제 2형 성인 당뇨병 유병자의 혈당조절 취약군 예측: 제7기(2016-2018년도) 국민건강영양조사 자료 활용 (Identification of Subgroups with Poor Glycemic Control among Patients with Type 2 Diabetes Mellitus: Based on the Korean National Health and Nutrition Examination Survey from KNHANES VII (2016 to 2018))

  • Purpose: This study was performed to assess the level of blood glucose and to identify poor glycemic control groups among patients with type 2 diabetes mellitus (DM). Methods: Data of 1,022 Korean type 2 DM patients aged 30-64 years were extracted from the Korea National Health and Nutrition Examination Survey VII. Complex samples analysis and a decision-tree analysis were performed using the SPSS WIN 26.0 program. Results: The mean level of hemoglobin A1c (HbA1c) was 7.22±0.25%, and 69.0% of the participants showed abnormal glycemic control (HbA1c≥6.5%). The characteristics of participants associated with poor glycemic control groups were presented with six different pathways by the decision-tree analysis. Poor glycemic control groups were classified according to the patients' characteristics such as period after DM diagnosis, awareness of DM, sleep duration, gender, alcohol drinking, occupation, income status, low density lipoprotein-cholesterol, abdominal obesity, and number of walking days per week. Period of DM diagnosis with a cut-off point of 6 years was the most significant predictor of the poor glycemic control group. Conclusion: The findings showed the predictable characteristics of the poor glycemic control groups, and they can be used to screen the poor glycemic control groups among adults with type 2 DM.

Six-year clinical performance of lithium disilicate glass-ceramic CAD-CAM versus metal-ceramic crowns

  • PURPOSE. To assess the clinical performance of monolithic CAD-CAM lithium disilicate glass-ceramic (LDGC) crowns and metal-ceramic (MC) crowns provided by predoctoral students. This study also assessed the effects of patient and provider-related factors on their clinical performance as well as patient preference for these types of crowns. MATERIALS AND METHODS. Twenty-five patients who received 50 crowns (25 LDGC CAD-CAM and 25 MC) provided by predoctoral students were retrospectively examined. LDGC CAD-CAM crowns were milled in-house using the CEREC Bluecam system and cemented with either RelyX Unicem or Calibra Esthetic resin cements. MC crowns were cemented with RelyX Unicem cement. Clinical assessment of the crowns and the supporting periodontal structures were performed following the modified California Dental Association (CDA) criteria. Patients' preference was recorded using a visual analog scale (VAS). The results were statistically analyzed using log-rank test, Pearson Chi-squared test and Kaplan-Meier survival analysis. RESULTS. Twelve complications were observed in the MC crown group (9-esthetic, 2-technical and 1-biological). In comparison, 2 complications in the LDGC CAD-CAM crown group were observed (1-technical and 1-esthetic). The 6-year cumulative survival rates for MC crowns and LDGC CAD-CAM were 90.8% and 96%, respectively, whereas the success rates were 83.4% and 96%, respectively. Overall, patients preferred the esthetic outcomes of LDGC CAD-CAM crowns over MC crowns. CONCLUSION. The high survival and success rates, low number of complications, and the high level of patients' acceptance of monolithic LDGC CAD-CAM crowns lend them well as predictable and viable alternatives to the "gold standard" MC crowns.

Real-time wireless marine radioactivity monitoring system using a SiPM-based mobile gamma spectroscopy mounted on an unmanned marine vehicle

  • Marine radioactivity monitoring is critical for taking immediate action in case of unexpected nuclear accidents at nuclear facilities located near coastal areas. Especially when the level of contamination is not predictable, mobile monitoring systems will be useful for wide-area ocean radiation survey and for determination of the level of radioactivity. Here, we used a silicon photomultiplier and a high-efficiency GAGG crystal to fabricate a compact, battery-powered gamma spectroscopy that can be used in an ocean environment. The developed spectroscopy has compact dimensions of 6.5 × 6.5× 8 cm3 and weighs 560 g. We used LoRa, a low-power wireless protocol for communication. Successful data transmission was achieved within 1.4 m water depth. The developed gamma spectroscopy was able to detect radioactivity from a 137Cs point source (3.7 kBq) at a distance of 20 cm in water. Moreover, we demonstrated an unmanned radioactivity monitoring system in a real sea by combining unmanned surface vehicle with the developed gamma spectroscopy. A hidden 137Cs source (3.07 MBq) was detected by the unmanned system at a distance of 3 m. After successfully testing the developed mobile spectroscopy in an ocean environment, we believe that our proposed system will be an effective solution for mobile real-time marine radioactivity monitoring.

