Ethnography of Caring Experience for the Senile Dementia (노인성 치매 환자의 돌봄경험에 대한 문화기술지)

  • Senile Dementia is one of the dispositional mental disorder which has been known to the world since Hippocratic age. It has become a wide-spread social problem all over the world because of chronic disease processes and the demands of dependent care for several years as well as improbability of treatment of it at the causal level. Essentially, life styles of the older generation differ from those of the younger generation. While the fomer is used to the patriarchal system and the spirit of filial piet and respect, the latter is pragmatized and individualized under the effects of the Western material civilization. These differences between the two generations cause conflict between family members. In particular, the pain and conflict of care-givers who take care of a totally dependent dementia patient not only is inciting to the collapse of the family union, but is expanding into a serious social problem. According to this practical difficulty, this study has tried to compare dementia care-givers' experiences inter-culturally and to help set up more proper nursing interventions, describing and explaining them through ethnographies by participant observation and in-depth interviews that enable seeing them in a more close, honest and certain way. It also tries to provide a theoetical model of nusing care for dementia patients which is proper to Korean culture. This study is composed of 12 participants (4 males, 8 females) whose ages range from 37-71 years. The relations of patients are 5 spouses(3 husbands, 2 wives), 4 daughters-in-law, 2 daughters, and 1 son-in-law. The following are the care-givers' meaning of experiences that results of the study shows. The first is "psychological conflict". It contains the minds of getting angry, reproaching, being driven to dispair, blaming oneself, giving up lives, and being afraid, hopeless, and resigned. The second is "physical, social and psychological pressure" . At this stage, care-givers are shown to be under stress of both body and soul for the lack of freedom and tiredness. They also feel constraint because they hardly cope with the care and live through others' eyes. The third is "isolation". It makes the relationship of patient care-giver to be estranged, without understanding each other. They, also, experience indifference such as being upset and left alone. The forth is "acceptance" They gradually have compassion, bear up and then adapt themselves to the circumstances they are in. The fifth is "love". Now they learn to reward the other with love. It is also shown that this stage contains the process of winning others' recognition. The final is "hope". In this stage they really want situations to go smoothly and hope everything will be O.K. These consequences enable us to summarize the principles of cue experience such as, in the early stage, negative response such as physical·psychological confusion, pain and conflict are primary. Then the stage of acceptance emerges. It is an initial positive response phase when care-givers may admit their situations. As time passes by a positive response stage emerges. At last they have love and hope. Three stages we noted above : however, there are never consistent situations. Rather it gradually comes into the stage of acceptance, repeating continuous conflict, pressure and isolation. If any interest and understanding of families or the support of surrounding society lack, it will again be converted to negative responses sooner or later. Otherwise, positive responses like hope and love can be encouraged if the family and the surroundings give active aids and understanding. After all, the principles of dementia care experiences neither stay at any stage, nor develop from negative stages to positive stages steadily. They are cycling systems in which negative responses and positive responses are constantly being converted. I would like to suggest the following based on the above conclusions : First, the systematic and planned education of dementia should be performed in order to enhance public relations. Second, a special medical treatment center which deals with dementia, under government's charge, should be managed. Third, the various studies approaching dementia care experiences result in the development of more reasonable and useful nursing guidelines.

A Comparative Analysis over News Framing of the Abolition of the Family Headship (Hoju) System: Examining Three Major Korean Dailies: Chosun, Kukmin, Hankyoreh (호주제 폐지에 대한 뉴스 프레이밍 비교 연구: 조선일보, 국민일보, 한겨레신문을 중심으로)

