고혈압 및 당뇨병 환자에서 한식 섭취가 혈청 GGT와 심혈관질환 위험인자에 미치는 영향 (Effects of adherence to Korean diets on serum GGT and cardiovascular disease risk factors in patients with hypertension and diabetes)

  • 본 연구는 고혈압 및 당뇨병 치료 중인 환자 41명을 대상으로 한식 섭취가 12주간의 경과 과정에서 4차례의 방문(0주, 4주, 8주, 12주)을 통해 측정한 혈청 GGT, 혈압 및 glycemic control data, 심혈관계 위험지표 및 식이섭취의 변화를 비교 평가한 결과는 다음과 같았다. 1. 연구대상자는 평균 연령은 $61.8{\pm}1.9$세로 한식군 (21명)의 경우 건강한 한식을 1일 3끼씩 12주간 제공된 식사를 섭취하였고, 대조군 (당뇨병 식이 가이드라인에 따른 관리교육을 받은 자) 20명은 평소 섭취하던 당뇨조절 식사를 그대로 유지하게 하였다. 2. 수축기혈압과 이완기혈압은 두군 간 유의한 차이를 확인할 수 없었으나 맥박수는 한식군에서 대조군에 비해 유의적으로 감소하였다 (p = 0.007). 3. 당화혈색소 (HbA1c)는 한식군에서 연구 참여 전과 후 각각 $6.8{\pm}.2%$, $6.1{\pm}.2%$로 감소하고 대조군은 각각 $6.8{\pm}.2%$, $6.5{\pm}.2%$로 감소되어 한식군이 대조군에 비해 유의적으로 감소폭이 컸다 (p = 0.004). 4. 혈청 GGT는 한식군에서 연구 참여 전과 후 각각 $28.8{\pm}4.5IU/L$, $19.8{\pm}3.2IU/L$로 감소하고 대조군은 각각 $30.0{\pm}4.1IU/L$, $33.9{\pm}5.0IU/L$로 증가되어 한식군이 대조군에 비해 유의적으로 감소하였다 (p < 0.001). 5. 총콜레스테롤, LDL 콜레스테롤 및 중성지방은 한식군에서 대조군에 비해 감소한 경향이었으나 두군 간 유의적인 차이가 없었으며 free fatty acid는 한식군에서 대조군 보다 유의적으로 감소하였다 (p = 0.002). 6. 신체계측 지표인 체중 (p = 0.002), 체질량지수 (p = 0.002), 체지방량 (p < 0.001), 체지방율 (p < 0.001)은 연구 참여 전에 비해 연구 참여 12주 후에 한식군에서 대조군에 비해 유의적으로 감소하였다. 7. 치료약물의 변화량은 두군 간 유의한 차이는 없었으나 한식군에서 대조군보다 많은 수의 환자가 실제로 고혈압, 당뇨병 및 고지혈증 치료 약물을 감량할 수 있었다. 8. 한식군은 연구 참여 동안에 전곡류의 밥, 채소류인 나물류, 김치 및 전통 콩발효식품 등의 섭취량은 대조군에 비해 유의하게 증가하였다 (p < 0.001). 9. 한식군은 동물성식품에서 유래한 동물성 단백질 (p < 0.001), 지질 (p < 0.001) 및 콜레스테롤 (p = 0.034)의 섭취량은 대조군에 비해 유의적으로 감소한 반면, 총칼로리(p < 0.001), 엽산 (p < 0.001), 식이섬유 (p < 0.001), 나트륨(p < 0.001), 칼륨 (p < 0.001), 비타민 A, C, E (p < 0.001) 및 비타민 B류 (p < 0.001)의 섭취는 유의적으로 증가하였다. 이상의 결과를 종합해 보면, 적극적인 전곡류로 만든 밥을 중심으로 한 한식 섭취는 당뇨병 및 고혈압 환자에서 대사성질환 및 심혈관계질환 위험인자인 혈청 GGT, 맥박수, 혈당조절 지표 및 비만지표 개선에 긍정적인 영향을 주었다.

