Comparison of three-dimensional digital technique with two-dimensional replica method for measuring marginal and internal fit of full coverage restorations

  • PURPOSE. This study compared digital (reference point matching) and replica methods for measuring marginal and internal fit of full coverage restorations. MATERIALS AND METHODS. A maxillary left first molar typodent was fixed on to an aluminum base and prepared to receive all-ceramic full coverage restoration. The model was scanned with an intraoral scanner (CEREC Omnicam, Sirona, York, PA, USA). Twelve crowns were fabricated from lithium disilicate blocks (IPS emax CAD, Ivoclar Vivadent) and then crystalized. Marginal and internal fit of each restoration was measured by two examiners using replica and a new digital three-dimensional technique. Reliability between the two methods and two examiners was assessed by correlation and Cronbach's Alpha coefficient (P<.05). A Bland-Altman assessment for agreement was used to compare the two methods. RESULTS. Bland-Altman assessment showed that the mean of difference for marginal, absolute marginal, and axial gap was respectively -1.04 ㎛, -41.9 ㎛, and -29.53 ㎛ with limit of agreement (LOA) between -37.26 to 35.18 ㎛ for marginal, -105.85 to 22.05 ㎛ for absolute marginal and -80.52 to 22.02 ㎛ for axial gap. Positive correlation for repeatability (P<.05) in determining marginal and internal gaps by the two examiners in both techniques was revealed. Reliability of both techniques in all sites of measurements was at least good (0.8 ≤ α < 0.9). CONCLUSION. Both measuring techniques appeared highly reliable for evaluating fit of fixed dental restorations, while reference point matching provided higher values in axial and absolute marginal gap assessment.

MARGINAL FIT OF THE DIGIDENT CAD/CAM ZIRCONIA CERAMIC CROWNS

  • Statement of problem. There have been many studies about marginal discrepancy of single restorations made by various systems and materials. However most of the statistical inferences are not definite because of sample size, measurement number, measuring instruments, etc, and there have been few studies about the marginal fit of the Digident CAD/ CAM zirconia ceramic crowns. Purpose. The purpose of this study was to compare the marginal fit of the anterior single restorations made by using the Digident CAD / CAM zirconia ceramic crowns with metal-ceramic restorations and to obtain more accurate information by using a large enough sample size and by making sufficient measurements per specimen. Material and Methods. The crowns were made from one extracted maxillary central incisor pre-pared with a 1mm shoulder margin and $6^{\circ}$ taper walls by milling machine. The in vitro marginal discrepancies of the digident CAD / CAM zirconia ceramic crowns and control groups(metal ceramic crowns) were evaluated and compared. Twenty crowns per each system were fabricated. Measurements of a crown were recorded at 50 points that were randomly selected for marginal gap evaluation. Parametric statistical analysis was performed for the results. Conclusion. Within the limitations of this in vitro study, the following conclusions were drawn: 1. Mean gap dimensions and standard deviations at the marginal opening for maxillary incisal crowns were $88{\pm}10{\mu}m$ for the control (metal-ceramic crowns), $92{\pm}4{\mu}m$ for Digident CAD / CAM zirconia ceramic crowns. 2. Marginal gap between Digident CAD / CAM zirconia ceramic crowns and metal ceramic crowns did not show significant difference (P>.05). 3. The Digident CAD/ CAM zirconia ceramic crowns and metal ceramic crowns showed clinically acceptable marginal discrepancy.

Marginal and internal fit of 3D printed provisional crowns according to build directions

  • PURPOSE. This study aimed to fabricate provisional crowns at varying build directions using the digital light processing (DLP)-based 3D printing and evaluate the marginal and internal fit of the provisional crowns using the silicone replica technique (SRT). MATERIALS AND METHODS. The prepared resin tooth was scanned and a single crown was designed using computer-aided design (CAD) software. Provisional crowns were printed using a DLP-based 3D printer at 6 directions (120°, 135°, 150°, 180°, 210°, 225°) with 10 crowns in each direction. In total, sixty crowns were printed. To measure the marginal and internal fit, a silicone replica was fabricated and the thickness of the silicone impression material was measured using a digital microscope. Sixteen reference points were set and divided into the following 4 groups: marginal gap (MG), cervical gap (CG), axial gap (AG), and occlusal gap (OG). The measurements were statistically analyzed using one-way ANOVA and Dunnett T3. RESULTS. MG, CG, and OG were significantly different by build angle groups (P<.05). The MG and CG were significantly larger in the 120° group than in other groups. OG was the smallest in the 150° and 180° and the largest in the 120° and 135° groups. CONCLUSION. The marginal and internal fit of the 3D-printed provisional crowns can vary depending on the build angle and the best fit was achieved with build angles of 150° and 180°.

