Impulse Trafficking in Neurons of the Mesencephalic Trigeminal Nucleus

  • In the primary sensory neuron of the mesencephalic trigeminal nucleus (MTN), the peripheral axon supplies a large number of annulospiral endings surrounding intrafusal fibers encapsulated in single muscle spindles while the central axon sends only a few number of synapses onto single ${\alpha}-motoneurons({\alpha}-MNs)$. Therefore, the ${\alpha}-{\gamma}$ linkage is thought to be very crucial in the jaw-closing movement. Spike activity in a ${\gamma}-motoneuron\;({\gamma}-MN)$ would induce a large number of impulses in single peripheral axons by activating many intrafusal fibers simultaneously, subsequently causing an activation of ${\alpha}-MNs$ in spite of the small number of synapses. Thus, the activity of ${\gamma}-MNs$ may be vital for modulation of jaw-closing movements. Independently of such a spindle activity modulated by ${\gamma}-MNs$, somatic depolarization in MTN neurons is known to trigger the oscillatory spike activity. Nevertheless, the trafficking of these spikes arising from the two distinct sources of MTN neurons is not well understood. In this short review, switching among multiple functional modes of MTN neurons is discussed. Subsequently, it will be discussed which mode can support the ${\alpha}-{\gamma}$ linkage. In our most recent study, simultaneous patch-clamp recordings from the soma and axon hillock revealed a spike-back-propagation from the spike-initiation site in the stem axon to the soma in response to a somatic current pulse. The persistent $Na^+$ current was found to be responsible for the spike-initiation in the stem axon, the activation threshold of which was lower than those of soma spikes. Somatic inputs or impulses arising from the sensory ending, whichever trigger spikes in the stem axon first, would be forwarded through the central axon to the target synapse. We also demonstrated that at hyperpolarized membrane potentials, 4-AP-sensitive $K^+$ current ($IK_{4-AP}$) exerts two opposing effects on spikes depending on their origins; the suppression of spike initiation by increasing the apparent electrotonic distance between the soma and the spike-initiation site, and the facilitation of axonal spike invasion at higher frequencies by decreasing the spike duration and the refractory period. Through this mechanism, the spindle activity caused by ${\gamma}-MNs$ would be safely forwarded to ${\alpha}-MNs$. Thus, soma spikes shaped differentially by this $IK_{4-AP}$ depending on their origins would reflect which one of the two inputs was forwarded to the target synapses.

사상체질별(四象體質別) 두면부(頭面部)의 형태학적(形態學的) 특징(特徵) (A Morphologic Study of head and face for Sasang Constitution)

  • 1. 연구배경 : 사상의학의 가장 중요한 요점은 체질진단이다. 그간 다양한 체질변증에 관한 연구가 있었지만 신체 각부의 형태학적인 연구, 특히 얼굴을 대상으로 한 연구는 거의 전무하였다. 저자들은 그 동안 전일적이고 직관적으로 표현된 동의수세보원의 형태적 묘사를 정량화하여 정리하고, 이 자료를 체질진단의 근거자료로 삼고자 하는 연구의 일환으로 먼저 두안부(頭顔部)의 형태를 태음인 소음인 소양인 별로 그 형상적 특징을 찾아내고자 하였다. 2. 방법 : 1995년 7월부터 1995년 12월까지 경희의료원 부속한방병원 외래환자 및 직원을 대상으로 설문지 및 임상적 치료 경과를 통하여 체질적 경향성이 뚜렷한 대상자를 선별하고 동일촬영 조건을 통하여 얻은 얼굴 사진 중 형태특이자를 제외한 170례의 고경(전두고외 26항목) 방사경(두최대장외 22항목) 및 폭경(안최대폭외 18항목)등 총 69항목을 측정한 후 이를 분석하여 체질별 상이점을 도출하였다. 3. 결과 : 체질별 두면부의 형태학적인 특징을 수치화하여 설명할 수 있게 되었고, 나아가 직관적이고 전일적인 형태를 구체화시킬 수 있는 특징을 도출할 수 있었다. 4. 결론 : 이상의 결과를 이용하여 체질별 각 부분의 형태를 계량화하여 비교 설명하고, 이를 이용하여 체질판별공식을 만들었다.