비만아동 및 청소년의 체중조절 경험과 비만관리 의도에 영향을 미치는 요인 (Factors influencing weight control behavior and intention of obese children and adolescents)

  • This study was undertaken to explain weight control behavior and intention of obese children and adolescents as measured by the elements of the health belief model. A total of 732 obese students from 28 schools in Seoul metropolitan area and their mothers were assessed with a self-administered questionnaire. The analyzed results are as follows; 1. Among obese students, 45.3% of male students and 57.2% of female students, a significantly higher portion than male students, reported that they had tried to lose weight within the recent year. Exercise was the most frequently used method to lose weight followed by diet control, drug use, and specialized clinic visits, in descending order. 2. Male students were more likely to try to lose weight if they perceived a low threat level and their mother had a job, and female students were more likely to try to lose weight if they were younger in age, perceived a low threat level and had strong external motivating factors. 3. Female students showed a significantly higher level of intention to obesity control than male students, and the intention level of their mothers also showed the same trend. 4. In male students, the degree of weight dissatisfaction, weight control experience, the level of obesity related beliefs of students, the educational level of the mother and economic status of the family were significant predictors of intention to obesity control, and in females, age, the level of obesity related beliefs of students and intention of their mothers were significant. In the mothers of male students, obesity index of students, age of the mother and the level of obesity related beliefs of the mother were significant predictors of intention of the mother, and in the mothers of female students, obesity index of students, occupational status of the mother and obesity related beliefs of the mother were significant. 5. According to the path model of intention to obesity control, the degree of weight dissatisfaction had the most powerful effect in male students, and perceived net benefit level was the most important variable in female students. Since the weight control behavior and intention of obese students were more predictable by the degree of weight dissatisfaction than the obesity index, we can conclude that only the students dissatisfied with their weight are well motivated for obesity control. There can be a discrepancy between the mother and her child's beliefs and intention status(especially in male students), so the therapists should also assess the student's opinion as well as the mother's. In female students, the perceived net benefit level wag the most important predictor of intention to obesity control, therefore the intervention program should pay particular attention to the positive benefits of weight control rather than negative aspects(threats) of obesity.

당근의 삼투건조시 물질이동 특성 (Mass Transfer Characteristics in the Osmotic Dehydration Process of Carrots)

  • 삼투건조시 물질이동 특성을 알아보기 위하여 수분과 용질의 이동을 확산식으로 평가하여 침지온도와 농도에 따른 영향을 조사하였으며 또한 품질변화 정도는 carotene 함량을 지표로 하여 반응속도식으로 구명하고자 하였으며 삼투공정시 수분의 이동을 기존의 건조모델로 표현하고자 적합도를 알아보았다. 수분손실이나 용질의 증가는 온도와 농도가 증가함에 따라 높은값을 보였으며 농도보다는 온도의 영향을 많이 받는 것으로 나타났다. 확산계수 또한 고온과 고농도에서 높은 값을 가져 확산이 빠르게 일어남을 알 수 있었다. 고온에서는 수분 손실이 용질의 증가보다 높아 용질의 확산계수가 수분의 확산계수보다 높았으며, 품질변화를 나타내는 반응속도상수는 온도의 증가보다 농도의 증가에 따라 더 큰 값을 가져 고농도에서 파괴가 많이 일어남을 알 수 있었다. 확산계수와 품질변화 속도상수에 대한 온도의 영향을 알아보고자 Arrhenius 식에 적합시켜 본 결과 낮은 농도인 $20^{\circ}Brix$에서 확산에 많은 활성화에너지가 필요하고 고농도인 $60^{\circ}Brix$에서는 상대적으로 낮은 활성에너지로도 용질의 확산이 일어난다는 것을 알 수 있었고 활성화 에너지의 크기로 보아 수분의 이동은 $40^{\circ}Brix$에서, 용질의 이동은 $60^{\circ}Brix$에서 이동이 효과적으로 일어남을 알 수 있었다. 침지온도와 농도를 독립변수로 하여 확산계수와 반응속도상수를 예측하고자 각 독립 변수의 최적함수를 구하여 수립한 최적 함수 모델식과 polynomial 형태로 모델화 하였을 경우 수분이나 용질의 확산계수는 높은 적합도를 결정계수를 가지나 품질변화를 나타내는 반응속도강수는 다소 낮아 온도와 농도의 함수로 예측하기에는 미흡한 것으로 나타났다. 삼투건조 공정 중 수분의 이동을 시간의 함수로 표현하기 위한 가장 적합한 모델은 quadratic 모델의 $R^{2}$값이 침지온도와 농도에 관계없이 전반적으로 0.99 이상으로 나타나 다른 모델보다 더 높은 적용 가능성을 보였다. 따라서 quadratic 모델을 이용하여 삼투건조공정에서의 시간에 따른 수분함량을 예측할 수 있으며 확산계수와 아울러 물질이동 특성을 나타낼 수 있을 것으로 생각된다.