IN VITRO MARGINAL FIT OF THE COMPUTERAIDED MILLED CERCON CROWNS

Purpose. The purpose of this study was to compare the marginal fit of the anterior single restorations made using computer-aided milled Cercon crowns with metal-ceramic restorations and to obtain more accurate information by using a large enough sample size and by making sufficient measurements per specimen. Material and methods. The in vitro marginal discrepancies of computer-aided milled Cercon crowns and control groups (metal ceramic crowns) were evaluated and compared. The crowns were made from one extracted maxillary central incisor prepared by milling machine. 30 crowns per each system were fabricated. Measurements of a crown were recorded at 50 points that were randomly selected for marginal gap evaluation. Parametric statistical analysis was performed for the results. Results. The means and standard deviations of the marginal fit were 85$\pm$22$\mu$m for the control group and 91$\pm$15$\mu$m for the Cercon crowns. The t-test of the marginal discrepancies between Cercon crowns and metal-ceramic crowns were performed. Significant differences were not found between groups (P=0.230>.05). Based on the criterion of 120$\mu$m as the limit of clinical acceptability, the mean marginal fits of Cercon crowns and metal-ceramic crowns were acceptable. Conclusion. Within the limitations of this in vitro study, the following conclusions were drawn: 1. Mean gap dimensions and standard deviations at the marginal opening for maxillary incisal crowns were 85$\pm$22$\mu$m for the control (metal-ceramic crowns), 91$\pm$15$\mu$m for Cercon crowns. 2. The Cercon crowns showed slightly larger marginal gap discrepancy than the control but marginal gap between Computer-aided milled Cercon crowns and metal ceramic crowns did not showed significant difference (P>.05). 3. The Cercon crowns and metal ceramic crowns showed clinically acceptable marginal discrepancy.

도재 소성 과정에서의 고온이 지르코니아 코어의 변연적합도에 미치는 영향 (Influence of High Temperature of the Porcelain Firing Process on the Marginal Fit of Zirconia Core)

  • 본 연구는 치과에서 사용되는 심미 보철물 중에 하나인 지르코니아 기반의 전부도재관 제작 시 지르코니아 코어 위에 상부 도재를 축성하고 소성하는 과정에서 발생되는 고온이 완성된 전부도재관의 변연적합도에 미치는 영향을 분석함으로써 임상적 허용이 가능한지 알아볼 뿐 아니라, 치과보철치료를 위한 치과의사, 치과위생사, 치과기공사의 보철물 선택 시 임상적 참고자료로 제공할 목적으로 수행되었다. 심미에 가장 많은 영향을 미치는 상악 중절치를 지대치로 선정하여 동일한 모형 10개 제작 후 각각의 지대치에 적합한 지르코니아 코어를 제작하였다. 제작된 코어의 변연적합도 측정 후 코어 위에 상부 도재를 축성하여 전부도재관을 완성한 뒤 2차 측정을 실시하였으며, 측정 후 비교 분석 된 결과는 다음과 같다. 변연적합도는 지르코니아 코어 제작한 후 전부도재관으로 제작되는 과정에 따라 더 커졌으며, 통계적으로도 유의한 차이를 보였다(p<0.001). 그러나 전부도재관에서 총 80회 측정된 변연적합도의 값에서 임상적 허용 수치인 $120{\mu}m$을 넘지 않는 결과를 보였으며, 이와 같은 결과를 토대로 지르코니아 코어 위에 상부 도재 소성 시 변연적합도가 커지긴하나 임상적으로 허용이 가능하다는 결론을 도출하였다.