  • The main purpose of this study is based on the comparative analysis over news framing of the family headship(Hoju) abolition in Korean society. This study examined the newspaper articles involving the Hoju abolition, which had been printed on the three major dailies, Chosun, Kukmin, Hankyoreh through February of 1990 to July of 2005. First, the news articles were analyzed and classified on the basis of their lengths, news types, main characters, news framing and systematic framing. Second, the articles that this study looked into were divided into the five major periods when the issue of the Hoju abolition in Korean society surfaced as a main social agenda to be discussed. Third, the main differences between the noticeable frame and unnoticeable frame in each period were analyzed through the three different perspectives which can also can be sub-divided into the six different attributes. This study found that the Hoju abolition as an attribute had developed into political, legal and social fields. The analysis of the research shows that the articles related to the patriarchy abolition showed more dominant frame which reflected the social change or the general tendency of the times. However, the analysis indicates that the articles in the level of an attribute included more dominant frame which mirrored a male chauvinism society. It also points out that the articles contained more dominant frame which was be used as a standard to find out the readers' political inclination. The articles also showed the dominant frame which included the revision and legal process of family laws before presidential or general election campaigns. The study also found that there were major differences among the three dailies. First of all, Chosun, regarded the Hoju as a custom by stressing that 'it is necessary to keep Hoju system to intensify the role and crisis of family if the Hoju will be abolished'. However, Hankyoreh recognized the issue as an important one to improve feminism and female rights by maintaining that 'it is the time to balance the inequality out between men and women with the abolition of patriarchy'. Finally, Kukmin treated the issue as an first step to acknowledge the dignity of females by emphasizing that 'a revision of the law is essential to accept the changing ethics of the times'.

Antioxidant Effects and Improvement of Lipid Metabolism of Acanthopanacis cortex Water Extract in Rats Fed High Fat Diet (고지방 식이 흰쥐에서 오가피 추출물의 항산화 효과 및 지질 개선 효과)

  • The effects of an Acanthopanacis cortex water extract on lipid levels, lipid peroxide, total antioxidant status and antioxidant enzyme activities were evaluated in rats fed one of the following diets for six weeks: normal diet and deionized water (ND), normal diet and Acanthopanacis cortex water extract (NDC), high fat diet and deionized water (HFD), high fat diet and Acanthopanacis cortex water extract (HFDC). The food intakes were significantly lower, but the food efficiency ratios were significantly higher in the high fat diet groups. The level of HDL-cholesterol in the plasma was significantly increased and the levels of LDL-cholesterol and triglyceride in the plasma were significantly decreased by the Acanthopanacis cortex water extract in the high fat diet groups. As a a result, the AI (atherogenic index) and CRF (cardiac risk factor) were significantly lower in the high fat diet groups that were treated with Acanthopanacis cortex water extract. The triglyceride and the total cholesterol of the liver were also significantly upregulated in the high fat diet groups, while the total cholesterol of the liver decreased in response to treatment with Acanthopanacis cortex water extract (HFDC). The plasma and liver concentrations of thiobarbituric acid reactive substances (TBARS) were significantly reduced by the addition of Acanthopanacis cortex water extract to the normal diet groups. The total antioxidant status (TAS) in the plasma was significantly upregulated by adding Acanthopanacis cortex water extract to the high fat diet groups. The activities of SOD, catalase and GST were also significantly higher in the Acanthopanacis cortex water extract groups when compared to the ionized water groups. The activity of GSH-Px and the concentration of GSH in the liver were significantly higher following the addition of Acanthopanacis cortex water extract to the high fat diet groups. Taken together, these results suggest that a supplementation of the diet of rats fed a high fat diet with Acanthopanacis cortex water extract improves lipid metabolism, reduces lipid peroxide and improves the activities of antioxidant enzymes, which may have favorable effects on antioxidant systems by improving the total antioxidant status (TAS).

