  • Two cases of plasma cell granuloma of the lung, whose incidence is 0.7% in the tumor of the lung and bronchus, are managed at department of thoracic and cardiovascular surgery, College of Medicine, Choong Nam national University Hospital from March, 1983 to July, 1984. The plasma cell granuloma of the lung, which is usually asymptomatic, is most commonly detected in routine chest films. The prognosis of the plasma cell granuloma is good after surgical resection. One case [female] was undertaken right pneumonectomy, the other, left lower lobectomy. Both cases had uneventful postoperative courses and are well being now.

자카스펭귄에서 발생한 Aspergillus fumigatus 감염증 (Aspergillus fumigatus infection in Jackass penguin)

  • A three years old male Jackass penguin(Spheniscus demersus) showed anorexia, depression, respiratory problem for a few days, followed by clinical treatment for 20 days resulted in death. Grossly, multiple white nodules measuring 2~3 mm in diameter were observed in the surface of and inside lung, liver, kidney, thoracic wall. Microscopically, granuloma formations were observed in the lung and liver. The margin of granuloma surrounded by connective tissue barrier and infiltrated lymphocytes, and also observed giant cell near the granuloma. By Periodic acid Schiff reaction, hyphae were detected in granuloma of lung and liver. This case was a chronic and systemic aspergillosis caused by Aspergillus fumigatus infection in a Jackass penguin at a zoo.

타조(Struthio camelus)에서 발생한 아스퍼질러스증 (Aspergillosis in an Ostrich (Struthio camelus))

  • A nine-month-old male ostrich (Struthio camelus) exhibited loss of appetite and dyspnea for 10 days, followed by emaciation and death. Grossly, multiple white nodules measuring 1-3 mm in diameter were observed in the surface of and inside of the lung and in the mucosa of the air sac. Microscopically, the granuloma formations were observed in the lung and air sac. The core of granuloma consisted mainly of macrophages and fibroblasts. The thin layer comprising the giant cells and macrophages surrounded the granuloma. By Periodic acid Schiff reaction, mycelia were detected especially in the core of granuloma. From the present results, the causative agent inducing the death of an ostrich was thought to be Aspergillus sp. This is the first report of the occurrence of acute Aspergillus pneumonia (brooder pneumonia) in an adult male ostrich in Korea.

  • Plasma cell granuloma of the lung is very rare and most commonly detected in routine chest films. The prognosis of this disease is usually good with surgical resection. Recently, we experienced a case of this, the diagnosis of which was confirmed by postoperative histopathological examination. The operation was right upper lobectomy under the impression of benign tumor of the lung. The postoperative course was uneventful and the patients was discharged without any complication. Now we report this with literature reviews.

A Lung Granuloma Case Possibly Associated with a Working Environment: A Case Report

  • Lung granulomas are uncommon in Thailand. The disease typically develops from an occupational environment and is mostly caused by infection. Herein is a case report of a female patient, aged 48, working as a nurse in an Accident and Emergency Department at a hospital. Eighteen years prior to admission the patient was diagnosed with myasthenia gravis and pulmonary tuberculosis. The chest X-ray and CT scans showed a solitary pulmonary nodule in the lower left lung. The patient received an open thoracotomy with a left lobectomy. Granulomatous and nonseptate hyphae were found in the pathology diagnosis. The patient was thus diagnosed as having a lung granuloma. The galactomannan antigen test was positive. The solitary pulmonary nodule-found from the use of a Polymerase Chain Reaction (PCR) test-was an Aspergillus spp. The fungus culture was collected from air samples. The air samples were collected by the impaction technique using a microbial air sampler. Three types of Aspergillus spp. were found as well as Penicillium spp. and Monilia sitophila. The Aspergillus spp. was a match for the patient's disease. The patient was diagnosed as having a lung granuloma possibly Aspergillus nodule which was caused by airborne Aspergillus spp. from the occupational environment.

  • A 31-year-old female visited O.P.D. of internal medicine, Kyung Hee Medical center, with the chief complaint of generalized weakness and headache. She took a routine chest P-A and there was a 6.5 x 7.5 cm sized round mass in the right middle lung field. She admitted to the Dept. of Thoracic and Cardiovascular Surgery under the impression of malignant bronchogenic carcinoma with the remit of bronchoscopy and sputum cytology and tomogram. Middle and lower lobectomy of right lung was performed and postoperative surgical biopsy revealed out the plasma cell granuloma. The plasma cell granuloma may occur as a solitary nodule in the lung or be associated with systemic disease, plasma protein imbalance, or nonspecific local inflammatory reaction . More than two third of the reported patients were less than 30 year of age. Grossly the lesion appears reddish-brown and microscopic features include pallisade or a cartwheel distribution of plasma cells with Russel bodies and amyloid. Local excision or lobectomy has been curative in most cases.

  • Aspegillosis in free-living birds can be occurred mostly under poor weather and climate. But, captive birds tend to be more susceptible to infection and diagnosis was made usually at post mortem. A two months old Whooper swan(Cygnus c. cygmus) dying suddenly was found in a zoo without prior clinical signs. At necropsy, numerous well- demarcated yellow to white firm nodules were scattered throughout the air sacs and the lungs. Microscopically, granuloma formations were observed in the lung and air sacs. The margin of granuloma was surrounded by connective tissue barrier and was infiltrated with lymphocytes, and also observed giant cell near the granuloma. By Periodic acid Schiff reaction, hyphae were detected in granuloma of lung and air sacs. This case was diagnosed as an invasive pulmonary aspergillosis caused by Aspergillus fumigatus infection in a Whooper swan at a zoo.

Silicone Granuloma Mimicking a Lymphatic Metastasis in a Lung Cancer Patient: A Case Report

  • Silicone granulomas are rare, benign lesions that may occur after breast augmentation. Occasionally, a careful differential diagnosis is necessary because lymphadenopathy or malignancy is suspected based on an imaging study. A 56-year-old woman who visited the hospital due to a lung nodule in the left upper lobe (LUL) underwent a staging work-up with the suspicion of lung cancer. Positron emission tomography/computed tomography and chest computed tomography revealed the LUL nodule and a lesion in the left internal mammary chain (IMC), suggesting lymphadenopathy. Diagnostic wedge resection was performed, followed by curative surgery. The final biopsy result confirmed that the LUL nodule was pathologic stage IB adenocarcinoma; unexpectedly, the lesion in the left IMC was a silicone granuloma.
