Very large haematoma following the nonoperative management of a blunt splenic injury in a patient with preexisting liver cirrhosis: a case report

  • The spleen is the most commonly injured organ after blunt abdominal trauma. Nonoperative management (NOM) is the standard treatment for blunt splenic injuries in haemodynamically stable patients without peritonitis. Complications of NOM include rebleeding, new pseudoaneurysm formation, splenic abscess, and symptomatic splenic infarction. These complications hinder the NOM of patients with blunt splenic injuries. We report a case in which a large haemorrhagic fluid collection that occurred after angio-embolisation was resolved by percutaneous drainage in a patient with liver cirrhosis who experienced a blunt spleen injury.

  • The hot spot on liver scan was demonstrated by many authors in various conditions such as SVC obstruction, Budd-Chiari syndrome, liver abscess, hemangioma of liver, hepatic venoocclusive diseases, IVC obstruction, and tricuspid insufficiency. And the appearance of hot spot in SVC obstruction is due to unsual collateral circulation. But there was no report of this hots pot on liver scan in our country. We have recently observed one patient with SVC obstruction who shows well-defined area of increased radioactivity between right and left lobe of liver on liver scan using $^{99m}Tc-tin$ colloid, and demonstrated collateral circulations with RI venography using $^{99m}Tc-O_4$. The injection site of radiocolloid was left antecubital vein. This hot spot did not appear when the radiocolloid was injected into right leg vein. We report here this hot spot on liver scan in SVC obstruction with review of some liter atures.

Is Laparoscopy-assisted Radical Gastrectomy Safe in Patients with Child-Pugh Class A Cirrhosis?

  • Purpose: We investigated early postoperative morbidity and mortality in patients with liver cirrhosis who had undergone radical gastrectomy for gastric cancer. Materials and Methods: We retrospectively reviewed the medical records of 41 patients who underwent radical gastrectomy at the Chonnam National University Hwasun Hospital (Hwasun-gun, Korea) between August 2004 and June 2009. There were few patients with Child-Pugh class B or C; therefore, we restricted patient selection to those with Child-Pugh class A. Results: Postoperative complications were observed in 22 (53.7%) patients. The most common complications were ascites (46.3%), postoperative hemorrhage (22.0%) and wound infection (12.2%). Intra-abdominal abscess developed in one (2.4%) patient who had undergone open gastrectomy. Massive ascites occurred in 4 (9.8%) patients. Of the patients who underwent open gastrectomy, nine (21.9%) patients required blood transfusions as a result of postoperative hemorrhage. However, most of these patients had advanced gastric cancer. In contrast, most patients who underwent laparoscopic gastrectomy had early stage gastric cancer, and when the confounding effect from the different stages between the two groups was corrected statistically, no statistically significant difference was found. There was also no significant difference between open and laparoscopic gastrectomy in the occurrence rate of other postoperative complications such as ascites, wound infection, and intra-abdominal abscess. No postoperative mortality occurred. Conclusions: Laparoscopic gastrectomy is a feasible surgical procedure for patients with moderate hepatic dysfunction.

[ $^{99m}Tc-RBC$ ] Hepatic Scintiscan in focal Hepatic Lesions ($^{99m}Tc-RBC$ 간신티스캔을 이용한 간내 공간점유병소의 감별진단)

  • 39 patients with focal hepatic lesions were evaluated by $^{99m}Tc-RBC$ liver scan. The diagnosis of focal hepatic lesions were made by percutaneous needle biopsy, angiography, surgery, or clinical courses. Thses diagnoses included 24 cases of hemangioma, 7 hepatomas, 6 metastatic disease, 1 abscess, and 1 cyst. 19 hemangiomas showed focal hot activity on delayed static planar images. 3 small deep seated hemangiomas were diagnosed by SPECT that would have been missed by planar images alone. 2 large hemangiomas had no radioisotope uptake within the lesions on delayed images and at surgery cavernous hemangioma with thrombosis, calcification, and fibrosis were found. For hepatic hemangiomas in our series, the sensitivity was 91.7% and the specificity was 100%. The remaining 15 patients including hepatomas, metastatic lesions, cyst and abscess showed cold defect on delayed blood pool images. It is concluded that $^{99m}Tc-RBC$ liver scan should be the choice of primary diagnostic procecure for clinically suspected hepatic hemangioma since it's inexpensive, non-invasive, and readly available.

Walled-off Pancreatic necrosis in a Dog

  • A 7-year-old, castrated, male Maltese dog presented with hyporexia and depression for 3 days. Elevated serum amylase, lipase activities, and liver enzyme values were found upon blood examination. An abdominal mass was seen on radiographs caudal to the gastric body in the left middle abdomen. In the left middle abdomen, abdominal ultrasonography also revealed a massive, irregularly marginated, heterogeneous mass of unknown origin, and in the right cranial abdomen, heterogeneously hypoechoic pancreatic tissue and hyperechoic change of adjacent mesenteric fat were observed. Contrast-enhanced computed tomography showed an irregular contour of the left pancreatic limb as well as heterogeneously enhanced parenchyma. A low-attenuating peripancreatic fluid collection with a thin and irregular wall was also seen. Based on these findings, an atypical pancreatic abscess with necrotizing pancreatitis which manifested as walled-off necrosis was suspected. The mass was excised, and the pancreatic abscess was confirmed by histopathologic examination. No complications were found in the patient after two months of follow-up examination.

