  • Adult patients who need implant and prosthodontic treatment often need treatment to improve the existing occlusion through orthodontic treatment for long-term stable treatment results. However, due to non-aesthetic and uncomfortable orthodontic treatment with orthodontic brackets and wires, many adult patients give up treatment even though they know the need for it. Recently, as digital dentistry has affected all areas of dentistry, clear aligner orthodontic systems have begun to be widely used, and their use is increasing in adults and old-aged people due to the esthetic advantage and convenient oral care. SERAFIN clear aligner system developed in Korea has been developed with the aim of implementing a functional occlusion harmony and is used not only for partial orthodontic treatment but also comprehensive orthodontic treatment. This patient presentation is shown the treatment using SERAFIN clear aligner system for the treatment of patient with TMD, severe extrusion of maxillary second molars, and extracted teeth.

Mediating Effect of Cognitive Function on the Relationship Between Vision, Hearing, and Activities of Daily Living (노인의 시각, 청각 및 일상생활활동의 관계에 대한 인지기능의 매개효과)

  • Objective : The typical decline in function due to aging includes limitations in vision, hearing, cognitive function, and activities of daily living. Therefore, this study evaluated the mediating effect of cognitive function on the relationship between vision and hearing and activities of daily living. Methods : The participants in this study were 36,767 persons from the long-term care database on older adults provided by the National Health Insurance Sharing Service. The extracted variables were: (i) independent variables: visual and hearing; (ii) dependent variables: basic and instrumental daily living activities; and (iii) mediator variables: cognitive function. Regression analysis was performed evaluate the mediating effects of the extracted variables. Results : Cognitive function showed a mediating effect of 30% on the relationship between vision and basic daily life activities and 42% in the relationship with instrumental daily life activities. The mediating effect was 50% in the relationship between hearing and basic activities of daily living, and 53% in the relationship with instrumental activities of daily living. Conclusion : Prevention programs for functional decline due to aging are in progress in Korea. Based on the results of this study, establishing a prevention program would help promote successful aging.

A Study on the Usefulness of Characteristic Past History Investigation for Life Care in People with Lumbar Instability (허리부위 불안정성자 라이프케어를 위한 특징적과거력 조사의 유용성에 관한 연구)

  • The purpose of this study was to investigate the relation between and subjective instability pain behavior (SIPB) and physical instability test(PIT) according to the presence of characteristic past histories(CPH) in people with chronic low back pain(CLBP). Forty CLBP subjects participated in this study. The presence of four characteristics past histories(long term history, traumatic experience, sports activities, neurologic sign) were examined. According to presence number(PN) of CPH, subjects were divided into 5 groups[group 1(PN:0): n=8, group 2(PN:1): n=8, group 3(PN:2): n=8, group 4(PN:3): n=8, group 5(PN:4): n=8]. After 16 items were examined for the SIPBs, then Seven PITs were conducted, and the results were scored. The SIPBs and PITs were compared according to the presence numbers of CPH, and the relation between them was analyzed. There was a significant difference(p<.05) in both SIPB scores and PIT scores in the comparison of groups according to the presence number of CPH. There was high positive correlation between the presence numbers of CPH and SIPB score(r=.819, p=.000) and PIT score(r=.606, p=.000). Also, there was a correlation between SIPB score and PIT score(r=.571, p=.000). Based on the findings in the present study, the presence of three or more CPH in people with CLBP may be a useful variable in the diagnosis of lumbar instability. The combined findings of the three variables such as CPH, SIPB, and PIT can improve the accuracy of lumbar instability diagnosis.

Factors Affecting Retirement Satisfaction of the Elderly: Comparing young-old and middle-old elders (노인의 은퇴만족에 영향을 미치는 요인: 전기·중기노인 비교)

  • The purposes of this study are to examine the factors affecting retirement satisfaction, focusing on health, social activity and social security anticipation, and to compare the results between young-old and middle-old. A total of 860 retired people who live alone or live with their spouses were selected from 6th Korean Longitudinal Study of Aging, and binomial logistic regression analysis was used to analyze 416 young-old(aged from 65 to 74) and 444 middle-old(aged from 75 to 83). The results are as follows. First, there was no significant difference in retirement satisfaction between young-old and middle-old. Second, when factors affecting retirement satisfaction was examined, young-olds showed hgher retirement satisfaction with higher perception of health condition and mental health condition, and showed lower retirement satisfaction with higher expectation of long-term care insurance. Middle-old was likely to show higher retirement satisfaction when they had better perspective on physical and mental health condition. The results of the study show that health condition of elderly is an important contributing factor in determining retirement satisfaction of the elderly. Based on the results of the study, political and practical suggestions were proposed to improve retirement satisfaction.

