Behavioral Strategies for Weight Loss Maintenance (체중감량 이후 장기적인 유지를 위한 다양한 행동전략)

  • Compensatory changes in energy consumption and neuro-hormonal changes following weight loss make it difficult to maintain the reduced weight and may cause weight regain. Therefore, establishing a long-term weight control plan and strategy starting from the initial weight loss period is necessary. Both the patient and doctor should know that weight loss cannot occur continuously, and that maintaining weight after the weight loss period is the basic course of obesity treatment. No single dietary pattern is effective for weight maintenance, and a variety of dietary control methods - such as calorie restriction and healthy proportions of carbohydrates, proteins, fats, and meal replacements - should be used to target an integrated and healthy dietary habit. An increase in physical activity is needed for weight loss and maintenance; however, rather than recommending an excessive amount of exercise, it is better to set realistic and long-term achievable goals. It is necessary to reset the goal according to the patient's weight maintenance stage and continuously apply behavioral therapies, such as self-monitoring and stress management. In previous studies, since the degree of weight loss and changes in behavioral patterns over the course of one year were important factors in maintaining long-term weight loss, obesity therapists should closely examine patient data and behavioral patterns across a period of one year and actively intervene when needed.

  • Dietary therapy is one of the most important treatments for obesity. In general, it is difficult to maintain the recommended diet for weight control for a long time; therefore, it is difficult to achieve weight loss or maintain weight. Intermittent fasting has recently become one of the most popular diets for weight loss. Intermittent fasting is a strategy of repeating intermittent energy restriction and eating, unlike conventional diets of continuous calorie restriction. Studies on intermittent fasting have shown positive results not only in terms of weight loss but also improvement in metabolic indicators, such as glucose control and reduction of blood pressure. Therefore, it is important to maintain a long-term dietary strategy to prevent weight loss in obese individuals. The effect on weight loss was similar to that of an existing continuous energy-restricted diet. However, long term studies and safety data are still lacking, and large-scale studies with various populations are needed. If more evidence is secured for various individuals, it can be expected that intermittent fasting, including time-restricted eating, will be applied clinically in the future.

  • We develop a mathematical model for the obesity dynamics to investigate the long term obesity trend with the consideration of psychological and social factors due to the increasing prevalence of obesity around the world. Many mathematical models for obesity dynamics adopted the modeling idea of infectious disease and treated overweight and obese people infectious and spreading obesity to normal weight. However, this modeling idea is not proper in obesity modeling because obesity is not an infectious disease. In fact, weight gain and loss are related to social interactions among different weight groups not only in the direction from overweight/obese to normal weight but also the other way around. Thus, we consider these aspects in our model and implement personal weight gain feature, a psychological factor such as body image dissatisfaction, and social interactions such as positive support on weight loss and negative criticism on weight status from various weight groups. We show that the equilibrium point with no normal weight population will be unstable and that an equilibrium point with positive normal weight population should have all other components positive. We conduct computer simulations on Korean demography data with our model and demonstrate the long term obesity trend of Korean male as an example of the use of our model.

An exploratory study of factors related to long-term hospitalization of inpatients using the quality assessment data for long-term care hospitals (요양병원 입원급여 적정성 평가 결과를 활용한 요양병원 입원환자의 장기입원 관련 요인 탐색 연구)

  • Purpose: The purpose of this study was to analyze the factors associated with long-term hospitalized patients in long-term care hospitals using the quality assessment data for long-term care hospitals by the Health Insurance Review. Methods: Among 1,376 long-term care hospitals, frequency analysis and descriptive statistics were used to analyze the characteristics of these hospitals. Multiple linear regression was conducted to examine the associations between infrastructure characteristics, medical personnel characteristics, health outcomes and the proportion of long-term hospitalized patients. Results: The research findings indicate that the number of patients per doctor, the number of patients per nurse, and the number of patients per nursing staff were positively associated with the proportion of long-term hospitalized patients. Among health outcomes, a higher proportion of patients with more than a 5% weight loss compared to the previous month and the proportion of patients showing improvement in ADL, were more likely to have a lower proportion of long-term hospitalized patients. However the proportion of diabetic patients with HbA1c test results within the appropriate range was positively associated with the proportion of long-term hospitalized patients. Conclusion: The present study results provide fundamental data for the establishment of policies for long-term care hospitals. Based on this study, it is important to suggest screening methods for unnecessary long-term hospitalizations, such as sufficient medical personnel to improve the quality of care in long-term care hospitals. It is also necessary to clearly separate the roles of medical institutions and long-term care facilities and implement policies to support patients' social reintegration.

Improvement of Track Tracking Performance Using Deep Learning-based LSTM Model (딥러닝 기반 LSTM 모형을 이용한 항적 추적성능 향상에 관한 연구)

  • This study applies a deep learning-based long short-term memory(LSTM) model to track tracking technology. In the case of existing track tracking technology, the weight of constant velocity, constant acceleration, stiff turn, and circular(3D) flight is automatically changed when tracking track in real time using LMIPDA based on Kalman filter according to flight characteristics of an aircraft such as constant velocity, constant acceleration, stiff turn, and circular(3D) flight. In this process, it is necessary to improve performance of changing flight characteristic weight, because changing flight characteristics such as stiff turn flight during constant velocity flight could incur the loss of track and decreasing of the tracking performance. This study is for improving track tracking performance by predicting the change of flight characteristics in advance and changing flight characteristic weigh rapidly. To get this result, this study makes deep learning-based Long Short-Term Memory(LSTM) model study the plot and target of simulator applied with radar error model, and compares the flight tracking results of using Kalman filter with those of deep learning-based Long Short-Term memory(LSTM) model.