Antioxidant Activity and Inhibition Activity against α-Glucosidase and PTP1B of Chrysanthemum indicum L. Extract and Fractions (감국(Chrysanthemum indicum L.) 추출물과 분획물의 항산화 및 α-Glucosidase와 PTP1B 저해 활성)

  • In this study, we evaluated the polyphenol content, flavonoid content, antioxidant and biological of 70% EtOH extract and its fractions (dichloromethane, ethyl acetate and water) of the plant Chrysanthemum indicum L. (CI) belonging to family chrysanthemum. antioxidant activity was determined 1,1-diphenyl-2-picrylhydrazyl (DPPH), 2,2'-azino-bis-3-ethylbenzthiazoline-6-sulphonic acid (ABTS), and superoxide scavenging activity. The biological activity tests included measurement of ${\alpha}$-glucosidase and PTP1B. The results show that content of polyphenol and flavonoid in 70% EtOH of CI 30.20 mg and 31.58 mg, respectively. Among the different samples, ethyl acetate (EA) fraction showed the highest level of polyphenol (87.87 mg) and flavonoid (65.12 mg). The DPPH, ABTS and superoxide dismutase(SOD) antioxidant assay of EA fraction showed 93.84%, 93.38% and 84.68% inhibition respectively. The EA fraction also showed the highest antioxidant activity compared to 70% EtOH extract and other fractions. EA fraction ($20{\mu}g/mL$) showed the highest 30.29% and 99.24%, respectively compared to other samples. The results of this study show that the EA fraction contained more antioxidant and antidiabetic components than 70% EtOH extract and other fractions. In conclusion, we believe that CI may be useful natural antioxidant and functional material.

Application of ECVAM as a Indicator for Monitoring National Environment in Korea (국토환경 모니터링 지표로서의 국토환경성평가지도 활용방안)

  • Objectives of the Korean Environmental Conservation Value Assessment Map (ECVAM) is to evaluate environmental value used in comprehensive environmental information in order to encourage eco-friendly land use and management. The first research was conducted in 2001 to establish the evaluation items and the criteria of the ECVAM, and the first nationwide map was established in the period of 2003 to 2005. The maps are updated annually to reflect environmental changes of land. The evaluation items and the criteria have been modified based on feasibility studies to improve the accuracy of the maps. This study re-evaluated the ECVAMs from 2005 to 2010 with criteria used in current environment and analyzed the changes in the area of the maps in 6 years. This is also an investigation on the maps whether they are appropriate as an index for sustainable environmental monitoring. The result shows that the 1st grade level of the ECVAM area with the highest conservation value had been expanding since 2005. These changes were analyzed in terms of updating the 4th Forest Map (2008) produced once every 10 years, reflecting the new legal protected areas such as Baekdudaegan Protected Area(2010), and the environmental/ecological assessment items such as the National Ecological Network (2009). This mean the ECVAM are a monitoring index that integrates individual environmental indexes including the increase of forest age and diameter due to sustainable management of forest areas, and the change of conservation areas. Therefore, ECVAM can be used as a new index integrating national environmental indicators for monitoring changes of national environment and policy. In order to utilize the ECVAM, improving accuracy and reducing renewal cycle time of thematic maps are required.

Development of a Climate Change Vulnerability Index on the Health Care Sector (기후변화 건강 취약성 평가지표 개발)

  • The aim of this research was to develop a climate change vulnerability index at the district level (Si, Gun, Gu) with respect to the health care sector in Korea. The climate change vulnerability index was esimated based on the four major causes of climate-related illnesses : vector, flood, heat waves, and air pollution/allergies. The vulnerability assessment framework consists of six layers, all of which are based on the IPCC vulnerability concepts (exposure, sensitivity, and adaptive capacity) and the pathway of direct and indirect impacts of climate change modulators on health. We collected proxy variables based on the conceptual framework of climate change vulnerability. Data were standardized using the min-max normalization method. We applied the analytic hierarchy process (AHP) weight and aggregated the variables using the non-compensatory multi-criteria approach. To verify the index, sensitivity analysis was conducted by using another aggregation method (geometric transformation method, which was applied to the index of multiple deprivation in the UK) and weight, calculated by the Budget Allocation method. The results showed that it would be possible to identify the vulnerable areas by applying the developed climate change vulnerability assessment index. The climate change vulnerability index could then be used as a valuable tool in setting climate change adaptation policies in the health care sector.

