Other Gynecologic Pathology in Endometrial Cancer Patients

  • Background: To evaluate the prevalence and features of other gynecologic or surgical lesions in endometrial cancer (EMC) patients. Materials and Methods: Clinico-pathological data of EMC patients who were treated in the institution from 1995 to 2012 were collected. Data collected were age, stage of disease according to the FIGO 2009 criteria (FIGO), histopathology, tumor grade, adjuvant therapy, other gynecologic or surgical lesions, follow-up period, and living status. Results: The mean age of 396 patients was $56.7{\pm}10.64years$. Abnormal uterine bleeding was the most common presenting symptom (90.1%). Bleeding was accompanied with pelvic mass in 7.7% and 5.4% had only a pelvic mass. Abnormal cervical cytology was found in 3.8%. Approximately 75% had early stage diseases and 86% had endometrioid histology. We found 55.8% of EMC patients had other gynecologic lesions: 89.6% benign and 9.5% malignant. Some 4.5% had pre-invasive cervical/vulva/vagina lesions. The two most common gynecologic lesions were myoma uteri and ovarian tumors. Focusing on the latter, approximately 14% were benign while 8% were malignant. Among 364 patients with available data, surgical lesions were found in 11.8%, 5.7% benign and 9.2% malignant. The most common benign surgical condition was chronic appendicitis while breast and colon cancers were the two most common malignant lesions found. Conclusions: More than half of EMC patients had other gynecologic lesions including benign and malignant tumors. Surgical lesions were also found in more than one-tenth of patients. Careful pre-operative evaluation and intra-operative inspection are advised for proper management and better prognosis.

Elastography for Breast Cancer Diagnosis: a Useful Tool for Small and BI-RADS 4 Lesions

  • The present study aimed at evaluating and comparing the diagnostic performance of B-mode ultrasound (US), elastography score (ES), and strain ratio (SR) for the differentiation of breast lesions. This retrospective study enrolled 431 lesions from 417 in-hospital patients. All patients were examined with both conventional ultrasound and elastography. Two experienced radiologists reviewed ultrasound and elasticity images. The histopathologic result obtained from ultrasound-guided core biopsy or operation excisions were used as the reference standard. Pathologic examination revealed 276 malignant lesions (64%) and 155 benign lesions (36%). A cut-off point of 4.15 (area under the curve, 0.891) allowed significant differentiation of malignant and benign lesions. ROC (receiver-operating characteristic) curves showed a higher value for combination of B-mode ultrasound and elastography for the diagnosis of breast lesions. Conventional ultrasound combined elastography showed high sensitivity, specificity, and accuracy for group II lesions (10mm${\leq}20mm$). Elastography combined with conventional ultrasound show high specificity and accuracy for differentiation of benign and malignant breast lesions. Elastography is particularly important for the diagnosis of BI-RADS 4 and small breast lesions.

치근단 병소에서 면역적격세포의 분포에 관한 연구 (A QUANTITATIVE ANALYSIS OF THE IMMUNOCOMPETENT CELLS IN PERIAPICAL LESIONS OF THE HUMAN TEETH)

