Nutritional approach to failure to thrive

  • Failure to thrive (FTT) is a term generally used to describe an infant or child whose current weight or rate of weight gains is significantly below that expected of similar children of the same age, sex and ethnicity. It usually describes infants in whom linear growth and head circumference are either not affected, or are affected to a lesser degree than weight. FTT is a common problem, usually recognized within the first 1-2 years of life, but may present at any time in childhood. Most cases of failure to thrive involve inadequate caloric intake caused by behavioral or psychosocial issues. The most important part of the outpatient evaluation is obtaining an accurate account of a child's eating habits and caloric intake. Routine laboratory testing rarely identifies a cause and is not generally recommended. FTT, its evaluation, and its therapeutic interventions are best approached by a multi-disciplinary team includes a nutritionist, a physical therapist, a psychologist and a gastroenterologist. Long term sequelae involving all areas of growth, behavior and development may be seen in children suffering from FTT. Early detection and early intervention by a multidisciplinary team will minimize its long term disadvantage. Appropriate nutritional counseling and anticipatory guidance at each well child visit may help prevent some cause of FTT.

Urine and Hair Metal Correlation in Subjects with Long term Intake of Herbal medicine (한약복용환자의 뇨와 머리카락중 금속간의 상관관계 -유해금속이 인체흡수에 미치는 영향연구-)

  • This research purpose was to get urine and hair metal correlation by Spearman methods in subjects with long term intake of herbal medicine. And that result can help research for metal absorption mechanism in body. Subjects have been intake 1-124 months(7.55 months mean) in decoction and 1-60 months (1.27 months means) for some disease treatment in sampling point. 1) Metal concentrations in urine and hair of study subjects showed higher than WHO reference limits, especially 0.32-6.17% in urine and 11.69-26.95% in hair. 2) Metal concentration in urine correlated 0.37(p〈0.001) Cu to Cd, 0.37(p〈0.001) Pb to Cu, 0.29(p〈0.001) Pb to Cu, 0.29(p〈0.001) Pb to Cd, and 0.36(p〈0.001) Pb to Cu, -0.13(p〈0.05) Hg to Cu in hair. Also between urine and hair correlated 0.21(p〈0.001) Cd to Cd. 0.17(p〈0.01) Pb to Pb, -0.06 Cu to Hg, -0.01 Hg to Cu. These results showed that occur physical and chemical reaction among many metals in body, respectively. However, further study related this subjects will be need.

  • Caffeine is widely consumed ingredient and it belongs to alkaloids. Many foods that we intake contain caffeine ; coffee, tea cocoa, chocolate, and coke. And it is also added to many commercial remedies ; cold tablets, headache tablets, etc. Effect of caffeine that is known to us so far is as follows; 1. Remaining awake for long hours 2. Increasing concentration and decreasing fatigue 3. Increasing basal metabolic rate 4. decomposing glycogen and body fat and providing energy 5. Stimulating gastric acid 6. Increasing urinary excretion. Caffeine containing beverages(especially, coffee)are also favorite food in adult. In case of children and youth, chocolate and coke are favorite food. So, to intake caffeine containing foods moderately can be a vitality of life. But, a long-term intake or overdose of caffeine can result in many side effects. For example, headache, irritability, restlessness, hypertension, fetal abnormality, etc. Therefore, it is desirable that caffeine intake is under 300-400mg per day. To decrease intake of caffeine, 1. Use decaffeinated coffee 2. Product of decaffeinated coffee bean through gene transformation 3. Indicate content and function of caffeine on caffeine-food container 4. Provide an information of caffeine to public.

Carbohydrate Consumption and Glycemic Index of the Usual Diet in Type 2 Diabetes Mellitus Patients (제2형 당뇨병 환자의 평소 식사 중 당질섭취량과 glycemic index 관련 연구)

  • The possibility that high, long-term intake of carbohydrates that are rapidly absorbed as glucose may increase the risk of type 2 diabetes has been long-standing controversy. A high consumption of carbohydrates with a high glycemic index produces greater insulin resistance than did the intake of low glycemic index carbohydrates. This study was designed to evaluate the cabohydrate intake status include glycemic index and correlation carbohydrtae intake status with anthropometry factors & other nutrients in usual diet of the Korean type 2 diabetes mellitus. In 104 tpye 2 diabetes mellitus patients(mean age : 51.8yr, male=44.femal=60), we determined carbohydrte intake status include glycemic index with 24hr recall method and measured anthropometry. Mean daily carbohydrtae intakes and glycemic index were 307.3g(male 323.1g, female 295.5g) and 90.7(male 93.4, female 88.8), respectively. We found a strong and statistically significant association between carbohydrate ratio and glycemic index in obese factors, other nutrient. But carbohydrate intake/kg of body weight was low a significant differences in obese factors, other nutrient. Also glycemic index was effected by total energy intake and carbohydrate ratio than carbohydrate intake/kg of body weight. In conclusion, emphasis for dietary modification should be total energy intake and carbohydrate ratio in diabetes mellitus patient.

