  • Background: Breast cancer is the second most common cancer in the world and by far the most frequent cancer among women. Objective: The present study was undertaken to assess the awareness of breast cancer warning signs and screening methods among the women of Pokhara valley, Nepal. Materials and Methods: A cross-sectional questionnaire survey was carried out in a community setting with the female population. The questionnaire was administered in face-to-face interviews by trained research assistants. Results: Nepalese women demonstrated poor awareness of warning signs like a breast lump, lump under the armpit, bleeding or discharge from the nipple, pulling of the nipple, changes in the position of the nipple, nipple rash, redness of the breast skin, changes in the size of the breast or nipple, changes in the shape of the breast or nipple, pain in the breast or armpit, and dimpling of the breast skin. While 100% of nurses were aware about breast self-examination(BSE), mammography and warning signs of breast cancer. Levels of knowledge were significantly poorer in women with other occupations. Graduates were more aware about BSE, mammogram and warning signs of breast cancer compared to those with other educational levels. Conclusions: The findings indicated that the level of awareness of breast cancer, including knowledge of warning signs and BSE, is sub-optimal among Nepalese women.

  • Background: Delay in diagnosis of breast cancer is associated with a poorer survival and a pivotal contribution to this delayed diagnosis comes from patient delay in presenting at a clinic. Reasons involved must be evaluated in order to decrease this reducible delay. Objectives: i) To evaluate the reasons for patient delay in diagnosis of breast cancer; ii) to investigate any association with other variables. Materials and Methods: A 6 month cross sectional study (from July 2012 to Dec 2012), was carried out in Surgical and Oncology Units of Civil Hospital, Karachi. A total of 100 females diagnosed with breast cancer of any histological type were interviewed after informed consent and relevant data were collected. Due ethical clearance was obtained. Results: Mean age was $47.5{\pm}12.1$ years with a range from 25-77 years. Mean duration of delay was $5.13{\pm}4.8$ months, from shortest 1 month to longest 36 months. Duration of delay was observed to be no delay (<1 month) in 28%, short delay (1-3 months) in 30% and long delay (>3 months) in 42% of patients. Considering the symptoms as "harmless" (39%) was the most frequent reason of delay followed by "temporary" (20%) and the "use of traditional methods" (12%). Most common reason for later approaches was an increase in the size of the lump (41%). Statistically significant association (p-value <0.05) of longer patient delay was obtained with being single, being illiterate, painless breast lump as the first symptom, negative family history of breast cancer and vague attribution of the symptoms. Conclusions: Significant delay in approach to health care facility was observed in our study due to variable reasons given by women. Sufficient awareness regarding breast cancer, its symptoms and favorable effects of a timely diagnosis on prognosis must be imparted to our general population.

  • Purpose: Abdominal cysts of gastrointestinal origin are rare. Their rarity and varied clinical presentations make their pre-operative diagnosis difficult. Methods: Fourteen patients with histological diagnosis of cysts of gastrointestinal origin admitted between 2009 and 2015 were retrospectively analyzed with respect to age, sex, clinical presentation, diagnostic modality, site and type of cyst, management, outcome and follow-up. Results: The mean age at presentation was 4 years and there were six males and eight females. Abdominal pain was the most common presenting symptom. Five patients had an acute presentation-three had distal ileal mesenteric cysts and two had ileal duplication cyst sharing a common wall with ileum. Six patients presented with chronic abdominal pain and lump-three patients had omental cysts and three had mesenteric cysts-two of these in distal ileum and one in sigmoid colon. Two patients presented with antenatally diagnosed palpable abdominal lump. One had a mesenteric cyst of the ileum and the other had a distal ileal duplication cyst which required excision with resection and anastomosis. One patient had an atypical presentation. He was a known case of sickle cell trait and had presented with vague abdominal pain, recurrent cough and multiple episodes of haemoptysis over a period of one year. At laparotomy, gastric duplication cyst was found which was excised completely. Histopathology confirmed the diagnosis. Conclusion: Cysts of gastrointestinal origin are rare and have varied presentation. Surgical excision is the mainstay of treatment. The results and prognosis are good.

