한국 어린이 및 청소년의 폐환기능에 관한 연구 - 특히 표준치 예측 수식에 관하여 - (Studies on the Ventilatory Functions of the Korean Children and Adolescents, with Special References to Prediction Formulas)

  • The maximum breathing capacity (MBC) and the maximum mid-expiratory flow rate (MMF) are widely used in evaluation of the ventilatory function, among various parameters of pulmonary function. The MBC volume is the amount of gas which can be exchanged per unit time during maximal voluntary hyperventilation. Performance of this test, unlike that of single breath maneuvers, is affected by the integrity of the respiratory bellows as a whole including such factors are respiratory muscle blood supply, fatigue, and progressive trapping of air. Because of this, the MBC and its relation to ventilatory requirement correlates more closely with subjective dyspnea than does any other test. The MMF is the average flow rate during expiration of the middle 50% of the vital capacity. The MMF is a measurement of a fast vital capacity related to the time required for the maneuver and the MMF relates much better to other dynamic tests of ventilatory function and to dyspnea than total vital capacity, because the MMF reflects the effective volume, or gas per unit of time. Therefore, it is important to have a prediction formula with one can compute the normal value for the subject and the compare with the measured value. However, the formulas for prediction of both MBC and MMF of the Korean children and adolescents are not yet available in the present. Hence, present investigation was attempt to derive the formulas for prediction of both MBC and MMF of the Korean children and adolescents. MBC and MMF were measured in 1,037 healthy Korean children and adolescents (1,035 male and 1,002 female) whose ages ranged from 8 to 18 years. A spirometer (9L, Collins) was used for the measurement of MBC and MMF. Both MBC and MMF were measured 3times in a standing position and the highest values were used. For measurement, the $CO_2$ absorber and sadd valve were removed from the spirometer in order to reduce the resistance in the breathing circuit and the subject was asked to breathe as fast and deeply as possible for 12 seconds in MBC and to exhale completely as fast as possible after maximum inspiration for MMF. During the measurement, investigator stood by the subject to give a constant encouragement. All the measured values were subsequently converted to values at BTPS. The formulas for MBC and MMF were derived by a manner similar to those for Baldwin et al (1949) and Im (1965) as function of age and BSA or age and height. Nicotine Suppresses TNF-${\alpha}$ Expression in Human Fetal Astrocyte through the Modulation of Nuclear Factor-${\kappa}B$ Activation

  • Parkinson's disease (PD) progresses severely by a gradual loss of dopaminergic neurons in the substantia nigra (SN). Epidemiological studies showed that the incidences of PD were reduced by smoking of which the major component, nicotine might be neuroprotective. But the function of nicotine, which might suppress the incidences of PD, is still unknown. Fortunately, recently it was reported that a glial reaction and inflammatory processes might participate in a selective loss of dopaminergic neurons in the SN. The levels of tumour necrosis factor (TNF)-${\alpha}$ synthesised by astrocytes and microglia are elevated in striatum and cerebrospinal fluid (CSF) in PD. TNF-${\alpha}$ kills the cultured dopaminergic neurons through the apoptosis mechanism. TNF-${\alpha}$ release from glial cells may mediate progression of nigral degeneration in PD. Nicotine pretreatment considerably decreases microglial activation with significant reduction of TNF-${\alpha}$ mRNA expression and TNF-${\alpha}$ release induced by lipopholysaccharide (LPS) stimulation. Thus, this study was intended to explore the role of nicotine pretreatment to inhibit the expressions of TNF-${\alpha}$ mRNA in human fetal astrocytes (HFA) stimulated with IL-$1{\beta}$. The results are as follows: HFA were pretreated with 0.1, 1, and $10{\mu}g/mL$ of nicotine and then stimulated with IL-$1{\beta}$ (100 pg/mL) for 2h. The inhibitory effect of nicotine on expressions of TNF-${\alpha}$ mRNA in HFA with pretreated $0.1{\mu}g/mL$ of nicotine was first noted at 8hr, and the inhibitory effect was maximal at 12 h. The inhibitory effect at $1{\mu}g/mL$ of nicotine was inhibited maximal at 24 h. Cytotoxic effects of nicotine were noted above $10{\mu}g/mL$ of nicotine. Moreover, Nicotine at 0.1, 1 and $10{\mu}g/mL$concentrations significantly inhibited IL-$1{\beta}$-induced TF-${\kappa}B$ activation. Collectively, these results indicate that in activated HFA, nicotine may inhibit the expression of TNF-${\alpha}$ mRNA through the pathway which suppresses the NF-${\kappa}B$ activation. This study suggests that nicotine might be neuroprotective to dopaminergic neurons in the SN and reduce the incidences of PD.

