MELAS 증후군과 미토콘드리아 근육병에서의 Tc-99m ECD 뇌단일 광전자방출 전산화단층촬영 소견: 자기공명영상과의 비교 (Tc-99m ECD Brain SPECT in MELAS Syndrome and Mitochondrial Myopathy: Comparison with MR findings)

  • 목적: 본 연구의 목적은 MELAS 증후군과 미토콘드리아 근육병의 뇌 SPECT 소견을 알아보고 SPECT 소견과 자기공명영상 소견을 비교 분석하여 MELAS증후군의 특징적인 영상 소견을 찾아보고자 하였고 MELAS 증후군에 있어서 뇌 SPECT의 역할을 평가해 보고자 하였다. 대상 및 방법: 뇌졸중 유사 증상이나 경련 또는 발달 지연을 주소로 하였고, 혈청 또는 뇌척수액의 lactic acid치가 상승되어 있는 1세에서 25세의 5명의 환자를 대상으로 하였고 남녀비는 4:1이었다. 모든 환자에서 Tc-99m ECD를 이용한 뇌혈류 단일광전자방출 전산화 단층촬영술(SPECT)와 자기공명영상을 시행하여 영상 소견을 분석하였다. 결과: 자기공명영상에서는 주로 두정엽(4/5)과 후두엽(4/5), 그리고 기저핵(1/5)에 백질과 회백질에 증가된 T2 신호강도를 나타내었는데, 특정한 혈관 영역에는 부합하지 않는 병변의 분포양상을 보였다. SPECT상에서는 자기공명영상에서 이상소견을 보인 모든 부위에서 관류 저하를 보였으며 추가적으로 두정엽(1예), 측두엽(1예), 전두엽(1예), 기저핵(1예)와 시상(2예)에서도 감소된 Tc-99m ECD의 섭취를 나타내어서, 자기공명영상과 SPECT에서 이상 소견을 보인 수를 비교하면 자기공명영상에서 나타난 해부학적인 이상소견보다 SPECT에서 보인 관류 저하가 더 광범위하였다. 결론: MELAS 증후군의 SPECT에서는 특정한 혈관 영역에는 부합하지 않는 두정엽과 후두엽, 기저핵, 시상, 측두엽등의 관류저하를 보여 주었는데, 본 연구의 여러 제한점으로 인하여 MELAS 증후군에서만 나타나는 특징적인 소견이라고 할 수는 없었다. 자기공명영상에서 상응하는 이상 소견이 없이 SPECT에서만 관류 저하를 보이는 경우의 중요성은 좀 더 많은 수의환자를 대상으로 한 연구를 통해 평가되어져야 할 것으로 생각한다. 나타내었다.속도를 향상시킬 수 있었다. 정상인의 뇌영상에 대해 위치 정합을 실시한 결과 평균 거리 오차는 2mm 이하였다. 가중정규화 방법을 사용하였을 때 합성된 영상의 정성적인 식별 명확도가 향상하였다. 결론: 견실한 PET 영상 경계점 추출과 거리지도를 이용한 계산 속도의 향상을 통해 뇌 PET과 MR 영상 합성기법의 성능을 개선할 수 있었으며 이를 이용하며 개발한 영상정합 프로그램은 임상 환경에서 유용하게 사용될 수 있을 것이다.은 환자군을 대상으로 한 추가 연구가 필요한 것으로 판단된다.07% ID/g 이하로 매우 낮았다. 결론: 이실험에서 표지한 Re-188 황 교질은 표지효율과 안정성이 높고 임상적으로 방사선 활액막 절제술 등에 사용할 수 있을 것으로 생각한다.}I$] 또는 [$^{131}I$]OMIMT는 종양의 아미노산 대사 영상제제로 이용될 수 있으며 앞으로 이에 대한 임상연구가 필요할 것으로 생각되었다.>$R_A,\;R_v$의 결과간에 좋은 상관관계를 가졌다. 따라서 이러한 약역학 컴퓨터시뮬레이션이 SPECT 영상을 이용한 도파민 운반체 또는 수용체 정량분석을 최적화하는데 매우 유용할 것으로 생각된다.TEX>-CIT SPECT는 파킨슨병의 조기진단 및 진행 추적에 임상적으로 유용할 것으로 판단된다., SCC 4예, AC 1예)였으며, 11예 중 9예(81.8%)에서 방사선학적 검사결과와 Tc-99m MIBI섭취율의 변화가 일치하였다. 결론적으로, Tc-99m MIBI SPECT는 폐암병소의 국소화 및 방사선치료 효과의 판정에 어느정도 유용하리라 사료되었다.냈고 4명에서는 low CBD obstruction을 나타내었으며 후에 CBD stone, CBD carcinoma, gall bladder Ca.의 porta hepatis 전이 및 clonorchis worms의 cluster에