Denta $Scan^R$을 이용한 즉시 임플랜트 시술시 최적의 식립 위치 대한 통계적 연구 (STATISTICAL STUDY ON OPTIMAL PLACEMENT OF IMMEDIATE DENIAL IMPLANTATION USING DENTA $SCAN^R$)

  • Purpose : The purpose of this study was to determine proper position and angulation of an implant for immediate implantation. Materials and Method : From the years 1997 to 2000. 52 Denta $scan^R$ views, 22 upper and 32 lower jaw with an average age of 43 and 40 respectively, were investigated, which comprise intact upper and lower 6 anterior teeth and premolars. On the Denta $scan^R$, the optimal placement for the immediated implantation was simulated. The measuring methods included 1) Angulation difference between tooth long axis and alveolar bone process. 2) Angulation difference of long axis between tooth and installing fixture 3) Distance between center of tooth at cervical area and center of fixture. 4) Distance from root apex to the bone limit of vital structure. One sample t-test was used for statistical analysis. Result : The results were as follows. 1) At the maxillary central incisor and lateral incisor, angulation difference of long axis between tooth and installing fixture was respectively 0.5 and 3.2 degrees with the fixture center's palatally positioned 2mm apart from tooth center. 2) At the lower anterior 6 teeth, that was about $-2.8^{\circ}\;to\;-4.6^{\circ}$ with the fixture center's lingually positioned 1mm apart from tooth center. 3) At the maxillary canine and premolar, that was respectively $11.8^{\circ}\;and \;7.2^{\circ}$ with the fixture center palatally positioned $2\sim2.4mm$ apart from tooth center. 4) At the lower premolar area, that was about $0^{\circ}\;to\;2^{\circ}$ with the fixture center's lingually positioned $0.5{\sim}1mm$ apart from tooth center. 5) Distance from root apex to the bone limit of vital structure, at the maxillary anterior and premolars. was the range of 10 to 12mm, and at the mandibular anterior teeth and the 1st premolar, that was the range of 18 to 20mm. Conclusion : The proper implant position of maxillary anterior and premolar teeth is as paralleled as or more buccally angulated than long axis of tooth with the fixture center's palatally positioned. In mandiblular anterior region, long axis of implants is lingully angulated compared with long axis of tooth and in premolar, almost parelleled with long axis of tooth and alveolar process.

골격성 III급 부정교합자의 편악(하악)수술후 연조직 변화의 평가 (Soft Tissue Change After Single Jaw(mandible) Surgery in Skeletal Class III Malocclusion)

  • 본 연구는 골격성 III 급 부정교합 환자에서 하악골 후방이동수술을 시행하였을 때 경, 연조직 변화를 관찰하고 수술전후의 경, 연조직 변화의 상관성을 산출하여 교정-악교정 수술 복합 치료 계획의 수립과 결과의 예측에 이용하고자 하였다. 영남의료원 치과에 내원하여 상하악골의 수직적인 부조화는 경미하고 전후방적인 부조화가 심한 골격성 III급 부정교합으로 진단되어 수술전 교정치료를 받고 1989년 10월부터 1997년 1월 중에 하악골 시상분할 골절단술로 하악골의 후방이동을 시행한 성인 환자 25명(남자 12명, 여자 13명)을 대상으로 수술전후 측모두부방사선사진을 계측, 분석하여 다음의 결과를 얻었다. 1. 하악골의 시상분할 골절단술로 하악골의 후방이동에 따른 하안면부의 연조직의 수평적인 후방 이동은 상당한 유의성을 가지고 나타났으나 수직적인 변화는 거의 없었다. 2. 하악골의 시상분할 골절단술로 하악골의 후방이동을 시행한 경우 상순의 상대적인 돌출도는 증가하고 (p<0.01) 하순의 상대적인 돌출도는 감소하였으며(p<0.01) 하악골 전방부의 연조직의 두께는 증가하였다(99% 유의수준).

성장기 골격성 III급 부정교합 환자의 상악골 전방 견인 시 하안모 형태에 따른 치료 효과 비교 (Treatment effect of face mask therapy for Class III malocclusion patients according to low facial morphology)

  • 성장기 아동의 상악골 전방 견인을 이용한 치료 효과 중 하악골의 후하방 회전은 III급 골격관계를 개선시키지만 하안모의 길이가 길어져 face mask가 장안모 환자에게는 비적응증 이라는 것이 선학들의 일반적 견해였다. 그러나 실제로 하안모의 길이를 분류 기준으로 삼아 이에 따른 치료 효과를 비교한 연구는 부족한 실정이며 경조직 변화에 따른 연조직 변화의 연구 또한 미비하였다. 본 연구에서는 상악골 열성장을 보이는 성장기 골격성 III급 부정교합 환자 44명을 Ricketts의 lower facial height (LFH)기준에 따라 LFH가 정상 수치 이상인 군(1군)과 미만인 군(2군)으로 분류하여, 상악골 전방 견인 시 두 군간의 경조직, 연조직 치료 효과를 비교하고 경조직과 연조직의 상호 관계를 비교 분석하여 다음과 같은 결론을 얻었다. 상악의 경조직, 연조직 전방 이동량은 두 군간에 유의한 차이가 없었다. 하악의 경조직 후방이동량은 두 군간에 유의한 차이가 없었으나 연조직 후방이동량은 2군에서 더 크게 나타났다. 하안면의 경조직 수직 비율 변화는 두 군간에 유의한 차이가 없었으나 연조직 수직 비율 변화는 2군에서 더 크게 나타났다. 상악과 하악의 전후방적 경조직, 연조직 변화는 유의한 상관관계를 갖는 반면 수직적 하안면 비율 변화는 유의한 상관관계를 갖지 않았다. 위의 결과들을 고려해 볼 때 Face mask 사용 결과 상악골의 전방 이동과 하악골의 후하방회전이 일어났으며 하악의 연조직 후방 이동량은 단안모에서 더 크게 나타났다.