도재전장금관을 위한 코핑의 변형에 관한 연구 (A STUDY ON THE DISTORTION OF THE COPINGS FOR CERAMOMETAL CROWNS DURING REPEATED FIRING)

  • Ceramometal crowns are common restorations in fixed prosthodontics because of their casting accuracy, the high strength properties of the metal, and the cosmetic appearance of porcelain. However, deterioration of the initial fit of the metal coping has been observed after the porcelain firing cycle. The distortion due to repeated firing makes it difficult to fit crown margin and elicits microleakage. The major causes of distortion are the residual stress that accumulate during wax-up, casting, cold work and the induced stress caused by the mismatch of porcelain-metal thermal contraction. This study examined the marginal fit changes of metal copings in relation to repeated firing and the effects of heat treatment that reduce the distortion resulted from residual stress. The marginal changes of the copings that were treated with conventional method and those treated with heat before repeated firing, were evaluated. The metal die which represented preparations of a maxillary central incisor was fabricated, and 45 wax patterns were cast with nonprecious metal alloys. The heat treatment of each group was performed as follows. Group 1(control) : Casting - Devesting - Cold work - Firing Group 2 : Casting - Heat treatment - Devesting - Cold work - Firing Group 3 : Casting - Devesting - Cold work - Reinvesting - Heat treatment - Devesting - Firing The copings were fired 3 times. After each firing, the marginal fit changes were measured with inverted metallurgical microscope at the 4 reference points located at labial, lingual, and both proximal surface. Measurements were compared, and statistically analyzed. The results were as follows ; 1. In all groups, the highest value of marginal fit changes of the copings studied were found after the first firing cycle. 2. When the distortion of each experimental group at the first firing cycle were compared, group 1 exhibited the greatest changes($20-27{\mu}m$), followed by group 2($9-13{\mu}m$), and group 3($8-10{\mu}m$). 3. The copings treated with heat before devesting(group 2) revealed significantly smaller marginal fit changes than the copings treated with conventional method(group 1). (p<0.01) 4. The copings treated with heat after reinvesting(group 3) revealed significantly smaller marginal fit changes than the copings treated with conventional method(group 1). (p<0.01) 5. No siginificant differences in marginal fit changes were found between the copings treated with heat before devesting(group 2) and the copings treated with heat after reinvesting(group 3). (p>0.01)

Fracture resistance and marginal fit of the zirconia crowns with varied occlusal thickness

  • PURPOSE. The present study aimed to evaluate the clinical applicability of monolithic zirconia (MZ) crowns of different thickness via determination of fracture resistance and marginal fit. MATERIALS AND METHODS. MZ crowns with 0.5, 0.8, 1.0, and 1.5 mm thickness and porcelain fused to metal (PFM) crowns were prepared, ten crowns in each group. Marginal gaps of the crowns were measured. All crowns were aged with thermal cycling (5 - 55℃/10000 cycle) and chewing simulator (50 N/1 Hz/lateral movement: 2 mm, mouth opening: 2 mm/240000 cycles). After aging, fracture resistance of crowns was determined. Statistical analysis was performed with one-way ANOVA and Tukey's HDS post hoc test. RESULTS. Fracture loads were higher in the PFM and 1 mm MZ crowns compared to 0.5 mm and 0.8 mm crowns. 1.5 mm MZ crowns were not broken even with the highest force applied (10 kN). All marginal gap values were below 86 ㎛ even in the PFM crowns, and PFM crowns had a higher marginal gap than the MZ crowns. CONCLUSION. The monolithic zirconia exhibited high fracture resistance and good marginal fit even with the 0.5 mm thickness, which might be used with reduced occlusal thickness and be beneficial in challengingly narrow interocclusal space.