주의력결핍 과잉운동장애 남아에서 중추신경자극제가 주의력과제 수행 중의 대뇌 기능적 국소화에 미치는 효과 (EFFECT OF METHYLPHENIDATE ON FUNCTIONAL CEREBRAL LOCALIZATION DURING ATTENTIONAL TASKS IN BOYS WITH ATTENTION-DEFICIT/HYPERACTIVITY DISORDER)

  • 본 연구는 주의력결핍 과잉운동장애(ADHD) 남아에서 전형적인 중추신경자극제인 methylphenidate를 투여한 전후의 상태 간에 주의집중력을 요하는 과제를 수행하는 동안의 정량화 뇌파검사(Quantitative Encephalography;Q-EEG) 소견을 비교하였다. 이를 통하여 대뇌의 전기생리학적 활성도의 위치적 변화를 알아봄으로써 methylphenidate가 대뇌의 기능적 국소화에 미치는 영향을 알아보고자 하였다. 연구대상으로는 DSM-IV기준으로 ADHD로 진단된 $6{\sim}12$세의 남아 13명이었고, 단축형 Conners척도와 한국판 아동행동척도(Child Behavior Checklist;CBCL)을 포함한 임상평가를 하였다. 모든 대상에게 methylphe-nidate를 투여하기 전에 안정상태와 주의력과제인 TOVA(Test of Variables of Attention)를 수행하는 동안에 Q-EEG를 시행하였다. 그리고 1주일 후에 methylphenidate 0.7mg/kg을 1회 경구로 투여하고 1시간이 경과한 시점에서 안정상태와 활성상태에서 Q-EEG를 시행하여, 각 상태의 Q-EEG소견을 비교분석하였다. 연구결과는 다음과 같다. 안정상태에서는 약물을 투여한 전후 시기 간에 Q-EEG 소견의 유의한 차이를 보이지 않았으나, 활성상태에서는 약물투여 전에 비하여 약물투여 후의 뇌파소견 중 theta파와 beta파의 진폭 비율(theta/beta)이 대뇌 우측의 전두엽과 두정-후두엽부위, 좌측의 측두-두정엽에서 유의하게 증가하였다. 본 연구의 결과로 미루어 methylphenidate는 주의력과제를 수행하는 동안에 대뇌의 전기생리적인 활성도에 영향을 미침을 알 수있다. 특히 주의력과 관련되어 methylphenidate가 대뇌기능에 미치는 국소적 영향이 우측 전두엽과 두정-후두엽, 좌측 측두-두정엽에 나타나고 있음을 알 수 있었다. 이는 중추신경자극제가 ADHD 환자에서 주의력 증진과 관련되어 대뇌기능의 국소적인 영향을 주는 약물임을 시사한다.

ADCP 정지법 측정 시 미계측 영역의 유량 산정 정확도 분석 (Accuracy Analysis of ADCP Stationary Discharge Measurement for Unmeasured Regions)

  • ADCP는 하천의 3차원 유속과 수심 자료를 매우 효율적이고 빠르게 측정할 수 있으며, 그 자료의 공간 및 시간적 해상도는 기존의 전통적인 유속 측정 방법들과 비교하여 매우 정밀하다는 장점이 있다. 하지만 ADCP는 하상 부근과 센서 근처에서의 미계측 영역이 발생하고 이 미계측 영역의 유속을 얼마나 정확하게 산정하느냐에 따라 ADCP 유량 측정의 정확도에 영향을 미친다. 본 연구에서는 ADCP 유량 산정 시 범용적으로 활용되고 있는 1/6 멱법칙(power law)을 활용한 미계측 영역의 유량 측정 결과의 정확도를 분석하였다. 이를 위해 실규모 직선수로에서 ADCP를 고정시킨 상태에서 측정한 유속 자료를 1/6 멱법칙과 대수 법칙(log law)을 적용하여 외삽 한 유속분포와 유량 산정 결과를 ADV를 이용하여 정밀하게 측정한 결과와 비교하였다. 비교 결과 전체적으로 대수 법칙으로 외삽한 경우가 높은 정확도를 나타냈으며, 수표면 근처 미계측 영역에서는 1/6 멱법칙은 유량을 작게 산정하는 경향을 나타냈고, 하상 근처의 미계측 영역에서는 유량을 크게 산정하는 경향을 나타냈다. 이 결과는 기존 1/6 멱법칙을 활용한 하상 및 수표면 부근 미계측 영역 유량 추정 방법이 오차를 수반함을 의미한다. 따라서 ADCP 정지법 측정 방식을 사용할 경우, 대수 법칙이 1/6 멱법칙보다 정확한 상하부 미계측 유량 추정 결과를 보여주었으므로 대안으로 고려되어야 할 것이다. 또한 제방 근처 미계측 영역의 유량 측정 정확도를 높이기 위해서는 수심이 0.6 m 이상을 확보한 측선을 기준으로 유량을 산정할 경우 신뢰도 높은 유량 측정 결과를 보였다. 향후, ADCP 정지법 측정 방식에 비해 보다 많이 활용되고 있는 보트탑재 이동식 ADCP의 경우도 이와 같은 검증이 필요하다고 하겠다.

