Analysis of the propensity of medical expenses for auto insurance patients by type of medical institution (의료기관 종류별 자동차보험 환자의 진료비 성향 분석)

  • This study aims to provide basic information necessary to find an efficient management plan for patients using auto insurance. The analysis was conducted on the five-year auto insurance medical expenses review data registered in the health care bigdata Hub from 2016 to 2020. As a result of the analysis, the number one composition ratio of auto insurance inpatient treatment expenses was treatment and surgery fees for Certified tertiary hospitals, hospitalization fees for general hospitals, hospitals and clinics, and treatment and surgery fees for oriental medical institutions and dental hospitals. outpatient treatment expenses was doctor's fee for medical institution, treatment and surgery fees for oriental medical institutions and dental hospitals. The ratio of medication, anesthesia, and special equipment significantly affected the cost of inpatient. And the ratio of physical therapy significantly affected the cost of outpatient.

  • This paper provides the design of system software for the management of radiation dose that is generated by using computerized tomography(CT). Recently, the radiation leakage incident of Japanese nuclear power plant was in the news internationally and there is a growing interest not only in nuclear power plant but in medical radiation exposure. In spite of the fact that currently safety management of radiation is under control only the workers of the radiation involved, now the exposure management of patients have been required. As surgery and inspections using the radiation have increased, this medical radiation exposure is increasing too. But it is a real situation that medical institutions don't know the level of radiation exposure applied to the patient. Therefore, a system for managing the radiation exposure of a patient from the medical institution is required. This paper proposes a design of a software program that manages the radiation exposure of CT which is a typical imaging tool to use the radiation in the medical institution. By check the amount of radiation dose and set the limit of dose, we would be of help to optimize the medical exposure of the patient.

  • When we are totalizing the lawsrelated to the medical, as the Laboratory medical testing is a kind of the medical act, it is the regulation that the medical technologist can analyze the specimen using in vitro diagnostic devices and diagnosticdrugs under the guidance of doctor or dentist from a corresponding medical institution and can report through verification and interpretation. However, in real medical fields, 'the guidance of doctor' is seriously in-sufficient or even the person who is not the medical technologist is executing. Furthermore the cases that produce inspection results with devices or reagents which are not validated nor approved have been frequently occurred. The result of Laboratory medical testing derived from this procedure can become the important information for the disease control of a country, and also can be decisive to the definite diagnosis and the prognostic monitoring about the patient disease. In spite of its significant medical act to be applicable to an unique proof with the related expert appraisal result in the medical mal-practice lawsuit, our reality in which the quality control is not properly working due by the costs and the labor shortage related to the Laboratory medical testing is quietly in bad condition. Even from now, the government should recognize the significance of the Laboratory medical testing and must achieve more strict administrative management as well as the law maintenance.

Comparative study on the Satisfaction difference of Korean and Western Medical Services (한양방 의료서비스 만족도 비교연구)

  • Objective : In South Korea, there are two kinds of medical systems that have Western medical institution and Korean medical institution. Patients who use medical services have different satisfaction in this environment. This study explores the satisfaction difference of korean/western medical services. Method : The data for this study were collected through a self-administered survey to 680 subject from patients who use medical services. frequency analysis, factor analysis, t-test, ANOVA were used for the statistical analysis. Results : The result of factor analysis, three factors were extracted. That was Information, Environment, Human services. Overall, the satisfaction scores of Korean medical service was higher than western medical service. Especially the human service satisfaction of the korean clinic was higher than western clinic. Conclusion : The satisfaction score of korean/western medical services is different. It is necessary to develop the medical management strategy and medical policy including korean medical system.

Classification on Patient Severity Score among Hemodialysis Patients (혈액투석 환자의 중증도 분류에 관한 연구)

  • Purpose: This study was to classify patient severity score for hemodialysis patients. Method: The subject of this study was 1,575 patients. To study the severity of the patients, we used t-test and ANOVA. The congruity was measured by Kappa coefficient and the severity in each medical facility was analyzed by ANOVA. Result: The results showed that there was a significant difference according to the levels of medical center (F=171.187, p<.0001). Categorizing the severity of the patients in each medical facility, group II and III of the secondary medical institution had higher ratio than the primary medical institution. There was not a single patient coming under group IV in both of the primary or secondary medical institutions. However, the tertiary medical institutions had more subjects in group II and III than the primary and secondary medical institutions. The group IV with the highest severity had 11 patients(1.5%), demonstrating that the tertiary medical institution had higher severity patients than the primary or secondary medical institutions. Conclusion: The results of this study appropriately reflects the repayment system of medical expenses by the government. Also, it provides the fundamental information to develop nursing fee system taken into account of the systemic differences among the primary, secondary and tertiary medical institutions.

