  • The purpose of this study was to compare the soft tissue movements in facial profile predicted by a computer package with those that had actually accured following a sagittal split ramal osteotomy. The reliability of predicting the results of orthodontic surgical treatment was analysed. The study was based on the serial records of 30 consecutive patients who had been treated by means of a sagittal split ramal osteotomy. The serial lateral cephalometric radiographs used for the study were taken at the following stages: immediate preoperative : immediate postoperative : 6 months postoperative. A superimposition T1, T2-3 was generated to allow visual comparision. The results can be considered in relation to four important parts of the facial profile :the nose, upper lip, lower lip, and chin. The nose & Upper lip:The amount of movement of the upper lipwas not badly predicted for the average case. The lower lip: There was a significant trend over the whole sample for vertical positionof the lower lip to be less well predicted. The chin: The soft tissue movements of the chin were well predicted.

악교정 수술후 하악 근원심 골편의 위치 변화와 안정성에 관한 연구 (A STUDY OF THE CHANGE OF MANDIBLE POSITION AND THE STABILITY AFTER ORTHOGNATHIC SURGERY)

  • The purpose of this study was to evaluate the patterns of skeletal changes of proximal and distal segments after one jaw surgery and two jaw surgery with posterior impaction using SSRO on mandible in order to determine the skeletal origin of relapse and compare the stability of surgical methods in anterior open bite. The points and lines from lateral cephalometrics were measured before, after surgery, and at least 6-month follow up period. And then, the positional change of the proximal and distal segment were evaluated respectively. The results obtained were as follows; In cases of two jaw surgery, the results were stabler because they had less relapse factors. In cases of one jaw surgery, the value of APD were increased but it didn't relapse to the original value. Both of proximal and distal segments were responsible for the relapse tendency. But in one jaw surgery, the rotation of proximal segment was more responsible, and in two jaw surgery, the rotation of distal segment was.

  • This investigation was undertaken to know how the bony facial profile could be changed with age. The 5 serial lateral cephalometric roentgenograms of the fourteen boys and fourteen girls between 7 and 11 years of age were studied and the findings seems to warrant the following conclusions. 1. The manaible tended to become more prognathic in relation to the cranial base (S-N) during growth, but the maxilla showed very little change. 2. There was increase in the inclination of the lower border of mandible associated with the increase in mandibular prognathism. 3. There was a tendency to being straight in bony facial profile due to the decrease in angle ANB with age. 4. The chin portion had a tendency to forward and downward growth in relation to the cranial base but there was slight strong tendency in downward growth. 5. The vertical growth was more prominant in the maxilla than in the mandible. 6. There was uniform and gradual increase of all measurements during growth. 7. Dimension of the craniofacial complex was larger in the boys than in the girls but this difference was not statistically significant. 8. Individual variation in skeletal growth was a normal occurrence.

  • Patients who have repaired cleft lip and palate generally undergo restriction of maxillary growth. Concave facial profile is often exhibited with relatively normalized mandible. Horizontal and sagittal deficiency of the maxilla could cause anterior and posterior crossbites. In growing patients, ortho-dontic and orthopedic treatment is acceptable with maxillary expansion and protraction. However, surgical approach has to be accompanied with orthodontic treatment in skeletally matured patients. We used SARPE and BSSRO to expand the constricted maxilla and retract the mandible in a patient who had cleft palate repaired in infancy. Through SARPE, orthodontic treatment and BSSRO, we sufficiently expanded the maxillla and improved facial profile.

  • Angelman syndrome(AS) is a rare genetic neurological disorder. The main clinical characteristics of this syndrome are delayed neuropsychological development, intellectual disability, speech impairment, jerky movements especially hand-lapping, frequent laughter or smiling. AS is a classic example of genetic imprinting in that it is usually caused by deletion or inactivation of genes on the maternally inherited chromosome 15. The syndrome has oral manifestations such as diastemas, tongue thrusting, sucking/swallowing disorder, mandibular prognathism, frequent drooling, and excessive chewing behavior. The purpose of this paper is to describe the interesting aspects of the dental treatment of a childe with AS.

