폐동정맥루를 동반한 팔로사징환자의 치험 -1례보고- (Tetralogy of Fallot with Pulmonary Arteriovenous Fistula -A Case Report-)

  • Pulmonary arteriovenous fistula can occur in a variety of clinical situations including liver diseases, infections, metastatic carcinomas, systemic disorders, and after the palliation of congenital heart diseases. A 72-day-old male infant with Tetralogy of Fallot and pulmonary atresia underwent surgical correction without difficulty. However, ventilator weaning in the ICU failed initially because of an unexplained postoperative hypoxemia(FiO2: 0.8, PaO2: 40 mmHg, SaO2: 80∼90%). Postoperative follow-up lung perfusin scan at postoperative 15 days showed right-to-left shunt(33.6%) and ventilator weaning was performed on the 20th day after the operation (FiO2: 0.4, PaO2, 50mmHg, SaO2: 86.9%). Arterial oxygen saturation under room air was 80∼85% at 7 months postoperatively. One and half year follow-up lung perfusion scan showed decreased amount of right-to-left shunt (11.2%). We report a case with a review of the literatures.

Diagnostic imaging features of hepatic myelolipoma incarcerated in a peritoneopericardial diaphragmatic hernia in a cat

  • A 1-year-old male Persian cat was presented for castration. Liver incarcerated in a peritoneopericardial diaphragmatic hernia (PPDH) was diagnosed through pre-anesthetic tests. Multiple homogeneous hyperechoic nodules in the hepatic parenchyma were identified using ultrasound. The nodules showed decreased attenuation compared with normal hepatic parenchyma, and the herniated hepatic parenchyma showed increased arterial and decreased portal enhancement on computed tomography. From the histopathology, we diagnosed hydropic degeneration with portal fibrosis and myelolipoma. This report presents diagnostic imaging features of hepatic myelolipoma incarcerated in a PPDH in a cat. When perfusion of the hepatic parenchyma is altered, surgical treatment should be considered.

Effects of Protein Kinase C Modulation on Hepatic Hemodynamics and Glucoregulation

  • This study evaluated the effects of PKC activation using phorbol 12-myristate 13-acetate (PMA) and PKC inhibition using the isoquinoline sulfomide derivative H-7 on hemodynamics and glucoregulation in the isolated perfused rat liver. Livers were isolated from fed male Holtzman rats and perfused with Krebs Ringer bicarbonate solution under a constant flow of 50 ml/min at $35^{\circ}C.$ Portal vein pressure, glucose and lactate concentrations in the medium and oxygen consumption rates were continuously monitored by a Grass polygraph, YSI glucose and lactate monitors, and a YSI oxygen monitor, respectively. PMA at concentration of 2 to 200 nM increased the portal vein pressure, glucose and lactate production, but decreased oxygen consumption rate in a dose-dependent fashion. H-7 $(200\;{\mu}M)$ attenuated PMA (50 nM)-induced vasoconstriction $(15.1{\pm}1.36\;vs\;10.56{\pm}1.17\;mmHg),$ glucose production rate $(91.3{\pm}6.15\;vs\;71.8{\pm}2.50\;{\mu}moles/g/hr),$ lactate production rate $(72.4{\pm}6.82\;vs\;53.6{\pm}4.82\;{\mu}moles/g/hr)$ and oxygen consumption rate $(33.7{\pm}1.41\;vs\;27.9{\pm}1.75\;{\mu}l/g/min).$ The effects of PMA were blocked either by addition of verapamil $(9\;{\mu}M)$ or perfusion with $Ca^{2+}-free$ KRB. These results suggest that the hemodynamic and glucoregulatory changes in the perfused rat liver are mediated by protein kinase C activation and require $Ca^{2+}$ influx from the extracellular fluid.

A clinical Comparison of Lobaplatin or Cisplatin with Mitomycine and Vincristine in Treating Patients with Cervical Squamous Carcinoma

