Treatment Experiences of Abdominal Cutaneous Nerve Entrapment Syndrome -A report of 3 cases- (복부 피신경 포착 증후군 환자의 치료경험 -증례보고-)

  • The diagnosis of chronic abdominal pain due to abdominal cutaneous nerve entrapment can be elusive. Tenderness in patients with abdominal pain is naturally assumed to be of either peritoneal or visceral origin. Studies have shown that some patients suffer from prolonged pain in the abdominal wall and are often misdiagnosed, even after unnecessary and expensive diagnostic tests, including potentially dangerous invasive procedures, and treated as having a visceral source for their complaints, even in the presence of negative X-ray findings and atypical symptoms. Abdominal cutaneous nerve entrapment syndrome is rarely diagnosed, which is possibly due to failure to recognize the condition rather than the lack of occurrence. The accepted treatment for abdominal cutaneous nerve entrapment syndrome is a local injection, with infiltration of anesthetic agents coupled with steroids. Careful history taking and physical examination, in conjunction with the use of trigger zone injections, can advocate the diagnosis of abdominal cutaneous nerve entrapment and preclude any unnecessary workup of these patients. Herein, 3 cases of abdominal cutaneous nerve entrapment syndrome, which were successfully treated with local anesthetics and steroid, are reported.

Dynamic Masterplan of the Saemangeum Grain Complex for Progressive Development (점진적 개발 단계를 고려한 새만금 복합곡물단지의 동태적 마스터플랜 수립)

  • The grain complex of Saemangeum is created for promoting the foundation of agriculture combined the global competitiveness. However, the masterplan is being also revised with changing of local conditions and social needs. Thus, the dynamic masterplan is needed to consider the change of time for Saemangeum project. The present study was made to set up the dynamic masterplan of Saemangeum grain complex for handling the change such as project progress, local environment, and project conditions flexibly. In this study, the dynamic masterplan for the progressive development of water supply, farmland composition, and introduction facilities is presented to the 6-2 zone in three stages. We believed that the water supply would be possible through the pumping and desalination facilities with the development stages. The farmland composition proceeded for each complex with reclamation, soil preparation, and soft soil processing. And it is planned to carry out crop cultivation from the complex where the construction is completed first. The introduction facilities were analyzed focusing on the silos and forage loading facilities, and the optimal location of them was selected using road and accessibility. The concept of dynamic masterplan may provide the direction for the planning and progress of reclamation project.

  • The number of malaria cases has been undulating for the past 10 years in Korea since the reemergence of malaria in early 1990's. Considering the spatial variations of malaria incidence across the northmost border areas near the demilitarized zone (DMZ), the occurrence of the disease seems to be influenced by the natural and human environment in the region. Malaria is an infectious disease that is transmitted to humans by the bites of vector-mosquitoes that carry malaria parasites, and its incidence rate depends on specific climatic and sociodemographic factors. This study found that the spatial characteristics of malaria incidence have varied depending on relative proportions of mosquito habitats, distance between mosquito habitats and human residence, the physical and sociodemographic environments of the city by urbanization, and local topography.

  • Fishing, aquaculture, coastal tourism and port activities in southern coastal seas are important to the Korean economy Conventional strategies or quick-fixes may not be the best means of addressing coastal issues. Coastal issues in Korean coastal sea include harmful algal blooms (HAB), oxygen depletion, and sea grass disappearance. Regional coastal management plans have been developed during 2001~present after inauguration of Coastal Management Law in 1999. Activities such as eco-pioneer cities, pollution reduction, constitutional rearrangement, environmental regulation, monitoring indicators, and budgeting would be included in regional coastal management Planning. The successful implementation of integrated coastal management is largely dependent on the engagement of government and non-government organizations to increase the chorus of concern. The need to involve coastal area residents in restoring activities was addressed with the creation of NGOs' Association for Masan Bay Restoration (NAMR). Several restoration efforts by NAMR are currently underway in Masan Bay coastal zone with scientific and local knowledge. A new level of dialogue was achieved suggesting a sustainable picture of Masan Bay coastal area regarding to a new port construction.