악교정 환자의 악교정 수술전후 발음양상에 대한 비교연구 (The Comparative Study of Effect on Speech before and after Orthognathic Surgery of Patients)

  • The purpose of this study was undertaken to determine the effects of orthognathic surgery on speech. The hyposis stated herein is that functional behaviors of the dentofacial complex, such as speech production, may be adversely affected by deviations of a structural nature(especially, Class III malocclusion). Twenty adults with Class III malocclusion(13 female and 7 male) were studied preoperative, immediate postoperative and either 6 or 12 months postoperative lateral cephalograms. They had mandibular prognathism and had undergone mandible setback operation. The position of tongue, soft palate(Uvula), hyoid bone, respiratory track width, and pharyngeal depth were assessed on lateral cephalograms with 23 cephalometric variables, ANOVA, Paired t-tests and Pearson's product-moment correlation coefficient tests were used to evalute the operative changes in all cephalometric parameters. A experienced speech and language pathologists performed narrow phonetic transcriptions of tape-recorded words and sentences produced by each of the ninth patients and the recording tapes were analyzed by phonetic computer program(Computerized Speech Lab(CSL) Model 4300BI(U.S.A.)) These judges also recorded their ratings of each patient's overall consonants, hypernasality, hyponasality, and articulation proficiency. The results obtained are as follows; 1. There were significant changes in distance of posterior pharyngeal wall to tongue (TI-TW2, TS-TW3) after the surgery at 6 months postoperatively(each p<0.01 p<0.05). 2. The posterior tongue point(TI, TS, PPT) moved posteriorly after surgery and remained to its changed position at 6 months postoperatively(p<0.05). The displacement of tongue was correlated with the movement of mandibular setback amount(p<0.05). The hyoid bone moved posteriorly superiorly after immediate postoperative period. There was significant changes in hyoid bone movement after immediated postoperative period(p<0.05), but returned to its original position during the follow-up period(p>0.05) 3. The soft palate was displaced posteriorly superiorly after immediated operative period and remained to its changed position at 6 months postoperatively(p<0.05). ANS-PNS-SPT angle increasing, PPU-PPPo distance narrowing was showed after surgery, and remained its appearance 6 months postoperatively(p<0.05). 4. There were significant changes in formant value and squre diagram of vowel sound after the orthognathic surgery and the follow-up period. There were significant changes in /ㅅ/sound and posterior tongue sound. 5. The posterior movement of tongue and the posteriosuperior movement of soft palate was correlated with mandibular setback amount after orthognathic surgery. On the vowel squre diagram, the author found that the place of articulation after operation moved downward, backward, upward. 6. In assessing speech abnormalities, dental occlusion should be considered as a contributing factor. The vast majority of subjects with preoperative misarticulations eliminated or reduced their errors following orthognathic surgery. There was significant difference in speech impovement between pre- and postoperation.

  • Cerebral palsy(CP) is one of the most common motor disease, due to brain injury during fetal and neonatal development which results in neuromotor paralysis and associated neuromuscular symptoms. Features of CP include motor disability due to the lack of muscle control, often accompanied by sensory disorders, mental retardation, speech disorders, hearing loss, epilepsy, behavior disorders, etc. There are increasing chances of treatment of dental patients with cerebral palsy, as the occurrence of CP is increasing with the decrease in infant mortality and an increase in immature birth and premature birth and also, there is a trend to pursue of higher quality of life. Reports on the relationship between CP and maxillofacial deformity are uncommon, but it is well known that the unbalance and discontrol of the facial muscles, lip, tongue and the jaws leads to malocclusion and temporomandibular joint disorders, and statistics show that class 2 relationship of the jaws and open bite is frequently reported. However, it is difficult to perform maxillofacial deformity treatment, which consists of orthodontic treatment, maxillofacial surgery and muscle adaptation training, due to difficulties in communication and problems of muscle adaptation caused by difficulties in motor control which leads to a high recurrence rate. This case report is to trearment of maxillofacial deformity in CP patient. A 26 year old female patient came to the department with the chief complaint of prognathism of the mandible and facial asymmetry. According to the past medical history, she was diagnosed as cerebral palsy 1 week after birth, classified as GMFC, classII accompanied with left side torticollis. The patient's intelligence was moderate, and there were no serious problems in communication. For two years time, the patient underwent lingual frenectomy, pre-operation orthodontic treatment and then bimaxillary orthognathic surgery to treat mandibular prognathism and facial asymmetry followed by rehabilitatory exercise of facial muscle. After 6 months of follow up, there was a good result. This is to report to the typical signs and symptoms of DFD in CP patient and the limitation of the usual method of the treatment of DFD in CP patient with literature review.