  • Periapical lesions are developed as a result of inflammatory response to irritants from root canal system. Clinicians remove these irritants from root canal system and seal the root canal space to induce healing of the periapical lesions. Immunopathologic responses may play an important role in development and progression of periapical lesions and periapical lesions contain immunocompetent cells. The purposes of the present study were to analys and to compare the distribution of the immunocompetent cells in the human periapical lesions according to the stage of endodontic treatment using indirect immunoperoxdase technique. Obtained 94 human periapical lesions were devided into four groups: Group 1 : no endodontic treatment(28 samples) Group 2 : root canal enlarged and irrigated(28 samples) Group 3 : root canal filled(29 samples) Group 4: unknown(9 samples) Monoclonal antibodies to examine target cells were UCHL-1 for T lymphocytes(1 : 200, Dakopatt, Denmark), L26 for B lymphocytes(1 : 200, Dakopatt, Denmark), OPD4 for helper T lymphocytes(l : 200, Dakopatt, Denmark) and alpha-1-antichymotrypsin for macrophages(l : 2000, Dakopatt, Denmark). The following results were obtained : 1. All the periapical lesions studied were infiltrated by T lymphocytes, plasma cells, B lymphocytes, and macrophages. T lymphocytes were more infiltrated than B lymphocytes, and B lymphocytes and macrophages were less infiltrated than T lymphocytes and plasma cells(P<0.05 : Oneway ANOVA test). 2. In untreated group and canal irrigated and enlarged group of all the periapical lesions, helper T lymphocytes were predominently infiltrated(P>0.05 : Oneway ANOVA test). 3. In canal filled groups of all lesions except periapical cyst, plasma cells were predominently infiltrated. But, in canal filled group of periapical cyst, helper T lymphocytes were the predominent cells(P>0.05 : Oneway ANOVA test). The above results shows that the immunologic responses play important role in pathogenesis of periapical lesions and the immunologic response involved undergoes certain changes after endodontic therapy.

미만성 뇌축삭손상 환자의 자기공명영상 소견과 예후와의 상관관계 (The Correlation of MRI Findings to Outcome in Diffuse Axonal Injury Patients)

  • Object : We intended to investigate the relationship between the degree of injury on MRI and the outcome of the patients with diffuse axonal inury. Method : From january, 1995 to march, 1999, 22 patients were supposed to have diffuse axonal injuries by means of their neurologic signs and MRI. We investigated their prognosis according to CT, MRI and initial neurologic findings. Result : 1) The lesions were mainly located at white matter of cerebrum, corpus callosum, brainstem, and basal ganglia. 2) The lesions of white matter were most commonly in the frontal lobe and temporal lobe. 3) The majority of corpus callosal lesions were located in the posterior body and splenium, but anterior corpus callosal lesions combined with posterior lesions were not found. 4) Brainstem lesions, all non-hemorrhagic, were mostly located in the dorsolateral aspect, not be found on CT. 5) The brainstem lesions were found in 10 cases among total 22 cases, and corpus callosal lesions were accompanied with 8 cases of brainstem lesions. 6) The patients with brainstem lesions had worse prognosis. Conclusion : It is important and reasonable to take brain MRI to identify the brainstem lesions in any cases of suspicious diffuse axonal injury, and we should remind that the diffuse axonal injury with stem lesion has worse prognosis.

피부병변의 정량적 평가를 위한 디지털 컬러 영상 시스템 (Digital Color Imaging Systems for Quantitative Evaluation of Skin Lesions)

  • In this paper, we introduce a digital cross-polarization and fluorescent color imaging system for quantitative evaluation of skin lesions. We describe the characterization of the imaging systems and the quantitative image analysis methods to show the feasibility for quantitative evaluation of skin lesions. The polarization color image was used to compute erythema and melanin index image which are useful for quantitative evaluation of pigmentation and vascular skin lesions, respectively. The fluorescent color image was used to quantitatively evaluate "sebum" and "vitiligo". In quantitative evaluation of various skin lesions, we confirmed the clinical efficacy of the imaging systems for dermatological applications. Finally, we sure that the imaging systems can be utilized as important assistant tools for the evaluation of skin lesions by providing reproducible quantitative result for widely distributed skin lesions.

성대 양성점막 병변의 수술적 치료 (Surgical Intervention for Benign Vocal Fold Lesions : When and How?)

  • Several distinct pathologic entities are encompassed in benign vocal fold lesions, including intracordal cysts, vascular ectasia, as well as vocal fold nodules and vocal fold polyps. Treatment options for theses lesions include both medical/conservative and surgical techniques. First approaches should be focus on correcting the underlying causative factors, largely through voice therapy and education. There are several laryngomicrosurgery techniques for removal of benign lesions. Much debate continues regarding the relative merits of cold instruments versus carbon dioxide laser removal of benign vocal fold lesions. Both techniques have the merits and the demerits each other. Therefore the surgeon should well comprehend the merits of each techniques and choose the proper procedure for patient's lesions.