정상교합자의 골격형에 따른 치아치조보상 (Dentoalveolar Compensation according to Skeletal Patterns of Normal Occlusion)

  • 일반적으로 교정 환자의 문제점을 파악하고 치료 계획을 수립할 때에 여러 분석법의 정상치를 기준으로 하는 경우가 많다. 하지만 다양한 악골관계 하에서 정상적인 악궁관계를 유지하기 위해 3차원적으로 발생하는 치아치조부 보상 기전을 고려한다면 좀 더 개별화된 치료 목표와 치료계획을 수립하는데 도움이 될 수 있을 것이다. 이에 본 연구에서는 정상교합자 125명의 측모 두부계측방사선사진을 계측하여 정상교합자의 골격양상의 다양성을 살펴보았다. 또한 여러 골격계측 기준을 이용한 군집분석을 적용하여 정상교합자의 골격형을 전후방으로 세 군, 수직적으로 세 군으로 분류하고 각 골격형에 따른 치아치조부 보상양상을 조사하여 다음과 같은 결과를 얻었다. 1. 정상교합자의 전후방적, 수직적 악골관계는 매우 다양하였다. 2. 하악골이 상악골에 비해 전방 위치할수록 상악 전치는 순측으로 경사 되고 하악 전치는 설측으로 경사 되며 교합평면의 경사도는 완만해졌다. 3. 수직적인 골격양상일수록 상악 전치부의 치아치조 고경이 증가하고, 하악 구치부의 치아치조 고경이 감소하였으며 구개평면에 대한 상악 구치와 하악평면에 대한 하악 전치 및 하악 구치는 직립 하였고 교합평면과 하악평면사이의 각(OMA)은 증가하였다. 4. 상악 전치보다 하악 전치에서 전후방 및 수직적인 악골관계에 대해 더 많은 치아치조부 보상양상을 보였다.

정상교합과 부정교합에서의 상${\cdot}$하악골과 제 1 대구치 위치에 관한 비교연구 (A COMPARATIVE STUDY ABOUT THE POSITION OF UPPER AND LOWER JAWS, AND FIRST MOLARS IN NORMAL OCCLUSION AND ANGLE'S CLASS $I{\cdot}II{\cdot}III$ MALOCCLUSIONS)

  • There has been so much controversies about the position of upper and lower jaws, and their first permanent molars in normal occlusion and Angle's class $I{\cdot}II{\cdot}III$ malocclusions. So, the purpose of this study is to compare the position of upper and lower jaws, and their first molars in normal occlusion and Angle's class $I{\cdot}II{\cdot}III$ malocclusions by lateral cephalometric analysis. The sample consisted of one hundred and twenty girls(thirty in each group) who had completed growth. The findings of this study were as follows : 1. In class I malocclusion, both maxilla and mandible were slightly posterior position than normal occlusion, but they showed harmonious relationship. 2. In class II malocclusion, the mandible was greatly retruded, and the maxilla was also slightly retruded to the cranial base as compared with normal occlusion. 3. In class III malocclusion, the maxilla was significantly retruded to the cranial base, but no significant difference was found in mandibular position as compared with normal occlusion. 4. The maxillary first molar was located at posterior position in class II malocclusion, and anterior position in class III malocclusion to the cranium, so that the rotation of mandible was influenced by that. 5. The mandibular first molar showed constant relationship to the mandible in all four groups, but different position to the cranial base in direct proportion to the mandibular position. 6. On the treatment planning of class III malocclusion, it seems to be better to promote the mandibular horizontal growth by inhibiting the vertical growth of maxillary molar area, and on the treatment planning of class III malocclusion, it seems to be better to promote the antero-inferior growth of maxilla mi to promote the mandibular vertical growth by inducing the vertical growth of maxillary molar area.