  • Stable posterior occlusal support is crucial for adequate masticatory function and facial aesthetics. In elderly patients over the age of 65, masticatory ability has a significant impact on nutritional intake and overall health. This case report presents a prosthetic treatment of an elderly patient with edentulous maxilla and bilateral posterior edentulous mandible. The upper jaw was restored with a complete denture to establish an ideal occlusal plane, and the three-dimensional positions of the mandibular implants were determined accordingly. The implants were placed through computer-guided implant surgery and were immediately loaded with fixed provisional restorations. The implant-fixed zirconia final restorations were inserted in the lower posterior region, and the occlusal surface of posterior artificial teeth in the upper denture was substituted with cobalt-chrome alloy to resist occlusal wear. The patient's posterior occlusal support and masticatory function were promptly restored through this treatment process, and long-term stability against wear was also obtained.

Effectiveness of NQ-E index-based individual nutrition counseling for community-care elderly: an intervention study on improving nutritional status, complex chronic diseases, and quality of life (커뮤니티케어 대상 노인의 NQ-E 지수 기반 개별영양상담 효과성: 복합만성질환과 삶의 질 개선에 관한 중재 연구)

  • Objectives: This study sought to assess the effectiveness of community-based nutrition counseling on improving nutritional status, managing complex chronic diseases, and enhancing the quality of life for elderly individuals with chronic conditions, particularly in older adults with high levels of food insecurity and multiple chronic illnesses. Methods: Thirty elderly subjects with diabetes and hypertension who were registered at local Senior Welfare Center received individualized nutrition counseling, based on their Nutrition Quotient for the Elderly (NQ-E) index. Over a 16-week period, they received tailored counseling and underwent various health and nutritional assessments. The final analysis included 28 participants after two dropped out. Data analysis was conducted using the SPSS v28.0. Results: The subjects were over 70, with multiple chronic diseases including diabetes and hypertension and predominantly female. After 16 weeks, significant improvements were observed in the subjects' grip strength, and HbA1c levels, as well as in their NQ-E scores, indicating improved dietary balance and diversity. There were no significant improvements in the 'Moderation' subdomain of the NQ-E index, suggesting that this aspect requires further attention in nutritional counseling. The subjects' nutritional risk scores (NSI) were also significantly decreased, indicating less nutritional risk. Lastly, as measured by the SF-36K, the subjects' quality of life showed significant improvement in several domains including physical role performance and social function. Conclusions: This study demonstrates that tailored nutrition counseling, based on the NQ-E index, can improve elderly health, manage chronic diseases, and enhance quality of life. This approach potentially broadens the scope of community nutritionists' roles within an aging society. However, additional research is necessary to evaluate these interventions' long-term effects and sustainability.

Non Face-to-Face Treatment and Not-informed Medication to Persons with Mental Disorders (정신질환자에 대한 비대면진료 및 비고지투약 -치료적 대화의 복원을 위한 모색적 고찰-)

  • People with mental illness are generally either unaware of their illness or unwilling to voluntarily seek treatment, which makes treatment difficult and the pain mainly passed on to their families. Accordingly, non-face-to-face treatment, in which the patient is diagnosed by interviews with the family and unannounced medication, in which medication is secretly administered through the family, can be performed, and this has been considered a necessary evil. Even considering realistic aspects such as the special nature of mental health care and families' suffer, not-informed treatment without consent violates not only medical laws, but also human rights of mentally ill patients. Above all, if the patient finds out about this late, the trust between the patient, family, and doctor is completely broken, and a treatment is absolutely refused. Japan's Chiba decision, which presents exceptional conditions for allowance might be a solution. However, it would not be a right solution, considering that it could lead to long-term unannounced medication and completely cut off treatment through therapeutic dialogue. Ultimately, it need to approach this problem and seek alternatives through restoration of therapeutic dialogue.

