Childhood Obesity Patients treated by Korean Medicine Treatment: A Case Series (한의 치료로 호전된 소아비만 환자 9례에 대한 임상증례군 보고)

  • Objectives This study was written to report Korean medicine treatment for obese children. Methods This study included obese children who visited the Department of Korean Pediatrics at the Daejeon Korean Medicine Hospital of Daejeon University from September 18, 2020 to March 30, 2023. Height and weight at the first visit and on the day of the last herbal medicine termination were measured, and changes before and after treatment were observed. Liver function tests were performed before and after the treatment. Results Of the nine obese children who received Korean medicine treatment, five were male (55.6%) and four, female (44.4%). In term of age, there were five 8-year-olds (55.6%), two 6-year-olds, and two 10-year-olds (22.2%); the patients' mean age was 8.00 ± 1.414 years. The treatment period was more than three months, and the average treatment period was 177.11 ± 106.013 days. After treatment with Korean medicine, the patients' heights increased, and the body mass index (BMI) percentile and obesity decreased. Liver levels were maintained within the normal range without abnormal liver function after herbal medicine treatment, and no adverse reactions were observed during Korean medicine treatment. As a result of the pattern identification analysis, gastrointestinal damp-heat type and yin deficiency with internal heat were reported at a high frequency. Conclusions Korean medical treatment can reduce the degree of obesity without hindering the growth of childhood obesity.

Distinctive Features of Hepatic Steatosis in Children: Is It Primary or Secondary to Inborn Errors of Metabolism?

  • Purpose: The incidence of hepatic steatosis among children has been increasing; however, data distinguishing simple steatosis from a more complex disorder are lacking. Methods: This study identified the etiologies resulting in hepatic steatosis through a retrospective review of pediatric liver biopsies performed in the last 10 years. A total of 158 patients with hepatic steatosis proven by histopathological evaluation were enrolled in the study, and baseline demographic features, anthropometric measurements, physical examination findings, laboratory data, ultrasonographic findings, and liver histopathologies were noted. Results: The two most common diagnoses were inborn errors of metabolism (IEM) (52.5%) and nonalcoholic fatty liver disease/steatohepatitis (NAFLD/NASH) (29.7%). The three most common diseases in the IEM group were glycogen storage disorders, Wilson's disease, and mitochondrial disease. The rates of consanguineous marriage (75.6%; odds ratio [OR], 26.040) and positive family history (26.5%; OR, 8.115) were significantly higher (p=0.002, p<0.001, respectively) in the IEM group than those in the NAFLD/NASH group. Younger age (p=0.001), normal anthropometric measurements (p=0.03), increased aspartate aminotransferase levels (p<0.001), triglyceride levels (p=0.001), and cholestatic biochemical parameters with disrupted liver function tests, as well as severe liver destruction of hepatic architecture, cholestasis, fibrosis, and nodule formation, were also common in the IEM group. Conclusion: Parents with consanguinity and positive family history, together with clinical and biochemical findings, may provide a high index of suspicion for IEM to distinguish primary steatosis from the consequence of a more complex disorder.

  • Objectives : The purpose of this study is to report a case of Rhabdomyolysis that occurred after shock related to overworking, presenting myalgia(especially in the neck), muscular weakness, and dark urine. Methods : A patient diagnosed with Rhabdomyolysis received Korean medical treatment from November $17_{th}$ to $24_{th}$. Clinical improvement was evaluated using a numerical rating scale(NRS), neck disability index(NDI), and laboratory tests which included complete blood count, kidney function, liver function, serum eletrolytes and Creatinine kinase(CK). Results : After treatment, myalgia, muscular weakness and dark urine each improved. Laboratory results, including CK, had decreased to within normal range. Conclusions : Korean medical treatment could be effective for Rhabdomyolysis patients. Further extensive studies should be carried out.