National Registry Data from Korean Neonatal Network: Two-Year Outcomes of Korean Very Low Birth Weight Infants Born in 2013-2014

  • Background: The aim of this study was to observe long-term outcomes of very low birth weight infants (VLBWIs) born between 2013 and 2014 in Korea, especially focusing on neurodevelopmental outcomes. Methods: The data were collected from Korean Neonatal Network (KNN) registry from 43 and 54 participating units in 2013 and 2014, respectively. A standardized electronic case report form containing 30 items related to long-term follow up was used after data validation. Results: Of 2,660 VLBWI, the mean gestational age and birth weight were $29^{1/7}{\pm}2^{6/7}$ weeks and $1,093{\pm}268g$ in 2013 and $29^{2/7}{\pm}2^{6/7}$ weeks and $1,125{\pm}261g$ in 2014, respectively. The post-discharge mortality rate was 1.2%-1.5%. Weight < 50th percentile was 46.5% in 2013 and 66.1% in 2014. The overall prevalence of cerebral palsy among the follow up infants was 6.2% in 2013 and 6.6% in 2014. The Bayley Scales of Infant Developmental Outcomes version II showed 14%-25% of infants had developmental delay and 3%-8% of infants in Bayley version III. For the Korean developmental screening test for infants and children, the area "Further evaluation needed" was 5%-12%. Blindness in both eyes was reported to be 0.2%-0.3%. For hearing impairment, 0.8%-1.9% showed bilateral hearing loss. Almost 50% were readmitted to hospital with respiratory illness as a leading cause. Conclusion: The overall prevalence of long-term outcomes was not largely different among the VLBWI born between 2013 and 2014. This study is the first large national data study of long-term outcomes.

Study of the Fall Detection System Applying the Parameters Claculated from the 3-axis Acceleration Sensor to Long Short-term Memory (3축 가속 센서의 가공 파라미터를 장단기 메모리에 적용한 낙상감지 시스템 연구)

  • In this paper, we introduce a long short-term memory (LSTM)-based fall detection system using TensorFlow that can detect falls occurring in the elderly in daily living. 3-axis accelerometer data are aggregated for fall detection, and then three types of parameter are calculated. 4 types of activity of daily living (ADL) and 3 types of fall situation patterns are classified. The parameterized data applied to LSTM. Learning proceeds until the Loss value becomes 0.5 or less. The results are calculated for each parameter θ, SVM, and GSVM. The best result was GSVM, which showed Sensitivity 98.75%, Specificity 99.68%, and Accuracy 99.28%.

Clinical Application of Ma Huang in the Obesity Treatment (비만처방에서 마황의 임상활용에 대한 고찰;용량, 효과, 부작용 등의 측면에서)

  • Background : Use of Ma Huang is controversial. Multiple reports of adverse effects led the U.S. Federal Drug Administration (FDA) to announce about prohibiting dietary supplements containing ephedrine alkaloids/Ma Huang. Objective : The objective of the study was to review the evidence from literature and rigorous clinical trials on the dosage, effectiveness, and adverse effects. Methods : Literature searches were conducted in U.S. FDA database after 2003 and digital Uibujeonok(醫部全錄). Hand searches of medical journal and bibliographies of identified articles were conducted. Results : Ma Huang has been used in Korean Oriental Medicine to treatment asthma and other ailments. Recently, Ephedrine/Ma Huang are used for weight loss or to enhance athletic performance in Europe and America, Korea. Ephedrine/Ma Huang are associated with increased risk of psychiatric, autonomic, or gastrointestinal symptoms, and heart palpitations. In addition to that, a risk of serious adverse events, including heart attack, stroke, epilepsy and psychiatric problem are also have been reported. Therefore, max dose of total alkaloids of ephedrine is studied in need of a specialist's part. Conclusion : In most reports, Ma Huang effectively promoted short-term weight loss and enhancement of athletic performance, But, safety with long-term use requires further investigation.

Cold Storage Characteristics of Early Variety of Citrus unshiu Produced in Cheju with Various Treatments (처리조건에 따른 조생온주밀감의 저온저장 특성)

  • The conditions of cold storage of Citrus unshiu Marc. var. miyagawa produced in Cheju were investigated. Compared to the citrus fruits stored at room temperature, the content of soluble solids, total sugar, vitamin C and specific gravity decreased slightly on the fruits stored at $4^{\circ}C$ and 87% relative humidity. Decay ratio and weight loss were below 10% on keeping freshness relatively till late of March during cold storage. Weight loss, decrease of firmness and soluble solids occured gradually during cold storage. Nevertheless seal-packaging with 0.02 mm LDPE film or wax-coating of citrus fruits were effective on weight loss and appearance, decay ratio increased for long-term storage. It was needed to control humidity in cold chamber for preventing from decay and weight loss. Edible part ratio was decreased gradually by respiration, and peel and tissue of fruits were softened slightly by 60 days of cold storage. Decay ratio and weight loss of full-ripened citrus fruits were little during the storage for 100 days, compared to the fruits harvested early. Appearance and taste of citrus fruits stored for 4 month were good relatively, but decay occured rapidly on seal-packaging fruits putting out of cold room for a few days. Because of the difficulty of long-term storage for early variety of Citrus unshiu, the conditions and periods of cold storage would be determined after considering the physicochemical properties of fruits.
