Pathological Mechanistic Study of Conducting Fire Back to Its Origin (인화귀원(引火歸原)의 병기론 연구)

  • The fire not to back to its origin(火不歸原) is said that source yang(元陽) of sea of qi(氣海) rises because fire(火) of lower energizer(下焦) can't return to its origin. Successive medical men regarded the cause of it as yang deficiency(陽虛) or yin deficiency(陰虛) generally, but Jangseoksun(張錫純) presented eight kinds of cause, they are syndrome of upcast yang(戴陽證), deficiency of qi(氣虛), yin deficiency, yin and yang deficiency(陰陽虛), thoroughfare qi ascending counterflow(衝氣上衝), heart fire(心火), yang deficiency with cold fluid retention(寒飮) in middle energizer(中焦寒飮), yang deficiency with sunken cold locked in(沈寒錮冷). The method of conducting fire back to its origin may be the treatment of fire not to back to its origin as an interpretation of the phrase in a broad sense, but it is limited to yang deficiency with sunken cold locked in besides syndrome of upcast yang as the treatment based on pathological conditions. By this standpoint Eunsuryong(殷壽龍) used conducting fire back to its origin to remove hidden cold(伏寒) and make rising false fire(假火) settle. The meaning of conducting fire back to its origin is not just raise yang qi(陽氣) but break sunken cold locked in by using the drugs like Buja(附子), Yukgye(肉桂). Jakyak(芍藥) can concentrate yang qi on the life gate(命門) by converging it, Sukjihwang(熟地黃) can supply yin essence(陰精) and check the intense nature of tonifing yang(補陽) drugs. So if we want to use the method of conducting fire back to its origin, we should confirm the symptoms of sunken cold locked in and yang deficiency not to misdiagnose yin deficiency.

Patient Improvement Following Acupuncture in the Treatment of Locked-In Syndrome Caused by Bilateral Pontine Infarction - A Case Report (침술치료로 호전된 양측성 뇌교경색으로 인한 감금증후군 환자 1례 - 증례보고)

  • Introduction: This report describes a case of locked-in syndrome (LIS) due to a bilateral pontine infarction. Case Presentation: A patient with LIS showed worsening despite treatment with thrombolytics, anticoagulants, and antiplatelet agents for acute pontine infarction. On the 8th day post-infarction, the patient was referred to the Internal Korean Medicine Department and received 83 days of acupuncture therapy. Treatments were conducted daily, excluding holidays and Sundays, for 15 minutes at various acupoints, including Baihui (GV20), Qubin (GB7), Jianyu (LI15), and Hegu (LI4). Muscle strength was assessed using manual muscle testing (MMT). Post-treatment, improvements were observed: right upper extremity strength increased from grades 0 and 1 to 2 and 4, while right lower extremity strength increased from 0 and 0 to 2 and 3; the results were similar for both left extremities. Conclusion: Acupuncture therapy led to a noticeable improvement in muscle strength in the LIS patient.

Development of Online Speller using Non-contact Blink Detection Glasses (비접촉 눈 깜박임 측정 안경형 디바이스를 이용한 실시간 스펠러의 구현)

  • We proposed blink based online speller for the locked-in syndrome (LIS) patients, paralyzed in nearly all voluntary muscles expect for the eyes, with a simple and easy-to-use eye blink detection glasses. Electrooculogram (EOG) is the golden standard method of eye movement or blink measurement with Ag/AgCl electrodes. However, this method has several drawbacks such as skin irritation and dehydration of conductive gel. To resolve the shortcomings, we used a blink detection system based on a transparent capacitively coupled electrode, which is conductive indium tin oxide (ITO) films. The films make it possible to measure eye blink without direct skin contact and obstruction of field of view. We finally developed user-friendly blink based online speller with the blink detection system. To classify voluntary and non-voluntary blink, we used the double blink for command of the speller. The online speller experiment result with six healthy subjects shows that mean accuracy is 98.96% and letter per minute (LPM) is 4.73, which are better result by comparison with conventional P300 or auditory brain-computer interface (BCI) paradigm. The result of the experiment demonstrates the possibility of applying the proposed system as a communication method for the LIS patients.
