Morbidity and Medical Facilities Utilization Patterns of High School Students in Urban and Rural Areas (도시(都市)와 농촌(農村), 고등학생(高等學生)의 상병(傷病) 및 의료이용(醫療利用) 양상(樣相))

  • This study was conducted to assess the morbidity and medical facilities utilization patterns of high school students in urban and rural areas, and to contribute to establishment of school health policies. A questionnaire survey was carried out for 1,979 of one boy's and one girl's high school in Pusan and 1,315 of one boy's and one girl's high school in Kyungnam province from March 27 to April 8, 1989. The summarized result is as follows. The number of students who were sick or injured in one month period was 378.0 of 1,000 students. One sick or injured student experienced 1.2 events on the average and thus the incidence rate was 453.2 per 1,000 students for a month. The morbidity rate of the urban area (550.8) was higher than that of the rural area (306.5) while the rate for girl students (561.9) was higher than that of the boy students (328.3) (P<0.01). Especially, the girl students (740.7) in the urban area showed two times higher morbidity rate than that of the girl students in the rural area. Out of all morbidity respiratory disease for 67.6 percent in urban area and 68.5 percent in rural area and it was followed by gastrointestinal disease and nervous & sensory diseases. The morbid conditions took place at school in 37.5 percent, and at home in 59.8 percent. The absence rate due to morbidity was 4.4 percent out of all morbid students. In addition, 73.5 percent of all morbid student utilized medical treatment, of which the pharmacy accounted for 53.8 percent and 57.5 percent in the urban and rural areas, respectively. The second most common facility utilized was hospital OPD. While there was no student who utilized the school nursing room in the rural area, 1.5 percent of the sick students utilized the school nursing room in the urban area. The most important in selecting medical facility was the distance. By low medical expense was more frequent cited reason for selecting medical facility in rural area (13.6 percent) than in urban area (3.2 percent). Mild illness accounted for 70.3 percent of the reasons for no treatment and 21.8 percent was due to the conflict between school hour and clinic hour. The morbid students mostly suffered from mild respiratory and gastrointestinal disease, and its incidence was the highest rate in the school. Although there was relation between the worry of absence and no-treatment, the school nursing room utilization of students was very insufficient. Therefore, it is required to activate the operation of the high school nursing room by utilizing the specialized personnel.

  • This study used the financial information of 29 Regional public hospitals registered in the HASPA. Regional public hospitals analyzed the ratio of operating financing by management activities and how they affect net profit. The results of the study, Regional Public Hospitals ratio of fund financing for management activities was 83.50 percent for medical revenues, 9.53 percent for Incidental Revenue of Medical, 4.54 percent for Contribution Revenue, 4.42 percent for Other non-medical profits, 1.21 percent for depreciation Expense and 0.73 percent for Amount of reserve for unique purpose project. As a result of examining the effect of financing by management operation on net profit, the ratio of operating funds by medical revenue, by incidental revenue of medical, by contribution revenue and by other non-medical profits had a positive impact on net profit of Regional public hospitals. And It has been confirmed that the ratio of amount of reserve for unique purpose project has a negative impact on net profit. Therefore, it is necessary for Regional public hospitals to manage costs in consideration of revenues and internal reserves.

Demographic and Clinical Characteristics of Patients with Restless Legs Syndrome in Spine Clinic

  • Objective : The restless legs syndrome (RLS) is a common disorder affecting up to 5% to 15% of the general population, in which the incidence increases with age, and includes paresthesia in the legs. The purpose of this study is to investigate the incidence of RLS in spine center and to review clinical manifestations of this syndrome and its current treatments. Methods : Over a period of a year, retrospective medical record review and lumbar magnetic resonance images were performed on 32 patients with RLS in spine clinic who were diagnosed by National Institutes of Health criteria. Affected limbs were classified as five. Two grading systems were used in the evaluation of neural compromises. Results : The incidence of RLS was 5.00% (32/639). There were 16 males (50%) and 16 females (50%). The median age at diagnosis was 55.4 years (range, 25-93 years). There are no correlation between the affected limbs of RLS and neural compromises on the lumbar spine. Conclusion : The RLS is a clearly common neurologic disorder of the limbs, usually the legs. The awareness of this syndrome can help reduce diagnostic error; thereby, avoiding the morbidity and expense associated with unnecessary studies or inappropriate treatments in RLS patients.

A Study on the Utilization Status of Community Living Facilities in Rural Area (농어촌지역 공동생활시설 이용실태 고찰)

  • The housing environment of the weak class in rural area is inferior, the elderly ratio is high, and senior citizens whose living by themselves are increasing. This study was to analyze the actual conditions of utilization status, characterization of residency, management method of the federation of construction associate's love house and local government's case, and usage of the village hall. For better study, we reviewed the literatures and conducted field surveys. The purpose of this study is to suggest a development plan of rural community living homes. The results of this study were as follows; First, it is important for proper site of rural community living home plans to select adjacent location in the surrounding medical and welfare facilities(including community facilities). Second, the living space for one person does not need such a space; about $33m^2$ is suitable. Third, it is important to use surplus area to make kitchen garden in the site planning. Also the architectural plans must be considered to minimize the maintenance costs for the sustainability of the house and winter heating expense problem.

