폐결핵의 외과적 요법에 대한 임상적 고찰: 3,566예의 분석 (Surgical Management of Pulmonary Tuberculosis - A Review of 3,566 Cases -)

  • Between October 1964 and August 1979, 2,537 patients underwent operation for pulmonary disease including tuberculosis at the National Kongju Hospital. Ages at operation ranged from 6.6 to 67.6 years with a mean age of 30.8 years and the male to female ratio was 3.07: l. Indications for operation were, respectively, as follows; destroyed lobe or segment with /without cavity[68.6%], ipsilateral total destroyed lung[14.8%], empyema with/wit-hout fistula[6.0%], atelectasis with bronchostenosis[2.6%], bronchiectasis[2.4%], tuberculoma[1.6%], lung abscess[1.7%], etc. There were 1,416 thoracoplasties[most common], 915 lobectomies, 591 pneumonectomies, etc. 1,073 patients with hemoptysis were found, streaking[23.8%], mild[42.3%], moderate[26.6%], severe[3.4%] and massive[3.9%] as the severity. Tuberculosis was the most common cause of severe and massive hem-optysis. Postoperative complications were developed 16.% of the operated cases, bleeding was the most frequent complication found in these cases. The mortality of operation was 8%, the most common cause of death was bleeding also.

폐동정맥루 수술치험 1례 (Pulmonary arteriovenous fistula -One case report-)

  • Pulmonary arteriovenous fistula is unusually congenital malformation consisting of an abnormal connection between a pulmonary artery and vein. It may appear as an isolated anomaly or with hereditary hemorrhagic telangiectasis. In the classic form, symptoms and signs are caused by the abnormal right to left shunt. since severe clinical symptoms such as paradoxical embolism, infection, and spontaneous rupture with massive bleeding may occur, surgical treatment is often indicated. Recently we have experienced a surgical treatment of pulmonary arteriovenous fistula in 54 years old housewife. She was admitted in May 1986 with recurrent hemoptysis for 1 month. On admission, the PaO2 was 65.2 mmHg without specific findings. Pulmonary arteriovenous fistula was confirmed by preoperative pulmonary arteriography. A well circumscribed cystic mass was noted in visceral subpleural and inferior lingular segment of left upper lobe. Left upper lobectomy was performed with good results. Histologically angiomatous dilatation of abnormal vessels embedded in lung parenchyma was noted. Postoperative PaO2 was 90 mmHg. Postoperative results were good.

다량 객혈을 합병한 폐동정맥루 (Pulmonary Arteriovenous Fistula)

  • Pulmonary arteriovenous fistula is a congenital malformation of the pulmonary vasculature in which there is a persistence of one or more sizable communications that bypass the pulmonary capillary bed that thus directs unoxygenated pulmonary arterial blood directly into the pulmonary venous system. The developmental cause of pulmonary arteriovenous fistula is unknown, but it is postulated that the abnormal channels represent persistent vessels of the primitive splanchnic capillary bed that do not resorb during growth and maturation. We have experienced a case of pulmonary arteriovenous fistula. The 45 years old woman admitted to Kosin Medical College Hospital via emergency room because of massive hemoptysis, and was diagnosed pulmonary arteriovenous fistula by twice postoperative histopathologic examination. The postoperative course was uneventful and she was discharged on the 16th, second postoperative day in a healthy condition.

  • in general rapid and complete resolution of pulmonary emboli, even massive, is the natural history. However, rarely, the emboli do not resolve but rather became fibrotic organization and densely adherent to the arterial wall, therefore, may lead to significant clinical disability. In patients with chronic pulmonary embolism, medical management usually has little effect and only surgical treatment can offer improvement. The case was 30-year-old man who had admission to the Hanyang University Hospital due to fall-down from 11th floor 407 days before operation and then transferred to our department for surgical management under the diagnosis of chronic pulmonary embolism, Pulmonary angiogram demonstrated multifocal thromboembolism with infarction and lung scans showed no improvement in spite of anticoagulant and thrombolytic therapy. At median sternotomy for pulmonary artery thromboembolectomy, the well organized and multiple septic emboli could be removed by gallstone forceps. But reoperation of left upper lobectomy was performed because of the repeated hemoptysis and suspicious pulmonary arterio-bronchial fistula 19 days postoperatively. Despite of ventilatory support and drug treatment, the patient died due to right heart failure associated with cor pulmonale 27 days after first operation. Discussion of the operative and perioperative problems are offered.