  • Background: The research was to compare the efficacy and side effects of cisplatin or lobaplatin in combination with mitomycine (MMC) and vincristine in treating patients with cervical squamous carcinoma. Materials and Methods: Cervical squamous carcinoma patients who were pathologically diagnosed with stage Ib-IIb from April 2012 to May 2013 in the general hospital of Chinese People's Libration Amy were enrolled. All patients were confirmed without prior treatment and were randomly divided into two groups, Group A and B. Efficacy and side effects were evaluated after one cycle of chemotherapy. Results: Group A (n=42) were treated with Loubo$^{(R)}$ (Lobaplatin) $50mg/m^2$, MMC $16mg/m^2$ and Vincristine $2mg/m^2$ every 21 days. Group B (n=44) were treated with Cisplatin $100mg/m^2$, MMC $16mg/m^2$ and Vincristine $2mg/m^2$ every 21 days. All 86 patients completed one cycle of chemotherapy with cisplatin or lobaplatin in combination with MMC and vincristine. No difference was observed regardiing short-term effect between two groups. Main side effects were bone marrow suppression and gastrointestinal reactions including decrease of white blood cells, platelet and nausea/vomiting. Grade III-VI liver and kidney impairment was not reported in two groups. In group A the incidence of uterine artery spasm in the process of drug delivery was significantly lower than the group B. Conclusions: Cisplatin or lobaplatin with MMC and Vincristine in the interventional treatment of cervical squamous carcinoma were effective, especially after uterine artery perfusion chemotherapy at tumor reduction and tumor downstaging period. The adverse reactions of concurrent chemotherapy are tolerable, and low physical and mental pressure even more less stimulation of vascular in treatment with lobaplatin. However, the long-term effects of this treatment need further observation.

홍삼 성분의 혈당강하작용 연구 (ll) :쥐의 배양 간세포의 당대사 관련 효소 활성에 미치는 홍삼 지용성 분획의 영향 조사 (Hypoglycemic Action of Red Ginseng Components (II). Investigation of the Effect of Fat Soluble Fraction from Red Ginseng on Enzymes Related to Glucose Metabolism in Cultured Rat Hapatocytes)

  • In this study, rat hepatocytes known to have active glucose metabolism were obtained to investigate the hypoglycemic action of fat soluble fraction of red ginseng by using the liver perfusion technique and incubated in two different media-one containing insulin and glucagon (control group), and the other containing glucagon only The activities of main regulating enzymes, such as glucokinase, glucose 6-phosphate dehydrogenase, 6-phosphogluconate dehydrogenate, and glucose 6-phosphatase, related to metabolic pathways of glucose in these two kinds of hepatocytes were compared between these two groups and the effects of addition of fat soluble fraction ($10^1$~$10^4$%) from red ginseng to these two groups on these enzymes were also detected. The results were as follows. The specific activity of enzymes such as glucokinase, flucorse 6-phosphate dehydrogenase, and 6-phosphogluconate dehydrogenase related to glucose-consuming pathways of insulin-deficient group was much less than control one. However, their decreased activity was recovered after the addition of fat-soluble fraction at all range of concentrations. The specific activity of these enzymes after the addition of ginseng components to the control group was also increased. On the other hand, the specific activity of glucose 6-phosphatase related to glucose-producing pathway of insulin-deficient group was much higher than control one, but their increased activity was decreased obviously after the addition of fat soluble fraction at all range of concentrations. The same results were observed after the addition of fat-soluble fraction to the control group. These results suggest that the red ginseng saponin components might be effective on diabetic hyperglycemia by regulating the activity of enzymes related to glucose metabolism directly and/or indirectly. The effects of fat-soluble fraction ($10^2$%) and ginsenosides (mixture, $Rb_1$ and $Rg_1$, $10^4$%) on hypoglycemic action were compared. As a result, they showed considerable effect on hyperglycemia, but the best eff ect on the activities of glucokinase and glucose 6-phosphate dehydrogenase was appeared by ginsenoside $Rb_1$ and that of 6-phosphogluconate dehydrogenase and glucose 6-phosphatase was by ginsenoside mixture.

담즙분비촉진 식물추출물을 이용한 심근관류영상 검사법 개선 (A Modified Protocol for Myocardial Perfusion SPECT Using Natural Plant Extracts for Enhancement of Biliorg Excretion)

