Role of Kupffer Cells in the Vasoregulatory Gene Expression during Hepatic Ischemia/Reperfusion

  • Hepatic microcirculatory failure is a major component of reperfusion injury in the liver. Recent data provided some evidence that endothelium-derived vasoconstrictors and vasodilators may be functionally important to the control of the total hepatic blood flow under these conditions of circulatory failure. Since Kupffer cells provide signals that regulate the hepatic response in ischemia/reperfusion (I/R), the aim of this study was to investigate the role of Kupffer cells in the I/R-induced imbalance of vasoregulatory gene expression. Rats were subjected to 60 min hepatic ischemia, followed by 5 h of reperfusion. The Kupffer cells were inactivated by gadolinium chloride ($GdCl_3$, 7.5 mg/kg body weight, intravenously) 1 day prior to ischemia. Liver samples were obtained 5 hrs after reperfusion for RT-PCR analysis of the mRNA for genes of interest: endothelin-1 (ET-1), its receptors $ET_A and ET_B$, endothelial nitric oxide synthase (eNOS), inducible nitric oxide synthase (iNOS) and heme oxygenase-1 (HO-1). ET-1 mRNA expression was increased by I/R. mRNA levels for $ET_A$ receptors showed no change, whereas $ET_B$ receptor transcripts increased in the I/R group. The increases in ET-1 and $ET_B$ mRNA were not prevented by the $GdCI_3$ pretreatment. The mRNA levels for iNOS and eNOS significantly increased within the I/R group with no significant difference between the I/R group and the $GdCl_3$-treated I/R group. HO-1 mRNA expression significantly increased in the I/R group and this increase was attenuated by $GdCI_3$. In conclusion, we have demonstrated that an imbalance in hepatic vasoregulatory gene expression occurs during I/R. Our findings suggest that the activation of Kupffer cells is not required for I/R-induced hepatic microvascular dysfunction.

Mild stimulation of stomach 36 acupuncture point by organic nanoscale SP6 patch improves cellular physiologic functional status of different organs

  • Acupuncture works by stimulating peripheral sensory nerves and their endings causing an increase in cutaneous blood flow and microcirculation, as well as release of neurotransmitters, neuropeptides, and hormones. SP6 Patch is a nanoscale nontransdermal device that mildly stimulates Stomach 36 (zusanli) and other acupuncture points. As stimulation of these points has been indicated to have an effect on hypothalamic function, it is of great research interest to investigate the effect of SP6 Patch on the physiology of organs that are affected by hypothalamic regulation. Bioelectrical tissue impedance data indicative of cellular physiologic organ function, using an Electro Interstitial Scan (EIS) system, were acquired from hypothalamus, pancreas, liver, intestines, kidneys, thyroid and adrenal glands in 10 (1 male, 9 females) volunteers while wearing the SP6 Patch daily for 1 week. EIS testing was performed at baseline with no patch, 30 min after wearing the patch, and after wearing the patch 12 h/day for 1 week. Subjects were instructed to keep well hydrated during the study period. All subjects served as their own control. The hypothesis was: The SP6 Patch worn 12 h/day on the Stomach 36 acupuncture point for 1 week, may significantly improve cellular physiologic functional status of different organs measured by EIS. All tested organs achieved significant improvement in their functional physiologic status after wearing the SP6 Pach 12 h/day for 1 week compared to baseline with an overall average statistical power > 89%. Based upon these results the hypothesis was accepted as true.

간암에서 색전술의 효과를 평가하는데 있어서 $^{99m}Tc$-MAA SPECT를 이용한 간동맥 혈류 검사의 의의 (Evaluation of Embolization Effect of Hepatocellular Carcinoma by Hepatic Arterial Flow Study with $^{99m}Tc$-MAA SPECT)

  • 본 저자들은 $^{99m}Tc$-MAA를 이용하여 정상 간조직에 대한 간암의 상대적인 미세 혈류량을 조사하고 간암 환자에서 간동맥 색전술을 시행 후 상대적인 혈류량의 변화를 검사함으로써 간동맥 색전술의 효과를 알아보고자 하였다. 여덟명의 간암 환자들을 대상으로 하였는데, 이 중 다섯 명은 조직 생검으로, 그리고 나머지 세 명은 임상적, 그리고 방사선학적으로 진단되었다. 3내지 4mCi의 $^{99m}Tc$-MAA를 2cc의 생리식염수에 혼합하여 간동맥내에 위치된 카테터(catheter)를 통하여 1분간에 걸쳐 서서히 주입한 후 바로 동적영상을 4초 간격으로 1분 동안 얻었으며, 이어서 정적 영상과 SPECT를 시행하였다. 간동맥 혈관 조영술만을 시행한 세명의 환자에서 $^{99m}Tc$-MAA를 주입하자마자 주변 간 조직에서 비해 간암내에 강한 방사능 섭취를 보였으며, 주변 간조직에 대한 간암의 방사능 섭취율은 6.5 이상이었고, 그 범위는 6.5에서 19였으며 평균은 12.5였다. 간동맥 색전술을 시행한 5명 중 4명에서 주변 간조직에 대한 간암의 방사능 섭취율이 심하게 감소하였다($0.5{\sim}1.3$). 또한 색전된 부위는 색전술 후 시행한 추적 간동맥 혈관 촬영상에서 보다 동위원소 검사에서 잘 관찰되었다. 간동맥혈관 촬영상에서 완전히 색전된 것으로 생각된 한 환자에서 동위원소 검사상 간암에 강한 방사능 섭취를 보였다(주변 간조직에 대한 간암의 방사능 섭취율 7.0). 따라서 본 저자들은 $^{99m}Tc$-MAA를 이용한 간 동맥 혈류 검사는 간암에서 색전술의 효과를 정확히 평가할 수 있는 유용한 검사법으로 이용될 수 있으리라 생각한다.
