Background: It has been argued that changes in muscle activity in the upper trapezius and serratus anterior may or may not cause shoulder joint pain and dysfunction. Objective: To investigate the effects of active vibration exercise on muscle activity regarding scapular stabilization using a flexi-bar. Design: A randomized controlled trial. Methods: A total of 24 subjects were randomly assigned to a flexi-bar group with active vibration and general stick group with non-active vibration. Both groups performed the same four action programs for 6 weeks, three times a week for 30 minutes at a time. The upper trapezius muscle, middle trapezius muscle, lower trapezius muscle and serratus anterior muscle of the dominant side was measured by electromyography before and after the 6 weeks of exercise. The independent t-test and paired t-test were used to analyze data. Results: There was a significant difference between groups in upper trapezius muscle and serratus anterior muscle activity after exercise (P<.05). Also, there was a significant difference in upper trapezius muscle and serratus anterior muscle activity before and after exercise in the flexi-bar group (P<.05). Conclusion: This study demonstrates that active vibration exercise using a flexi-Bar contribute to reduce the activity of the upper trapezius muscle and promote the activity of the serratus anterior muscle.
Background: Although the scapular posterior tilt movement could facilitate the lower trapezius (LT) muscle activity, no study identified the effects of the scapular posterior tilt movement on the selective activation of the LT muscle during prone shoulder extension. Objectives: To examine the influences of additional scapular posterior tilt on electromyography (EMG) of the upper trapezius (UT) and the LT muscles during prone shoulder extension. Design: Cross-sectional study. Methods: There were 15 asymptomatic male participants in this study who performed prone shoulder extension with and without scapular posterior tilt movements. For the scapular posterior tilt movements, participants performed visual biofeedback training for scapular movement using motion sensor. During the exercises, the EMG activity of the UT and LT was recorded using surface EMG system. Results: The EMG activity of the LT significantly increased during prone shoulder extension with scapular posterior tilt compared to that of general prone shoulder extension, whereas that of the UT was not significantly different between the two exercises. Moreover, scapular posterior tilt application significantly decreased UT/LT muscle activity ratio. Conclusion: Scapular posterior tilt movement may be emphasized during exercise when facilitating LT muscle activation.
Purpose: The purpose of this study was to assess how much neurac technic affects the stability muscles of the shoulder according to the supporting surface in crawling position. Method: 29 healthy adult males participated in this experiment. The experiment consists of 2 experimental groups and 1 control group. It is conducted for 3 weeks. Each group exercised for 7 seconds and then rested for 3 seconds. Using EMG, the average value of serratus anterior, upper trapezius, and lower trapezius was repeated three times. Experimental group 1 conducted push up plus exercise applied neurac technic with sling on unstable supporting surface. Experimental group 2 conducted push up plus exercise applied neurac technic with sling on stable supporting surface. Control group conducted the push up plus exercise on stable supporting surface. One-way ANOVA was used for statistical analysis. Result: It was noticeable that the result of Experimental group 1 affects stability muscles of more than Experimental group 2 and Control group. Conclusion: Push up plus position applied neurac technic with sling on unstable supporting surface was the most effective in activating the stability muscles of the shoulder.
PURPOSE: Excessive activity of the upper trapezius (UT) muscle contributes to a rounded shoulder posture (RSP) through abnormal rotation of the scapula that weakens the lower trapezius (LT) and serratus anterior (SA) muscles. This study compared the effects of two arm-lifting exercises with and without the use of a Thera-band on the activities of LT, SA, and UT muscles, and the LT/UT and SA/UT activity ratio in subjects with a rounded shoulder posture. METHODS: Sixteen subjects with RSP participated in this study. All subjects performed arm-lifting (AL), diagonal arm-lifting (DAL), arm lifting with isometric adduction (ALIA), and diagonal arm-lifting with isometric adduction (DALIA) exercises. The surface electromyography data, LT, SA, UT, and the LT/UT and SA/UT activity ratios were measured. A paired t-test was used to compare the differences between two arm-lifting exercises and two arm lifting with isometric adduction exercises. RESULTS: In ALIA and DALIA exercises, the UT muscle activity decreased significantly, whereas the LT/UT, SA/UT activity ratio increased significantly. The activity of SA muscle increased significantly more with DALIA than that with DAL. CONCLUSION: Arm-lifting exercises using a Thera-band can be implemented as an effective way to reduce the UT overactivity and increase the SA activity and LT/UT, SA/UT activity ratio in subjects with RSP.