  • Background: There is strong evidence that delayed diagnosis of breast cancer is associated with poor survival. Our objectives were to determine the frequency of breast cancer patients with delayed presentation, the reasons of delay and its association with different socio-demographic variables in our North Pakistan setting. Materials and Methods: We interviewed 315 histologically confirmed breast cancer patients. Delay was defined as more than 3 months from appearance of symptoms to consultation with a doctor. Questions were asked from each patient which could reflect their understanding about the disease and which could be the likely reasons for their delayed presentation. Results: 39.0 % (n=123) of patients presented late and out of those, 40.7% wasted time using alternative medicines; 25.2 % did not having enough resources; 17.1 % presented late due to painless lump; 10.6% felt shyness and 6.5% presented late due to other reasons. Higher age, negative family history, < 8 school years of education and low to middle socio-economic status were significantly associated with delayed presentation (p< 0.05). Education and socioeconomic status were two independent variables related to the delayed presentation after adjustment for others (OR of 2.26, 2.29 and 95%CI was 1.25-4.10, 1.06-4.94 respectively). Conclusions: Significant numbers of women with breast cancer in North Pakistan experience presentation delay due to their misconceptions about the disease. Coordinated efforts with public health departments are needed to educate the focused groups and removing the barriers identified in the study. Long term impact will be reduced overall burden of the disease in the region.

  • In case of analyzing PAHs (EPA 16 compounds) in oil-contaminated soils, the lump of peaks may occur because of the aliphatic and polar compounds in oil. This phenomenon is due to the lower accuracy of the analysis. To solve this problem, evaluation of application of silicagel-alumina multi-layer fraction was performed using standard substances and oil-contaminated soils. As a result of application of silicagel-alumina multi-layer fraction cleanup method using standard substances, recovery rates of surrogate standards (5 compounds including Naphthalene-d8) were 83~100% and those of target standards were 75~129%. These were to meet the target values (60~130%) in this study. When used 4% water-silicagel column analyze PAHs in oil-contaminated soils, Some problems were generated for quantitative analysis of PAHs; concentration of PAHs was underestimated due to an upward baseline of internal standard (recovery rate: less than 60%) and overestimated by the lump of peaks which were not purified (the biggest recovery rate: more than 400%). On the other hand, in case of silicagel-alumina multi-layer fraction cleanup method, recovery rate of surrogate standards were 61~101.6%. Therefore this cleanup method was considered a valid method to improve accuracy of analysis of PAHs in oil-contaminated soils.

  • Objectives: This study aims to search the clinical cases that treated leiomyoma through the traditional Chinese medicine, to understand the study trend on leiomyoma in China, and to clairfy the cause of leiomyoma and its treatment by the oriental medicine herbs. Methods: We searched the key word "leiomyoma" in the China National Knowledge Infrastructure (CNKI)[http://www.cnki.net], and narrowed its searching area to the papers published in the year 2010. Of 720 study papers, we chose 21 clinical papers on the leiomyoma, and studied the patient case, treating method, result of treatment, and checked the category, cause, treatment method of leiomyoma. Results: The results are shown in the Table 1-3. Conclusions: Leiomyoma is categorized in the traditional Chinese medicine as both lump occuring inside the body and stony abdominal lump. The main cause of leiomyoma is qi-stagnation and blood stasis, essence deficiency and pathogen affluence. The treatment method is used by activating blood and resolving stasis but harmonizing the spleen and stomach to prevent damaging healthy qi. The development herbal medicine of treating leiomyoma is Gyehongbyeolgaphwan, Gyejibongnyeonggyonang, Gungryusogyonang, Angonpyoen etc. In treating leiomyoma, the herbs were much used to regulate qi as Baekbokryung, Danggwi, Baekchul, Doin, Moryeo, Achul, Mokdanpi etc. Traditional Chinese Medicine(TCM) treatment of leiomyoma has efficiency of improve symptom but low ratio of complete recovery.

  • In various fields, several studies based on carbon nanotubes (CNTs) have been conducted. The results of previous studies, wherein CNT coatings have been incorporated as solid lubricants, demonstrate that the friction and wear characteristics of CNT coatings can be improved through the absorption/dispersion of the contact pressure by controlling the stiffness of the nanomesh structure comprising CNT strands. In this study, the friction and wear characteristics of the following are compared: CNT coating formed by spin coating of CNT solution, compressed CNT coating, and compressed/heated CNT coating (wherein CNT strands are squeezed through compression and/or heating). It is observed that the friction coefficient of the CNT coating having the largest number of voids between the CNT strands is significantly lower than those of the compressed CNT coating and the compressed/heated CNT coating. The wear tracks of the compressed CNT coating and the compressed/heated CNT coating indicate that some parts become torn or adhere into a lump. However, in the case of the CNT coating, a smooth wear surface is formed by rubbing. Furthermore, as the void space between the squeezed and adhered CNT strands decreases, the resistance to structural deformation increases, thereby resulting in an increased frictional force and a wear pattern that becomes torn or forms a lump. Hence, the results obtained from this study corroborate that the friction and wear characteristics of CNT coatings can be enhanced through the absorption/dispersion of the contact pressure by controlling the stiffness of the nanomesh structure of CNT coatings.
