성별에 따른 노인의 성인식 및 성교육 수요에 관한 연구 (A Study on Sexual Perception and Sexual-Education Demands of the Elderly According to Gender)

  • Objectives : The aim of this study is to suggest a direction for sexual education suitable for the aged by analyzing senior citizens' perception level and analyzing the demand for sexual education among the elderly, such as their feelings on the necessity of sexual education and the patterns of sex education. Methods : The population of this survey targeted elderly residents aged over 60 who are taking lectures at 4 university lifelong education centers that are located in Busan. The period of data collection was from May 10, 2013 to May 30, 2013. The methods of analyzing data were frequency analysis, t-test, and correlation analysis. Results : The general characteristic of the survey were that it included 60 men (41.1%) and 86 women(58.9%). As a result of the comparison of sexual perceptions and sexual-education demand according to gender, it was found that men had greater sexual perception and greater need for sexual education than did women. A correlation according to sexual-life perception and sexual-education demand showed a positive relationship and was statistically significant as well. Conclusions : This study will form a basis for formation of a direction for the development of sexual education programs.

Analysis of changes in dose distribution due to respiration during IMRT

  • Purpose: Intensity modulated radiation therapy (IMRT) is a high precision therapy technique that can achieve a conformal dose distribution on a given target. However, organ motion induced by respiration can result in significant dosimetric error. Therefore, this study explores the dosimetric error that result from various patterns of respiration. Materials and Methods: Experiments were designed to deliver a treatment plan made for a real patient to an in-house developed motion phantom. The motion pattern; the amplitude and period as well as inhale-exhale period, could be controlled by in-house developed software. Dose distribution was measured using EDR2 film and analysis was performed by RIT113 software. Three respiratory patterns were generated for the purpose of this study; first the 'even inhale-exhale pattern', second the slightly long exhale pattern (0.35 seconds longer than inhale period) named 'general signal pattern', and third a 'long exhale pattern' (0.7 seconds longer than inhale period). One dimensional dose profile comparisons and gamma index analysis on 2 dimensions were performed. Results: In one-dimensional dose profile comparisons, 5% in the target and 30% dose difference at the boundary were observed in the long exhale pattern. The center of high dose region in the profile was shifted 1 mm to inhale (caudal) direction for the 'even inhale-exhale pattern', 2 mm and 5 mm shifts to exhale (cranial) direction were observed for 'slightly long exhale pattern' and 'long exhale pattern', respectively. The areas of gamma index >1 were 11.88 %, 15.11%, and 24.33% for 'even inhale-exhale pattern', 'general pattern', and 'long exhale pattern', respectively. The long exhale pattern showed largest errors. Conclusion: To reduce the dosimetric error due to respiratory motions, controlling patient's breathing to be closer to even inhaleexhale period is helpful with minimizing the motion amplitude.

Activities of Upper Limb Muscles Related to the Direction of Elastic Tape Application in Healthy Adults: A Randomized Trial of Parallel-Aligned Versus Cross-Aligned Tape Application

  • The purpose of this study was to evaluate the differences in electromyographic (EMG) activities of upper limb muscles between cross- and parallel-aligned taping and to compare the effects of these 2 taping methods in healthy adults. Thirty subjects, who volunteered for this study, were tested under 3 taping conditions in random order: (1) no taping, (2) cross-aligned taping, and (3) parallel-aligned taping. EMG activities of the biceps brachii, triceps brachii, flexor carpi ulnaris, and extensor carpi radialis muscles were measured. All muscles showed significant differences in EMG activity among the 3 conditions (p<.05). In the post hoc test, biceps brachii and triceps brachii muscles showed significant differences in EMG activity between the no taping and the cross-aligned taping conditions and between the no taping and the parallel-aligned taping conditions. Additionally, the EMG activities of the flexor carpi radialis and extensor carpi radialis muscles appeared to be significantly different between the no taping and parallel-aligned taping conditions. These findings demonstrate that taping may be helpful for decreasing muscle activity, regardless of the direction of tape application. This study provides useful information to future researchers regarding the effects of taping on muscle activity.

우리나라 의사양성체제의 관점에서 본 의과대학 교육의 문제점과 개선방향 (Current Issues and Future Considerations in Undergraduate Medical Education from the Perspective of the Korean Medical Doctor Development System)