Effects of ${\alpha}$-lipoic acid and L-carnosine supplementation on antioxidant activities and lipid profiles in rats

  • ${\alpha}$-Lipoic acid and L-carnosine are powerful antioxidants and are often used as a health supplement and as an ergogenic aid. The objective of this study was to investigate the effects of ${\alpha}$-lipoic acid and/or L-carnosine supplementation on antioxidant activity in serum, skin, and liver of rats and blood lipid profiles for 6 weeks. Four treatment groups received diets containing regular rat chow diet (control, CON), 0.5% ${\alpha}$-lipoic acid (ALA), 0.25% ${\alpha}$-lipoic acid+0.25% L-carnosine (ALA+LC), or 0.5% L-carnosine (LC). Superoxide dismutase (SOD) and glutathione peroxidase (GSH-Px) and lipid peroxidation products, malondialdehyde (MDA) concentrations, were analyzed in serum, skin, and liver. Blood lipid profiles were measured, including triglycerides (TG), total cholesterol (TC), high density lipoprotein cholesterol (HDL-C), and low density lipoprotein cholesterol (LDL-C). Skin and liver SOD activities of the ALA and LC groups were higher than those of the CON group (P<0.05), but serum SOD activity was higher only in the LC group compared to that in the CON group (P<0.05). Additionally, only liver GSH-Px activity in the LC group was higher than that of the CON and the other groups. Serum and skin MDA levels in the ALA and LC groups were lower than those in the CON group (P<0.05). Serum TG and TC in the ALA and ALA+LC groups were lower than those in the CON and LC groups (P<0.05). The HDL-C level in the LC group was higher than that in any other group (P<0.05). LDL-C level was lower in the ALA+LC and LC groups than that in the CON group (P<0.05). Thus, ${\alpha}$-lipoic acid and L-carnosine supplementation increased antioxidant activity, decreased lipid peroxidation in the serum, liver, and skin of rats and positively modified blood lipid profiles.

12종 어류의 일반성분조성과 콜레스테롤 및 $\alpha$-토코페롤 함량의 계절변화 (Seasonal Variation in Proximate Composition, Cholesterol and $\alpha$-Tocopherol Content of 12 Species of Korean Fish)

  • 한국 남해 연근해에서 어획된 12종의 주요 어류 (표$\cdot$중충회유어 5종: 멸치, 숭어, 전어, 방어, 고등어: 연안암초어 3종: 조피볼낙, 참돔, 감성돔: 저서어 4종: 도다리, 붕장어, 넙치, 말쥐치)를 대상으로 하여 그들의 일반성분조성과 CHOL 및 $\alpha$-Toc 함량의 계절변화를 검토하였다. 지질과 수분함량은 역의 상관관계(r=-0.86, p<0.001)로 계절에 따라 뚜렸한 변화를 나타내었으나, 단백질 및 회분함량은 거의 변화가 없었다. 멸치, 전어, 감성돔, 도다리, 붕장어, 넙치, 말쥐치는 각각 12월, 10월, 11월, 9월, 1월, 1월, 11월에 지질함량이 높았고, 산란기에 해당하는 계절에 지질함량이 낮아 산란의 영향이 큰 어종이었다. 그러나 숭어, 방어, 고등어, 조피볼낙, 참돔의 지질함량 변화는 산란의 영향보다는 수온과 섭이량의 영향이 큰 것으로 생각되었다. CHOL의 함량변화는 지질함량변화와 정의 상관관계 (r=0.89, p<0.001)를 나타냈으나, $\alpha$-Toc 함량은 계절에 따른 변화가 인정되지 않았다.

손바닥선인장 복합물이 당뇨 쥐의 혈당 및 지질대사에 미치는 영향 (Effects of Opuntia ficus-indica Complexes B(OCB) on Blood Glucose and Lipid Metabolism in Streptozotocin-induced Diabetic Rats)

