The principal objective of this study was to analyze and conceptualize the socio-psychological burden in utilizing a nursing home for elderly. The subjects were five elderly from a private nursing home located in Seoul and their familial caregivers. An old male and three females were currently staying at the facility, and a female had been discharged already from there. Data were collected through depth interviews, observations and review of records at the facility For analysis, the data were classified by similar contents among significant expressions and factors in common. The subjects' motives to consider admission to the nursing home might be attributed to familial caregivers' burden, a shortage of support, environmental improvement and feeling of helplessness for the case elderly. The concept of burden is including family members' being badly off in living, their weariness, complications among family members, feeling psychological uneasiness, and hospital expenses. The identified image of nursing homes for the elderly in Korea was generally negative at the point of high cost, unreasonable requisites and limitations for admission to the facilities, inferior situations, and especially in that there were few long-term care facilities within the community boundary. From their experience of nursing homes, the interviewees have felt the sentiments of sorry for their old parents, with the thought of being an undutiful, bitterness, and empathy. Additionally, they expressed a sense of anxiety of relative deprivation against the fact that there were no long-term care facilities available for the middle class. On the basis of these, multi-dimensional needs could be identified for the elderly with chronic illnesses.
With a rapidly aging population, the proportion of elderly households with low income has been increasing. Despite the poor housing environment, it is not easy to improve their housing environment due to the high cost of modification. However, as many elderly want to keep living in their current houses, it is urgent to improve their housing environment. The purpose of this study was to develop the guidelines on home modification for low-income elderly. This study set the scope of home modification categories through literature analysis in advance to develop the guidelines. Based on the literature analysis and small group workshops, the primary and secondary guidelines were derived and a total of 169 final guidelines were produced based on the scope of home modification categories. Those guidelines were composed of the categories by space, divided into mandatory and recommended by details. Those guidelines proposed in this study were classified and composed under the objective standards, so that they were systemic and objective based on the verification of experts. They are considered to get closer to the user's demand on the basis of the demand of low income elderly for home modification and the improvement categories under the system to support home modification for low income elderly at home and abroad. In addition, as the standards to apply each guideline, separated into mandatory and recommended, was suggested, those guidelines may help expand the scope of improvement under the economic conditions for home modification.
This study investigated the field application and the acceptance test of the meal service for the elderly (${\ge}$ 65 years) community halls in Jeonnam and Gyeongbuk. The acceptance scores of bean rice and glutinous rice were the highest among the tested cooked rice varieties in Jeonnam and in Gyeongbuk, respectively (P<0.05). The acceptance scores of tofu soybean paste soup and spinach bean paste soup were the highest among different soups, in Jeonnam and Gyeongbuk, respectively. The scores of menu 5 (bean rice, tofu and bean paste soup, pan-fried pork, green laver salad, kimchi, soy milk) and menu 1 (cereal rice, fermented soy bean soup, hard-boiled quail's egg, crown daisy salad, kimchi, mandarin) were the highest in Jeonnam and Gyeongbuk, respectively, in terms of overall acceptance (P<0.05). The average cost of each meal in two provinces was $2012{\pm}323$ won. The amount of leftovers from menu 5 (Bean rice, Tofu and soybean paste soup, Pan-fried pork, Seasoned green laver, Kimchi, Soy milk) was the lowest in Jeonnamin, while the one from menu 2 (rice, leaf beet and soybean paste soup, sated vegetables with potato noodle, hard-boiled potato, kimchi, cherry tomato) was the lowest in Gyeongbuk.

This study was designed to search for the direction to develop housing service which assists the older people to live independently in their community and provides the systematization of community care. The subjects of the research include 533 older people between the age of 60 to 79 residing in Seoul and 84 welfare officials at a dong(in)-office and 67 social workers working at comprehensive social welfare institutions. The results were as follows: Both demanders and deliverers recognized that housing service for older people was necessary for community care, and, in particular, the deliverers had higher degree of necessity as people charged in welfare service. The most needed service was community alarm service which currently is proceeded partially for low-income and living-alone olders. It needed to expand the scope of the people to be served. It appeared that the need for inside safety measures rather than for housing space or structural problems. It requires less effort and cost for those who need simple repair, such as safety grips or slippery protection, and safety checks. Among financial support services, the need for tax benefits appeared the most necessary service for the older people. It suggested that more 'direct' financial support service is needed for the elderly people.
This study looked into the current conditions in Korean and Japanese care management for the elderly, cases of consumer counseling in these countries, and the supporting institutions for Japanese users regarding the use of care management for the elderly. The number of recipients of care management for the elderly in both Korea and Japan is growing every year, and more Japanese users receive various services compared to Korean users. The results of an analysis on consumer counseling regarding the use of Korean and Japanese care management revealed, two types of complaints: counseling for the improvement of the institution and complaints related to the procurement of service. Regarding the insurance system, the complaints were mostly related to affirmation of a rating and the burden incurred by cost-sharing. Regarding the use of service, such key impairment cases were related to in-service medical accidents, illegal acts including caregiver contract violations and forced retirement, careless service by workers, and human rights violations of the elderly. Japan has developed governmental and, related-administrative and business services as well as a local governmental system to redress customer impairment issues as this pertains to service for better quality care management. In addition, they have enlarged the locally closed service, provided exact information about the services offered, and improved service appraisal techniques. The Japanese cases will serve as a good reference to improve the Korean system. In particular, the construction of a system that reduces customer losses and the availability of more counseling information are urgently required to improve the system in terms of customer experiences when they seek care management.
This study was carried out to assess the utilization of urban health center and its related factors among the urban residents. The survey was carried out for 2,394 households in Taegu and Kyongju by the structured questionnaire from 28 March to 4 April and from 2 July to 9 July, 1994. Well trained interviewers visited 2,630 households in Taegu and Kyongju, and interviewed with housewives. Of the target households, 91.0%(2,394 households) were responded through three-time visiting. The major results were summarized as follows : The fourth and fifth decad utilized the health center more frequently than any other age groups.. The lower income group showed higher rate of health center utilization than those in higher income group in Taegu City. The mean length of residence among residents of Kyongju City is longer than those of Taegu City, and the longer length of residence, the higher rate of the health center utilization. Those who are living together with neonate and infant or elderly people showed higher rate of health center utilization than those who are living without neonate and infant or elderly people in both Taegu and Kyongju. Background: This study is to investigate the association between the distribution of multimorbidity and length of stay and medical expenses among inpatients in a municipal hospital to achieve an integrated care setting. Methods: We used the exploratory factor analysis and the generalized estimating equation model to analyze the data from patients living in the northeast region of Seoul, who were hospitalized from January 2017 to December 2017 in a municipal hospital. Results: As a result of the factor analysis, seven types of multiple chronic diseases were classified. Among the elderly patients admitted to municipal hospitals, the burden of medical expenses was mainly influenced by the length of stay (B=310,719, p-value <0.0001), not the type of disease (all not significant). Length of stay were mainly due to psychiatric illness (factor 1: B=4.323, p-value <0.0001) related to the brain and metabolic diseases (factor 2: B=2.364, p-value=0.003). Conclusion: This study showed that the medical expenses of the elderly patients were largely due to prolonged hospitalization, not multimorbidity. Therefore, it is necessary to develop an integrated care paradigm strategy cope with the multimorbidity of the elderly in the community and to alleviate the socio-economic burden.