Streptozotocin으로 당뇨병을 유발시킨 흰쥐의 지방세포에서 일어나는 Insulin-Sensitive Phosphodiesterase의 조절에 관한 연구 (Regulation of Insulin-Sensitive Cyclic Nucleotide Phosphodiesterase in Adipocytes of Streptozotocin-Induced Diabetic Rats)

  • Streptozotocin으로 당뇨병을 유발시킨 흰쥐를 모델로 하여 당뇨병으로 인한 인슐린의 antilipolytic action을 매개하는 insulin-sensitive cyclic nucleotide phosphodiesterase의 역할의 변화 가능성을 연구하였다. 흰쥐의 epididymal adipose tissue로부터 분리한 지방세포를 여러 약물과 toxin으로 전처치한 다음, insulin을 처치 또는 처치하지 않고 $37^{\circ}C$에서 15분 동안 incubation하였다. 그리고 나서 differential centrifugation으로 3 fractions로 분리한 다음 cAMP phosphodiesterase activity를 assay하였다. Insulin에 의한 PDE activities의 증가는 당뇨병군과 대조군 모두 crude microsomal (P2) fraction에서만 볼 수 있었다. P2 fraction을 2 nM insulin, $100\;{\mu}M$ isoproterenol, 또는 두 약물을 함께 처치하여 나타난 maximal effect는 두 군 모두에서 유의한 차이가 없었다. 그러나 basal PDE activities는 당뇨병균이 대조군에 비해 증가한 것으로 나타났다. 당뇨병군의 P2 fraction의 insulin-sensitive PDE activities는 $A_{1}$ adenosine receptor agonist 인 PIA에 의해서 영향을 받지 않은 반면, 대조군의 경우 PIA에 의해 basal PDE activities와 같게 감소하였다. 그리고 지방세포의 pertussis toxin에 대한 sensitivity는 당뇨병군이 대조군보다 더욱 민감하였다. 그러나 cholera toxin에 대한 sensitivity는 당뇨병군과 대조군 사이에 큰 차이를 볼 수 없었다. 이러한 결과로 보아 streptozotocin으로 당뇨병을 유발시킨 흰쥐의 지방세포에서, adenosine receptor와 같은 inhibitory receptor를 경유한 signalling의 감소는 $G_{i}$ proteins의 소실 또는 기능의 감소와 관련이 있으며, 또한 basal state에서 insulin-dependent PDE의 활성을 증가시키는 것으로 사료된다.

Effect of phosphorus application on appearance of algal water bloom and rice yield in rice-barley double cropping system