이모장치 착용 후 하악 이부의 헝태변화 (Changes of symphysis morphology after chincup treatment)

  • 성장기 아동에서의 골격성 III급 부정교합을 치료하기 위하여 사용되는 이모장치는 하악골의 후하방 회전 또는 하악골 길이 성장의 억제에 의하여 하악 전돌감을 감소시키는 것으로 알려져 있다. 그러나 이에 관한 연구는 하악골 성장의 주요 부분을 담당하는 하악 과두부위와 관련된 것이 대부분이었다. 본 연구는 장기간 이모장치를 착용하였을 때 구외력이 직접 적용되는 부위인 하악 이부의 형태 변화를 평가하기 위해 시행하였다. 전북대학교병원 치과교정과에서 교정치료를 받은 혼합치열기 아동 62명 (이모장치군 32명, 대조군 30명)을 대상으로 측모 두부규격 방사선사진을 채득하여(치료 또는 관찰기간 평균 2.1년) 계측치 (각도 4개 거리 5개, 비율 2개)를 평가하여 다음과 같은 결론을 얻었다. 1. 치료 후 symphysis height는 이모장치를 사용하지 않은 군과 이모장치 착용군에서 모두 치료 전보다 증가하였다. symphysis depth는 이모장치를 사용하지 않은 군에서 치료 후 증가하였으나 이모장치 착용군에서는 치료 전과 차이가 없었다. 특히, posterior symphysis depth가 이모장치 착용군과 대조군에서 모두 감소한 반면, anterior symphysis depth는 대조군에서는 증가하였고 이모장치 착용군에서는 치료 후 감소하는 경향을 보였다. 2. chin depth와 chin curvature는 이모장치를 사용하지 않은 군에서는 증가하였으나 이모장치 착용군에서는 치료 후에도 변화를 보이지 않았거나 감소하였다. 대조군에서 chin angle, menton angle, symphysis angle은 감소하는 경향을 보였으나 이모장치 치료군에서는 오히려 증가하였다. 이것은 하악골의 성장에 따라 정상적으로 나타나는 pogonion 부위의 골침착이 이모장치 착용군에서 chincup의 직접 접촉에 의해서 억제된 결과로 생각된다. 3. 성장기 아동에서 이모장치를 착용하였을 때 하악 이부의 전방 성장 억제에 의하여 치료 전과 유사한 하악 이부 형태가 유지되었으며 이것은 주로 하악 이부의 전방부의 골침착 억제에 의한 결과인 것으로 생각된다.

하악전돌증환자의 악교정수술후 하악각변화에 관한 임상적 분석 (CLINICAL ANALYSIS OF GONIAL ANGLE CHANGE AFTER ORTHOGNATHIC SUGERY IN PATIENTS WITH THE MANDIBULAR PROGNATHISM)

