A Study on Sources of Energy & Macronutrients from Korean Dishes by Area (지역에 따른 주요영양성분의 공급음식에 관한 연구(I) -에너지 및 3대 영양소를 중심으로-)

  • The dietary intake of nutritional elements by Koreans as determined by the Ministry of Health and Welfare's National Nutrition Survey have been reported for 19691993. But these data were based on not dish but food. Therefore, the purpose of this study was to compare dish sources of energy and macronutrients (carbohydrate, fat, protein) in the diets of three area (large city, small city and rural). Dish sources were evaluated from two-day record obtained in the secondary analysis of the 1993 Korean National Nutrition Survey. The result, large city and small city had higher fat and protein intake than did rural. Otherwise, carbohydrate intake among rural was higher than those among large and small city (p<0.05). The primary dish source of energy and macronutrients was cooked rice for all area. The percentage of cooked rice in daily carbohydrate intake was 52.64% for nationwide, 48.40% for large city, 50.52% for small city, 61.79% for rural. The cumulative percent of top 10 dish sources to carbohydrate for large city, small city, rural were 78.02%, 81.16%, 85.69%, respectively. These was higher than cumulative percent of other macronutrients. The milk as good protein source ranked 3 for large city, 6 for small city, 22 for rural. The major dish sources to fat were cooked rice and pork that prepared by various cooking way. These results show that the major dish sources of energy and macronutrients were cooked rice based of Korea traditional consumption pattern. The most of nutrient intake consumed some dishes for all area. The dish consumption pattern was generally similar between large and small city. But rural was different from other area. Threfore, nutrition educations and interventions should be targeted to each area and should be attended with studies that comparision of dish sources to nutritional elements by specific age-sex groups.

No Effect of Energy Intake Overall on Risk of Endometrial Cancers: a Meta-analysis

  • Background: Previous epidemiologic studies on the association between energy intake and endometrial cancer risk have only generated contradictory results. The role of energy intake in endometrial carcinogenesis thus remains unclear. To quantitatively assess the potential association between energy intake and endometrial cancer, a meta-analysis of case-control and cohort studies was here conducted. Materials and Methods: Eligible studies were retrieved via both computerized searches and review of references. Fixed-or random-effect models were used to summarize the estimates of OR with 95%CIs. Stratified analyses on study design, region and macronutrients' calorie were performed. Results: Sixteen studies met the inclusion criteria of the meta-analysis. No association between total energy intake and endometrial cancer was observed in either overall group (OR=1.11, 95%CI 0.92-1.30) or subgroups stratified by study design and region. In the specific macronutrients' calorie analysis, higher fat energy intake was found to be associated with increased endometrial risk (OR=1.72, 95%CI 1.12-2.32) while energy from carbohydrate and protein was not related to endometrial cancer risk. Conclusions: Our analysis did not support that total energy intake is related to endometrial cancer risk, in contrast to fat energy.

  • In experiment 1, dietary self-selection of the 3 macronutrients, protein, fat, and carbohydrate were examined in male rats given 3 food cups of 80% carbohydrate, 80% protein, and 70% fat diets simultaneously. All the rats showed normal growth pattern and organ weight, which means they have ability to select just right kinds and amounts of nurients in order to support their growth and development. Mean values of caloric intake, body weight gain, serum lipid values and empty carcass compositions were not significantly differ between the upper and lower quartile groups of fat proportion of empty carcass compared to the lower quartile group(LF). Same feeding design was employed in experiment 2 where the effect of mild electric stress on food selection was studied. The rats in both control and electric stress group revealed a normal growth curve and organ weights. The stress group showed higher caloric intake and body weight gain than control group, but no significant effects of stress on serum and empty carcass components was found. Even though normal rats seemed to select macronutrients according to their physiolosical needs, there were individual differences in food selection whether they were exposed to stress or not. Therefore life long individual food selection pattern may have a great influence on nutritional status and chronic degenerative diseases of eldery, and on aging process.

Intake of energy and macronutrients according to household income among elementary, middle, and high school students before and during the COVID-19 pandemic: a cross-sectional study (코로나19 팬데믹 전후 초·중·고등학생의 가구소득별 에너지 및 다량영양소 섭취: 국민건강영양조사 (2016-2022) 자료 활용)