  • 손바닥선인장 복합물(OCB)의 항당뇨 효과 및 기작을 알아보기 위한 연구의 일환으로, OCB의 급여가 STZ으로 당뇨가 유발된 8주령 수컷 흰쥐의 음수량, 식이 섭취량, 체중, 공복혈당, 당내성에 미치는 효과와 당뇨의 합병증인 고지혈증(중성지방, 유리 지방산, 총 콜레스테롤, LDL-콜레스테롤, VLDL-콜레스테롤, HDL-콜레스테롤)의 개선효과를 조사하였다. OCB는 손바닥선인장의 줄기, 여주, 천화분, 목단피, 산약, 강황과 호로파를 조합하여 제조하였고, 기본사료에 2%와 5%의 수준으로 첨가하여 8주령 rat 수컷에게 3주간 자유 채식시켰다. OCB의 수분함량 약 12%, 조회분 15.4%, 조지방 1.26%, 조단백질 4.9%, 조섬유량 6.3% 및 탄수화물 함량은 60% 이상으로 주성분은 탄수화물이었다. 체중은 NC에 비해 DC, OCB-2와 OCB-5에서 감소하였다. 음수량과 사료섭취량은 NC에 비해 DC가 각각 약 8배 및 1. 5배 이상 증가하였고, OCB-2와 OCB-5는 DC와 비교해 유의적 차이가 없었다. NC의 공복 혈당은 평균 100 $mg/d{\ell}$, DC는 416 $mg/d{\ell}$이었으며, OCB-2는 362 $mg/d{\ell}$, 그리고 OCB-5는 329 $mg/d{\ell}$로 혈당농도의 감소를 보였다. 당내성 실험에서 모든 실험군은 포도당을 급여한지 30분 후에 최고 혈당을 나타냈고, OCB-5는 유의적인 감소를 보였다(p<0.05). 혈장 인슐린 함량도 DC와 비교해 NC, OCB-2, 그리고 OCB-5에서 유의적인 증가를 보였다(p<0.05). 이상의 결과에서 OCB의 급여는 STZ으로 당뇨가 유발된 쥐에서 당뇨의 개선효과가 있는 것으로 판단된다. 또한, OCB의 급여는 당뇨의 합병증인 지질대사에서 혈장의 중성지방, 유리지방산, 총콜레스테롤, LDL-콜레스테롤 그리고 VLDL-콜레스테롤을 모두 DC와 비교해 유의적인 수준으로 감소시켰다(p<0.05). 이러한 결과에서 볼 때, OCB의 급여는 당뇨의 합병증인 지질대사의 이상도 개선하는 것으로 관찰되었다.

상추 추출물(Lactuca sativa L.)의 혈관내피세포에서 항염증 작용과 고지방 식이 생쥐에서 혈중 지질농도 개선에 미치는 영향 (Anti-inflammatory Effect of Lactuca sativa L. Extract in Human Umbilical Vein Endothelial Cells and Improvement of Lipid Levels in Mice Fed a High-fat Diet)

  • 본 연구에서는 상추 추출물이 혈관내피세포의 염증반응에 미치는 영향 및 지질대사 개선 효과를 확인하기 위해 HUVEC을 이용한 in vitro 실험 및 고콜레스테롤혈증 모델을 이용한 동물실험을 수행하였다. 상추 추출물은 HUVEC에서 TNF-${\alpha}$에 의해 증가되는 세포접착단백질 ICAM-1, VCAM-1의 발현을 억제하였고 TNF-${\alpha}$에 의해 촉진되는 HUVEC와 단핵구 세포의 부착을 감소시켰다. 또한 상추 추출물은 TNF-${\alpha}$에 의해 증가되는 염증성 사이토카인 IL-6, IL-8 및 동맥경화 유발 케모카인 MCP-1의 생성을 농도 의존적으로 억제하였다. 고콜레스테롤혈증 동물모델에서 고콜레스테롤 식이군 및 상추 추출물이 첨가된 고콜레스테롤 식이군의 체중증가량 및 식이섭취량은 유의적인 차이를 보이지 않았다. 고콜레스테롤 식이군의 혈중 총 콜레스테롤 함량은 정상 식이군에 비해 크게 증가하였고 상추추출물이 첨가된 고콜레스테롤 식이군의 콜레스테롤 함량은 유의적으로 감소하였다. 고콜레스테롤 식이군의 혈중 중성지방 함량은 정상 식이군에 비해 약간 감소하였지만 유의적인 차이는 보이지 않았다. 상추 추출물이 첨가된 고콜레스테롤 식이군의 경우 혈중 중성지방 함량은 고콜레스테롤 식이군과 비교하였을 때 유의적인 차이를 보이지 않았다. 고콜레스테롤 식이군의 HDL-콜레스테롤 함량은 정상 식이군에 비해 감소하였고 상추 추출물이 첨가된 경우 HDL-콜레스테롤 함량은 고콜레스테롤 식이군에 비해 유의적으로 증가하였다. LDL-콜레스테롤 함량은 고콜레스테롤 식이군에서 크게 증가하였고 상추 추출물이 첨가된 경우 감소하였다. 혈청의 동맥경화지수 및 심혈관지수는 고콜레스테롤 식이군에서 정상 식이군에 비해 유의적으로 증가하였고 상추 추출물이 첨가된 고콜레스테롤 식이군에서 유의한 감소를 나타내었다. 이상의 연구결과는 상추 추출물이 혈관내피세포의 염증반응을 억제하여 혈관협착을 방지할 수 있고 혈중 지질대사의 개선 효과를 보여 혈액순환개선 및 동맥경화, 심혈관질환의 예방과 치료에 효과가 있다는 것을 의미한다.