Spatial and temporal trends in food security during the COVID-19 pandemic in Asia Pacific countries: India, Indonesia, Myanmar, and Vietnam

  • BACKGROUND/OBJECTIVES: The economic recession caused by the coronavirus disease 2019 pandemic disproportionately affected poor and vulnerable populations globally. Better uunderstanding of vulnerability to shocks in food supply and demand in the Asia Pacific region is needed. SUBJECTS/METHODS: Using secondary data from rapid assessment surveys during the pandemic response (n = 10,420 in mid-2020; n = 6,004 in mid-2021) in India, Indonesia, Myanmar, and Vietnam, this study examined the risk factors for reported income reduction or job loss in mid-2021 and the temporal trend in food security status (household food availability, and market availability and affordability of essential items) from mid-2020 to mid-2021. RESULTS: The proportion of job loss/reduced household income was highest in India (60.4%) and lowest in Indonesia (39.0%). Urban residence (odds ratio [OR] range, 2.20-4.11; countries with significant results only), female respondents (OR range, 1.40-1.69), engagement in daily waged labor (OR range, 1.54-1.68), and running a small trade/business (OR range, 1.66-2.71) were significantly associated with income reduction or job loss in three out of 4 countries (all P < 0.05). Food stock availability increased significantly in 2021 compared to 2020 in all four countries (OR range, 1.91-4.45) (all P < 0.05). Availability of all essential items at markets increased in India (OR range, 1.45-3.99) but decreased for basic foods, hygiene items, and medicine in Vietnam (OR range, 0.81-0.86) in 2021 compared to 2020 (all P < 0.05). In 2021, the affordability of all essential items significantly improved in India (OR range, 1.18-3.49) while the affordability of rent, health care, and loans deteriorated in Indonesia (OR range, 0.23-0.71) when compared to 2020 (all P < 0.05). CONCLUSIONS: Long-term social protection programs need to be carefully designed and implemented to address food insecurity among vulnerable groups, considering each country's market conditions, consumer food purchasing behaviors, and financial support capacity.

Long-term Outcome of Fontan-Associated Protein-Losing Enteropathy: Treatment Modality and Predictive Factor of Mortality

  • Background and Objectives: Protein-losing enteropathy (PLE) is a devastating complication after the Fontan operation. This study aimed to investigate the clinical characteristics, treatment response, and outcomes of Fontan-associated PLE. Methods: We reviewed the medical records of 38 patients with Fontan-associated PLE from 1992 to 2018 in 2 institutions in Korea. Results: PLE occurred in 4.6% of the total 832 patients after the Fontan operation. After a mean period of 7.7 years after Fontan operation, PLE was diagnosed at a mean age of 11.6 years. The mean follow-up period was 8.9 years. The survival rates were 81.6% at 5 years and 76.5% at 10 years. In the multivariate analysis, New York Heart Association Functional classification III or IV (p=0.002), low aortic oxygen saturation (<90%) (p=0.003), and ventricular dysfunction (p=0.032) at the time of PLE diagnosis were found as predictors of mortality. PLE was resolved in 10 of the 38 patients after treatment. Among medical managements, an initial heparin response was associated with survival (p=0.043). Heparin treatment resulted in resolution in 4 patients. We found no evidence on pulmonary vasodilator therapy alone. PLE was also resolved after surgical Fontan fenestration (2/6), aortopulmonary collateral ligation (1/1), and transplantation (1/1). Conclusions: The survival rate of patients with Fontan-associated PLE has improved with the advancement of conservative care. Although there is no definitive method, some treatments led to the resolution of PLE in one-fourth of the patients. Further investigations are needed to develop the best prevention and therapeutic strategies for PLE.

A Basic Study for the Introduction of Green Prescription and Establishment of Policy System in Korea - Through Comparative Analysis of U.K. and U.S. Cases - (국내 녹색처방 도입과 정책체계 수립을 위한 기초연구 - 영국과 미국 사례 비교 분석을 통해 -)

  • The burden of medical expenses and the loss of social capital due to chronic diseases are becoming problems worldwide, and comprehensive and inclusive measures across various fields are required to prevent and manage their impacts. Social prescriptions have been shown to be effective in resolving the fundamental causes of health problems in patients with chronic diseases and in supporting existing treatments. In particular, green prescriptions that utilize the healing effects of nature and green spaces based on social prescriptions are being introduced in many countries overseas. Green prescription is the practice of a healthcare provider recommending activities in green spaces or experiences in the natural environment to patients for the prevention and management of chronic diseases. This study analyzed cases focusing on the policy system, the cases of the United Kingdom and the United States, which have introduced and operated green prescriptions under a national system. For this purpose, this study compared the background of green prescription introduction, related policies, and operation methods. Based on this, four implications were proposed to establish an effective plan for introducing green prescriptions in Korea. First, prior to establishing a policy for green prescriptions, interest in and research on green prescriptions are essential. Second, an implementation plan that fits the national health care system should be established, and policies should support the plan. Third, the introduction of green prescriptions from a long-term and gradual perspective is required. Fourth, comprehensive cooperation is required for the introduction and implementation of the green prescription system. This study can be used as basic data for discussion before introducing green prescriptions in Korea in the future.
