요양병원 입원 노인의 수면 양상 및 영향요인 (Sleep patterns and it's influencing factors of hospitalized elderly in long-term care hospital)

  • 본 연구는 노인요양병원 입원노인의 수면양상 및 영향요인을 파악하여 이들의 수면의 질 향상을 위한 중재프로그램 개발 시 기초자료로 활용하고자 시도된 서술적 조사연구이다. G시 노인요양병원에 입원한 65세 이상의 노인환자 142명을 대상으로 구조화된 설문지와 24시간 수면기록지를 사용하여 자료 수집하였다. 요양병원 입원노인의 일일 평균 수면시간은 10.7시간이었으며 낮 수면시간은 평균 3.9시간, 밤 수면시간은 평균 6.8시간 이었다. 총 수면규칙성은 71.7%였고, 낮 수면규칙성은 평균 58.1%, 밤 수면규칙성은 평균 80.5%였다. 요양병원 입원노인의 수면시간에 영향을 주는 예측요인은 병실 내 치매환자 유무로 10.3%의 설명력을 나타냈고, 수면 규칙성에 영향을 미치는 예측요인은 통증, 병실 내 치매환자 유무, 신체기능으로 16.1%의 설명력을 나타냈다. 따라서 요양병원 입원노인의 수면양상에 영향을 미치는 추가요인 규명을 위한 후속 연구의 필요성과, 본 연구에서 확인된 요양병원 입원노인의 수면에 영향을 미치는 요인을 고려한 중재프로그램 개발 및 적용이 필요하다.

Effects of Dietary Supplement Containing Melatonin on Reproductive Activity in Male Golden Hamsters

  • Melatonin is a pineal hormone that is synthesized and released at night under the light and dark cycles of a day. Its effects on the reproductive activities have well been established by the administration through various routes in photoperiodic animals. It was also identified in plants and named phytomelatonin. The capacity of the phytomelatonin was investigated in this investigation whether it affects the reproductive function in male golden hamster. As expected, animals housed in long photoperiod (long photoperiod, LP>12.5 hours of lights in a day) had large testes and animals kept in short photoperiod ($$SP{\leq_-}12.5$$ hours of lights in a day) showed remarkably reduced testes. The dietary supplement with melatonin itself induced the complete involution of testes. Pistachios that were reported to contain a large amount of melatonin demonstrated no effects at all in male golden hamsters. These results suggest that dietary supplement containing melatonin-rich foodstuff used in this investigation may not be enough to affect the reproductive endocrine system in male golden hamsters.

Measuring the Impact of Change Orders on Project Performances by Building Type

  • The project performances can be measured in terms of meeting the project schedule, budget, and conformance to functional and technical specifications. Numerous studies have been conducted to examine the causes and effects of change orders for both vertical and horizontal construction, respectively. However, these studies mainly focus on a single project type, so this paper examines the impact of change order for cost growth and schedule overruns using four different building types to close the gap in the change order research area. A total of 211 building projects are collected from four building types: healthcare, residential, office, and education. Statistical analyses using ANOVA tests and linear regression models are used to examine the created metric $CO/day on the cost and schedule impacts. The results found that mean $CO/day values were not statistically different among building types, and that the sum of change orders is a statistically significant predictor of $CO/day. The results will help project stakeholders mitigate the negative change orders effects can be a challenge for project managers and researchers alike.

  • 심부 처분환경조건에서 붕규산유리고화체의 장기침출거동을 규명하기 위하여 3종의 모의붕규산유리고화체에 대한 장기침출실험이 1997년에 착수되었다. 5년간의 침출결과는 붕소가 본 붕규산유리고화체의 장기침출지표물질로 사용될 수 있음을 확인시켜 주었고, 비록 고화체들의 조성은 약간씩 다르지만, 초기 1년여 기간동안의 침출률을 제외한 장기침출률은 S/V에도 무관하게 0.03g/$m^2$-day 에 근접하는 경향을 보여주고 있다.