Alcohol-induced Hyperlipidemia Is Ameliorated by Orally Administered DWP208, a Sodium Succinate Form of ZYM201

  • DWP208 is a sodium succinate form of ZYM-201 which is a triterpenoid glycoside isolated from Sanguisorba officinalis, a medicinal plant prescribed for various diseases, such as duodenal ulcers and bleeding in East Asian counties. We demonstrated that this compound is able to normalize the altered lipid metabolism induced by hyperglycemia and a high fat diet. In this study, we determined whether hyperlipidemic conditions induced with chronically treated alcohol can also be restored by DWP208. Similar to our previous results, orally administered DWP208 (1 to 10 mg/kg) also ameliorated the hyperlipidemia that was induced by alcohol. This compound reversed the alcohol-induced hyperlipidemia including (i) up-regulated hyperlipidemic parameters such as low-density lipoprotein (LDL), very low-density lipoprotein (VLDL), atherosclerotic index (AI), triglyceride, and total cholesterol, and (ii) down-regulated hyperlipidemic parameters such as absolute body weight, superoxide dismutase (SOD) activity, and high-density lipoprotein (HDL) in serum and liver. According to our data, the ameliorative activity of DWP208 is due to its indirect anti-oxidative activity as a result of which lipid peroxide and hydroxyl radical levels were reduced and the activity of SOD was enhanced. Therefore, our data strongly suggest that DWP208 can be used as a remedy against alcohol-induced hyperlipidemia.

한방 비만 치료 후 간기능 개선을 보인 비알코올성 지방간 환자 1례 보고 (A Case Study on A Non-alcoholic fatty liver disease(NAFLD) Patient Improved by The Oriental Obesity Therapy Program)

  • Objectives The purpose of this study was to investigate the clinical effect of oriental medical obesity therapy on obese patient with non-alcoholic fatty liver disease. We supposed to treat patient 21 years old that had non-alcoholic fatty liver with oriental medical obesity therapy. Methods The patient was treated from the 19th of July to the 22th of August for obesity treatment. And the patient had intensive care through admission treatment from the 19th of July to the 30th of July. Herbal medicine, low calorie and low cholesterol diet, electrolipolysis, auricular acupuncture, aerobic exercise was done during the treatment period. Results His body weight decreased from 107.10㎏ to 98.85㎏. The BMI decreased from 36.2㎏/$m^2$ to 34.5㎏/$m^2$. Abdominal visceral fat tissue decreased from 190.80$cm^2$ to 145.05$cm^2$. AST was decreased from 113 to 45. ALT was decreased from 145 to 40. r-GT was decreased from 123 to 51. Oriental obesity therapy could be effective to treat obese patient with non-alcoholic fatty liver disease. Conclusions The obese patient with non-alcoholic fatty liver disease had improved liver function after oriental obesity treatment.

Oily Fish Consumption Modifies the Association between CD36 rs6969989 Polymorphism and Lipid Profiles in Korean Women

  • The aim of this study was to investigate the association of CD36, a class B scavenger receptor, rs6969989 polymorphism with the serum lipid profiles in Korean women, together with their modulation by oily fish consumption. Subjects were participants from the Korean Genome Epidemiology Study (KoGES), which was initiated in 2001 as a large-scale. A total of 4,210 women aged 39 to 70 were included in this study. Data were collected using self-administered questionnaires, anthropometric measurements, and blood chemical analysis. Dietary intake was analyzed using a semi-quantitative food frequency questionnaire. The minor allele frequency for rs6969989 was found in 12% of this population. Homozygotes minor G allele at the rs6868989 exhibited significantly higher high density lipoprotein cholesterol (HDLC) concentrations (P-trend=0.043) and lower fasting glucose (P-trend=0.013) than major allele A carriers. The risk of low HDL-C was significantly lower in homozygotes for the G allele than the A allele carriers (P-trend=0.032). Gene-diet interaction effects between rs6969989 and oily fish intake were significantly associated with the risk of dyslipidemia (P-interaction=0.004). Subjects with homozygotes minor G allele and high oily fish intake generally had a lower risk of dyslipidemia than did those with major allele homozygotes and low oily fish intake. These findings supported that oily fish consumption may modulate the contributions of CD36 rs6969989 on genetic predisposition to the risk of dyslipidemia.