  • This study reviews the time while the homeless patients Length of Stay emergency medical institution according to their medical treatment when they visit a hospital and characteristics of pathogenesis to understand the related factors affecting the case. Such review aims at providing basic data and information on how to improve medical care services of our society. 691 homeless patients visited an emergency medical care institution in Chungnam-si for one year from January 1, 2012 until December 31, of the same year were surveyed. Methods adopted were the analysis of frequency, ANOVA, correlation analysis and multiple regression analysis was conducted by making an independent variable as a dummy. This study came to a conclusion that first of all a medical care institution is required to avoid negative awareness and it should provide the homeless patients with medical care of better quality, having emergency care support system. Second, as most of the homeless patients are in their 40 or 50's, they are still in the age of high productivity of our society. Therefore, proper policy should be established and managed by the government on the program for their returning to the society as well as providing them with better medical care and support.

  • Emergency medical information center performs role of medical direction about disease consult and pre-hospital emergency handling scheme work to people. Emergency medical information system plays a major role to be decreased mortality and disability of emergency patient by providing information of medical institution especially when emergency patient has appeared. But, various attacks as a hacking have been happened in Emergency medical information system recently. In this paper, we proposed security structure which can protect the system securely by detecting attacks from outside effectively. Intrusion detection was performed using rule based detection technique according to protocol for every packet to detect attack and intrusion was reported to control center if intrusion was detected also. Intrusion detection was performed again using decision tree for packet which intrusion detection was not done. We experimented effectiveness using attacks as TCP-SYN, UDP flooding and ICMP flooding for proposed security structure in this paper.

The Utilization of Health Service by Psychiatric Outpatients (정신과 외래환자들의 의료이용에 관한 연구)

  • Objective of the study represents experiences of medical utilization of Psychiatric Outpatients. This research draws on information obtained from the 2008 National-wide sampling study of the Korean Healthcare Panel(KHP) conducted. The results of our study suggest the significance of variables such as the gender(p<.001) in the social demographical characteristics, the medical security type(p<.016), medical institution utilization(p<.012) treatment type(p<.004) in the utilization factors. In the case of medical utilization cost, the financial support factor(p<.018) showed a significant relationship. Depending on the particularities of gender, medical security type, financial support, medical institution utilization, treatment type were determined through a multiple logistics regression analysis. Gender showed that, among Psychiatric outpatient age of 30-59 level was 5.358 times and 60 years and older, their the second medical examination level was 4.490 times higher than Psychiatric outpatients under the age of 29. Health insurance type showed for the others medical allowance was 6.712 times higher than job health insurance and the other treatment was 0.395 times lower than drug treatment. Psychiatric outpatients utilization are rooted in the thoughts and factor levels of the Psychiatric patients and must be understood from the Psychiatric outpatients's perspective. The point is not only to gauge the patients research during Psychiatric medical utilization. The important of social community mental healthcare services levels goes beyond the goal of enhancing healthcare.

A Study on Cooperative Treatment with Both the Western and Oriental Medical Department in C.V.A patients (뇌졸중 환자의 양${\cdot}$한방 협진에 대한 연구)

  • The primary purpose of this study was to provide the basic materials for C.V.A inpatients' actual use of medical service at the hospital with both the western and oriental medical department and the attitude on cooperative treatment. The results of this study were as follow: First, the subjects' general characteristics including job, monthly mean income and age made statistical differences to their pathological characteristics such as part of primary paralysis, detailed name of disease and cause of elicitation. Second, their general characteristics including religion and job produce statistical difference to their actual use of medical service, like medical institution form, term of treatment and type of medical institution at first-aid. Third, through the awareness of cooperative treatment system, the effect of C.V.A treatment and the shorten of the C.V.A treatment term were higher at oriental medical department inpatients and cooperative treatment serviced inpatients than western medical department inpatients and cooperative treatment non-serviced inpatients. Fourth, the biggest problem on current dual medical system is increase medical expenses and the biggest reason on not vitalized cooperative service is prejudice of both parts.

A study on the recognition of medical institution workers and the development of the certification system after electronic medical record certification (전자의무기록 인증 후 의료기관 종사자 인식과 인증제 발전을 위한 연구)

  • In this study, senior general hospital, general hospital medical institutions certified by the Electronic Medical Record System Management Portal in 2021 were surveyed on the recognition of changes in medical information management and EMR system functionality. Through verification, it was implemented to promote future development of the certification system and long-term development of the electronic mandatory record certification system. A total of 1,189 respondents were used for final analysis using structured questionnaires, and in particular, differences in recognition of certification systems and system functionality after EMR certification were verified by conducting average analysis and ANOVA. As a result of analysis, the electronic mandatory record certification system was confirmed to affect positive work changes and perceptions on medical institution workers, and after certification of the electronic mandatory record system, it showed operational effects in many ways. Based on the results of this study in the future, communication follow-up research is needed.
