Evaluating Paraspinal Back Muscles Using Computed Tomography (CT) and Magnetic Resonance Imaging (MRI): Reliability Analysis and Correlation with Intervertebral Disc Pathology

  • Purpose: To investigate the reliability of CT and MRI for quantitative and qualitative analyses of lumbar paraspinal muscle fatty infiltration (PSFI) and correlation of PSFI with intervertebral disc pathology. Materials and Methods: Lumbar spine CT and MRI of 36 subjects were reviewed retrospectively. Two observers independently outlined lumbar paraspinal muscles at each mid-intervertebral disc level. Paraspinal muscles on CT and MRI were graded according to the Goutallier grading system (GGS). The area, mean value, and standard deviation (SD) of the Hounsfield unit (HU) were obtained. Intervertebral discs were assessed on axial image of T2WI at each level. Correlations between qualitative and quantitative data and intervertebral disc pathology, age, and sex were evaluated. Results: Inter- and intra-observer agreements for results of GGS on MRI were substantial (κ = 0.79) and moderate (κ = 0.59), respectively. Inter- and intra-observer agreements for results of GGS on CT were almost perfect (κ = 0.88) and substantial (κ = 0.66), respectively. Quantitative measurements of HU showed almost perfect inter- and intra-observer reliabilities (κ = 0.82 and κ = 0.99, respectively). There were statistically significant correlations between intervertebral disc pathology and PSFI at L1-2, L2-3, and L4-5 levels on MRI and at L1-2 and L3-4 levels on CT. Age showed significant correlation with results of GGS at all levels on CT and MRI. Conclusion: This study showed that GGS results and HU measurements could be useful for evaluating PSFI because they showed correlations with intervertebral disc pathology results at certain levels.

Evaluation of Knee Joint after Double-Bundle ACL Reconstruction with Three-Dimensional Isotropic MRI

  • Purpose: To evaluate the knee joint after double-bundle anterior cruciate ligament (ACL) reconstruction with three-dimensional (3D) isotropic magnetic resonance (MR) image, and to directly compare the ACL graft findings on 3D MR with the clinical results. Materials and Methods: From January 2009 to December 2014, we retrospectively reviewed MRIs of 39 patients who had reconstructed ACL with double bundle technique. The subjects were examined using 3D isotropic proton-density sequence and routine two-dimensional (2D) sequence on 3.0T scanner. The MR images were qualitatively evaluated for the intraarticular curvature, graft tear, bony impingement, intraosseous tunnel cyst, and synovitis of anteromedial and posterolateral bundles (AMB, PLB). In addition anterior tibial translation, PCL angle, PCL ratio were quantitatively measured. KT arthrometric values were reviewed for anterior tibial translation as positive or negative. The second look arthroscopy results including tear and laxity were reviewed. Results: Significant correlations were found between an AMB tear on 3D-isotropic proton density MR images and arthroscopic proven AMB tear or laxity (P < 0.05). Also, a significant correlation was observed between increased PCL ratio on 3D isotropic MRI and the arthroscopic findings such as tear, laxities of grafts (P < 0.05). KT arthrometric results were found to be significantly correlated with AMB tears (P < 0.05) and tibial tunnel cysts (P < 0.05). Conclusion: An AMB tear on 3D-isotropic MRI was correlated with arthroscopic results qualitatively and quantitatively. 3D isotropic MRI findings can aid the evaluation of ACL grafts after double bundle reconstruction.

  • We present image findings, especially rare MRI of a primary breast angiosarcoma with its histopathology, and also analyze the relevant medical literature reports in terms of the MRI findings. As our patient had unique features of a primary breast angiosarcoma, this case could be very helpful for future diagnosis of this rare breast malignancy by MRI.

An Implementation of Effective CNN Model for AD Detection

  • This paper focuses on detecting Alzheimer's Disease (AD). The most usual form of dementia is Alzheimer's disease, which causes permanent cause memory cell damage. Alzheimer's disease, a neurodegenerative disease, increases slowly over time. For this matter, early detection of Alzheimer's disease is important. The purpose of this work is using Magnetic Resonance Imaging (MRI) to diagnose AD. A Convolution Neural Network (CNN) model, Reset, and VGG the pre-trained learning models are used. Performing analysis and validation of layers affects the effectiveness of the model. T1-weighted MRI images are taken for preprocessing from ADNI. The Dataset images are taken from the Alzheimer's Disease Neuroimaging Initiative (ADNI). 3D MRI scans into 2D image slices shows the optimization method in the training process while achieving 96% and 94% accuracy in VGG 16 and ResNet 18 respectively. This study aims to classify AD from brain 3D MRI images and obtain better results.

Benefit of Using Early Contrast-Enhanced 2D T2-Weighted Fluid-Attenuated Inversion Recovery Image to Detect Leptomeningeal Metastasis in Lung-Cancer Staging