  • Observation of the current Korean medical education and training system shows that certain negative traits of unchangeable solidification engraft themselves so deeply into the overarching system that they are now hampering the state of the national health welfare. Focusing only on undergraduate medical education, we can point out some glaring side-effects that should be of concern to any stakeholder. For instance, a graduate can legally begin his career as an independent practitioner immediately after passing the licensing exam and return to the old stuck school-year system of 2-year-premedical and 4-year-medical programs where outcome-based and integrated curricula are incomplete and unsatisfactory. In terms of learning opportunities, the balance between patient care and public health, as well as that between in-hospital highly specialized practice and community-based general practice, has worsened. Every stakeholder should be aware of these considerations in order to obtain the insight to forge a new direction. Moreover, our medical schools must prepare our students to take on the global roles of patient care within the Fourth Industrial Revolution, health advocacy for the imminent super-aged society, and education and research in the bio-health industry, by building and applying the concept of academic medicine. We will need to invest more resources, including educational specialists, into the current undergraduate medical education system in order to produce proper outcomes, smart curriculum, innovative methods of teaching and learning, and valid and reliable monitoring and evaluation. The improved quality of undergraduate medical education is the starting point for the success of the national system for public health and medical care as a whole, and therefore its urgency and significance should be emphasized to the public. The medical society should go beyond fixing what is broken and usher in a new era of cooperation and collaboration that invites other health professionals, governmental partners, law-makers, opinion leaders, and the general public in its steps toward the future.

Direction of Tissue Contraction after Microwave Ablation: A Comparative Experimental Study in Ex Vivo Bovine Liver

  • Objective: This study aimed to investigate the direction of tissue contraction after microwave ablation in ex vivo bovine liver models. Materials and Methods: Ablation procedures were conducted in a total of 90 sites in ex vivo bovine liver models, including the surface (n = 60) and parenchyma (n = 30), to examine the direction of contraction of the tissue in the peripheral and central regions from the microwave antenna. Three commercially available 2.45-GHz microwave systems (Emprint, Neuwave, and Surblate) were used. For surface ablation, the lengths of two overlapped square markers were measured after 2.5- and 5-minutes ablations (n = 10 ablations for each system for each ablation time). For parenchyma ablation, seven predetermined distances between the markers were measured on the cutting plane after 5- and 10-minutes ablations (n = 5 ablations for each system for each ablation time). The contraction in the radial and longitudinal directions and the sphericity index (SI) of the ablation zones were compared between the three systems using analysis of variance. Results: In the surface ablation experiment, the mean longitudinal contraction ratio and SI from a 5-minutes ablation using the Emprint, Neuwave, and Surblate systems were 28.92% and 1.04, 20.10% and 0.53, and 24.90% and 0.45, respectively (p < 0.001). A positive correlation between longitudinal contraction and SI was noted, and a similar radial contraction was observed. In the parenchyma ablation experiment, the mean longitudinal contraction ratio and SI from a 10-minutes ablation using the three pieces of equipment were 38.60% and 1.06, 32.45% and 0.61, and 28.50% and 0.50, respectively (p < 0.001). There was a significant difference in the longitudinal contraction properties, whereas there was no significant difference in the radial contraction properties. Conclusion: The degree of longitudinal contraction showed significant differences depending on the microwave ablation equipment, which may affect the SI of the ablation zone.

임상병리사 인력의 수급전망과 정책방향 (Prospective Supply and Demand of Medical Technologists in Korea through 2030)

  • 본 연구는 임상병리사 인력의 인력의 수급전망을 추계하여 인력계획 수립에 필요한 정책자료를 제공하는 것을 목적으로 한다. 공급은 기초추계(baseline projection) 모형에 근거한 인구학적 방법(demographic method)을 이용하여 추계하였으며, 수요추계는 임상병리사가 검사하는 임상병리검사 건수를 이용하는 의료수요에 의한 방법을 적용하였다. 전반적인 임상병리사 인력수급 추계결과는 생산성의 시나리오에 따라 공급이 과잉되기도 하고 부족하기도 할 것으로 전망되었다. 이렇게 임상병리사의 수급 비교 결과는 임상병리사의 생산성 가정에 따라 달라지지만, 어느 시나리오를 선택할 것인가는 궁극적으로 정부의 정책방향에 따라 달라진다. 즉 임상병리사의 생산성을 현재보다 높게 채택하는지 혹은 낮게 책정하는지는 보험재정 여건 등을 고려해야 하는 정부 정책에 달려있는 것이다. 이에 본 연구에서 정부의 정책방향이 고려되지 않은 2012년 현재의 생산성을 기준으로 한 '생산성 시나리오 3'을 살펴보면, ARIMA모델을 적용한 수요시나리오를 중심으로 보면 근무일수에 따라 2030년에는 2821명에서 4,530명의 임상병리사 공급이 과잉될 것으로 전망된다. 이러한 공급과잉은 전체에서 차지하는 비중이 10%미만이기 때문에 크게 문제가 되지 않을 것으로 판단된다. 그러나 임상병리사사 취업률이 60%대인 점을 감안하면 미취업자를 활용하는 정책도 함께 고려해야한다. 이러한 대책으로는 미취업인력에 대한 취업기회를 확대하는 방향으로 나아가야 할 것이고, 이를 위해서는 보건소 검사실의 기능강화 및 임상병리사 정원증원 및 신분보장, 통원치료 환자를 위한 상설 검사체제 확립, 산업재해 분야 및 의원급 검사기능 강화, 무면허 검사요원의 통제, 해외인력수출 확대 등이 필요할 것으로 사료된다.