A Study of the Factors Affecting the Term of Engraftment During Hematopoietic Stem Cell Transplantation with a Focus on the Inhibitors of Oral Intake and the Period of Nutritional Support (조혈모세포이식 시 조기생착에 영향을 주는 요인 분석 - 경구섭취 저해인자 및 영양지원 시기를 중심으로 -)

  • Hematopoietic stem cell tranntation is being widely used in an attempt to treat many hematological diseases such as leukemia, anemia, and lymphoma. To evaluate the success of hematopoietic stem cell transplantation, it is very important to determine how rapidly engraftment occurs. Therefore, this retrospective study was conducted to determine which factors affected the term of engraftment during hematopoietic stem cell transplantation, while focusing on the oral intake status. To accomplish this, 416 patients who underwent transplant operations at St. Mary's hospital from May 2006 to April 2008 were evaluated. The long-term engraftment group was characterized as having longer fasting days and more frequent vomiting, diarrhea, and oral mucositis incidences than the short-term engraftment group. In addition, the inhibitors of oral intake such as vomiting, diarrhea, and oral mucositis developed frequently between the pre-transplantation and 2 weeks after transplantation. A significantly negative correlation was observed between the oral intake volume and the duration of the oral intake inhibitors. A multiple regression analysis revealed that the frequency of vomiting and oral mucositis during hematopoietic stem cell transplantation, the length of hospitalization, and the hematocrit level in the 2 weeks after hematopoietic stem cell transplantation were significant predictors of engraftment. The results of this study could be used to establish a guideline for nutritional assessment, nutritional goals, and nutritional support for patients during hematopoietic stem cell transplantation.

Effects of Intaking of Red Ginseng Products on Human Platelet Aggregation and Blood Lipids (홍삼류의 섭취가 사람 혈소판의 응집반응 및 혈중 지질에 미치는 영향)

  • Thrombogenesis and atherosclerosis are mainly caused by platelet aggregation, blood coagulation, and hyperlipidemia. Platelet aggrelation, activated platelet thromboplastin time (APTT) were measured as indexes of blood coagulation and lipid contents in the subjects who have taken red ginseng products (e.g. water extract, tea, drink etc.) for 4 to 5 years. The platelet aggregation in the red ginseng-taking group was significantly decreased, as compared with the non-red ginseng-intaking group, when platelets were stimulated by 100 $\mu\textrm{g}$/ml of collagen (P<0.01). The atherogenic index and the ratio of triglyceride to HDL-cholesterol in blood, the risk factors of atherosclerosis, were decreased in the subjects of ginseng group, compared with that in control group. APTT was also prolonged to greater extent in ginseng group than in control group. These results suggest that long-term intake of ginseng products may help to prevent the risks of thrombogenesis and atherosclerosis.

Salt Intake Behavior and Blood Pressure: the effect of taste sensitivity and preference (소금 섭취 행태와 혈압: 맛에 대한 민감도와 선호도의 영향)

  • The literature suggested that a small reduction in overall blood pressure can have a large effect on overall prevalence of hypertension, and therefore, the affect of taste preferences of the population on salt intake should be considered for long-term blood pressure intervention programs. The purpose of this study is to investigate the influence of salt taste preference and salt taste sensitivity on salt intake behavior as risk factors for high blood pressure. We collected information on blood pressure, diet and lifestyle behaviors, salt taste preference and salt taste sensitivity from 540 respondents from Suseo-dong, Seoul. Salt taste sensitivity was assessed by administering a 1% NaCl solution to the subject's tongue and measuring the perceived intensity on 10 level scale. Salt intake behavior was classified into 3 categories: frequency of high-sodium foods, practice of salt-reducing behavior and frequency of vegetable and fruit intake. Salt taste preference showed a significant relation to the subjects' blood pressure, i.e. subjects with a higher salt preference had higher blood pressure. Salt taste sensitivity did not show a significant relation to blood pressure. However, there was a positive correlation between salt taste preference and salt taste sensitivity. Among the 3 indicators used to measure salt intake behavior, the practice of salt-reducing behavior remained significantly correlated to blood pressure. Moreover, salt-reducing behavior and salt taste preference showed a significant correlation, i.e. people who do not like salty foods tend to practice more salt-reducing behavior, leading to reduced levels in blood pressure. In a population, a small reduction in overall blood pressure can have large effects in overall prevalence of hypertension, in contrast to clinical studies where achievement of an individual's normal blood pressure is emphasized. Therefore, taste preference of the population should be considered for long-term blood pressure intervention programs.