  • Purpose: To evaluate the clinical benefit of 2D contrast-enhanced T2 fluid-attenuated inversion recovery (CE-T2 FLAIR) image for detecting leptomeningeal metastasis (LM) in the brain metastasis work-up for lung cancer. Materials and Methods: From June 2017 to July 2019, we collected all consecutive patients with lung cancer who underwent brain magnetic resonance image (MRI), including contrast-enhanced 3D fast spin echo T1 black-blood image (CE-T1WI) and CE-T2 FLAIR; we recruited clinico-radiologically suspected LM cases. Two independent readers analyzed the images for LM in three sessions: CE-T1WI, CE-T2 FLAIR, and their combination. Results: We recruited 526 patients with suspected lung cancer who underwent brain MRI; of these, we excluded 77 (insufficient image protocol, unclear pathology, different contrast media, poor image quality). Of the 449 patients, 34 were clinico-radiologically suspected to have LM; among them, 23 were diagnosed with true LM. The calculated detection performance of CE-T1WI, CE-T2 FLAIR, and combined analysis obtained from the 34 suspected LM were highest in the combined analysis (AUC: 0.80, 0.82, and 0.89, respectively). The inter-observer agreement was also the highest in the combined analysis (0.68, 0.72, and 0.86, respectively). In quantitative analyses, CNR of CE-T2 FLAIR was significantly higher than that of CE-T1WI (Wilcoxon signed rank test, P < 0.05). Conclusion: Adding CE-T2 FLAIR might provide better detection for LM in the brain-metastasis screening for lung cancer.

정형보철용 금속을 이용한 자기공명영상왜곡의 정량적 평가 (Quantitative evaluation of MRI distortion using orthopedic prosthetic metal)

  • 자기공명영상은 많은 장점에도 불구하고 체내에 정형보철이 있는 경우 자화율 차이로 영상왜곡이 발생한다. 스테인리스와 티타늄 시험편을 $65{\times}15{\times}2mm$로 동일하게 제작하여 phantom과 돼지고기에 부착 한 후 자기공명영상과의 관계를 알아보았다. phantom 영상을 통해 영상왜곡의 길이와 돼지 뼈를 통한 신호강도의 변화, 피하조직의 지방억제 정도를 정량적으로 평가 하였다. 스테인리스는 티타늄에 비해 길이 방향으로 4.4배, 높이 방향으로 2.8배 영상왜곡이 크며, 신호강도는 58.5%로 상대적으로 낮았다. 피하조직 지방억제는 Normal의 56.2%에 비해 스테인리스는 16.04%, 티타늄은 54.53%로 나타났다. 실험 결과 자기공명영상의 영상학적 진단가치는 스테인리스 보다 티타늄 금속이 부착 된 경우 더 우수한 것을 알 수 있었다.

Late Onset Postpartum Seizure and Magnetic Resonance Image Findings

  • Two young women were brought to the Emergency room with generalized tonic and clonic seizures. Seizure developed seven and ten days after delivery respectively without the clinical signs of pre-eclampsia throughout the pregnancies. Magnetic resonance(MR) image of the brain showed characteristically symmetrical abnormal signals in the parietal and occipital regions. After several days of medical treatment, they were discharged without neurologic sequelae and follow-up MR images taken three months after discharge showed complete disappearance of the previous abnormal signals.

  • 간세포암 위험이 있는 환자들에 대해 국제 가이드라인들은 6개월마다 초음파로 감시할 것을 권고하고 있다. 그러나 초음파는 초기 간세포암을 발견하는 데에 낮은 민감도를 보인다. 한편, 자기공명영상(MRI)은 간세포암을 비침습적으로 진단하는 데 중요한 역할을 하지만, 검사 시간이 길고 비용이 높아서 감시 검사로는 적합하지 않다. 따라서 비조영증강 단축 MRI, 역동적 조영증강 단축 MRI, 그리고 간담도기영상을 포함한 단축 MRI 등 다양한 단축 MRI 전략들을 간세포암 감시에 이용한 여러 연구들이 있었다. 이 종설에서는 다양한 단축 MRI 전략들을 살펴보고, 비용-효과적인 측면을 고려하여 간세포암 감시의 앞으로 나아가야 할 방향에 대해 살펴보고자 한다.

  • Bone marrow aspirates concentrate (BMAC) transplantation is a well-known technique for cartilage regeneration with good clinical outcomes for symptoms in patients with osteoarthritis (OA). Magnetic resonance imaging (MRI) has an important role in evaluating the degree of cartilage repair in cartilage regeneration therapy instead of a second assessment via an arthroscopy. We experienced a case of hypertrophic regeneration of the cartilage and a presumed simultaneous regeneration of the posterior horn of the lateral meniscus after BMAC transplantation for a cartilage defect at the lateral tibial and femoral condyle. This report provides the details of a case of an unusual treatment response after a BMAC transplant. This report is the first of its kind to demonstrate a MR image that displays the simultaneous regeneration of the cartilage and meniscus with a differentiation ability of the mesenchymal stem cell to the desired cell lineage.

Magnetic Resonance Findings in Two Episodes of Repeated Cerebral Fat Embolisms in a Patient with Autologous Fat Injection into the Face

  • We report magnetic resonance image (MRI) and magnetic resonance spectroscopy (MRS) findings in a patient of cerebral fat embolism (CFE) occurred in a 26-year-old woman after an autologous fat injection into the face. After initial neurologic symptom onset, MRI and MRS data were obtained two times to investigate repeated CFE. We obtained the MRS data in the two different time intervals and two different echo times to compare the lesions with normal brain parenchyma. The results of MRS data showed that a decrease in N-acetyl-aspartate, an increase in lactate and a very high early peak of free lipids between 0.9 and 1.4 ppm were obtained at the acute infarcted lesion as compared with normal brain parenchyma. In addition, these findings were more clearly detected on short echo time spectrum rather than long spectrum. A close relationship between the clinical manifestations and MRI and MRS findings of the brain can helpful to distinguish CFE with other conditions and to evaluate the cause materials of infarctions rather than conventional MRI or diffusion-weighted imaging.
