This study was conducted on 342 patients(male 157, female 185)aged 8 to 15 years old, who visited Dankook University Dental Hospital. Pre-orthodontic treatment orthopantomograms were used to assess the dental calcification stages of mandibular 3rd molar, 2nd molar, 2nd premolar and 1st premolar by 8 stages.(by Demirjian) Hand-wrist radiogrms were used to evaluate the skeletal maturity in 11 stages.(by Fishman) Following results were obtained after investigating the correlationship between dental calcification and skeletal maturity 1. Chronologic age showed high correlation to dental calcification and skeletal maturity. 2. Dental calcification and skeletal maturity showed high correlation and no statistical difference was observed between male and female. 3. SMI stages 1 to 4 showed high statiscal significance to mandibular 2nd molar, 2nd premolar and 1st premolar. SMI stages 5 to 8 showed high stastical significance to mandibular 2nd molar, 2nd premolar. SMI stages 9 to 11 showed high statistical significance to mandibular 3rd molar.
Objective: To examine the effect of bite force on the displacement and stress distribution of orthodontic mini-implants (OMIs) in the molar region according to placement site, insertion angle, and loading direction. Methods: Five finite element models were created using micro-computed tomography (microCT) images of the maxilla and mandible. OMIs were placed at one maxillary and two mandibular positions: between the maxillary second premolar and first molar, between the mandibular second premolar and first molar, and between the mandibular first and second molars. The OMIs were inserted at angles of $45^{\circ}$ and $90^{\circ}$ to the buccal surface of the cortical bone. A bite force of 25 kg was applied to the 10 occlusal contact points of the second premolar, first molar, and second molar. The loading directions were $0^{\circ}$, $5^{\circ}$, and $10^{\circ}$ to the long axis of the tooth. Results: With regard to placement site, the displacement and stress were greatest for the OMI placed between the mandibular first molar and second molar, and smallest for the OMI placed between the maxillary second premolar and first molar. In the mandibular molar region, the angled OMI showed slightly less displacement than the OMI placed at $90^{\circ}$. The maximum Von Mises stress increased with the inclination of the loading direction. Conclusions: These results suggest that placement of OMIs between the second premolar and first molar at $45^{\circ}$ to the cortical bone reduces the effect of bite force on OMIs.
This case report describes the management of a 30-year-old woman with hopeless mandibular first molars and right maxillary second premolar. The treatment plan included mandibular second and third molar protraction after extraction of mandibular first molars. Mini-implants were placed between roots of first and second premolar. Sliding mechanics with lever arm was used to prevent inclination of molars. A good functional occlusion was achieved in 38 months without clinically significant side effects. Most of the extraction space of mandibular first molar was closed by protraction of second and third molars. The skeletal Class II pattern was improved by counterclockwise rotation of mandible through reduction of wedge effect. Mandibular molar protraction with orthodontic mini-implants in adequate cases would be a great alternative to prosthetic implant and reduce the financial and surgical burden of patients.
Mandibular premolars show a wide variety of root canal anatomy. Especially, the occurrence of three canals with three separate foramina in mandibular second premolars is very rare. This case report describes the root canal treatment of an unusual morphological configuration of the root canal system and supplements previous reports of the existence of such configuration in mandibular second premolar.
One of the most important factors for successful endodontic therapy is an accurate length determination of physiological root apex. Some methods suggested for the measurement of root canal length, include digital-tactile sense and roentgenographic technique with measuring wire, scale and grid. But these methods do not derermine an accurate working length to physiological root apex. Recently electronic measuring devices are used to locate the physiological root apex in root canal length determination and these devices are accepted as an effective apparatus. The 89 patients (116 teeth, 144 canals) among the out-patients of Yonsei University Dental Infirmary, who had had an endodontic treatment in the Department of Operative Dentistry, were measured by the Root-Canal Meter$^{(R)}$ as an electronic device, and radiographs to determine the distribution and location of physiological root apex, then the following results were made: (1) Range of ${\pm}$1mm from the radiographic root apex were present in 88.88% (128 canals) of the subjects. (2) Physiological root apex and radiographic root apex were coincided in 31.94% (46 canals) of the subjects. (3) The actual length of the physiological root apex of the teeth were as follow; A : in the maxillary central incisor : 0.46mm B : in the maxillary lateral incisor : 0.44mm C : in the maxillary canine : 0.44mm D : in the maxillary 1st premolar : a) Buccal : 0.59mm b) Lingual : 0.34mm E : in the maxillary 2nd premolar : 0.54mm F : in the maxillary 1st molar : a) Mesio-buccal : 0.50mm b) Disto-buccal : 0.42mm c) Lingual : 0.56mm G : in the mandibular central incisor : 0.62mm H : in the mandibular lateral incisor : 0.45mm in the mandibular canine : 0.54mm J : in the mandibular 1st premolar : 0.47mm K : in the mandibular 2nd premolar : 0.34mm L : in the mandibular 1st molar : a) Mesio-buccal : 0.54mm b) Mesio-lingual : 0.31mm c) Distal : 0.37mm.
