Survival Analysis of Patients with Breast Cancer using Weibull Parametric Model

  • Background: The Cox model is known as one of the most frequently-used methods for analyzing survival data. However, in some situations parametric methods may provide better estimates. In this study, a Weibull parametric model was employed to assess possible prognostic factors that may affect the survival of patients with breast cancer. Materials and Methods: We studied 438 patients with breast cancer who visited and were treated at the Cancer Research Center in Shahid Beheshti University of Medical Sciences during 1992 to 2012; the patients were followed up until October 2014. Patients or family members were contacted via telephone calls to confirm whether they were still alive. Clinical, pathological, and biological variables as potential prognostic factors were entered in univariate and multivariate analyses. The log-rank test and the Weibull parametric model with a forward approach, respectively, were used for univariate and multivariate analyses. All analyses were performed using STATA version 11. A P-value lower than 0.05 was defined as significant. Results: On univariate analysis, age at diagnosis, level of education, type of surgery, lymph node status, tumor size, stage, histologic grade, estrogen receptor, progesterone receptor, and lymphovascular invasion had a statistically significant effect on survival time. On multivariate analysis, lymph node status, stage, histologic grade, and lymphovascular invasion were statistically significant. The one-year overall survival rate was 98%. Conclusions: Based on these data and using Weibull parametric model with a forward approach, we found out that patients with lymphovascular invasion were at 2.13 times greater risk of death due to breast cancer.

Studies on Wax Gourd Wine (다이어트와인 동아주 제조에 관한 연구)

  • Three kinds of the mashes of 15%, 20% and 25% of boiled wax gourd containing 20% of sugar were fermented by yeast at 5~1$0^{\circ}C$ for 100 days. After fermentation, total sugar content of the 15% mash was 8.0%, 20% mash 8.6%, and 25% mash 8.3%, respectively. Also, it revealed that in reducing sugar content, 15% mash was 7.4%, 20% mash 7.6%, and 25% mash 7.4%, respectively, In protein content, 15% mash was 10.3mg/ml, 20% mash 9.8mg/ml, and 25% mash 11.3mg/ml, and in amino acid content, 15% mash was 0.13 $\mu$mo1/ml, 20% mash 0.03 $\mu$mo1/ml, and 25% mash 0.03 $\mu$mo1/ml, each. In case of pH, 15% mash was 3.88, 20% mash 3.99, and 25% mash 3.97, respectively. In acidity, 15% mash was 0.37, 20% mash 0.44. and 25% mash 0.43, respectively. In the number of yeast cell, 15% mash was 8.33 logCFU/ml, 20% mash 8.56 logCFU/ml, and 25% mash 8.57 logCFU/ml. respectively. In ethanol content, 15% mash was 13.4%, 20% mash 14.9%, and 25% mash 15.5%, respectively. In organic acid content, acetic acid, pyruvic acid, citric acid, and lactic acid were produced, and succinic acid content ranges from 1,407~1,800 mg/l. Five-grade scoring test of sensory evaluation showed that in its taste degree, 15% mash was 3.93, 20% mash 3.66 and 25% mash 3.40, respectively.

Overall Survival of Filipino Patients with Squamous Cell Carcinoma of the Head and Neck: A Single-Institution Experience

  • This paper is the first to present the incidence and overall survival of patients with squamous cell carcinoma of the head and neck (SCCHN) from the extreme northern part of the Philippines. We retrospectively retrieved the records of patients with histologically-confirmed squamous cell carcinoma of the oral cavity, oropharynx, hypopharynx and larynx at the Mariano Marcos Memorial Hospital and Medical Center, Ilocos Norte, Philippines, from 2003 to 2012 and analysed prognostic factors associated with survival. Of the 150 cases, only 80 (53.3%) were still living when the study was terminated. Median age at initial diagnosis was 61.5 years and the male to female ratio was 7:3. The majority of the cases had tumours in the oral cavity (50.7%), followed by the larynx (36.7%). Sex (log rank=1.94, p value/${\alpha}$=0.16), tumor site (log rank=0.02, p value/${\alpha}$=0.90), tumor grade (log rank=1.74, p value/${\alpha}$=0.42), and node stage (log rank=0.07, p value/${\alpha}$=0.80) were not shown to be associated with the survival of our cases. Only 45 (30.0%) had no regional lymph node involvement (N0) at presentation and 12 (8.0%) had already developed distant metastases. Among the 150 patients, 71 (47.3%) were not able to receive treatment of any kind. Oddly, treatment (log rank=1.65, p value/${\alpha}$=0.20) was also shown to be not associated with survival. The survival rate of those who underwent surgery, radiotherapy, or both was not statistically different from those who did not receive any treatment. Only the tumor stage (log rank=4.51, p value/${\alpha}$=0.03) was associated with patient survival. The overall mean survival was 49.3 months, with survival rate diminishing from 88.3% during the 1st year to 1.80% by end of the study. This relatively low survival rate of our cases only reflects their poor access to quality diagnostic and treatment facilities.