PURPOSE: This study was implemented to measure the muscle activity of muscles surrounding shoulder during push-up exercise according to the slope angle. METHODS: This study has research subject consisting of 25 normal male adults without neurologic or musculoskeletal injury in shoulder part. EMG was used as a tool for measuring muscle activity and four muscles including upper trapezius (UT), middle trapezius (MT), Lower Trapezius (LT), and Serratus Anterior (SA) were chosen for measuring the muscle activity of muscles surrounding shoulder. Tilting table slope was set at $0^{\circ}$, $30^{\circ}$, $45^{\circ}$, and $60^{\circ}$ and push-up motion was performed three time for each case to use the average value. The measured value was used after generalization process to create %RVC value where the measured value was standardized by using $0^{\circ}$ as a standard. RESULTS: There was a difference of muscle activity according to the tilting table slope during push-up exercise. In UT, MT, and SA, there was no significant $0^{\circ}$ and $30^{\circ}$, $45^{\circ}$ and $60^{\circ}$. But there was significant $30^{\circ}$ and $45^{\circ}$. LT was significant difference between $45^{\circ}$ and $60^{\circ}$ (p<.05). CONCLUSION: There are cases where push-up exercise should be selectively implemented due to shoulder problems. According to this results between $0^{\circ}$ and $30^{\circ}$, push up will be able to more effectively exercise. Applying proper angle of tilting table for push-up to patients who have difficulties in performing motions along with physical consumption can deliver effective and easy exercise program.
Objective: This study covered the effect of stabilization exercise on muscle activity, shoulder painanddisability index(SPADI) oftheelderly people with chronic shoulder pain by tele-rehabilitation village hall in rural area. Design: A randomized controlled trial. Methods: The study subjects are 29 elders with chronic shoulder pain aged over 65-year-old who reside in the rural area (experimental group: 14 elders and control group: 15). The elderly people in the experimental group were asked to perform shoulder stabilization exercise 60 minutes per one time for 4 weeks and three times in a week and the others in the control group were asked to receive education related to pain management for the initial one time. Before and after the experiment, we measured the subjects' muscle activity of upper trapezius, serratus anterior and the lower trapezius. In addition, SPADI is measured through the questionnaire. Results: Compared to the control group, the experimental group showed significant differences in the muscle activity of the three muscles (upper trapezius, serratus anterior, lower trapezius), and SPADI. Conclusions: It is found that shoulder stabilization exercise is effective in muscleactivity, pain and ability of the elderly people through tele-rehabilitation service. This study should be used for improving the quality of the elderly people's lives through active tele-rehabilitation service for areas where have poor medical benefits.
Purpose: The purpose of this study was to compare therapeutic climbing exercise and general isometric exercise in patients with shoulder impingement syndrome. Methods: Among 20 adults, study subjects were arbitrarily classified into an experimental group of 10 and a control group of 10. The control group performed general isometric exercise (ISE) and the experimental group performed therapeutic climbing exercise (TC) (3 sets, 3 times per week for 8 weeks). To evaluate the effects of exercise, subjects were evaluated using a Disabilities of the arm, shoulder and hand score (DASH), a goniometer for range of motion, and shoulder activity measured serratus anterior, upper trapezius, and lower trapezius. Independent and paired t-test were used for comparison of the effect between groups. Results: DASH scores showed a significant decrease in both groups after 8 weeks of treatment (p<0.001) and significant difference was observed between the TC groups (p<0.01). Flexion and abduction were significantly increased after 8 weeks of treatment in the ISE group (p<0.001) and flexion, abduction, external and internal rotation were significantly increased after 8 weeks of treatment in the TC group (p<0.001). Serratus anterior and lower trapezius activity were significantly increased after 8 weeks of treatment (p<0.001) and upper trapezius activity was not significantly increased after 8 weeks of treatment in the ISE group (p>0.05). Serratus anterior, lower trapezius, and upper trapezius activity were significantly increased after 8 weeks of treatment in the TC group (p<0.001). Conclusion: Scapular stabilizing exercise using a therapeutic climbing exercise increases range of motion and decreases DASH, and increases activity of shoulder muscles in patients with impingement syndrome.
Purpose : The purpose of this study was to examine activities of the scapular stabilizers during push-up plus and PNF backward rocking exercises. Methods : 9 subjects performed randomly push-up plus and PNF backward rocking exercises in four point kneeling. The activities of 4 scapular stabilizing muscles were compared between push-up plus and PNF backward rocking exercises. Results : PNF backward rocking exercise showed significantly higher lower trapezius and lower serratus anterior activations compared to push-up plus exercise. Push-up plus exercise showed significantly higher middle serratus anterior activation than PNF backward rocking exercise. Conclusion : PNF backward rocking exercise facilitates the lower trapezius and serratus anterior activations compared to push-up plus exercise.
Purpose : The purpose of this study was to compare the effect of exercise on the neck muscles activities between scapular stability and McKenzie stretch exercise in the forward head posture subjects. Method : After measuring cervical alignment using the Global Posture System, 20 volunteers with forward head posture posture were selected and divided into two groups. The experimental group A(n=15) and Experimental group B(n=15) participated in respectively and McKenzie neck stretching exercise, three times per week for 4 weeks. The scapular stabilization exercise program was comprised middle and lower trapezius strength exercises and the stretching exercise program was comprised levator scapulae and upper trapezius stretching exercise. The activities of the muscles of the posterior neck was then measured using electromyography. Result : After the intervention, there was significant difference of a electromyography activity changes between the pre-test and post-test in the experimental group. Conclusion : This study showed that both scapular stabilization and McKenzie neck stretching exercises are more effective for reducing neck muscles activities.