Objective: In this study, we measured the cortical bone thickness in the mandibular buccal and lingual areas using computed tomography in order to evaluate the suitability of these areas for application of temporary anchorage devices (TADs) and to suggest a clinical guide for TADs. Methods: The buccal and lingual cortical bone thickness was measured in 15 men and 15 women. Bone thickness was measured 4 mm apical to the interdental cementoenamel junction between the mandibular canine and the 2nd molar using the transaxial slices in computed tomography images. Results: The cortical bone in the mandibular buccal and lingual areas was thicker in men than in women. In men, the mandibular lingual cortical bone was thicker than the buccal cortical bone, except between the 1st and 2nd molars on both sides. In women, the mandibular lingual cortical bone was thicker in all regions when compared to the buccal cortical bone. The mandibular buccal cortical bone thickness increased from the canine to the molars. The mandibular lingual cortical bone was thickest between the 1st and 2nd premolars, followed by the areas between the canine and 1st premolar, between the 2nd premolar and 1st molar, and between the 1st molar and 2nd molar. Conclusions: There is sufficient cortical bone for TAD applications in the mandibular buccal and lingual areas. This provides the basis and guidelines for the clinical use of TADs in the mandibular buccal and lingual areas.
PURPOSE. The purpose of this study was to identify the complex course of the mandibular canal using 3D reconstruction of microCT images and to provide the diagram for clinicians to help them understand at the interforaminal region in Korean. MATERIALS AND METHODS. Twenty-six hemimandibles obtained from cadavers were examined using microCT, and the images were reconstructed. At both the midpoint of mental foramen and the tip of anterior loop, the bucco-lingual position, the height from the mandibular inferior border, the horizontal distance between two points, and position relative to tooth site on the mandibular canal were measured. The angle that the mental canal diverges from the mandibular canal was measured in posterior-superior and lateral-superior direction. RESULTS. The buccal distance from the mandibular canal was significantly much shorter than lingual distance at both the mental foramen and the tip of anterior loop. The mandibular canal at the tip of anterior loop was significantly located closer to buccal side and higher than at the mental foramen. And the mental canal most commonly diverged from the mandibular canal below the first premolar by approximately $50^{\circ}$ posterior-superior and $41^{\circ}$ lateral-superior direction, which had with a mean length of 5.19 mm in front of the mental foramen, and exited to the mental foramen below the second premolar. CONCLUSION. These results suggest that it could form a hazardous tetrahedron space at the interforaminal region, thus, the clinician need to pay attention to the width of a premolar tooth from the mental foramen during dental implant placement.

Objectives: Localization of the mandibular canal (MC) and measurement of the height and width of the available alveolar bone at the proposed implant site in the posterior segment of the mandible using cone-beam computed tomography (CBCT) in patients with a single missing tooth. Materials and Methods: A cross-sectional study was performed where CBCT scans of the patients with a single missing tooth in the posterior segment of the mandible-premolar, I (1st) molar, and II (2nd) molar were used. The scans were assessed using OnDemand3D software (version 1.0; CyberMed Inc., Seoul, Korea) for localization of the MC asnd remaining alveolar bone both vertically (from the superior position of the MC to the crest of the alveolar ridge) and horizontally (buccolingual, 3 mm below the crest of the alveolar ridge). The findings were statistically analyzed using independent t-test. Results: A total of 120 mandibular sites (40 sites for each of the three missing premolar, I molar, and II molar) from 91 CBCT scans were analyzed. The average heights (from the alveolar crest to the superior margin of the MC) at the premolar, I molar, and II molar areas were $15.19{\pm}2.12mm$, $14.53{\pm}2.34mm$, and $14.21{\pm}2.23mm$, respectively. The average widths, measured 3 mm below the crest of the alveolar ridge, at the premolar, I molar, and II molar areas were $6.22{\pm}1.96mm$, $6.51{\pm}1.75mm$, and $7.60{\pm}2.08mm$, respectively. There was no statistically significant difference between males and females regarding the vertical and horizontal measurements of the alveolar ridges. Conclusion: In the study, the measurements were averaged separately for each of the single missing teeth (premolar, I molar, or II molar), giving more accurate information for dental implant placement.