In ethanol content, 15% mash was 11.4%, 20% mash 12,4%, and 25% mash 11.5%, respectively. Succinic acid content ranges from 1,128~1,385mg/l. In Acetic acid, 20% mash showed 2,806 mg/l and 25% mash 4,952 mg/l by acidification. Five-grade scoring test of sensory evaluation showed that in its taste degree, 15% mash was 3.26, 20% mash 3.00 and 25% mash 2.46, respectively.

Galvanic Anode Charactristics of Grounding Cell Design for Corrosion Protection of Pipings (배관 방식용 접지전지 설계를 위한 유전양극의 특성에 관한 연구)

  • The galvanic anodes have three kinds of Zn alloy anode, Al alloy anode and Mg alloy anode, which are widely used in cathodic protection for all metal structures in water or under ground. This paper to be used for designing of the grounding cell has reached the following conclusion as the results of an experimental study on the characteristics of such galvanic anodes for corrosion protection of pipings: 1) Zn alloy anode was the best when the specific resistance of the environment was bellow 1000 $\Omega$.cm, and when above 1000 $\Omega$.cm, Mg alloy anode to be used for designing of the grounding cell was the best. 2) Al alloy anode was better than Mg alloy anode for grounding cell when the specific resistance was bellow 500 $\Omega$.cm, but the Al alloy anode in all the environments reduced the characteristics of galvanic anode to the lower grade than those of Zn alloy anode. 3) Each impressed voltage (E) of the anodes at which drainage current density ($\rho$) begins rapidly increasing is quantitatively presented as follows: \circled1 E sub(Zn)=log (4.9465/$\rho$super(0.0639))+11$\times$10 super(-6)$\rho$super(0.8923i) \circled2 E sub(Al)=log (4.9306/$\rho$super(0.0525))+13$\times$10 super(-6)$\rho$super(0.9314i) \circled3 E sub(Mg)= log (3.7086/$\rho$super(0.0988))+181$\times$10 super(-6)$\rho$super(0.5406i) 4) The empirical equations between the drainage current density (i) and impressed environment are modeled as the following type. logi=g+root(n.E+r)(g,n,r; constants)

Clinical Features and Treatment Outcome of Chordoid Meningiomas in a Single Institute

  • Objective : Meningioma is the second most common primary central nervous system neoplasm. In contrast, chordoid meningioma is rare; due to the paucity of cases, little is known about its clinical features or treatment outcomes. The objectives of this study were to describe the clinical characteristics and outcomes for patients with chordoid meningioma. Methods : In total, 16 patients, with newly diagnosed chordoid meningioma who underwent surgical excision between 1999 and 2012 were included. We retrospectively evaluated the medical records, radiological findings, and pathological findings. The median follow-up period was 56.5 (range, 3-170) months. The MIB-1 labeling index ranged from 1 to 26.60% (median, 5.04). Results : Simpson grade I, II, and III resections were performed in four, nine, and three patients, respectively. The overall recurrence rate was 37.5%. Overall progression-free survival (PFS) after resection was 94.7 months (95% CI=62.9-126.6). Of the 4 patients with Simpson grade I resection, recurrence occurred in one patient. Among the Simpson grade II and III resection groups, eight patients underwent adjuvant radiation therapy and they showed significantly longer PFS (121 months, 95% CI=82.1-159.9) than the patients who underwent surgery alone (40.5 months, 95% CI=9.6-71.3) by the log-rank test (p<0.05). Conclusion : Chordoid meningiomas are difficult to manage and have a high rate of recurrence. Complete resection of the tumor is a key determinant of better outcomes. Adjuvant radiation therapy is recommended, eparticulary when Simpson grade I resection was not achieved.