장기요양 서비스 이용자와 미이용자의 의료비 지출 차이 및 의료비 지출에 미치는 영향 - 요양병원 의료비 지출을 중심으로 - (The Effect on Health Care Utilization of the Non-Use of Beneficiaries of Long-Term Care Insurance Service - around of Geriatric Hospital's Medical Cost -)

  • 본 연구는 장기요양 1등급 건강보험가입자를 대상으로 장기요양서비스 이용자와 미이용자의 의료비 지출 차이를 비교하고, 의료비 지출에 미치는 영향 요인을 규명해 보고자 시도하였다. 연구 대상은 건강보험가입자로 2009년 1월 1일부터 12월 31일까지 1등급 판정을 받은 21,213명 전수를 대상으로 국민건강보험공단의 장기요양급여, 건강보험급여 자료를 활용하였다. 연구결과 2007년부터 2009년의 연간 총 진료비 변화량의 경우 서비스 이용자에 비해 미이용자는 5,337천원 증가하였으며, 연간 요양병원 진료비 변화량은 5,449천원 증가하였다. 연간 총 입원일 변화량의 경우 서비스 이용자에 비해 미이용자는 87.31일 증가하였으며, 요양병원 입원일 변화량은 79.47일 증가한 것으로 나타났다. 이 같은 결과는 장기요양 서비스 미이용자의 의료이용, 특히 요양병원의 의료이용이 높게 나타남에 따라 미이용자에 대한 적정의료와 요양서비스 지원 정책을 통해 장기요양과 의료서비스의 효율적 연계가 필요로 된다.

Relative Contribution from Short-term to Long-term Flaring rate to Predicting Major Flares

  • We investigate a relative contribution from short to long-term flaring rate to predicting M and X-class flare probabilities. In this study, we consider magnetic parameters summarizing distribution and non-potentiality by Solar Dynamics Observatory/Helioseimic and Magnetic Imager and flare list by Geostationary Operational Environmental Satellites. A short-term rate is the number of major flares that occurred in an given active region (AR) within one day before the prediction time. A mid-term rate is a mean flaring rate from the AR appearance day to one day before the prediction time. A long-term rate is a rate determined from a relationship between magnetic parameter values of ARs and their flaring rates from 2010 May to 2015 April. In our model, the predicted rate is given by the combination of weighted three rates satisfying that their sum of the weights is 1. We calculate Brier skill scores (BSSs) for investigating weights of three terms giving the best prediction performance using ARs from 2015 April to 2018 April. The BSS (0.22) of the model with only long-term is higher than that with only short-term or mid-term. When short or mid-term are considered additionally, the BSSs are improved. Our model has the best performance (BSS = 0.29) when all three terms are considered, and their relative contribution from short to long-term rate are 19%, 23%, and 58%, respectively. This model seems to be more effective when predicting active solar ARs having several major flares.

국민기초생활보장수급자의 장기요양 서비스 이용 여부가 의료 이용에 미치는 영향 (Effect of Long-term Care Utilization on Health Care Utilization of the Medicaid Elderly)

  • 본 논문은 기초생활보장수급자를 대상으로 장기요양서비스 이용 여부가 의료 이용에 미치는 영향을 파악하기 위한 서술적 연구이다. 연구 대상은 장기요양 1등급 판정을 받은 기초생활보장수급자 5,834명을 대상으로 하였다. 기초생활보장수급자의 의료 이용에 미치는 영향을 다중회귀분석으로 분석한 결과 2007-2009년 총 진료비 변화량을 설명하는 $R^2$은 22.6%이었으며, 장기요양 서비스 이용자에 비해 미이용자는 8,297,329원 증가하였다(${\beta}=.29$, p< .001). 입원일 변화량의 $R^2$은 22.4%이었으며, 서비스 이용자에 비해 미이용자는 119.013일 증가하였다(${\beta}=.33$, p< .001). 총 진료비와 입원일 변화량에 영향을 미치는 요인은 장기요양서비스 이용 여부, 수발자 여부, 2009년 일상생활수행능력, 간호처치, 재활기능이 유의한 영향을 미치는 것으로 나타났다. 따라서 장기요양 서비스 미이용자의 의료 이용이 높게 나타남에 따라 미이용자에 대한 적정 의료와 요양서비스 이용을 지원할 수 있는 정책 마련이 필요로 된다.