  • 목적: 심근관류 영상은 관상동맥 질환을 진단하고 환자의 예후를 평가하는데 널리 사용되고 있다. $^{99m}Tc$ 제재를 이용한 심근관류 영상은 간 방사능을 줄이고 좋은 영상을 얻기 위해 주사 후 일정시간을 대기하고, 지방식을 섭취하는 검사방법을 사용하고있다. 이번 연구에서는 식물 추출물 혼합제재를 이용하여 간 방사능의 배출을 촉진함으로써 검사시간을 단축시키고, 심근영상의 질을 향상시킬 수 있는지 알아보고자 하였다. 대상 및 방법: 프로토콜 1은 식물추출물의 환약제재(이하 혼합제재)를 투여한 개선법이 간 방사능의 배출을 촉진하고 영상의 질을 향상시키는 지 알아보기 위해 건강하고 젊은 자원자 10명(남: 녀=7: 3, 평균 연령 $24.1{\pm}2.4$세)을 대상으로 혼합제재를 복용하고 15분 후 MIBI를 주사하였으며, 주사 후 30분, 60분, 90분, 그리고 120분째 영상을 얻어 분석하였다. 프로토콜 2는 $^{99m}Tc$ 심근관류 영상법에서 널리 이용되고 있는 1일 휴식-부하방법에 혼합제재를 사용하였을 때의 개선법의 효과를 알아보기 위해 다른 자원자 10명(남: 녀=8: 3, 평균 연령 $26.1{\pm}2.9$세)을 대상으로 개선법과 고식적 방법(우유와 아이스크림 등 지방식 투여)을 적용한 영상을 얻어 분석하였다. 결과: 프로토콜 1에서 개선법과 고식적 방법의 심근/폐 방사능 비는 90분 영상까지 유의한 차이를 보이지 않았으며, 간/폐 방사능 비는 120분 영상까지 개선법이 고식적 방법에 비해 유의하게 낮은 값을 보였다. 심근/간 방사능 비는 개선법에서 지속적으로 높은 값을 보였다. 프로토콜 2에서 첫 번째 MIBI를 주사한 후 심근/페 방사능은 유의한 차이가 없었으나 심근/간 방사능비는 45분과 60분 영상에서 개선법이 유의하게 높은값을 보였다. 두 번째 MIBI를 주사한 후에는 20분 영상부터 지속적으로 개선법의 심근/간 방사능 비가 높은 값을 보였으나, 첫 번째 주사와 마찬가지로 심근/폐 방사능 비는 차이가 없었다. 결론: 담즙 배출을 촉진하는 천연식물 추출 혼합제재를 심근관류 영상을 획득하는데 적용하면 지방식 섭취없이 검사 소요 시간을 2시간대로 단축시킬 수 있어 환자의 불편을 최소화시키면서 영상의 질을 향상시킨 수 있을 것으로 판단된다.

적출간 관류법을 이용한 카드뮴과 니켈의 간독성 비교 (Comparative Hepatotoxicity Assessment of Cadmium and Nickel with Isolated Perfused Rat Liver(IPRL))

  • 목적 : 이 연구의 목적은 적출간 관류법을 통하여 염화니켈과 염화카드뮴의 간 독성을 비교하는 데에 두었다. 방법 : AST(aspartate aminotransferase ferase), ALT(alanine aminotransferase), LDH(lactate dehydrogenase) 등의 간기능을 나타내는 생화학적 지표와 관류속도를 간독성 지표로 이용하였으며 산소 소모율은 생존력을 나타내는 지표로 이용하였다. $300({\pm}50)g$ 정도의 흰쥐를 대조군과 각 노출군($50{\mu}M$$200{\mu}M$의 염화니켈과 염화카드뮴)에 5마리씩 총 25마리를 무작위로 할당하였다. Krebs-Ringer bicarbonate buffer solution을 관류액 용도로 제조하여 간문맥에 투입하고 간세포를 통과하여 vena cava로 배출되는 관류액을 일정한 시간 간격을 두고 계속적으로 채취하였으며 채취한 관류액은 간독성 지표를 측정하는 데에 이용하였다. 결과 : AST, ALT, LDH 모두 염하니켈 보다는 염화카드뮴 투여군에서 시간의 경과에 따라 큰 폭으로 증가하였으며 반복 측정된 2요인 분산분석을 실시한 결과, 통계적으로 유의한 차이를 나타냈다. 생존력은 두 물질 모두에서 시간의 경과에 따라 감소하는 경향을 나타냈다. 결론 : 결론적으로, 이 실험상에서는 염화니켈 보다 염화카드뮴의 간독성이 더욱 강한 것으로 드러났다. 적출간 더욱 강한 것으로 드러났다. 적출간 관류법의 특성상 급성 간독성 평가에서 적절한 기법으로 보이며 국내 연구에서도 이 방법이 널리 이용되기를 기대한다.