디지털 방식을 이용하여 제작한 전부 지르코니아 수복물의 변연 적합도 평가 (Evaluation of marginal fit of monolithic zirconia restoration by digital impression)

  • Purpose : The purpose of this study was to compare the marginal fit of monolithic zirconia restorations fabricated form the conventional and the digital impression technique. Materials and methods : Ten patients were restored with monolithic zirconia restorations fabricated from the digital and the conventional impression technique. Before definitive insertion, silicone replicas were produced from all twenty crowns. The silicone replicas were cut in three sections; each section was evaluated at two points. The measurement was carried out by using a measuring microscope (Olympus BX 51) and I-Solution. Data from the silicone replica scores were analyzed by Shapiro-Wilk test for normal distribution and the t-test for equality of two population's mean. All tests were performed with ${\alpha}-level$ of 0.05. Results : The average marginal gaps of monolithic zirconia restorations were $133.81{\pm}36.46{\mu}m$ for the conventional impression technique, $90.07{\pm}9.47{\mu}m$ for the digital impression technique. No statistical differences were found between the two impression techniques Conclusion : Both prostheses presented clinically acceptable results with comparing the marginal fit.

도재소부전장금관용 합금과 순측치경부 변연만곡도에 따른 변연적합도에 관한 연구 (AN EXPERIMENTAL STUDY ON THE FITNESS OF THE CERVICAL MARGIN IN THE CERAMO-METAL CROWN ACCORDING TO THE ALLOY TYPES AND THE CURVATURE OF LABIO-CERVICAL MARGIN)

  • The Purpose of this study was to evaluate the marginal fit of ceramo-metal crown according to the different ceramo-metal alloy types and the curvature of labio-cervical margin. Degudent $G^{(R)}$ as precious and $Verabond^{(R)}$ as non-precious ceramo-metal alloy were used. The abutment was preparaed with two different curvature types : a normal curvature type and a pronounced curvature type. 20 crowns were farbricated using four different combinations and their marginal fits were measured at 3 consecutive stages (before degassing, after degassing, after glazing) using microscope under 200 magnification. The results were as follows: 1 . Marginal fitness before degassing. The groups of precious ceramo-metal exhibited better marginal fit than the groups of non-precious ceramo-metal with significant difference(P<0.05) . In the same ceramo-metal groups, the normal curvature group exhibited better marginal fit than the pronounced curvature group but without significant difference(P>0.05). 2. Marginal fitness after degassing. By degassing, the group of pronounced curvature and non-precious ceramo-metal was deformed the most, and the degree of margin fitness of each group was the same as before degassing. 3. Marginal fitness after glazing The group of normal curvature and precious ceramo-metal exhibited better marginal fit than the group of pronounced curvature and non-precious ceramo-metal with significant difference(P>0.05), and the degree of margin fitness of each group was the same as before degassing.

Replica technique과 Triple-scan protocol을 이용한 지르코니아 전부도재관의 변연 및 내면 적합도에 관한 비교 연구 (Marginal and internal fit of all ceramic crown using the replica technique and the triple-scan protocol)

  • 목적: 본 연구는 보철물의 적합도를 계측하는 두 가지의 측정방법을 평가하기 위해 지르코니아로 전부도재관을 제작하고 replica technique과 triple-scan protocol을 이용하여 전부도재관의 변연 및 내면 적합도를 비교 연구하고자 하였다. 재료 및 방법: 타이타늄의 지대치 모형을 제작하여 20개의 지르코니아 전부도재관을 제작하였다. 제작된 지르코니아 전부도재관 10개는 replica technique을 이용해 복제하고 복제된 시편을 협설, 근원심으로 절단하여 변연 및 내면 적합도를 측정하였고, 다른 10개의 전부도재관은 triple-scan protocol을 이용하여 협설과 근원심으로 단면을 형성하고 변연 및 내면 적합도를 계측하여 통계 분석하였다 (${\alpha}=.05$). 결과: 변연과 내면 적합도는 replica technique $49.86{\pm}29.69{\mu}m$, triple-scan protocol $75.35{\pm}64.73{\mu}m$로 유의한 차이가 있었고 (P < .001), 내면 적합도만을 측정한 결과 각각 $58.38{\pm}31.77{\mu}m$, $64.00{\pm}46.43{\mu}m$로 유의한 차이가 없었다 (P > .343). 결론: Replica technique과 triple-scan protocol을 이용하여 전부도재관의 변연 및 내면 적합도를 비교 측정한 결과 triple-scan protocol에서의 변연 적합도가 크게 측정되었고, 내면 적합도만 비교해 본 결과 통계적인 유의한 차이가 없었다.