Yield and Production Forecasting of Paddy Rice at a Sub-county Scale Resolution by Using Crop Simulation and Weather Interpolation Techniques (기상자료 공간내삽과 작물 생육모의기법에 의한 전국의 읍면 단위 쌀 생산량 예측)

  • Crop status monitoring and yield prediction at higher spatial resolution is a valuable tool in various decision making processes including agricultural policy making by the national and local governments. A prototype crop forecasting system was developed to project the size of rice crop across geographic areas nationwide, based on daily weather pattern. The system consists of crop models and the input data for 1,455 cultivation zone units (the smallest administrative unit of local government in South Korea called "Myun") making up the coterminous South Korea. CERES-rice, a rice crop growth simulation model, was tuned to have genetic characteristics pertinent to domestic cultivars. Daily maximum/minimum temperature, solar radiation, and precipitation surface on 1km by 1km grid spacing were prepared by a spatial interpolation of 63 point observations from the Korea Meteorological Administration network. Spatial mean weather data were derived for each Myun and transformed to the model input format. Soil characteristics and management information at each Myun were available from the Rural Development Administration. The system was applied to the forecasting of national rice production for the recent 3 years (1997 to 1999). The model was run with the past weather data as of September 15 each year, which is about a month earlier than the actual harvest date. Simulated yields of 1,455 Myuns were grouped into 162 counties by acreage-weighted summation to enable the validation, since the official production statistics from the Ministry of Agriculture and Forestry is on the county basis. Forecast yields were less sensitive to the changes in annual climate than the reported yields and there was a relatively weak correlation between the forecast and the reported yields. However, the projected size of rice crop at each county, which was obtained by multiplication of the mean yield with the acreage, was close to the reported production with the $r^2$ values higher than 0.97 in all three years.

An Analysis on Traffic Networks of Local Metropolitan area Based on Express Bus and Car O/D (고속버스, 승용차 O/D를 활용한 지방도시권의 교통네트워크 분석)

  • This study examined flow between regions based on the spatial structure of the territory and characterized the flow patterns by analyzing regional networks via transportation connectivity. To accomplish this, transportation connectivity of the entire nation was examined using 2010 national express bus OD data from the Korea Transport Database. After the initial analysis, 2010 car OD data describing networks in seven regions (125 cities and districts), Gangwon-do, Gyeongsangnam-do, Gyeongsangbuk-do, Jeollanam-do, Jeollabuk-do, Chungcheong-do and Seoul, were analyzed to identify transportation connectivity. The results revealed that Korea has strong triangular-belt-shaped transportation connectivity that connects among metropolitan areas in the Jeolla and Gyeongsang areas. Particular zones are set by regional characteristics and functional connectivity for each zone. The results of this study will be useful as a basic material to establish development strategies and customized regional policy development, as well as balanced development.

Epidemiological Characteristics of Re-emerging Vivax Malaria in the Republic of Korea (1993-2017)

  • Historically, Plasmodium vivax malaria has been one of the most highly endemic parasitic diseases in the Korean Peninsula. Until the 1970s, vivax malaria was rarely directly lethal and was controlled through the Korean Government Program administered by the National Malaria Eradication Service in association with the World Health Organization's Global Malaria Eradication Program. Vivax malaria has re-emerged in 1993 near the Demilitarized Zone between South and North Korea and has since become an endemic infectious disease that now poses a serious public health threat through local transmission in the Republic of Korea. This review presents major lessons learned from past and current malaria research, including epidemiological and biological characteristics of the re-emergent disease, and considers some interesting patterns of diversity. Among other features, this review highlights temporal changes in the genetic makeup of the parasitic population, patient demographic features, and spatial distribution of cases, which all provide insight into the factors contributing to local transmission. The data indicate that vivax malaria in Korea is not expanding exponentially. However, continued surveillance is needed to prevent future resurgence.