폐 국균증에 대한 외과적 치료 (Surgical Treatment of Pulmonary Aspergillosis)

  • We experienced eleven cases of pulmonary aspergllosis treated surgically in the period from 1981 to 1992. There were 5 men and 6 women, ranging in age from 28 to 64 years [mean age 40.4 years]. The most common chief complaint of the patients was hemoptysis and blood tinged sputum[7 cases, 63.6%], On preoperativechest film, the case of cavity with fungus ball[7 cases] and only cavity[4 cases] were seen. The location of the lesion were both upper lobe[6 cases] and lower lobe[5 cases]. The underlying disease were tuberculosis[5 cases], bronchiectasis[2 cases], tuberculosis and bronchiectasis[1 case], pneumonia[1 case] and none[2 cases]. The operative procedures of pulmonary aspergillosis were lobectomy[8 cases], cavernostomy[1 case] and thoracoplasty[2 cases]. The postoperative complications were postoperative massive bleeding[reoperation, 2 cases], wound infection[2 cases] and no operative mortality.

  • To maintain an adequate airway in a patient with tracheobronchial narrowing coming from various causes, prosthetic tracheobronchial stents provide palliative treatment for narrowed airways where surgical resection is inadvisable. After insertion, previous reported complications were granuloma formation, dysphagia, suction catheter entrapment and fatal massive hemoptysis. We report, a case of complication associated with expandible metallic stent with endobronchial stenosis due to tuberculosis.

폐격리증 2례 보고 (Pulmonary Sequestration - Report of 2 cases -)

  • Pulmonary sequestration is applied to the congenital malformation characterized by an area of embryonic lung tissue that derives its blood supply from an anomalous systemic artery. Two cases of pulmonary sequestration were treated at the department of thoracic and cardiovascular surgery, college of medicine, Hallym University. One case was extralobar type, associated with the pneumothorax due to rupture of bulla. The other was intralobar type with symptom of massive and recurrent hemoptysis. The supplying arteries of both cases arose from the thoracic aorta. The venous return of the extralobar type was systemic into the hemiazygos vein, and that of the intralobar type was normal into the inferior pulmonary vein. Treatment for the former was resection of the sequestrated lung, and that for the latter was lobectomy of the left lower lobe. With the brief review of literature, we report the cases.

  • 기관지 정맥류는 기관지 정맥압이 상승하는 승모판 협착증이나 폐정맥 폐쇄증에서 이차적으로 발생할 수 있다. 기관지 정맥류는 주로 좌측 주기관지에서 관찰되며, 드물지만 기관지 정맥류의 파열로 대량 객혈이 가능하고 사망할 수도 있다. 저자들은 중증 승모판 협착증 환자에서 동반된 기관지 정맥류를 기관지내시경에서 우연히 관찰하였고 승모판치환술 후 기관지 정맥류가 호전되어 이를 문헌고찰과 함께 보고하는 바이다.

  • 저자들은 과거력상 반복되는 구강내 아프타성 궤양 및 결절홍반양 피부병변이 있었고, 객혈을 주소로 내원한 37세 남자환자에서 혈관조영술상 다발성 폐동맥류, 심부 정맥의 협착 및 폐색소견이 관찰되었고, 조직학적으로는 폐혈관염 소견을 보여, Hughes-Stovin 증후군으로 진단하였으며, 코일을 이용한 폐동맥색전술로 폐동맥류를 성공적으로 치료하였기에 문헌고찰과 함께 보고하는 바이다.