  • In this double-blind, placebo-controlled trial, we assessed the efficacy and safety of beef tallow extract (BTE) including Cis-9-cetylmyristoleate in patients with arthritis. Between May and December 2003, we selected 80 patients (n=80) who showed/manifested arthritic symptoms and whose radiological findings were suggestive of arthritis, and randomly assigned them to placebo-controlled (n=40) and treatment group (n=40). The placebo (corn starch 350 mg) and BTE (208 mg) were orally administered to placebo-controlled and treatment group three times a day, respectively. We assessed the efficacy and safety based on the visual analogue scale (V AS) and modified knee society knee scores (MKSKS) at baseline and endpoint, respectively. To assess the safety, we monitored the adverse effects noted in liver, kidney, cardiovascular and gastrointestinal system for 3 weeks. Then, we performed not only a questionnaire study but also laboratory tests (e.g., liver function test, kidney function test, urinalysis, electrocardiography [EKG], complete blood cell counts [CBC] and chest X-ray). For statistical analysis, Student (-test and paired (-test were done using SPSS■ Version 11.0. Statistical significance was set at p < 0.05. The scores between V AS and MKSKS showed statistical significance (p < 0.05) with an improvement of $69.2\%$ (27/39) and $3.8\%$ (21/39) of treatment-group patients, respectively. Abnormal laboratory findings were noted in neither placebo-controlled nor treatment group. In conclusion, our results indicate that the administration of BTE was a safe and effective treatment regimen for patients with arthritis. In addition, the efficacy of BTE was more remarkable in alleviating the symptoms rather than improving the function.

Study of Efficacy and Safety of Ginseng Seed Oil in Heathy Subjects Who Have Mild Liver Dysfunction : A Randomized, Double Blinded, Placebo-Controlled Study (경증의 간 기능 이상 소견을 보이는 건강한 성인에 대한 인삼종자오일의 유효성 및 안전성 연구 : 무작위배정, 이중눈가림, 위약대조 연구)

  • Objectives: The purpose of this study was to investigate the effectiveness and safety of ginseng seed oil in healthy subjects who have mild liver dysfunction. Methods: A randomized, double blinded, placebo-controlled trial was conducted. A total of 167 subjects visited Semyung University Hospital from July 1st, 2016 to June 10th 2017. Except for the 103 excluded subjects, 64 subjects were randomized into one of the two groups: an treatment group(n=33) and control group(n=31). Subjects were randomly given either ginseng oil seed capsules or indistinguishable placebo capsules(2 capsules per dose, twice per day). Laboratory tests(aspartate aminotransferase, alanine aminotransferase, gamma glutamyl transpeptidase, triglyceride, total cholesterol, high density lipoprotein-cholesterol, low density lipoprotein-cholesterol) were performed to evaluate the effectiveness after 6, 12 weeks of treatment. Vital sign, laboratory test were performed to assess safety at every visit. Results: There were no significant differences in efficacy between treatment group and control group. There were some adverse events with no significant difference in symptoms and frequency between treatment group and control group. Conclusions: Although the efficacy of ginseng seed oil was not proved, ginseng seed oil did not worsen liver function and proved its safety. More study of ginseng seed oil and clinical trials are necessary to increase the usefulness of above-ground parts of ginseng.

  • Halothane is usually a safe and effective inhalation anesthetic agent but it rarely has damaged liver. The authors selected 11 patients who had HBsAg positive and increased SGOT, SGPT at Yeungnam university hospital. Their physical status was ASA class 1 and 2. They had no previous history of operation or liver disease. The liver function tests were performed before surgery, and on 3rd, 7th and 10th postoperative days. The result were as follows : 1) The values of SGOT and SGPT were gradually increased on 3rd postoperative day and markedly increased on the 7th and 10th postoperative day 2) Alkaline phosphatase, total protein, albumin, total bilirubin and direct bilirubin were not significantly changed.

Evaluation of the Liver Cancer Diagnosis Function of PET-MRI Based on Decision Matrix Analysis (판정행렬분석을 통한 PET-MRI의 간암 진단성능 평가)

  • To evaluate the capability of integrated PET-MRI, which has recently been utilized in the clinical practices, on the diagnosis of liver cancer, its utility was assessed by $2{\times}2$ decision matrix. The numbers of abnormal and normal decisions on the liver cancer were 98 and 51 cases, respectively, upon PET-MRI scan results of the subjects, and the numbers of positive and negative decisions were 103 and 62, respectively, upon cytopathologic results. Out of the two tests, 95 cases were shown as true-positive and 3 were false positive, while 62 were true negative and 5 were false negative. Upon the results of PET-MRI test, its sensitivity, specificity, false negative rate, and false positive rate were 95.00%, 95.38%, 0.05%, and 95.15%, respectively. Therefore, it is considered to have the high potential to use the determination of the stage before the surgery, detections of recurrence and remote metastasis, assessment of uncertain remote lymph node metastasis, and so on in the diagnosis of the liver cancer, and also for the clinical utility of PET-MRI to be sufficient by integrated diagnosis and follow up scan with pathological studies.