  • Algal communities are important to maintain the aquatic ecosystems function. Algae have short life cycles, they respond quickly to environmental change and their diversity and density can indicate and the quality of their habitat. The bloom forms before the rice seedings have emerged, it may present a physical barrier that prevents the seedlings from penetrating the floodwater. Wind may also move the algal bloom, pushing the young plants beneath the surface. Another harmful action develops when the water dries up and the algae form a layer at the bottom of the field. The layer envelops the seedlings, which are not yet deeply rooted, and drag them to the surface when the water is let in again. Soil utilization pattern can be the mail facter affecting soil physico-chemical properties, especially in soil phosphorus (P). Solid content of the algae culture solution increased with the increase in the nitrogen rather the phosphors concentration. Phosphoric acid was treated with conventional treatments (100-0%, before transplanting time-tillering stage), 50-50%, 0-100%, and un-treated. The herbicide was treated on the 7 DAT (day after transplanting). Green algae samples were collected 20 DAT. Total phosphoric acid was the highest at 0.06 in 50-50% treatment in 20 DAT. The amount of green algae was about twice (9.8 mg/20ml) that of un-treated. Total number of green algae was 54 species(Green algae 35 species, Euglena 9 species, Stone wheel 10 species). Among the phosphoric acid treatment methods, the number of occurrences of green algae were the highest with 39 species in 0-100%, followed by 50-50%, 28 species, conventional treatments, 22 species, non-treatment, 18 species, respectively. Rice Yield was not significantly different by phosphoric treatment time, but slightly higher than un-treated. The maximal algal biomass was observed about 2weeks or 1 month after transplanting; the subsequent decrease of the biomass was related to the consumption by grazers and to a deficient light under the rice canopy. Maximal algal growth was observed just before tillering. To estimate the suitable method of phosphorus application in puddled-soil drill seeding of rice, available phosphorus appearance of algal water bloom, and rice yield were investigated in paddy soil of rice-barley double cropping system.

반복적인 공격적 행동과 혈소판 $^3H$ Imipramine 결합의 상관관계 연구 (REPEATED AGGRESSIVE BEHAVIOR AND PLATELET $^3H$ IMIPRAMINE BINDING)

  • 반복적으로 공격적인 행동 양상을 보이는 사람들의 세로토닌 반응(serotonergic responsivity)이 정상인과 비교하여 차이가 있을 것인지를 알아보고자 연구를 실시하였다. 나이를 대조하여 공격성군(소년원 재소자 16명)과 대조군(의과대학생 17명)으로 구분한 연구대상에게, 여러 심리검사 척도를 이용하여 공격성의 심한 정도를 정의하고, 두뇌의 세로토닌 기능과 일치하는 것으로 알려진 혈소판의 이미프라민 결합을 측정하여 다음과 같은 결과를 얻었다. 1) 공격성군은 대조군에 비하여 공격척도상 신체적 공격성(physical aggression)의 평균이 모두 유의하게 높은 값을 나타내었다. 이외에도 공격성군은 대조군에 비해 충동성(impulsivity), 적대감(hostility), 정신증(psychoticism)등이 통계적으로 유의하게 높은 평균값을 보였다. 2) 공격성군은 대조군에 비하여 혈소판 이미프라민 최대결합부위 밀도(Bmax)가 낮은 경향을 보였다. 3) 공격성군과 대조군의 혈소판 이미프라민 결합의 결합상수(Kd) 값은 통계적으로 유의한 차이를 보이지 않았다. 4) 연구대상군을 전체로 하였을 때, 공격성척도(PFAV)와 갈등해결 척도(CTS)의 신체적 공격성의 심한 정도는 혈소판 이미프라민 최대결합부위 밀도(Bmax)와 통계적으로 유의한 정도의 역비례 상관관계를 보였다.

광조사교합분석법을 이용한 머리 위치와 교합접촉면적의 관계 (Relation of head posture and occlusal contact area using photo occlusion analysis)

  • 목적: 교합접촉면적에 관련된 이전의 연구들은 교합두께 수준이나 교합측정방법 상 부정확성이 한계로 지적되어 왔다. 본 연구의 목적은 머리 위치가 교합접촉면적에 미치는 영향을 광조사교합분석법을 이용하여 조사하는 것이다. 연구 재료 및 방법: 건전한 치열을 갖는 54명(남성 44명, 여성 10명, 23 - 33세)을 대상으로 시험을 진행하였다. 머리 위치와 교합접촉면적의 관계를 알아보기 위해 시험대상은 치과용 유닛체어에 세 가지 자세(누운 자세($0^{\circ}$), 기울인 자세($45^{\circ}$), 직립 자세($90^{\circ}$))로 하여 최대 교두간 접촉위를 이용하여 악간관계기록을 채득하였다. 광조사교합분석기($BiteEye^{(R)}$)를 이용하여 교합접촉면적을 분석하였다. SPSS 25.0을 이용하여 유의수준 95%에서 통계분석을 시행하였다. 결과:머리 위치는 교합접촉면적과 유의미한 영향이 없는 것으로 나타났다(P > 0.05). 결론: 머리 위치는 교합접촉면적에 유의미한 영향이 없으므로 교합관계가 안정적이라면 최대 교두간접촉위의 악간관계 기록 채득 시 환자의 자세는 변수가 되지 않으며 광조사교합분석기를 이용한 교합접촉면적의 측정은 재료적, 용이성 측면으로 유용하다고 할 수 있다.