CHANGES IN CONTENTS AND LOCALIZATIONS OF CARBONIC ANHYDRASE II, PROCHYMOSIN AND PEPSINOGEN IN ABOMASAL MUCOSAE DURING LONG TERM MILK FEEDING GOATS

  • The present paper describes temporal changes of immunohistochemical localization and quantities of carbonic anhydrase isozyme II (CA-II) prochymosin (PC) and pepsinogen (PN) in goat's abomasal mucosae during long term milk feeding. The CA-II was not detected by day 14 after birth and then became positive on day 34 in the parietal cells, suggesting that the excretion of the hydrochloric acid (HCl) begins between days 14 and 34 under a feeding condition without solid materials. The quantity of the PC in the gastric chief cells detected by the ELISA showed rapid increase from the day of birth, making a peak on day 8 and then gradually decreased with age. The decrease in quantity of PC became started during the time period when HCl excretion had not started yet. The quantities of PN in the gastric chief cells were almost stable during the whole period examined. Expressions of these gastric enzymes did not seem to be regulated by the change of feeding condition.

Effects of Temperature and Day-Length on Heading Habit of Recently Developed Korean Rice Cultivars

  • To understand the heading habit of recently developed 20 Korean rice cultivars, rice plants in a phytotron were exposed to different temperature: $22.5^{\circ}C(day\;27^{\circ}C/night\;18^{\circ}C),\;27.5^{\circ}C(day\;32^{\circ}C/night\;23^{\circ}C)$, and day-length conditions: 10, 12, 13, 14, 15 hours. Four rice cultivars (Sobibyeo, Juanbyeo, Ilpumbyeo and Shindongjinbyeo) showed relatively short Basic Vegetative Phase (BVP) of 17 to 18 days, while Dasanbyeo showed the longest (35 days) BVP, compared to other remaining 15 tested cultivars which exhibited 24 to 31 days of BVP. In this experiment, it was tried out to separate the eliminable vegetative phase into photosensitivity and thermo-sensitivity with two different pathways. Many tested cultivars, however, exhibited quite different responses under low temperature and / or long day-length conditions. Especially, Surabyeo and Juanbyeo were the most difficult cultivars to separate into photo- or thermo-sensitivity in that the eliminable vegetative phase of these two cultivars increased greatly only under low temperature $(22.5^{\circ}C)$ and long day-length (15 hr.) conditions. Regarding the heading response to temperatures, tested cultivars could be categorized into 2 groups. In 1st group of rice cultivars, the eliminable vegetative phase decreased almost equally as the temperature changed from $20.0^{\circ}C\rightarrow22.5^{\circ}C\rightarrow25.0^{\circ}C\rightarrow27.5^{\circ}C$. In contrast, the 2nd group of rice cultivars exhibited eliminable vegetative phase slowly decreasing when the temperature changed from $22.5^{\circ}C\rightarrow25.0^{\circ}C\rightarrow27.5^{\circ}C$, but rapidly decreasing when the mean temperature changed from $20.0^{\circ}C\;to\;22.5^{\circ}C$. All the cultivars belonged to 2nd group, the heading date would be very delayed if cool summer comes.

Probable Evapotranspiration of Paddy Rice using Dry Day Index

  • To support some knowledge in planning irrigation system, short or long-term irrigation scheduling or determining irrigation reservoir capacity, it is necessary to estimate peak irrigation requirements and seasonal distribution of water demands for various return periods. In this paper Dry Day Index and Probable Evapotranspiration were evaluated to decide seasonal consumptive use of paddy rice for a design year using several decades' daily rainfall data and 5 years'('82~'86) actual evapotranspiration data, respectively. To obtain Dry Day Index that is defined as the number of probable dry days for a given period, Slade unsymmetrical distribution function was adopted. Dry Day Index was analysed for 5 and 10-day intervals. Each of them was evaluated with return periods of 1, 3, 5, 10 and 20 year. Their singnificance was tested by X$^2$ method. Based on these values, the Probable Evaportanspiration, that is the average daily ET both in dry days and rainy days during a given period, was estimated. Crop coefficient was also determined by the modified Penman equation proposed by Doorenbos & Pruitt.