  • This report describes 86 prescriptions related to the use of Herba Schizonepetae main blended from Dongeuibogam. The following conclusions were drawn through investigations on the prescriptions that use Herba Schizonepetae as a key component. Prescriptions that Herba Schizonepetae was taken as a monarch drug are utilized for22 therapeutic purposes, for example, eye disease, abscess disease, head and tooth, and woman disease. In particular, 16.3% of prescriptions appear in the chapter of eye. Prescriptions that utilize Herba Schizonepetae as the main component are used in the treatment of eye, abscess and woman disease, head disease, tooth disease and they are also used for treating 56 different types of disease. Herba Schizonepetae is used in pathogenic factors such as wind and used in pathology related to the liver and stomach system. The dosage of Herba Schizonepetae is 1pun 3li(about 0.49g) to 4don(about 14g), however 5pun 3li(about 1.99g)~1don(about 3.75g) has been taken the most for clinical application. Bangpungtang and Saenglyosamultang are the most useful base prescriptions which use the Herba Schizonepetae as the main ingredient.

  • Streptococcus intermedius is a Gram-positive, obligately anaerobic, nonsporeforming, and nonmotile cocci. S. intermedius is a member of oral flora and is endodontic infection, respiratory infections, infective endocarditis, brain abscess, and liver abscess. Streptococcus intermedius ChDC B718 (= KCOM 1545) was isolated from a human pulpitis lesion. Here, we present the complete genome sequence of S. intermedius ChDC B718.

  • The four principles of treatment of odontogenic infection are as follows : (1) removal of the cause, (2) establishment of drainage, (3) institution of antibiotic therapy, and (4) provision of supportive care, including proper rest and nutrition. A separate incision is required to establish drainage, especially in the case of extensive fascial space infections. There are four principle causes for active bleeding in the immediate incision & drainage phase; (1) vascular wall alteration (infection, scurvy, chemicals), (2) disorder of platelet function, (3) thrombocytopenic purpuras, (4) disorders of coagulation (liver disease, anticoagulation drug). If the hemorrhage from incision & drainage site is aggressive, the site must be packed with proper wet gauze and wound closure & drainage dressing are applied. The specific causes of bleeding may be associated with hypoxia, changes in the pH of blood & chemical changes affecting vascular contractility and blood clotting. This is a case report of bleeding control by the circumferential suture & drainage on active bleeding incision & drainage site of temporal space abscess due to advanced odontogenic infection in a multiple medically compromised disabled patient.

  • 1. King Hyeonjong(1641-1674) mainly suffered from eye disease and abscess. He specially took a hot spring bath for cure of eye and skin problems. He probably died of septicemia and the following gastro-intestinal infection at the age of 34, quite early for his age. 2. King Sukjong(1661-1720) was not very well all through his life, but lived quite longer than other Kings in the Joseon Dynasty. He suffered from various diseases like heart-based heat, abscess, edema, upper respiratory infections, etc. He frequently took the treatments of acupuncture and moxibustion. He presumably died of dysfunction of liver and kidney at the age of 60. 3. King Gyeongjong(1688-1724) suffered from political problems from birth to death, so he may have had excessive mental stress for his poor health. He mainly suffered from heart-based heat and abscess. It is quite not sure why he died in only one month from the onset of his symptoms, so many people thought that he was poisoned to death. He probably died of septicemia and the following gastro-intestinal infection at the age of 37. 4. King Yeongjo(1694-1776) lived for 83 years, which is the longest of all the Kings in the Joseon Dynasty. He mainly suffered from hypofunction of gastro-intestinal system with cold symptoms, coughs, uneasiness. He took various kinds of herbal medicine, of which he took ginseng the most. He is supposed to be dead due to his old age at 83.

Clinical Observation of Liver Scintigram (간(肝) Scintigram의 임상적(臨床的) 고찰(考察))

  • Although primary application of radioisotope scanning technics to the liver has been of use in the detection of the intra-hepatic space occupying lesion from the normal functioning liver parenchyme, there has been on increasing awareness of its use in evalution of Liver function. In this study, the diseases of the liver were classified into group A,B,C and D by the liver scanning findings, conventional liver function tests and clinical findings. Following were the results: 1. The colloidal radiogold liver scan appeared normal in the group A, also the albumin in serum, alkaline phosphatase activity and prothrombin time were within normal levels in this group. 2. In the group B, there were acute hepatitis 24(48%), chronic hepatitis 5(10%), toxic hepatitis 3(6%), subacute hepatic necrosis 3(6%), typhoid liver 4(8%), hepatic tuberculosis 2(4%), diabetes mellitus 3(6%) and others 3(6%). In this group, SGOT and SGPT were increased predominantly as compared with group A, and the liver scan showed small amount of mottling of activity and faintly visualized spleen. 3. In the group C, there were postnecrotic liver cirrhosis 30(60%), Laennec cirrhosis 10(20%), cardiac cirrhosis 1(2%), cholangiocarcinoma 1(2%), chronic active hepatitis 6(12%), hepatic milliary tuberculosis 1(2%) and gall bladder cancer 1(2%). In this group, the albumin in serum and prothrombin time were lowered significantly and the liver scan showed severe mottling of activity with extra-hepatic uptake in the spleen and bone marrow along the vertebral column. 4. In the group D, there were primary hepatoma 26(52%), hepatoma with liver cirrhosis 7(14%), metastatic liver cancer 5(10%), liver abscess 10(20%), multiple liver cyst 1(2%) and cystic duct adenoma 1(2%), In this group, the alkaline phosphatase activity was elevated with single or multiple intrahepatic space occupying lesion in the radiogold colloid liver scan.
