recovery of EAE were extracted as the difference in means and various analyses including heterogeneity, publication bias, funnel plot, and sensitivity index were reported. Meta-analysis was applied by CMA software (v.2), P<0.05 was considered a significant level, and the confidence interval (CI) was determined 95% (95% CI). Six eligible high-quality (approved by ARRIVE checklist) papers were gathered. The difference in means of peak and recovery phases were -0.775 (-1.325 to -0.225; P=0.006; I2=90.417%) and -1.230 (-1.759 to -0.700; P<0.001; I2=93.402%), respectively. The overall therapeutic effects of SCT of hUCMSCs on the EAE cases was -1.011 (95% CI=-1.392 to -0.629; P=0.001). hUCMSCs transplantation through the intravenous route to the animal MS model (EAE) seems a considerably effective procedure for the alleviation of motor defects in both phases of peak and recovery.

Implementation of Service Model to Exchange of Biosignal Information based on HL7 Fast Health Interoperability Resources for the hypertensive management (고혈압 관리를 위한 헬스레벨 7 FHIR 기반 생체정보 교환 서비스 모델 구현)

  • Hypertension is one of the major causes of death in the world as it is related with cardiovascular or cerebrovascular disease, so it is needed to provide continuos management for blood pressure. This study selected Health Level 7 Fast Health Interoperability Resources (HL7 FHIR) as a bio-signal data exchange service model that can provide constant blood pressure management in the rapidly growing mobile health care environment. The HL7 FHIR framework developed communicates with the IEEE 11073-10407 Personal Health Device (PHD) protocol through the bluetooth Health Device Profile (HDP) between the manager (smart phone) and the agent (hemomanometer) and acquires information about blood pressure. According to the test results, it performed its tasks successfully including hypertension patients' blood pressure monitoring, management on measured records, generation of document, or transmission of measured information. Because in the actual, clinical environment, it is possible to transmit measured information through the TCP/IP protocol, it will be needed to conduct constant research on it and vitalize it in the field of mobile health care afterwards.

The Implementation of Database Building System for Korean Medical Paper Database (한의학술논문 데이터베이스 구축을 위한 입력 및 검수 시스템 개발)

  • Objectives : KIOM(Korean Institute of Oriental Medicine) built up korean medical paper database and services it through information portal OASIS. The database are updated about 1,600 papers and 48,000 references annually. Because lots of manpower and time are needed to update database, it is very important to raise up efficiency and quality of it. Methods : In this paper, we implemented web based database building system utilizing pre-built OASIS' database to improve the working process, data quality and ease of management. Results : First we designed and implemented web based system to input bibliography of the paper efficiently. It raised efficiency using OASIS' paper and reference database. Second we improved the refining process using web based system to raise up data quality. And third we developed the manager functions of web based system to control and check the working process. Conclusions : If we add korean medical dictionary and link outside paper database in the future, we hope that work efficiency and data quality will be raised more. And because the database schema of OASIS system and developed system are different, we are implementing the data transformation system.

An Analysis of Factors Affecting Quality of Life through the Analysis of Public Health Big Data (클라우드 기반의 공개의료 빅데이터 분석을 통한 삶의 질에 영향을 미치는 요인분석)

  • In this study, we analyzed public health data analysis using the hadoop-based spack in the cloud environment using the data of the Community Health Survey from 2012 to 2014, and the factors affecting the quality of life and quality of life. In the proposed paper, we constructed a cloud manager for parallel processing support using Hadoop - based Spack for open medical big data analysis. And we analyzed the factors affecting the "quality of life" of the individual among open medical big data quickly without restriction of hardware. The effects of public health data on health - related quality of life were classified into personal characteristics and community characteristics. And multiple-level regression analysis (ANOVA, t-test). As a result of the experiment, the factors affecting the quality of life were 73.8 points for men and 70.0 points for women, indicating that men had higher health - related quality of life than women.

An Implementation of Context-Awareness Support System based on Voice Service for Medical Environments (의료 환경을 위한 음성 서비스 기반의 상황인식 지원 시스템의 구현)

  • As computing environments are more rapidly developed, an adaptive and intelligent services using post PC such as PDA. Laptop, and Tablet PC in case of rounding and examining patients are highly demanded. The objective of this study is to design and implement a context-awareness support system based on voice service for medical environments. To achieve it, we propose a context middleware which plays an important role in recognizing a client with PDA by using a Bluetooth wireless communication technology as well as in executing an appropriate execution module, like delivery for diagnosis information of patients, according to the staff's context acquired from a context server. In addition, the context server functions as a manager that efficiently stores context information such as client's current status, physical environment, and device resources into a database server. Finally, for verifying the usefulness of the proposed system, we develop an application system which provides voice playing services for notification of other physicians through our context middleware.