Eicosapentaenoic Acid, Docosahexaenoic Acid 농축어유와 들깨유가 저지방 식이를 섭취한 흰쥐의 지질대사에 미치는 영향 (The Effect of Dietary Concentrated Oils of Eicosapentaenoic Acid, Docosahexaenoic Acid and Perilla Oil on lipid Metabolism in Rata Fed Low Fat Diet)

  • To compare the effect of three kinds of n-3 fatty acids-eicosapentaenoic acid(EPA), docosahexaenoic acid(DHA) and perilla oil (PO)-on serum and liver lipid levels and fatty acid composition of liver phospholipid(PL) at low fat level(5%, w/w), 4-weeks old Sprague-Dawley rats were fed with one of five different oil diets for 4 weeks. Beef tallow(BT) and corn oil(CO) was used as control for sturated or n-6 fatty acid respectively. Se겨m concentrations of cholesterol(TC) and phospholipid(PL) were lower in PO DHA and EPA groups than in BT and CO groups. HDL-cholesterol levels were higher in CO and PO groups than in EPA, DHA and BT groups. Liver PL concentrations were higher in DHA and EPA groups than in CO, PO and BT groups, but liver TC and heal PL and TC concentrations did not show any significant difference among groups. Hepatic fatty acid composition of phosphatidylcholine (PC) and phosphatidylethanolamine (PE), two major phospholipids in liver, reflected their dietary fatty acid composition. In PC and PE, total percentage own-6 series was higher in CO group than in any other groups, and that own-3 series was higher in DHA and PO groups than in EPA, CO and BT groups. Moreover, the ratio of 20 : 4/18 : 2 was lower in PO and DHA groups than in EPA, CO and BT groups. On the contrary, the percentage of C22 : 6 was lower in EPA, CO and BT groups than in PO and DHA groups. These results revealed that n-3 series(EPA, DHA and PO) were more effective in lowering um lipids than n-6 fatty acids or saturated fatty acid. Based on the results of fatty acid composition of hepatic phospholipid, we suggest that the dietary effect of PO and DHA on antiatherogenic characteristics seems to be similar extent. In addition, the effect of EPA might not be significantly different from that of BT or CO in the view of eicosanoids production from the precursor fatty acid. These difference of hepatic fatty acid composition might come from other characteristics of dietary oil as well as the type of unsaturation, not from the carbon chain length or the degree of unsaturation of n-3 fatty acid.

행동교정요법이 비만아동의 신체지수, 습관 및 자아존중감에 미치는 효과 (Effects of Behavior Modification on Physical Variables, Habit and Self-esteem in Obese Elementary School Children)

  • Behavior modification is an approach to decrease degree of obesity through changing maladaptive eating habit and life style. It produces small amount of weight loss but it has few side effects and low dropout rate. It also has great effectiveness on maintenance of decreased weight. This study was performed to investigate effects of behavior modification on physical variables, fat and thin habit, and self-esteem in obese elementary school children. Sixty two students of 4th, 5th, and 6th grade were selected from two elementary schools located in the suburbs of Seoul. Thirty four children in one school were assigned to experimental group and twenty eight children in another school were assigned to control group. All subjects were healthy and were not on any type of obesity control. Behavior modification, in this study, was consisted of introduction, self-monitoring and stimuli control, education about diet and physical activities, individual counseling and reinforcement, requesting personal assistance, cognitive restructuring, and closing and long-term planning. Experimental group was received 60~70 minutes of behavior modification once a week for 8 weeks from October 16 to December 22, 2000. Control group was not received any intervention. Data were collected before and after intervention by measuring physical variables, fat and thin habit, and self-esteem. Physical variables consisted of obesity index, skinfold thickness, body fat(percentage of body fat, fat mass, fat free mass), serum lipids(total cholesterol, HDL-C, LDL-C, triglyceride) and serum leptin. The results were as follows ; 1. physical variables 1) Obesity index of the experimental group was significantly decreased after behavior modification. But there was no significant difference between two groups. 2) The increase of skinfold thickness was significantly low in the experimental group compared to the control group. 3) Percentage of body fat and fat mass were significantly decreased in the experimental group. Fat free mass was significantly increased in the experimental group. 4) Total cholesterol, HDL-C, triglyceride and serum leptin between the experimental group and control group showed no significant difference. LDL-C was significantly decreased in the control group. 2. Thin habit score was significantly increased in the experimental group. In subcategories of habit, thin scores of life style, attitude, social relationship, nutrition were signifi- cantly increased in the experimental group. 3. Self-esteem score was significantly increased in the experimental group. These results indicate that behavior modification is effective in decreasing percentage of body fat and fat mass, in less increasing skinfold thickness and in increasing fat free mass, thin habit, and self-esteem. In conclusion, behavior modification can be used as safe and effective strategy for managing obesity in elementary school children.