Assessment of Zeolite Soil Mixture as Adsorptive Fill Material at Industrial Zones (산업단지에서의 흡착 성토재로써 제올라이트 토양혼합물의 특성평가)

  • A number of industrial zones in South Korea were reported contaminated by heavy metals. Such contamination could cause severe damage to the subsurface environment including soil and groundwater. The treatment of zeolite mixing with soil at the bottom of such industrial zones might prevent, or at least reduce the damage of contamination by adsorption of the heavy metals from the leakage. However, such mixtures should maintain the proper bearing capacity as a foundation fill material from the geotechnical point of view at the same time. To investigate the effect of mixtures of zeolite with local soils for the adsorption of heavy metals (Zn, Pb) and sustainability of bearing capacity, adsorption isotherm tests and direct shear test with compaction tests were performed. Results showed that the mixing zeolite with local soils effectively reduces the spreading of the heavy metal contamination when maintaining its proper geotechnical properties as a fill material of industrial zones.

An In sight into Novel Drug Delivery System: In Situ Gels

  • In situ gelling devices, as they enter the body, are dosage forms in the shape of the sol but turn into gel types under physiological circumstances. Transition from sol to gel is contingent on one or a mixture of diverse stimuli, such as transition of pH control of temperature, irradiation by UV, by the occurrence of certain ions or molecules. Such characteristic features may be commonly employed in drug delivery systems for the production of bioactive molecules for continuous delivery vehicles. The technique of in situ gelling has been shown to be impactful in enhancing the potency of local or systemic drugs supplied by non-parenteral pathways, increasing their period of residence at the absorption site. Formulation efficacy is further improved with the use of mucoadhesive agents or the use of polymers with both in situ gelling properties and the ability to bind with the mucosa/mucus. The most popular and common approach in recent years has provided by the use of polymers with different in situ gelation mechanisms for synergistic action between polymers in the same formulation. In situ gelling medicine systems in recent decades have received considerable interest. Until administration, it is in a sol-zone and is able to form gels in response to various endogenous factors, for e.g elevated temperature, pH changes and ions. Such systems can be used in various ways for local or systemic supply of drugs and successfully also as vehicles for drug-induced nano- and micro-particles. In this review we will discuss about various aspects about use of these in situ gels as novel drug delivery systems.

Analysis of Microcystis Bloom in Daecheong Reservoir using ELCOM-CAEDYM (ELCOM-CAEDYM을 이용한 대청호 Microcystis Bloom 해석)

  • An abnormal mono-specific bloom of the cyanobacterium Microcystis aeruginosa had developed at a specific location (transitional zone, monitoring station of Hoenam) in Daecheong Reservoir from middle of July to early August, 2001. The maximum cell counts during the peak bloom reached 1,477,500 cells/mL, which was more than 6~10 times greater than those at other monitoring sites. The hypothesis of this study is that the timing and location of the algal bloom was highly correlated with the local environmental niche that was controled by physical processes such as hydrodynamic mixing and pollutant transport in the reservoir. A three-dimensional, coupled hydrodynamic and ecological model, ELCOM-CAEDYM, was applied to the period of development and subsequent decline of the bloom. The model was calibrated against observed water temperature profiles and water quality variables for different locations, and applied to reproduce the algal bloom event and justify the limiting factor that controled the Microcystis bloom at R3. The simulation results supported the hypothesis that the phosphorus loading induced from a contaminated tributary during several runoff events are closely related to the rapid growth of Microcystis during the period of bloom. Also the physical environments of the reservoir such as a strong thermal stratification and weak wind velocity conditions provided competitive advantage to Microcystis given its light adaptation capability. The results show how the ELCOM-CAEDYM captures the complex interactions between the hydrodynamic and biogeochemical processes, and the local environmental niche that is preferable for cyanobacterial species growth.