  • Objectives: This study examined the intake of energy and macronutrients among elementary, middle, and high school students according to household income before the COVID-19 pandemic (2016-2019), during the social distancing period (2020-2021), and after the social distancing measures were lifted (2022). Methods: We included 5,217 students aged 5-18 from the Korea National Health and Nutrition Examination Survey (KNHANES) conducted between 2016 and 2022. Dietary intake was assessed using one-day 24-hour dietary recalls. We estimated the least squares means (LS-means) of intake according to household income for each period using a weighted linear regression model, adjusted for age and sex. Differences in LS-means between the periods were analyzed using the t-test. Results: During the social distancing period, the LS-means of energy intake among students decreased significantly by 143.2 kcal/day compared to pre-pandemic levels (P < 0.001). Students from low-income households experienced a more pronounced decrease in energy intake (-379.1 kcal/day, P < 0.001) and macronutrient intake compared to those from other income groups. Energy intake at school significantly declined for all income groups during the social distancing period compared to before the pandemic. No significant changes in home energy intake were observed among low-income students, whereas there was an increase for students from higher-income groups. Before the pandemic, 8.5% of students from low-income households reported insufficient food due to economic difficulties; this figure rose to 21.3% during the pandemic. Conclusions: During the pandemic, students from low-income families experienced significantly lower intake of energy and macronutrients compared to pre-pandemic levels. The most substantial reductions were noted among low-income students, largely due to the lack of compensation for decreased school-based intake with increased intake at home.

  • New Dietary Reference Intakes (DRIs) forthe United States and Canada have recently been set for both macronutrients and micronutrients, and are likely to be of interest to health professionals in Korea as well. DRIs are now available for nutrients that did not have Recommended Dietary Allowances set in the past (amino acids, n-3 and n-6 fatty acids, total fiber, added sugar, choline, boron, nickel, and vanadium). Furthermore, the units for the DRIs do not always match those traditionally carried on food composition tables (FCTs). FCT developers will also need to consider carrying new variables to allow the calculation of folate intake in μg of dietary folate equivalents, vitamin E intake as mg of a-tocopherol (not as mg of a-tocopherol equivalents), and vitamin A intake as μg of retinol activity equivalents (not as μg of retinol equivalents). Because the new recommendations for upper levels of intake sometimes refer to a specific form or source of a nutrient, nutrients occurring in foods must be separated from added or supplemental forms for vitamin E, niacin, and folate; pharmacological magnesium must be carried as a separate variable; and preformed vitamin A must be separated from vitamin A from carotenoids.   • The Institute of Medicine of the National Academies Food and Nutrition Board in conjunction with Health Canada recently released the sixth in a series of reports on dietary reference values for the intake of energy and macronutrients. This paper discusses the Dietary Reference Intakes (DRIs) for energy and physical activity. Several new approaches were used with the DRIs for the macronutrients. Those discussed in this paper include: .Estimated Energy Requirements (EER) at four levels of energy expenditure based on doubly labeled water measurements of total energy expenditure (TEE). .Recommendations for levels of physical activity to decrease the risk of chronic disease.

Dietary intakes and obesity of Malaysian adults

  • BACKGROUND/OBJECTIVES: Over the past few decades, Malaysia has been experiencing an increased prevalence of overweight and obesity that threatens the health of Malaysians. Poor dietary intake is one of the major contributors to the development of obesity and many non-communicable diseases. The dietary intakes of adults in Malaysia were assessed to determine the association between the dietary intake variables and the body mass index (BMI) and waist circumference (WC) measurements. This study examined whether the dietary intake achieved the recommended nutritional guidelines and compared the intakes between both sexes. SUBJECTS/METHODS: The height, weight, and WC of four-hundred-and-ninety adults (n = 490) in Malaysia were measured using standard procedures. The three-day 24-hour dietary recalls were conducted on 422 out of the 490 adults and their dietary intakes were evaluated in detail. The selected dietary intake variables were used to determine the associations with the obesity indicators. RESULTS: Among the participants, 52.8% were overweight or obese. After data analysis, the mean energy intake was 1,550 kcal/day, in which male participants had a significantly higher energy and macronutrients intake than females. Protein consumption and its percentage of energy contribution exceeded the recommended range. The consumption of fruits, vegetables, and milk and milk products were lower than the recommended number of servings for a healthy diet. The male participants consumed significantly more servings of carbohydrate-based foods, meat, and fats, oils, and sweets than females. Among the selected dietary intake variables, only the carbohydrate intake was negatively associated with the BMI (Estimate b = -0.008) and WC measurements (Estimate b = -0.019) after adjusting for covariates. CONCLUSIONS: This study evaluated the dietary intakes of a sample of Malaysian adults and its association with the obesity indicators. The results highlight the need for improvements and modifications of the dietary intake of Malaysians to reduce the overweight and obesity rates.