취주악기를 사용한 음악치료가 경수손상환자의 호흡, 구강운동 및 조음에 미치는 영향 -사례연구- (The Effect of Wind-instrument Centered Music Therapy on Respiration, Oral Motor and Articulation for Patients with Cervical Cord Injury - Case Study -)

  • 본 연구는 호흡근의 지속적 훈련이 필요한 경수손상 환자에게 취주악기를 사용한 음악치료가 호흡, 구강 운동 및 조음에 미치는 영향에 대해 알아보고자 하였다. 시행되었던 프로그램의 참여자 중 경수 손상환자 군에 해당하는 3명의 결과를 선별하여 사후 데이터 분석 하였다. 프로그램에 사용할 취주악기로 호기와 흡기를 모두 활용하는 하모니카를 선택하였다. 기간은 총 6주간 주 1회 30 ~ 40분씩 시행하였고, 소그룹으로 진행 후 개별적으로 연습을 유지하도록 당일 진행내용이 자료로 제공되었다. 평가는 호흡관련 요소인 최대 호기량과 취주악기의 특징을 반영한 구강운동 그리고 조음검사로 시행하였고, 이에 대해 사전사후 결과를 비교하여 확인하였다. 분석결과 최대 호기량이 사전보다 평균 25ml 증가하였으며, 조음은 3.16점 증가하였다. 구강운동은 사전보다 사후에서 11.67점 움직임이 증가하였고, 세부항목 비교 시 입을 제외한 턱과 혀에 대한 점수에서 구강운동기능의 증가가 확인되었다. 이러한 연구결과를 토대로 본 연구는 취주악기를 사용한 음악치료가 경수손상 환자의 호흡재활 분야에 실천적이고 효과적 중재방안이 될 것으로 생각한다.

운동 후 극저온 냉각치료 적용이 운동유발성 근육 손상의 최대등척성근력과 관절가동범위에 미치는 효과 (The effect of whole-body cryotherapy intervention after an exercise on MVIC and ROM of EIMD)

  • Background: The purpose of this study was to investigate the effects of WBC (whole-body cryotherapy)on the MVIC (maximal voluntary isometric contraction)and active ROM (range of motion) recovery of after EIMD (exercise-induced muscle injury). Design: Randomized Controlled Trial. Methods: Thirty subjects who are student in their 20s at a university participated in this study, these subjects were assigned into three groups, a control group (n=10), experiment group I(n=10), and experiment group II (n=10). The subjects in the experimental group II were intervened by WBC (-130℃, 3 minutes) before induced EIMD, the experimental group were intervened by WBC (-130℃, 3 minutes) after induced EIMD, and the control group weren't by any intervened after induced EIMD. Results: First, In the comparison of the MVIC, there were significant variations with the lapse the time in three groups (p<.001) and there was a significant interaction of time and group (p<.001). In the among group comparison, the MVIC of experimental group II was significantly larger than those of other groups (p<.001). Second, In the comparison of the active extension angle, there were significant variations with the lapse the time in three groups (p<.001) and there was a significant interaction of time and group (p<.001). In the among group comparison, the active extension angle of experimental group II was significantly smaller than those of other groups (p<.001). Third, In the comparison of the active flexion angle, there were significant variations with the lapse the time in three groups (p<.01) and there was a significant interaction of time and group (p<.001). In the among group comparison, the active flexion angle of experimental group II was significantly larger than those of other groups (p<.05). Conclusion: The above results revealed that the WBC intervention after an exercise had a positive effect of muscle function after EIMD. Therefore we can consider the WBC as a considerable intervention method to prevent or reduce an exercise injury.