Long-term Ring Deflection Prediction of GFRP Pipe in Cooling Water Intake for the Nuclear Power Plant (원전 냉각수 취수용 GFRP관의 장기관변형 예측)

  • Recently, underground pipes are utilized in various fields of applications such as sewer lines, drain lines, water mains, gas lines, telephone and electrical conduits, culverts, oil lines, etc. Most of pipes are installed for long-term purposes and they should be safely installed in consideration of installation conditions because there are unexpected various terrestrial loading conditions. In this paper, we present the result of investigation pertaining to the structural behavior of glass fiber reinforced thermosetting polymer plastic (GFRP) flexible pipes buried underground. The mechanical properties of the GFRP flexible pipes produced in the domestic manufacturer are determined and the results are reported in this paper. In addition, ring deflection is measured by the field tests and the finite element analysis (FEA) is also conducted to simulate the structural behavior of GFRP pipes buried underground. From the field test results, we predicted long-term, up to 50 years, ring deflection of GFRP pipes buried underground based on the method suggested by the existing literature. It was found that the GFRP flexible pipe to be used for cooling water intake system in the nuclear power plant is appropriate because 5% ring deflection limitation for 50 years could be satisfied.

Effect of probiotics intake on oral environment changes of the elderly in long-term care facilities (프로바이오틱스 섭취가 장기요양시설 노인의 구강환경 변화에 미치는 효과)

  • Objectives: This study was aimed at evaluating the effects of changes in the oral environment of the elderly in long-term care facilities after the intake of Weissella cibaria. Methods: The test group ingested the W. cibaria strain Chonnam Medical University (CMU), and the control group ingested the control food. Of all randomized trial subjects, 62 were analyzed (32 in the experimental group and 30 in the control group). In this 8-week demographic study, we evaluated self-perceived halitosis, changes in halitosis, sensory test results, tongue plaque index scores, saliva buffering capacity, and the salivary flow rate. Results: The W. cibaria CMU intake in the elderly in long-term care facilities during the experimental period did not demonstrate statistically significant changes in the salivary flow rate. However, self-perceived halitosis, organoleptic test results, tongue plaque index scores, and salivary buffering capacity demonstrated statistically significant differences between the experimental and control groups. These findings partially confirmed the beneficial effects of the W. cibaria CMU on the oral environment in the elderly. Conclusions: Research results on the role of probiotics in the oral cavity should be summarized, and utilization plans should be sought to obtain a clearer understanding of the clinical efficacy and related factors. The value of probiotic use may be high in improving the oral health of people by enabling treatment and prevention.

The Effects of Oral Health Care Education for Nursing Staff on Halitosis and Nutrient Intake of the Elderly in a Long-term Care Facility (간호자 구강간호 교육프로그램이 요양시설 노인의 구취와 영양소 섭취량에 미치는 효과)

  • The purpose of this study was to investigate the effects of an oral health care education program for care providers on the nutritional status of the elderly in a long-term care facility. This study was conducted at a long-term care facility located in K metropolitan city using a nonequivalent control group non-synchronized design. Fifty-four nursing home residents aged 65 years or older were assigned either to the intervention group (n=27) or the control group (n=24). The intervention group received oral health care from the care providers' intervention group, who provided oral health care for 6 weeks after 6 weeks of oral health care education. Data were collected from the control group and intervention group at the baseline 6 and 12 weeks after oral care education and were analyzed using SPSS windows 16.0. The halitosis was lower in the intervention group than the control group at 12 weeks (P<0.01). Body mass index of the intervention group at 12 weeks was higher than that at 6 weeks. Iron intake of the intervention group at 12 weeks was lower than that at baseline. At baseline, the intakes of riboflavin, folate, and potassium were lower than 75% of dietary reference intakes. In conclusion, an oral care education program for care providers was effective in improving the oral hygiene of nursing home residents, and dietary plans are needed to improve the nutritional status of them.
