The structural designs of RC flat plates that have no flexural stiffness by boundary beams may be governed not by strength conditions but by serviceabilities. Specially, since over-loading and tensile cracking in early-aged slabs significantly increase the short- and long-term deflections of a flat plate system, a construction sequence and its impact on the slab deflections may be decisive factors in designs of flat plate systems. In this study, the procedure of simulating slab deflections with considering construction sequences, concrete cracking, and long-term effects is proposed. The proposed method is practically useful, as it can predict well the slab deflections at construction and service stages only with a few input data. The proposed method is verified by comparisons with measured results in a real-scale test.
In this paper we describe current activities on the project Multi-Scale Modeling and Analysis of convective boiling (MSMA), conducted jointly by the Paul Scherrer Institute (PSI) and the Swiss Nuclear Utilities (Swissnuclear). The long-term aim of the MSMA project is to formulate improved closure laws for Computational Fluid Dynamics (CFD) simulations for prediction of convective boiling and eventually of the Critical Heat Flux (CHF). As boiling is controlled by the competition of numerous phenomena at various length and time scales, a multi-scale approach is employed to tackle the problem at different scales. In the MSMA project, the scales on which we focus range from the CFD scale (macro-scale), bubble size scale (meso-scale), liquid micro-layer and triple interline scale (micro-scale), and molecular scale (nano-scale). The current focus of the project is on micro- and meso-scales modeling. The numerical framework comprises a highly efficient, parallel DNS solver, the PSI-BOIL code. The code has incorporated an Immersed Boundary Method (IBM) to tackle complex geometries. For simulation of meso-scales (bubbles), we use the Constrained Interpolation Profile method: Conservative Semi-Lagrangian $2^{nd}$ order (CIP-CSL2). The phase change is described either by applying conventional jump conditions at the interface, or by using the Phase Field (PF) approach. In this work, we present selected results for flows in complex geometry using the IBM, selected bubbly flow simulations using the CIP-CSL2 method and results for phase change using the PF approach. In the subsequent stage of the project, the importance of effects of nano-scale processes on the global boiling heat transfer will be evaluated. To validate the models, more experimental information will be needed in the future, so it is expected that the MSMA project will become the seed for a long-term, combined theoretical and experimental program.

Space transformation of hospital architecture happens more rapidly than other types such as school and office buildings. This requirement is caused by medical demand, service transformation, technical development and equipment changes. Grand scale extension and rapid change in hospital architecture affects whole planning; therefore, it is important to design in the macro perspective such like Life Cycle. Grand scale extension is difficult process to rebuild growth and transfer of hospital architecture and to adapt to the future change. That's why simple planning gives limits to some departments in a short-term and brings another reform such as more extension or interior renovation in a long-term. This study surveys various space transformation and grand scale extension of EB hospital extended recently, and it is to find planning condition in extension.
Objective : Gamma knife radiosurgery (GKRS) is the least invasive surgical option for patients with trigeminal neuralgia (TN). However, the indications and long term outcomes of GKRS are still controversial. Additionally, a series with uniform long-term follow-up data for all patients has been lacking. In the present study, the authors analyzed long-term outcomes in a series of patients with TN who underwent a single GKRS treatment followed by a minimum follow-up of 60 months. Methods : From 1994 to 2009, 40 consecutive patients with typical, intractable TN received GKRS. Among these, 22 patients were followed for >60 months. The mean maximum radiation dose was 77.1 Gy (65.2-83.6 Gy), and the 4 mm collimator was used to target the radiation to the root entry zone. Results : The mean age was 61.5 years (25-84 years). The mean follow-up period was 92.2 months (60-144 months). According to the pain intensity scale in the last follow-up, 6 cases were grades I-II (pain-free with or without medication; 27.3%) and 7 cases were grade IV-V (<50% pain relief with medication or no pain relief; 31.8%). There was 1 case (facial dysesthesia) with post-operative complications (4.54%). Conclusion : The long-term results of GKRS for TN are not as satisfactory as those of microvascular decompression and other conventional modalities, but GKRS is a safe, effective and minimally invasive technique which might be considered a first-line therapy for a limited group of patients for whom a more invasive kind of treatment is unsuitable.