Three sesquiterpene lactones suppress lung adenocarcinoma by blocking TMEM16A-mediated Ca2+-activated Cl- channels

  • Transmembrane protein TMEM16A, which encodes calcium-activated chloride channel has been implicated in tumorigenesis. Overexpression of TMEM16A is associated with poor prognosis and low overall survival in multiple cancers including lung adenocarcinoma, making it a promising biomarker and therapeutic target. In this study, three structure-related sesquiterpene lactones (mecheliolide, costunolide and dehydrocostus lactone) were extracted from the traditional Chinese medicine Aucklandiae Radix and identified as novel TMEM16A inhibitors with comparable inhibitory effects. Their effects on the proliferation and migration of lung adenocarcinoma cells were examined. Whole-cell patch clamp experiments showed that these sesquiterpene lactones potently inhibited recombinant TMEM16A currents in a concentration-dependent manner. The half-maximal concentration (IC50) values for three tested sesquiterpene lactones were 29.9 ± 1.1 µM, 19.7 ± 0.4 µM, and 24.5 ± 2.1 µM, while the maximal effect (Emax) values were 100.0% ± 2.8%, 85.8% ± 0.9%, and 88.3% ± 4.6%, respectively. These sesquiterpene lactones also significantly inhibited the endogenous TMEM16A currents and proliferation, and migration of LA795 lung cancer cells. These results demonstrate that mecheliolide, costunolide and dehydrocostus lactone are novel TMEM16A inhibitors and potential candidates for lung adenocarcinoma therapy.

계획된 수술전 교육이 수술후 회복에 미치는 영향에 관한 임상실험적 연구 (An Experimental Study on the Effects of Structured Preoperative Teaching on Postoperative Recovery)

  • The purpose of this study was to test the effect of the structured preoperative teaching on post-operative recovery and to observe the effects of an structured preoperative teaching on the adult surgical patient's ventilatory function ability, the length of hospital stay, the number of analgesics within a 72 hour postoperative period, the length of early ambulation. The research question investigated in this study was: What would be the effects of a structured preoperative teaching upon the adult surgical patients postoperative recovery? This study was based on a sample of 40 patients who were scheduled for abdominal surgery. They were asssigned alternately to experimental and control group. Among 40 subjects, 20 were placed in the experimental group and 20 in the control group. Preoperative ventilation function testing of control and experimental subjects was done the evening before surgery and before the patient received the structured preoperative teaching. A structured preoperative teaching was given to the subjects in the exporimental group only by writer. Postoperative testing was done the 5th postoperative day. The data were collected over a period of two months, from Aug. 8 to Oct. 31, 1983. For the analysis of the data and test for the hypotheses, the t-test with mean difference was used. The results of this study regarding the four-hypotheses were as follows: 1. Experimental group which received structured preoperative eaching will have more increase to-cough and deep breathe as measured byhis forced vital capacity(FVC), forced expiratory volume 1 (FEV1), maximal voluntary volume 15 (MVV 15) than control group without structured preoperative teaching. The ventilation function ability was more increase in experimental group than in control group, the mean difference was statistically significant at 0.01 level. Hypotheses 1 was supported. 2. Experimental group with structured preoperative teaching will have more reduced the length of hospital stay than control group without structured preoperative teaching. The length of hospital stay of the experimental group and control group were 11.90 days and 16.05 days respectively. However, the difference was. not statistically significant at .05 level. Therefore the hypothese 2 was not supported. 3. Experimental group with structured preoperative teaching will have more reduce the number of analgesics within a 72 hour postoperative period than control group. The number of analgesics within a 72 hour' postoperative period of experimental group and control group were 1.65 times and 2.4 times. The difference was not statically significant at .05 level. Therefore, the hypotheses 3 was not supported. 4. Experimental group with structured preoperative. teaching will have more reduce the length of early ambulation than control group without structured preoperative teaching. The length of early ambulation of experimental group and control group were 2.2 days and 3.5 days respectively The difference was statistically signficant at 0.05 level. Thus the hypothess 4 was supported.