Anti-inflammatory and antifatigue effect of Korean Red Ginseng in patients with nonalcoholic fatty liver disease

  • Background: Korean Red Ginseng (KRG) is a well-known natural product with anticarcinogenic and antioxidant effects. We evaluated the antifatigue effect of KRG in patients with nonalcoholic fatty liver disease (NAFLD). Methods: Eighty patients with NAFLD were prospectively randomized to receive 3 wk of KRG or placebo in addition to counseling on healthy eating and regular exercise. Liver function test, proinflammatory cytokines, adiponectin, antioxidant activity, and fatigue score were measured and compared according to the body mass index between the KRG and placebo groups. Results: The liver function tests were significantly improved after 3 wk of treatment in both groups. The mean levels (at baseline and after treatment) of tumor necrosis factor-${\alpha}$ were $108.0pg/mL{\pm}54.8pg/mL$ and $92.7pg/mL{\pm}39.0pg/mL$ (p = 0.018) in the KRG group and $123.1pg/mL{\pm}42.1pg/mL$ and $127.5pg/mL{\pm}62.2pg/mL$ (p = 0.694) in the placebo group, respectively. There was a significant difference in change of adiponectin levels between the KRG ($7,751.2pg/mL{\pm}3,108.1pg/mL$ and $8,197.3pg/mL{\pm}2,714.5pg/mL$) and placebo groups ($7,711.6pg/mL{\pm}3,041.3pg/mL$ and $7,286.1pg/mL{\pm}5,188.7pg/mL$, p = 0.027). In patients with overweight, the fatigue score was significantly decreased in the KRG group ($35.0{\pm}13.2$ and $24.5{\pm}8.9$, p = 0.019). Conclusion: Our results show that KRG might be effective in reducing proinflammatory cytokine and fatigue in overweight patients with NAFLD, in addition to improvements in adiponectin levels.

Pancytopenia Improves with Compromised Liver Function in A Hepatocellular Carcinoma Patient Treated by Modified Sipjeondaebo-tang (십전대보탕가감방을 투여한 간세포암종 환자에서 범혈구감소증 호전 및 간수치 상승 증례)

  • Objectives : To observe the therapeutic effects of an oriental herbal prescription in a hepatocellular carcinoma patient with splenomegaly and pancytopenia. Methods : Modified Sipjeondaebo-tang was prescribed three times a day to a hepatocellular carcinoma patient with splenomegaly and pancytopenia. Laboratory tests were carried out regularly to observe the therapeutic effects of the oriental herbal prescription for pancytopenia. Results : When treated with modified Sipjeondaebo-tang including Cinnamomi Cortex, the levels of white blood cells, red blood cells, hemoglobin, and hematocrit increased. However, aspartate transaminase, alanine transaminase, alkaline phosphatase, gamma-glutamic transpeptidase, total bilirubin, direct bilirubin levels also increased. Conclusions : Administering modified Sipjeondaebo-tang with Cinnamomi Cortex showed effect in improving pancytopenia but an increase in liver enzyme levels was also observed.

Clinical Report about the Efficacy of Herbal Medicine in Comparison with Western Medication Mixed for an Acute Liver Injured CVA Patient (급성 간손상을 동반한 CVA환자에서 한약, 양약 복합투여 시와 한약 단독 투여 시의 간기능 개선 효과에 관한 증례 보고)

  • This patient has been acute hepatic injured three times(2005.11.14-11.30, 2006.2.25-3.8, 2006.3.21-4.10) while he hadn't taken herb medicine or anything special. During these periods, his liver function tests showed a sharp elevation of AST, ALT and ALP level. The 1st period by western medication and herb medicine mixed and the 2nd, the 3rd by herb medicine only, he'd been treated. Each of it got significant results in short haul. It maybe means that herb medicine is not dangerous even for the acute hepatic injured person, moreover it can be more effective in comparison with western treatment.