  • 저자는 1996년 1월에서 1998년 10월까지 본원에서 하악지시상분할골절단술을 시행한 하악전돌증환자중 35명(남자15명, 여자 20명)을 무작위로 선정하여 수술전, 수술직후 및 술후 1년의 하악각변화를 분석한 결과 다음과 같은 결론을 얻었다. 1. 35명 환자의 하악각평균은 술직후에는 술전에 비해 $9.3^{\circ}$ 감소하였다가 술후 1년까지 $4.0^{\circ}$증가하여 술전에 비해 술후 1년의 하악각은 $5.3^{\circ}$감소하였다(p<0.01). 2. 35명 환자의 악교정수술전 하악각평균은 $129.4^{\circ}({\pm}6.27)$로 정상에 비해 유의있게 큰 값을 보였으며 하악지시상분할골절단술 후 1년의 하악각평균은 $124.1^{\circ}({\pm}6.33)$로 비교적 개선되었다. 3. 술전과 술후 1년 사이의 남녀의 하악각변화는 여자는 $5.4^{\circ}$감소, 남자는 $5.3^{\circ}$감소로서 남녀간에 유의한 차이는 없었다(p>0.05). 4. 35명의 하악지시상분할골절단술을 받은 환자의 술전과 술후 1년 사이의 하악각변화에서 하악각감소에 관여하는 가장 주요한 변수는 하악후퇴량이었다. 5. 35명의 하악지시상분할골절단술을 받은 환자의 술직후부터 술후 1년까지의 하악각변화에서 하악각증가에 관여하는 가장 주요한 요인은 수평회귀율이었으며 원심골편후방부의 흡수현상도 하악각증가에 관여하는 것으로 생각되었다. 결론적으로 하악전돌증 환자에 있어 하악각이 정상보다 큰 경우 하악지시상분할골절단술은 하악각개선에 효과적이었으며 하악각이 정상에 가깝고 과도한 하악후퇴량이 요구되는 경우는 Short lingual cut 방법, 부가적인 원심골편의 후방변연절제술, Obwegeser II 방법과 같은 하악우각부 형태를 유지하는 수술법을 선택함이 유리할 것으로 예상되고 술후 회귀방지책이 안정된 하악각유지에도 도움이 될 것으로 사료된다.

골격성 III급 부정교합자에서 전진이부성헝술을 동반한 하악 후퇴술 후 연조직 외형의 변화 (Soft tissue changes associated with advancement genioplasty in skeletal class III individuals receiving mandibular set-back surgery)

  • 본 연구는 전진이부성형술을 동반한 하악지 시상분할골절단술을 시행한 하악 전돌증을 가진 환자에서 하안면부, 특히 이부의 경조직과 연조직의 변화를 예측하는 데 있어서, 전후관계와 수직적 계측치를 통하여, 경조직의 변화에 따른 연조직의 상대적 변화량에 대해 비교, 분석하여 한국인의 골격성 III급 부정교합자의 악교정 수술시 전진이부성형술이 연조직 외형에 미치는 영향을 알아보고자 시행하였다. 교정치료와 악교정수술을 시행한 골격성 III급 부정교 합자 40명을 전진이부성형술을 시행하지 않고 하악지 시상분할골절단술만 시행한 환자 20명(BSSO group, 남자 10 명, 여자 10명)과 하악지 시상분할골절단술과 동시에 전진이부성형술을 시행한 환자 20명(Genie group, 남자 10명, 여자10명)으로 분류하여 수술직전(T1)과 수술직후(T2), 수술 최소 6개월 후(T3)의 측모 두부 방사선계측사진을 채득하여 계측 항목의 변화량을 분석하였다. BSSO군에서는 수술후 경조직 B점, Pogonion, Menton에 대하여 연조직 B점, Pogonion, Menton이 각각 수평적으로 0.997, 0.965, 1.022의 변화율을 보였으며, Genie군에서는 각각 0.824, 0.602, 0.887의 수평적 변화율을 보여서, Genie 군에서 연조직 B점, 연조직 Pogonion의 연조직 두께의 감소량이 더 크게 나타났다. 경조직 Pogonion(Pg)의 악교정수술 전과 후의 수평적 변화에 따른 하순의 하방점(Li), 연조직 B점 (B')의 변화간의 상관관계계수는 BSSO군에서는 각각 0.833, 0.922인 반면에, Genie 군에서는 각각 0.775, 0.799로 BSSO군에 비해서 상관관계가 다소 작은 것으로 나타났다. 이상의 결과에서 하악지 시상분할골절단술을 시행하는 환자에서 전진이부성형술을 동반할 경우에는 골격의 변화에 대해서 연조직의 두께 변화가 다소 적게 나타나므로 수술시에 이를 고려하여 전진이부성형술을 시행하여야 할 것이다.