Palaeomagnetism of the Cretaceous Yuchon Group in Kosong Area, Southern Kyongsang Basin (경상분지 남단 고성지역의 백악기 유천층군에 대한 고자기 연구)

  • A total of 165 independently oriented core samples were collected from 19 Cretaceous Yuchon Group sites in Kosong area, the southernmost part of the Miryang subbasin of the Kyongsang Basin in southern Korea. Stepwise AF and thermal cleaning revealed antipodal ChRM from 95 samples from 14 sites. Mean ChRM direction is d=26.0$^{\circ}$, i=49.4$^{\circ}$ (${\alpha}_{95}$=8.2$^{\circ}$, k=24.5, n= 14) before bedding correction and d=28.1$^{\circ}$, i=54.2$^{\circ}$ (${\alpha}_{95}$=4.8$^{\circ}$, k=70.6, n= 14) after bedding correction. A 2.88-fold increase of the precession parameter k by bedding correction indicates pre-folding age of the ChRM with 99% confidence level. Palaeomagnetic pole position calculated from the mean ChRM is 67.0$^{\circ}$N, 210.6$^{\circ}$E (dp=4.7$^{\circ}$, dm=6.7$^{\circ}$), which is significantly different neither from the poles of other part of the Kyongsang Basin nor those of Eurasia including SCB and NCB. This suggests stable relative position of the study area with regard to other parts of the Kyongsang Basin as well as to Eurasia continent since Cretaceous. Three ploarity reversals in the Kosong Formation in addition to the coexistence of normal and reversed polarities in the overlying Andesites and Welded Tuff suggest, in reference to the worldwide geomagnetic polarity time scale, an Albian to Maastrichtian (polarity chron 32r-31r) age of the Yuchon Group of the study area. An alleged hypothesis of stratigraphical correspondence between the Kosong Formation in the study area and the Tadaepo Formation in Pusan area is, however, not tenable: Not only because the latter shows a short reverse polarity only in its lowest part of the sequence but also because the Andesites overlying it is wholly normally magnetized, in contrast to the frequent reverals in the case of both the Kosong Formation and Andesites above it.

Study on the Legal Issues of New Draft of Civil Aviation Law in China (중국 민용항공법 개정 최근 동향과 주요 법적쟁점)

  • During more than 20 years of practice, Civil Aviation Law has experienced three times of amendments since it was enacted in 1995. But these revisions are limited to the technical level. The problems and limitations have become increasingly prominent in its implementation. Firstly, the civil aviation law is the result of interests game among several departments and some legal issues was left behind and the regulation was very vague as a result. Secondly, the process of aviation legislation is the process that the country has undergone profound changes and social transformation. The 20 years is long enough for the society to undergo tremendous changes and 1995 version of civil aviation law does not keep pace of development of economy. There was a serious lag between reality and the law. In order to actively promote the development of the aviation industry and overcome implementation issues of the Civil Aviation Law, Civil Aviation Administration of China (CAAC) initiated modification procedure the law and published new draft of Civil Aviation Law in August 2016. The spirit of this modification is to learn and absorb new achievements of domestic and foreign legislation and the International Convention on civil aviation. Furthermore, the purpose of the revision is to provide favorable policy for the development of civil aviation industry and improve aviation safety and supervision, strengthen and protection of consumer rights and interests, to enhance the safety of civil aviation activities, and promote the development of general aviation. This revision concerned to the 78 articles which are revised or deleted and 24 articles added. The highlights of the draft include but not limited to the enhancement of security management, clarification of the main responsibility for production safety. And also it added the provisions related to the construction of effective tracking capability of public air transport enterprises and license system on the transport of dangerous goods. Compared with the existing civil aviation law, the draft has made a great improvements. But there are several deficiencies and limitations in the drafts. These problems need to be supplemented and perfected through further amendments in near future.