안면골의 성장회전에 관한 연구 (VARIATION IN THE GROWTH PATTERN OF THE FACE: A LONGITUDINAL COMPUTERIZED RECTILINEAR CEPHALOMETRIC STUDY)

  • Variation in the facial pattern and effect of the rotational jaw growth on the facial proportion were studied in serial cephalometric radiographs of 40 Korean children (25 boys, 15 girls) ranging in age from 6 to 13 years. According to Y-axis (N-S-Gn) growth change during the 7 years of period, the subject who had exhibited 'wave-like' manner of Y-axis growth change within ${\pm}\;2^{\circ}$ was classified to the Parallelwise group and the subject who had exhibited Y-axis increase more than $+2^{\circ}$ was classified to the Clockwise rotation group and the subject who had exhibited Y-axis decrease more than $-2^{\circ}$ was classified to the Counterclockwise rotation group. For the comparison of each group, a total of 22 morphologic variables were employed and the data were analyzed by means of computer morphometrics and statistical methods. On the basis of the finding of this study, the following trends were established. 1. The Parallelwise group was $75\%$, the Clockwise rotation group was $12.5\%$ and the Counter-clockwise rotation group was $12.5\%$. 2. The growth pattern of cranial base was related to the rotation of mandible. 9. Maxillary prognathism was occured in the Counterclockwise rotation group and the rotation of palatal plane was occured in the Clockwise rotation group. 4. Mandibular prognathism was occured in the Parallelwise and the Counterclockwise rotation groups, especially in the Counterclockwise rotation group. 5. The degree of maxillo-mandibular divergency was constant in the Clockwise rotation group but decreased in the Counterclockwise rotation and the Parallelwise groups, especially in the Counterclockwise group. 6. There were no differences in the size of the anterior upper facial height (N-ANS) and the posterior lower facial height ((Go-Me)-PNS) but there were differences in the size of the anterior lower facial height (ANS-Me) and the posterior upper facial height ((S-N)-PNS) between each group. 7. The growth increment and the size of the facial depth was not related to the growth pattern of the face but the growth increment and the size of the facial height was related to the growth pattern of the face. 8. Proportional change due to the facial growth rotation was concentrated in the anterior lower face. 9. The most apparent difference between each group was happened in the size of the posterior cranial base (S-Ba) and the facial length (S-Gn).

The Relative Centrifugation Force Permits Visualization of the Germinal Vesicle in Pig Oocytes

  • Pig oocytes contain high levels of lipids in the ooplasm, which reduces the visibility of the germinal vesicle (GV) under microscopic examination. Therefore, the purposes of this study were to investigate the effects of relative centrifugation force (RCF) on the visibility and maturation rates of the GV stage oocytes after centrifugation. In Experiment 1, cumulus-oocyte-complexes (COCs) were collected from slaughterhouse ovaries and randomly allocated to different RCFs (3,000 rpm: 970 g; 6,000 rpm: 3,900 g; or 10,000 rpm: 10,840 g) for 10 or 20 min. Percentages of visible GV were 76-79% in the oocytes centrifuged with 10,000 rpm, which were significantly higher (p<0.01) than those with 3,000 and 6,000 rpm. No significant differences in GV visibility were observed among oocytes with different lengths of centrifugation (p<0.05) regardless of the RCFs. In esperiment 2, the maturation rate of the oocyte was found significantly lower in the 20 min than in the 10 min group received 10,840 g of RCF (30 vs. 75%, p<0.05). In conclusion, the GV of porcine oocytes can be clearly visible by centrifugation at 10,840 g for 10 min without compromising their subsequent maturation rates and a longer centrifugation time (20 min) had no beneficial influence on the visibility of GV stage pig oocytes.

심한 하악골 파괴를 동반한 화농성 육아종 (Pyogenic Granuloma with Severe Mandibular Bone Destruction)

  • Pyogenic granuloma is a overzealous proliferation of a vascular type connective tissue as a result of some minor trauma and is a well circumscribed elevated, pedunculated or sessile benign inflammatory lesion of skin and mucous membrane. The clinical features of pyogenic granuloma are indicative but not specific and nearly all cases of pyogenic granulomas are superficial in nature, and there is little if any mention in the literature of these lesions producing alveolar bone even jaw bone loss. This case is somewhat unique in that the lesion was an obvious histologic pyogenic granuloma; however, it appeared to invade the mandibular bone which resulted in the loss of the adjacent teeth. A 12-year-old boy came to Seoul National University Dental Hospital with chief complaints of left facial swelling. The features obtained were as follows; Plain radiograms showed a large well-circumscribed radiolucent lesion on left mandibular ramus area, which made severe expansion of lingual cortex and displacement of lower left 3rd molar tooth germ. Computed tomograms showed large soft tissue mass involving left masticator space with destruction of left mandibular ramus. Histologically, sections revealed loose edematous stroma with intense infiltration of inflammatory cells and proliferation of vascular channels. Also, there were focal areas of extensive capillary proliferation, bone destruction and peripheral new bone formation.