복강내 확산성 물질의 부위별 흡수속도 (Regional Differences of Entry Rate of Freely Diffusible Substances from Peritoneal Cavity)

  • The entry of antipyrine and urea from the peritoneal cavity of rabbit into organ tissue and blood plasma was studied. Two hundred mg of antipyrine plus 300 mg of urea in 10 ml Ringer's solution was injected into the peritoneal cavity of anesthetized rabbit. The injection was made from above of a rabbit kept tying right side down and it enabled part of the abdominal organs (liver, intestine, kidney) was immersed in the injected solution and kept high concentration gradient throughout the experimental period. The remaining part of the organs was revered only by a thin film of the test solution. Subsequently, in this part of the organs the concentration gradient of the diffusible substances during entry was presumed to decrease as time elapsed. Four pieces of the liver tissue were taken namely, the right superficial, right deep, left superficial and left deep portions. Two were taken from the small intestine, one from the portion which was immersed in. the fluid and the other from that above the fluid mass. Both kidneys were separately analyzed. As a remote organ the gastrocnemius muscle was taken from the right leg of the animal. The intervals which were the time periods elapsed after injections were 5,7,10,15 or 30 minutes. At each point 5 animals were sacrificed and the concentrations of the test substances in the tissue water were measured. The results obtained were as follows. 1. In the liver the right portion which was immersed in the fluid showed higher concentration if the test substances than the left portion and the superficial region exceeded the deep region. The concentrations diminished as the time elapsed after infusion, particulary in the case of antipyrine, suggesting circulatory removal of the substances. In urea such decreasing tendency of the concentration was not obvious, and suggested slower removal rate of it as compared with that of antipyrine. 2. In the small intestine there was no regional difference in the concentration of the test substances. Because of the intestinal motility different portions of the intestine were seemed to have bathed in the fluid of the same concentration. In general the concentrations in the intestinal wall exceeded those of the liver, suggesting a slower removal rate than in the latter. 3. In the kidney the accumulation of the endogenous urea was predominant, and the accumulating mechanism in the renal tissue went on during the period of the experiment. Therefore it revealed increasing tendencies as the time elapsed. The penetration of the test substances in this organ from the peritoneal cavity seemed to be slower than in other abdominal organs, namely liver or small intestine. Part of the test substances in the kidney were obviously brought by the blood stream. 4. Rapid exponential decay of the concentration of antipyrine and of the osmolality of the peritoneal fluid was attributed to the extensive removal through the whole dimension of the peritoneal surface, and the remote organ such as the gastrocnemius muscle attained a fairly close value to that of the abdominal organs in less than 30 minutes. The factors which related to the absorption rate were discussed. They were the concentration gradient, permeability and the regional perfusion rate.

음이온계 약물의 간수송과정에 있어서 대향수송의 약물동력학적 모델링 및 시뮬레이션 (Pharmacokinetic Modelling and Simulation of the Counter-transport in the Hepatic Transport of Organic Anions)

  • The purpose of the present study was to kinetically investigate the carrier-mediated uptake in the hepatic transport of organic anions, and to simulate the 'in vivo counter-transport' phenomena, using kinetic model which was developed in this study. The condition that the mobility of carrier-ligand complex is greater than that of free carrier is not essential for the occurrence of 'counter-transport' phenomenon. To examine the inhibitory effects on the initial uptake of organic anions by the liver, it is necessary to judge whether the true counter-transport mechanism (trans-stimulation) is working or not. Effects of bromophenol blue (BPB) or bromosulfophthalein (BSP) on the plasma disappearance curves of a 1-anilino-8-naphthalene sulfonate (ANS) were then kinetically analyzed based on a flow model, in which the ligand is eliminated only from the peripheral compartment (liver compartment). Moreover, 'in vivo counter-transport' phenomena were simulated based on the perfusion model which incorporated the carrier-mediated transport and the saturable intracellular binding. The 'in vivo counter-transport' phenomena in the hepatic transport of a organic anions were well demonstrated by incorporating the carrier-mediated process. However, the 'in vivo counter-transport' phenomena may be also explained by the enhancement of back diffusion due to the displacement of intracellular binding. In conclusion, one should be more cautious in interpreting data obtained from so-called 'in vivo counter-transport' experiments.

청색증으로 내원한 간폐증후군 1예 (A Case of Hepatopumonary Syndrome with Cyanosis)

  • 저자들은 심한 청색증을 동반한 만성 간질환에서, 폐 내 동정맥 단락과 동맥혈 저산 소혈증을 보인 간폐증 후군 1예를 경험하였기에 문헌고찰과 함께 보고한다. 단순흉부 X-선 사진에는 망상결절이 하엽 기저부에 주로 분포하였고 고해상 전산화 단층영상에서 확장된 폐혈관이 늑막까지 연장되어 보이고, 특히 비정상적으로 증가된 폐혈관종말지의 확장이 늑막하 폐에 분포하였다. 핵의학 관류검사 및 조영 심초음파 검사를 통해 폐내 단락을 진단하여 보고하는 바이다.