상악동 골이식술을 동반한 임플란트 식립 시 골이식재의 높이 변화에 대한 방사선학적 평가 (A radiographic evaluation of graft height changes after maxillary sinus augmentation and placement of dental implants)

  • The edentulous posterior maxilla generally provides a limited amount of bone height because of atrophy of the ridge and pneumatization of the maxillary sinus, Maxillary sinus augmentation is one of the surgical techniques for reconstruction of the severely resorbed posterior maxilla. The purpose of this study was to evaluate the survival rate of implants and the long-term changes of graft height after maxillary sinus augmentation by lateral window approach. From September 1996 to July 2004, maxillary sinus augmentation with mixed grafts of autograft, allograft, xenograft and alloplast were performed on 45 patients and 100 implants were placed. We evaluated the survival rate of implants and the changes of BL(bone length)/IL(implant length) according to time using panoramic radiographs. The survival rate of implants was 91.0% for follow-up period. The mean reduction of graft heights was 0.34mm(3.0%) for 6 months and 1.22mm(1O.66%) for 3 years after augmentation. The total mean BL/IL was $1.34{\pm}0.21$ during 5 year observation period after augmentation and decreased slightly over time. The result means that graft materials were stable above the implant apex. BL/ILs of 1stage procedure were significantly decreased at 1-2 year, 3-4 year after augmentation and no statistically significant changes were observed in those of 2 stage procedure. The graft materials of both procedures were stable above the implant apex. No statistically significant changes of BL/IL were observed in the grafts combined with low amount of autogenous bone or without autogenous bone. The graft materials of both groups were stable above the implant apex. The results indicated that the placement of dental implants with maxillary sinus augmentation showed predictable clinical results and the grafts combined with low amount of autogenous bone or without autogenous bone had long-term resistance to resorption in maxillary sinus.

  • 자동차산업과 ICT 기술의 융합 분야는 크게 상용서비스 분야와 Cooperative-ITS (C-ITS) 서비스 분야로 나뉠 수 있다. C-ITS 서비스 분야는 더 안전한 운송, 더 친환경적이고 효율적인 교통, 및 더 예측가능하고 생산적인 이동성을 제공하는 것을 목표로 하는 분야로서 V2X 통신기술이 사용되고 있다. 최근 이슈가 되고 있는 자율주행자동차와 커넥티드 카의 융합을 위해서는 높은 데이터 전송율과 낮은 전송 지연, 그리고, 낮은 전송 오류율을 요구한다. 이를 위하여 최근의 WAVE와 C-V2X (LTE-V2X, 5G-V2X) 성능 비교에 관심이 증폭되고 있으며, 통신 기술별 응용서비스들이 연구되고 있다. 본 논문에서 하이브리드 V2X 통신시스템의 응용서비스 성능평가 방법에 대해 설계하였고, 패킷에러율(PER) 성능의 감소는 차량속도가 아닌 통신 거리의 증가에 의해 발생하는 것으로 보이는 결과 등을 확인하였다.

돌발성 암성 통증 약물 선택 요인과 사용 경험: 의사 대상 설문조사 (Factors Influencing Pain Medication Preference for Breakthrough Cancer Patients and Their Application to Treatments: Survey on Physicians)