Clinical Significance of Tumor Markers in Gastric Cancer Patients after Curative Resection (근치적 절제술을 시행한 위암 환자에서 종양 표지자의 임상적 의미)

  • Purpose: This clinical study was conducted to evaluate the predictive value of tumor markers for recurrence and the clinical significance of false positive findings after curative gastrectomy in patients with gastric cancer. Materials and Methods: Two hundred ninety patients with gastric cancer who underwent gastrectomy with curative intent were evaluated retrospectively. We analyzed the correlations between changes in tumor markers (CEA, CA 19-9, AFP, and CA-125) and clinicopathologic data, and basis for changes in tumor markers without recurrence during the follow-up period. Results: The sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of tumor markers for recurrence were 75.0%, 64.6%, 23.1%, 94.8%, and 65.9% respectively. Among 36 patients with recurrences, 10 patients (27.8%) had elevated tumor markers prior to positive findings on imaging studies, while 13 patients (36.1%) had concomitant elevation in tumor markers. At least 1 of the 4 tumor markers increased in 90 of 290 patients during the follow-up period; however, there was no evidence of tumor recurrence. Twenty patients had persistently elevated tumor markers, while the tumor marker levels in 70 patients returned to normal level within $9.08\pm7.2$ months. The patients with pulmonary disease, hepatobiliary disease, diabetes, hypertension, or herbal medication users had elevated tumor markers more frequently than patients without disease (P<0.001). Conclusion: Although detecting recurrence of gastric cancer with tumor markers may be useful, false positive findings of tumor markers are common, so surgeons should consider other chronic benign diseases and medical conditions when tumor markers increase without evidence of recurrence.

Analysis of Relationship Between Dental Health Condition and Eating Habit (치아건강상태와 식습관과의 상관관계 분석)

  • The survey on dental health condition and eating habit of local residents in Daegu and Kyungbuk had been conducted. Through the analysis of influence of dental health condition on eating habit I could get the following result. The number of subjects of survey is 630. The period of survey is from June 2006 to September 2006. 1. General quality of subjects is that forty-four point four percent of subjects are male and fiftyfive pointsix percent are female. The age of thirty point one percent of subjects is between 20 and 29. Fifty-six point two percent of subjects are married. Fifty-one point four percent of subjects have above bachelor degree. Monthly income of forty-four point eight percent of subjects is less than one million won. Twenty-four point four percent of subjects are students. Fifty-five point seven percent of subjects are living in big cities. 2. Forty-seven point seven percent of male and fifty-one point eight percent of female answered their subjective dental health condition is bad. Marital status, age and academic background have relationship with answer. Forty-three point nine percent of married subjects and forty-one point five percent of divorced or bereaved subjects said they have bad dental health condition. The older he is or the lower academic background he has subjects think they have bad dental health condition. Forty-seven point four percent of non-educated subjects answered their dental health condition is bad. Forty-six percent of self-employed subjects and subjects who live in the country have tendency to think their dental health condition is bad. 3. About eating habit knowledge, male's knowledge of 2.03$\pm$0.20 is lower than female's of 2.08$\pm$0.21. This shows there is statistically significant difference(p<0.01). 4. There is significant relationship between subjective dental health condition and health condition of subjective. This means subject who has better health condition has also better dental health condition(37.5%). About subjective dental health condition, subject who eats restoratives has worse dental health condition. This shows there is a relationship between dental health condition and eating restoratives(p=0.004) and subject who works out steadily has better dental condition. 5. About relation between dental health condition and eating habit, subject who eats vegetables has worse dental health condition by fifty point seven percent. As cross tabulation result, p-value is 0.002. In level of significance of 0.05, there is statistically significant difference in eating habit. 6. It seems that eating habit has an effect on marital status, age, academic background, income and also on dental prosthetic treatment situation. Many subjects think their dental health condition is bad. About eating habit, subjects who eat meat have better dental health condition. Subjects who drink green tea and fruit juice has better dental health condition than who drink coffee and Balanced diet is good for dental health condition. As eating habit is important for developing dental health, government should make a proper program. Dental health education program especially for elderly, low-eduacted, residents in the country and poor people should be developed. Government, dental health organization, dental health specialist, associated research institution and people work in the press should be concerned and devote to improve quality of life. Primary prevention education will help for dental health.