  • This investigation studied the relationship between Body Mass Index(BMI) and dietary intake, levels of serum lipid, lipoprotein(a) and plasminogen activator inhibitor-1(PAI-1) of 28.449 Korean adults(16.937 men, 11.542 women) from 1995 to 1999. The dietary assessment was conducted using 24-hour dietary records and food frequency questionnaires. During this five year study, the BMI normal-weight group, as a percentage of the annual test population, decreased from 68.0% to 60.2%, while the BMI overweight and obese groups collectively increased from 25.0% to 29.7%. The levels of serum total cholesterol (TC), LDL-cholesterol(LDL-C), Triacylglycerol(TG) and PAI-1 increased as the values of BMI increased, while the levels of HDL-cholesterol(HDL-C) and lipoprotein(a)(men only) appeared to decrease as values of BMI increased. The levels of daily energy intake also increased as values of BMI increased in both men and women. The obese group had significantly higher levels of carbohydrate, protein, fat(men only), and cholesterol intake than those of the normal-weight, underweight, and overweight groups. In men, BMI positively correlated with the levels of macronutrients and cholesterol intake(p<0.001), % energy of protein, fat, and alcohol intake(p<0.001), and the levels of serum TC, LDL-C, TG and PAI-1(p<0.001), while BMI negatively correlated with % energy of carbohydrate intake, and the levels of Lp(a) and HDL-C(p<0.001). In women, BMI negatively correlated with level of cholesterol(p<0.01), fat(p<0.001), alcohol intake(p<0.05),% energy of fat (p<0.001), % energy of alcohol intake and level of and HDL-C(p<0.001). Subjects who had serum TC, LDL-C, HDL-C, and TG levels greater than the standard reference values(TC>240mg/㎗, LDL-C>130mg/㎗, HDL-C>35, TG>200mg/㎗) exhibited a higher intake of the three macronutrients, iron, calcium, meat, milk and fatty foods than those subjects who had serum lipid concentrations less-than-or-equal-to the standard reference values. Overall, there was positive correlation between the high risk factors of vascular disease variables, dietary intake, and BMI. Prevalence of hypertension and high blood sugar were increased as BMI increased, but the prevalence of hypertension is decreased as the year goes by. These findings showed that dietary intake, level of serum lipids and other vascular disease risk factors increased as BIM increased. Therefore, middle or upper class Korean adults who have high BMI should improve their eating habits. This involve reducing alcohol, animal fat, high carbohydrate foods, and overall food intake, and balancing intake in order to lower vascular disease risk factors, including obesity.

Benefits and Limitations of Low-Carbohydrate Diets: Healthy Carbohydrate Control (저탄수화물식의 효과와 한계: 건강한 탄수화물 조절)

  • The prevalence of obesity has been increasing worldwide. Several dietary treatments have been suggested to control weight, and recent guidelines recommend individualizing the composition of macronutrients. Carbohydrates are the most important nutrients in meals, and carbohydrate restriction is a dietary strategy that promotes weight loss. A low-carbohydrate diet is effective for short-term weight loss and can help improve glycated hemoglobin, systolic blood pressure, diastolic blood pressure, and triglyceride levels; however, the long-term effects and safety of this diet remains doubtful. In the short term, there is a risk of gastrointestinal symptoms such as vomiting, diarrhea, constipation, and gastroesophageal reflux, and type 1 diabetes patients are at risk of severe hypoglycemia, while in the long term, it can lead to malnutrition and decreased exercise capacity. Thus, rather than limiting the intake of carbohydrates, it is important to limit the intake of refined grains, sugar, honey, syrup, and sweetened beverages while maintaining the planned carbohydrate intake rate and improving meal quality.

Association of Daily Sleep Duration with Obesity, Macronutrient Intake, and Physical Activity (수면시간과 비만, 열량영양소 섭취비율 및 신체 활동량과의 관련성)

  • There are a few studies that reported the association of sleep duration with calorie intake and energy expenditure. Using cross-sectional data from a population-based prospective study, we evaluated the association of sleep duration with indicators of obesity including body mass index and waist circumference, calorie intake and its proportion of macronutrients, and physical activity. The study subjects were 4,226 male and female adults, who were aged 40 to 69 years and were free of diagnosed cardiovascular disease, diabetes mellitus, and dyslipidemia at baseline. Robust regression analysis was used to analyze associations. The study found that sleep duration is inversely associated with waist circumference, calorie intake, and percent of calories from fat intake and is positively associated with percent of calories from carbohydrate intake and physical activity. The inverse association between sleep duration and waist circumference was stronger among men than among women. The inverse association between sleep duration and calorie intake was stronger among women than among men and such association was also stronger among obese persons than those with a normal body mass index. The positive association between sleep duration and physical activity was strongly demonstrated regardless of sex or obesity. Physical activity is positively associated with sleep duration independent of potential confounding factors including age, sex, income, occupation, marital status, education, smoking status, waist circumference, calorie and macronutrient intake, and alcohol intake.