  • The purpose of the study is to identify land use characteristics which have influence on residents' mental health. Land use and housing types indicators including green areas and parks were selected by analyzing the related environment and health researches. Research database for every variable from 33 neighborhoods in Jeonju city were established through National Health Insurance Corporation, where is available for getting mental and behavioral disabilities (F code) expenses. The relation between those indexes and mental health were analysed. The findings of this study are as follows : First, the higher price area like apartments and raw houses the residents lived in, the more expenditure of F-code was paid. It could be interpreted by the relatively high frequency of medical treatment and interests on the health. Second, the more green area except park were there, the more F-code expenditure was given, which could be explicated with the spatial co-relation between location of green area and apartments of Jeonju city. It was very high, and apartment element's affect on the F-code was much more than the green zone. Third, the nearer to park were the residents, the less F-code expenses was paid.

Trend and Prediction of Urban Family Expenditure for Health Care (도시 가계의료비 지출의 추이와 예측)

  • The increase of health care expenditures is an important problem in the almost countries. Also, suppression of the health care expenditures is an important problem in the health field of Korea since the national health insurance for total people in 1989. Thus, it is very important to grasp the change of the health care expenditures of family and proportions of the health care expenditures to total expenditures of family, because they are the basis of national health care expenditures in Korea. While the health care expenditures of urban family were increased during 1980-1993 by 12.8% annually, the total expenditures of urban family were increased by 14.8% annually. Consequently, the proportions of health care expenditures to total expenditures were decreased from 5.98% to 4.76%. The proportions of health care expenditure for 3 years to come were predicted to 4.75% in 1994, 4.67% in 1995, and 4.63% in 1996 by the time-series analysis. That is, it was predicted that they would be decreasing slowly. The product elasticity of health care expenditure was less than 1 in the multiple regression analysis. so the health care is normal good rather than superior good. Therefore, it seems that the household economy is able to bear the expense pursuing the improvement of quality of health care by actualizing the medical insurance fee.

  • We propose the various Telemedicine applications using collaborative PACS environment. Traditionally, almost all medical images have been printed on radiological films. These films are expensive to produce and easy to misplace and usually only one copy exists, thus limiting the number of persons who have simultaneous access to the images. But the merit of this system including PACS(Picture Archiving and Communications System ) is very many which are small depository, low maintenance expense, good efficience(storage, speed) and good flexibility but the development costs is higher. This research has been done creating multimedia-based Telemedicine system for home PCs in network environment. Most of Telemedicine used hardware and exclusive line of high cost as CATV broadcasting technique. But this system will contribute to change the Home-PC for better use of collaborations among doctor and patients through various modes of interactions in cyberspace.

  • This study is planned to gather necessary data for setting up a system on students' health care in the university. In order to obtain statistical data on the students' health care problems especially in 'depression' among the mental health problems of the students, 'The Beck Depression Inventory' by A. T. Beck and 'Zung's Self-Rating Depression Scale' by W. W. K. Zung were used for gathering numerical data of scale of depression. It is evident that we have to prepare for further medical examination and health care educations for several students. For these students, it is clear that they have problems not only in mental health but also in physical health. I have screened out the high scored students for comparison and analysis. And it disclosed that we have to build up a periodical and continuous 'Health Screening System' utilizing Health Questionnaire for both physical and mental aspects, which will bring up us the very exact result to pick out any person who has healthy problem among the enormous number of the students with handy, convenient and effective procedures but with the least expense and effort. Also it is my firm confidence that this system will be applicable for primary health care control of mass population in local community or any organization.

Developing the administrative model using the data mining technique for injury in National Health Insurance (데이터마이닝 기법을 활용한 국민건강보험 상해상병 관리모형 개발)

  • We developed the hybrid model coupled with predictive model and business rule model for administration of injury by utilizing medical data of the National Health Insurance in Korea. We performed decision tree analysis using data mining methodology and used SAS Enterprise Miner 4.1. We also investigated under several business rule for benefits (expense paid by insurer) and claims of injury in National Health Insurance Corporation. We can see that the proposed hybrid model provides a quite efficient plausible results.

  • Objectives : This study aimed to analyze the structure and the trend of utilization and expenditure for Korean Medicine (KM) in Korea. Methods : Data were drawn from the 2008-2013 annual Korea Health Panel (Version 1.2.2), a national representative sample. We combined the data of household members with the data of outpatient KM service use. The volume of KM use was estimated based on the frequency of use and co-payment. Results : The KM utilization rate slightly increased in recent years, and it is presumed to be resulted from the increase of elderly population. Most KM outpatient visits were being concentrated in treating musculoskeletal diseases, and the procedures used frequently were acupuncture, moxibustion, cupping, and physical therapy. The imbalance of KM use between lower income group and higher income group was deepening. Conclusions : To expand restricted disease areas KM covered, the more herbal prescriptions should be insured and the insured form of herbal medicines need to be diversified.