  • 목적: 효과적인 돌발성 통증 조절은 암환자의 기능 유지와 삶의 질 향상에 매우 중요하다. 의사가 돌발성 통증 조절 약물 선택 시 고려하는 요인과 사용 경험에 대해 살펴보고자 한다. 방법: 2014년 9월부터 12월까지 한국 호스피스 완화의료학회에서 전국의 의사대상으로 시행한 온라인 설문 결과를 받아 이차적으로 분석하였다. 돌발성 통증 약물 선택 시 고려하는 사항으로 '빠른 효과 발현', '강력한 효과', '적은 부작용', '투약의 편리성', '장기간 지속적인 효과', '용량 비례하여 예측 가능한 효과', '많은 처방 경험'과 '풍부한 임상 데이터'에 5점 척도로 동의 여부를 조사하였다. 약물사용 경험에 대해서는 경점막 펜타닐, 경구 옥시코돈, 주사제 모르핀에 대해 앞서 언급한 9항목의 동의 척도를 조사하였다. 결과: 77명의 응답자는 빠른 효과를 가장 중요한 돌발성 통증 약물의 선택 요건으로 생각하였다. 그 외, 투약 시 편리성, 강력한 효과, 예측 가능한 효과와 적은 부작용이 중요하다고 응답하였다. 주사제 모르핀은 빠른 효과, 강력하고 예측 가능한 효과가 있으나, 투약의 편리함이나 적은 부작용에 대해서는 응답자 동의율이 낮았다. 경구 옥시코돈은 투약의 편리함이나 적은 부작용이 타 약제보다 우수한 것으로 나타났다. 다만, 빠른 효과는 주사제 모르핀보다 낮은 동의를 받았다. 경점막 펜타닐은 편리하지만, 예측 가능한 효과, 장기간 지속적인 효과와 수면장애 개선 효과에서 낮은 동의를 받았다. 결론: 돌발성 통증 조절 약물의 선택 시 빠른 효과 발현을 최적의 조건으로 꼽았으며, 돌발성 통증 조절 약제에 따라 의사의 사용 경험은 차이를 나타내었다.

Prognostic value of pretreatment 18F-FDG PET-CT in radiotherapy for patients with hepatocellular carcinoma

  • Purpose: The purpose of this study was to investigate the predictable value of pretreatment $^{18}F$-fluorodeoxyglucose positron emission tomography-computed tomography ($^{18}F$-FDG PET-CT) in radiotherapy (RT) for patients with hepatocellular carcinoma (HCC) or portal vein tumor thrombosis (PVTT). Materials and Methods: We conducted a retrospective analysis of 36 stage I-IV HCC patients treated with RT. $^{18}F$-FDG PET-CT was performed before RT. Treatment target was determined HCC or PVTT lesions by treatment aim. They were irradiated at a median prescription dose of 50 Gy. The response was evaluated within 3 months after completion of RT using the Response Evaluation Criteria in Solid Tumors (RECIST). Response rate, overall survival (OS), and the pattern of failure (POF) were analyzed. Results: The response rate was 61.1%. The statistically significant prognostic factor affecting response in RT field was maximal standardized uptake value (maxSUV) only. The high SUV group (maxSUV ${\geq}5.1$) showed the better radiologic response than the low SUV group (maxSUV < 5.1). The median OS were 996.0 days in definitive group and 144.0 days in palliative group. Factors affecting OS were the %reduction of alpha-fetoprotein (AFP) level in the definitive group and Child-Pugh class in the palliative group. To predict the POF, maxSUV based on the cutoff value of 5.1 was the only significant factor in distant metastasis group. Conclusion: The results of this study suggest that the maxSUV of $^{18}F$-FDG PET-CT may be a prognostic factor for treatment outcome and the POF after RT. A %reduction of AFP level and Child-Pugh class could be used to predict OS in HCC.

골 및 연부 조직 종양에서의 미세 침 흡입 세포 검사 (Fine Needle Aspiration Cytology in Bone and Soft Tissue Tumor)

  • From January 1994 to August 1995 we performed 270 cases of fine needle aspiration cytology for bone and soft tissue lesions. Among them 137 cases were diagnosed histologically. Sixty-seven cases were benign and 70 cases were malignant. We analysed these on the aspect of sensitivity, specifity, positive predictive value, negative predictive value of the cytologic examination. Malignant cases consisted of bone tumor 29 cases; soft tissue tumor 25 cases; and metastatic tumor 16 cases. The sensitivity and specifity of the aspiration cytology for malignant tumor were 67.8% and 97%, respectively. The positive and negative predictive value of aspiration cytology were 97.4% and 72.7%, respectively. Among them 40 cases were primary bony lesions; 14 benign lesions, 26 malignamt bone tumors. Sensitivity of aspiration cytology for these primary bony lesion was 90%(18/20) and its specifity was 100%(13/13). Three cases showed atypical cells in cytology but with biopsy we could find them as one benign and two malignancies. Two cases of manignancy showed negative result in cytology; one was parosteal sarcoma of proximal femur and the other was osteosarcoma of proximal tibia which was biopsied already at other hospital. Although the overall sensitivity and specifity for malignant bone and soft tissue tumors were relatively low and not so predictable, the fine needle aspiration cytology for primary bony lesion was useful in the differential diagnosis of benign or malignancy.