Supervised Competitive Learning Neural Network with Flexible Output Layer

  • In this paper, we present a new competitive learning algorithm called Dynamic Competitive Learning (DCL). DCL is a supervised learning method that dynamically generates output neurons and initializes automatically the weight vectors from training patterns. It introduces a new parameter called LOG (Limit of Grade) to decide whether an output neuron is created or not. If the class of at least one among the LOG number of nearest output neurons is the same as the class of the present training pattern, then DCL adjusts the weight vector associated with the output neuron to learn the pattern. If the classes of all the nearest output neurons are different from the class of the training pattern, a new output neuron is created and the given training pattern is used to initialize the weight vector of the created neuron. The proposed method is significantly different from the previous competitive learning algorithms in the point that the selected neuron for learning is not limited only to the winner and the output neurons are dynamically generated during the learning process. In addition, the proposed algorithm has a small number of parameters, which are easy to be determined and applied to real-world problems. Experimental results for pattern recognition of remote sensing data and handwritten numeral data indicate the superiority of DCL in comparison to the conventional competitive learning methods.

Evaluation of Standing Tree Characteristics by Development of the Criteria on Grading Hardwood Quality for Oaks Forests in Central Region of Korea (활엽수 입목형질등급 기준 개발을 통한 중부지역 참나무림의 입목특성 평가)

  • This study was carried out to improve the forest management method considering the use of high value added timber in the natural broadleaf forests. For this purpose, the criteria for evaluating the quality grade of standing trees were established and applied to the oak stand in the central region of Korea. The evaluation factors of the grade were bending of stem, branch, stem damage, and other defects. If the logs are divided into 2.1 m units and three logs up to 6.3 m are available, they are classified as Grade I (G-I). If two logs are available, they are classified as Grade II (G-II), If only one log is available, it is classified as Grade III (G-III). When any log is not available as timber, it is classified as Grade IV (G-IV). As a result of applying the grade to the oak stand, G-I was 6.7 %, G-II was 28.0 %, G-III was 38.3 %, and G-IV was 27.0 %. The ratio of standing trees by oak species of higher than G-III was 88.2 % for Quercus acutissima, 88.1 % for Q. variabilis, 83.5 % for Q. serrata, 56.3 % for Q. aliena, and 50.3 % for Q. mongolica, respectively. The G-IV ratio for Q. variabilis and Q. mongolica tended to decrease with increasing diameter at breast height. The order of major defect affecting the grading level was bending of stem > branch > stem damage > other defects. Considering the grade level and oak species distribution, it was concluded possible to produce high quality hardwood timber when we concentrate forest tending techniques on Q. acutissima and Q. variabilis stand. In order to improve the accuracy of grading, it is necessary to continuous complement through the monitoring research for evaluation factors.

Visual Log Grading and Evaluation of Lamina Yield for Manufacturing Structural Glued Laminated Timber of Pitch Pine (리기다소나무 원목형질 조사 및 구조용집성재 제조 수율 평가)

  • Pitch pine (Pinus rigida) has been planted in Korean forests for several decades, primarily for erosion control and use as a fuel supply. To enhance its value, and especially potential use as lamina for structural glued laminated timber (glulam), log quality and lumber yield of pitch pine were evaluated in this study. Trees from pure pitch pine stands with an average diameter at breast height of 32 cm were felled and bucked into 3.6m long 15 cm minimum butt-end diameter logs. Over 80% of the logs were classified to No.2 or No.3 visual grade group. Upon sawing total lumber yield was 55.2%, 39.9% for structural glulam lamina, 7.2% for louver, and 8.1% for miscellaneous use. The final lumber yield for manufacturing structural glulam, after cross-cutting to eliminate knots and finger jointing, was only 15.3%. To enhance this manufacturing yield requires that the rate of knot-included lumber used as lamina be raised. However arrangement of the knot-included lamina, whose mechanical properties need to be accurately evaluated, must be optimized to minimize any reduction to the structural glulam strength. The log quality and lumber yield of pitch pine evaluated in this study are expected to facilitate proper planning for wood product manufacture in the Korean lumbering and glulam industrial field, which has not previously dealt with this species.

Evaluation of Network Reshuffling Alternatives Based on Key Factors Affecting the Mode Share of Seoul Metro (서울시 도시철도 이용에 영향을 미치는 요소를 반영한 노선 조정 효과 분석)

  • Key factors affecting the mode share of Soul metro are investigated. The log-regression model, which can describe the elasticity of the factors with ease, is established rather than the conventional mode choice model is used. The log-regression model requires lower level of data availability for calibration and identifies the impact of the factors on mode share straightforwardly. As a result, it is found that the main reasons why the current mode share of railway is low are due to several problems such as winding lines, inconvenient transfers and unnecessary bypasses. The calibrated model is adopted to evaluate the network reshuffling alternatives. The network reshuffling is to rearrange the existing inefficient railway lines that have frequent transfers and many winding segments. The proposed network reshuffling, which includes straightening winding lines and changing grade separated transfers into cross-platform transfers, turned out to be a good measure to tackle the problems.