"의림(醫林)"지(誌)를 통해 살펴본 1950년대 동서의학 논쟁 (The Medical Dispute between Korean and Western Medicine in 1950s Examined through "EE LIM")

  • Objective: Although the government acknowledged Korean Medical Doctors as official health care providers since 1951, criticism regarding its modernity persisted. This paper focuses on the medical dispute between Korean and Western Medicine in 1950s risen from the problem of modernization of Korean Medicine. Method: This paper looked into the medical dispute between Korean and Western Medicine in 1950s through "EE LIM". "EE LIM" is a scholarly journal published by Korean Medical doctor Bae Won-Sik. Articles regarding the medical dispute between Korean and Western Medicine were selected, categorized and analyzed. Result: The main questions against Korean Medicine were: whether it was scientific or not, and whether there was a possibility of systematization. Several opinions were brought up on these issues. 1) Some people criticized Korean Medicine as being unscientific based on modern science. 2) Others emphasized the identity of Korean Medicine against the critics. 3) Another group of people wanted to study Korean Medicine using modern methods and obtain merits from both sides. Conclusion: The different understanding of the term 'science' led to diverse discussions on the direction of Korean Medicine's progression. The narrow meaning of science only considers Korean Medicine as an object of modern scientific and medical study. But if the meaning is broadened, containing empirical aspects, Korean Medicine can be included as a category of science based on its clinical utility.

전라남도 난임 여성 한방치료 지원 사업에 참여한 한의사를 대상으로 한 설문 연구 (Questionnaire Survey Analysis for Korean Medical Doctors within a Support Project of Korean Medical Treatment in Infertility)

  • Objectives: The research is aimed at identifying the necessity of the support project of Korean medical treatment in infertility by analyzing the outcome of those subject and providing data for Korean medical treatment in infertility by analyzing the medical treatment of Korean medical doctors involved in the project. Methods: 98 patients who participated in the support project of Korean medical treatment in infertility in Jeollanamdo and 57 Korean medical doctors who participated as therapists were surveyed. Patients were given four months of treatment at local Korean medical clinics. After the treatment was completed, we analyzed the characteristics of the patients' group. And we sent questionnaires to doctors to answer the message. The content of the questionnaire consisted of infertility treatments, a wish for support project. These data were analyzed by frequency and t-test. Results: Analysis of the answer to the questionnaire obtained data on Korean medical treatment in infertility. The most desired thing for future support program was the introduction of husband and wife care. Conclusions: This survey by Korean medical doctors gave us reference data for Korean medical treatment in infertility and the direction in which therapy business should proceed.

Long-Term Follow-Up Radiologic and Clinical Evaluation of Cylindrical Cage for Anterior Interbody Fusion in Degenerative Cervical Disc Disease

  • Objective : Various procedures have been introduced for anterior interbody fusion in degenerative cervical disc disease including plate systems with autologous iliac bone, carbon cages, and cylindrical cages. However, except for plate systems, the long-term results of other methods have not been established. In the present study, we evaluated radiologic findings for cylindrical cervical cages over long-term follow up periods. Methods : During 4 year period, radiologic findings of 138 patients who underwent anterior cervical fusion with cylindrical cage were evaluated at 6, 12, 24, and 36 postoperative months using plain radiographs. We investigated subsidence, osteophyte formation (anterior and posterior margin), cage direction change, kyphotic angle, and bone fusion on each radiograph. Results : Among the 138 patients, a minimum of 36 month follow-up was achieved in 99 patients (mean follow-up : 38.61 months) with 115 levels. Mean disc height was 7.32 mm for preoperative evaluations, 9.00 for immediate postoperative evaluations, and 4.87 more than 36 months after surgery. Osteophytes were observed in 107 levels (93%) of the anterior portion and 48 levels (41%) of the posterior margin. The mean kyphotic angle was $9.87^{\circ}$ in 35 levels showing cage directional change. There were several significant findings : 1) related subsidence [T-score (p=0.039) and anterior osteophyte (p=0.009)], 2) accompanying posterior osteophyte and outcome (p=0.05). Conclusion : Cage subsidence and osteophyte formation were radiologically observed in most cases. Low T-scores may have led to subsidence and kyphosis during bone fusion although severe neurologic aggravation was not found, and therefore cylindrical cages should be used in selected cases.

한국과 미국 치과대학의 의사소통 교육 비교 (Comparison of Communication Skills in Dental Education between Korean and United States Dental Colleges)

  • As the social interest in medical care has increased and the awareness and autonomy of patients have increased, the importance of communication skills with patients has been increasingly emphasized in providing high quality medical services. In medical education, such education has become an important area in medical school and lifelong medical education, with studies showing that communication skills can be improved through education and training, and that this learned communication skills can be maintained after becoming a doctor. The importance of communication education and research for dental college students is growing as practical examination on communication skills will also be introduced in dental license tests from 2021. Therefore, we aimed to find out the educational goals and educational methods of United States in which these studies are conducted and applied before our country, and compare them with Korean's current educational goals and methods, so that we can explore the desirable direction of education for dental college students in the future.