This study aimed to clarify the multifunctional services and space composition in the process of developing a multifunctional long-term care program in small elderly care facilities in Japan. We collected data about multifunctional long-term care at small facilities from the Community Life Support Center (CLC), a Japanese non-profit corporation, and conducted an interview survey of the members of the CLC's secretariat in 2014. Furthermore, we selected 3 Japanese pioneering care facilities (known as takurosho), and conducted interview surveys and data collection to clarify in detail the space composition and process of development of multifunctional long-term care at small facilities. Four distinct results were found. First, the facilities had gradually increased non-institutional services, including visitation, overnight stays, and long-term stays, to fit the needs of users and their families. Secondly, in the 1990s, they could offer both non-institutional and institutional services at the same facility, but after the long-term care insurance system began in 2000, non-institutional long-term stay services were not allowed. Third, the facilities had built extensions or extra rooms in response to increases in multifunctional services and users. These rooms had common characteristics, with sitting rooms at the center of the facility. Lastly, the maximum number of service users at each of the 3 facilities was limited to 15, to maintain a small scale. However, as the size of facilities was increased through building extensions or remodeling, the overall amount of area available to users increased.
Purpose: There has been a growing recognition that person-centered care enhances the quality of life of nursing home residents with dementia. This study was conducted to develop a person-centered dementia care online education program for direct care staff in long-term care facilities. Methods: Delphi method with expert group was used to validate contents. We developed 61 draft items based on literature review. Twenty experts participated in consecutive three round surveys including 5-point Likert scale questions and open-ended questions. Based on experts' opinions, the content validity ratio for content validity and the coefficient of variation for stability were calculated. Results: Three-round Delphi surveys and additional feedback from the expert panel established a consensus of core contents: 1) dementia (7 categories), 2) person-centered care (6 categories), 3) communication (8 categories), and 4) behavioral and psychological symptoms of dementia (6 categories). Specific sub-categories in each category were differentiated according to the job qualifications (65 sub-categories for registered nurses, 64 sub-categories for nursing aids, and 41 sub-categories for personal care workers). Conclusion: This delphi study identified person-centered dementia education curricula, in which the person-centered approach should be a key policy priority in Korean long-term care system. Now it is urgently needed to develop education programs utilizing online platforms that enable efficient and continuous learning for long-term care staff, which can contribute to behavior changes in the person-centered dementia care approach and improvement of care quality in long-term care facilities.
Recently, photovoltaic systems have been devolved into much larger systems up to MW-scale. Photovoltaic industry participants give their focus on power generation capability of photovoltaic modules because their benefits can be decided from the amount of generation. The information on long-term performance change of photovoltaic modules helps to estimate the amount of power generation and evaluate the economic cost-benefits. Long-term performance of a PV system has been analyzed with operation data for 12 years from 1999 to 2010. In the first year, the amount of yearly power generation was 57.7 MWh with 13.2% capacity factor. In 2007, the amount of yearly generation was 44.3 MWh with 10.14% capacity factor, and in 2010, the amount was decreased down to 38.1 MWh with 8.7% capacity factor. The result means that long-term capacity factor has been 4.5% decreased for 12 years and that the amount of generation has been decreased 34.0% for 12 years which is 2.8 % per year. The latter capacity factor has been decreased faster than 0.20%, the average rate for 10 years. The performance decrease of the PV system is meant to be accelerated. The decrease of performance and utilization is due to aged deterioration of photovoltaic modules and lowering conversion efficiency of PCS.
Objectives: The purpose of this study is to develop a survey instrument to assess intention to stay for dental hygienists based on validity and reliability. Methods: A survey was conducted targeting 317 dental hygienists in dental clinics. The data was used for the analysis of the study, using PASW Statistics 20.0 and IBM SPSS AMOS 18.0. Results: The preliminary instrument includes 44 item. 22 items were excluded by variable analysis. 17 final items was selected by exploratory factor analysis (EFA). The confirmatory factor analysis (CFA) was composed of four elements, 'organization fit', 'interpersonal relationship', 'identity', and 'job connectivity'. Conclusions: The validity and reliability of measurement tool for dental hygienist's intention to stay was proved. It could be used to help dental hygienist's long-term employment.
Purpose: This study synthesized the literature on non-pharmacological interventions for chronic pain in older adults in long-term care facilities. Methods: Scoping review and thematic analysis methods were combined. On June 1, 2023, searches of primary electronic databases, including PubMed, Embase, PsycINFO, CINAHL DBpia, KMbase, NDSL, and RISS, were performed, restricting the publication date from January 1, 2010 to December 31, 2022. Guidelines from the Joanna Briggs Institute were used as a framework to set and conduct the scoping review. Results: The review identified 1,095 abstracts, from which 14 studies were included in the review. Consequent to the study, there were 10 randomized controlled experimental study designs, and 6 out of 14 studies were conducted in China. The numeric rating scale was widely used for pain assessment. Exercise intervention was provided in 7 studies and physiotherapy was provided in three studies. The outcome variables measured were pain self-efficacy, physical functions, and depression. Conclusion: This comprehensive overview guided nursing staff in long-term care facilities for planning and intervention of effective non-pharmacological interventions for chronic pain in the elderly.

The use of reinforced earth walls in permanent structures is getting it's popularity. Despite a number of advantages of reinforced earth walls over conventional concrete retaining walls, there exit concerns over long-term residual deformation when subjected to repeated and/or cyclic loads, during their service period. In this investigation, the effect of preloading in reducing long term residiual deformation of back-to-back reinforced soil wall under sustained and/or repeated loading enviormentment using a series of reduced-scale model tests. It is found that the preloading technique can be an effective means of controlling residual deformations of reinforced soils under varisous loading conditions.