도축한우의 신장에 대한 병리학적 연구 (Abattoir survey of kidney lesions in Korean cattle)

  • To examine the prevalence and types of kidney lesions in Korean cattle, a survey was carried out at a abattoir in september 2000 and January 2001. The collected kidneys were examined grossly and histopathologically and investigated for the patterns of renal diseases with considering season and sex. Of 735 cattle (99 bulls and 246 cows in September and 140 bulls, 3 steers, and 247 cows in January) surveyed, 301 (41%) cattle had various renal lesions. The most common prominent finding was focal or multiple interstitial nephritis (36.2%). Other lesions included focal or multiple renal cyst (9.1%), infarction (0.1%), hemorrhage (0.3%), and renal atrophy (0.3%). Microscopically, main pathological lesions were acute or chronic interstitial nephritis with moderate infiltration of neutrophil or lymphocyte, respectively. Finally, the kidney lesions are more or less correlated with the meat quality, especially in female Korean cattle.

돼지의 위궤양에 관한 병리학적 관찰 (A Pathologic Study on the Gastric Ulcers of the Pigs)

  • A total of 499 stomachs consisted of 393 slaughter and 106 breeding pigs from industrial farm was examined for ulcerative lesions grossly and microscopically. The relationship between the ulcerative lesions and stomach contents was also examined. 1. The majority of stomachs has one or more lesions of ulcerative process, including parakeratosis(64.1%), erosions(10.2%), acute ulcers(2.8%), chronic ulcers(1.4%), and scars(0.8%). 2. The incidence and the degree of the esophagogastric lesions in the breeding pigs were similar to those of the slaughter pigs. But ulcerative lesions of the fundic lesions in the breeding pigs were found to be more remarkable than those of the slaughter pigs. 3. The stomach contents of the pigs having gastric ulcers were mostly fluid in nature, which were dry in the normal stomach. 4. Histologically, muscular hypertrophy, connective tissue hyperplasia and vascularity were shown in the ulcerative lesions of the esophagogastric portion. Thrombosis, necrosis and granulation tissue formation were found frequently.

  • Fibro-osseous lesions are a poorly defined category of conditions affecting the jaws and craniofacial bones, and include developmental lesions, reactive or dysplastic lesions, and neoplasms. Fibrous dysplasia and ossifying fibroma are the 2 main types of fibro-osseous lesions affecting the jaw, and ossifying fibroma is a true benign neoplasm of the bone-forming tissues with several well-recognized variants ranging from innocuous to extensively aggressive lesions. However, multiple simultaneous fibro-osseous lesions of the jaw bones involving all quadrants are exceedingly rare. One such case diagnosed by conventional radiography and computed tomography is discussed here.

CT Image Analysis of Hepatic Lesions Using CAD ; Fractal Texture Analysis

  • We investigated whether the CT images of hepatic lesions could be analyzed by computer-aided diagnosis (CAD) tool. We retrospectively reanalyzed 14 liver CT images (10 hepatocellular cancers and 4 benign liver lesions; patients who presented with hepatic masses). The hepatic lesions on CT were segmented by rectangular ROI technique and the morphologic features were extracted and quantitated using fractal texture analysis. The contrast enhancement of hepatic lesions was also quantified and added to the differential diagnosis. The best discriminating function combining the textural features and the values of contrast enhancement of the lesions was created using linear discriminant analysis. Textural feature analysis showed moderate accuracy in the differential diagnosis of hepatic lesions, but statistically insignificant. Combining textural analysis and contrast enhancement value resulted in improved diagnostic accuracy, but further studies are needed.