관동맥질환의 진단을 위한 아데노신 Tc-99m sestaMIBI SPECT의 재현성 (Reproducibility of Adenosine Tc-99m sestaMIBI SPECT for the Diagnosis of Coronary Artery Disease)

  • 목적: 아데노신 부하 Tc-99m MIBI SPECT는 관동맥질환의 진단, 예후 평가, 치료 방침의 설정, 치료 효과 판정 및 경과 관찰에 유용성이 있다고 알려져 있다. 치료 효과 판정 및 치료 방침 설정을 위해서는 검사 방법의 재현성이 높아야 한다. 본 연구는 아데노신 부하 Tc-99m MIBI SPECT의 재현 성능을 평가하기 위하여 동일인에서 2회 연속적으로 시행한 아데노신 부하 Tc-99m MIBI SPECT에서 약물주사에 따른 혈역학적 변화와 부작용의 빈도, 영상에 나타난 심근분절의 방사능섭취 정도의 차이를 알아 보고자 하였다. 대상 및 방법: 관동맥질환이 의심되거나 관동맥질환으로 진단된 후 추적검사를 위하여 아데노신 부하 Tc-99m MIBT SPECT를 시행한 30명의 환자를 대상으로 2-11 일(평균: 4.1 일)의 간격으로 검사를 반복하였다. 관류영상의 육안적 판독은 좌심실의 단층 영상을 18분절로 구분하여, 섭취 정도에 따라 4등급으로 분류하였고, 관류 정도의 평가는 2명의 핵의학 전공의사가 각각 평가한 후 일치하지 않은 경우에는 두 판독자 간에 토의로 합의된 소견으로 비교하였다. 결과: 아데노신 주사에 따른 수축기혈압, 이완기 혈압 및 맥박 수의 변화는 두 검사 사이에 유의한 차이가 없었고, 두 번의 검사를 시행하는 동안 검사를 중지하거나 부작용 치료를 위한 처치를 받아야 만큼 중독한 부작용의 발생은 없었다. 관류점수상 540분절 중 439분절에서 완전하게 일치하였고(일치도 81.3%, tau B index 0.73%), 1점 차이(97분절, 18%), 2점 차이(4분절, 0.7%)있었으나 3점 이상의 차이가 있은 경우는 없었다. 두 검사에서 측정된 관류결손의 범위와 중증도점수 사이에는 각각 $\gamma$-값 0.982와0.965의 높은 양의 상관성을 나타내었다(p<0.001, respectively). 결론: 연속적으로 2회 시행한 아데노신 부하 Tc-99m MIBI SPECT는 부하에 따른 혈역학적 변화 및 부작용의 발생이 유사하였고, 심근분절의 관류상태의 시각적 평가 및 관류결손의 정량적 판정에서 매우 높은 재현성을 보여주었다. 그러므로 아데노신 Tc-99m MIBI SPECT는 관동맥질환이 의심되거나 관동맥질환으로 진단된 환자에서 약물치료나 중재적 시술 또는 외과적 수술 치료 후 심근관류상태의 경시적 변화를 평가하는데 유용하게 이용될 수 있을 것이라 판단된다.

조선시대 사가(私家) 정원에서의 양학(養鶴) 사례 (A Cases of Crane Breeding(養鶴) at Private Homes(私家) in the Joseon Dynasty Period)

  • 본 연구의 목적은 조선시대 사가 정원에서 학을 기른 실제 사례를 살펴봄으로서 여기에 반영된 보편적 의미와 특징을 규명하는 것이다. 이러한 연구를 통해 조선시대 정원문화의 실체에 보다 가깝게 다가갈 수 있을 것이다. 본 연구의 시간적 범위는 조선 시대이며 내용적 범위는 사가(私家)에서의 사례로 한정하였다. 본 연구를 위한 문헌 자료는 한국고전종합데이터베이스에 탑재되어 있는 고전번역서를 대상으로 하였으며, 필요한 경우 관련 연구자의 번역 자료도 추가로 활용하였다. 본 연구의 결과를 요약하면 다음과 같다. 첫째, 종친, 사대부, 향촌 사족, 평민 등 다양한 신분의 인물이 학을 기른 것을 확인할 수 있었다. 학을 기른 인물 중에는 조선시대 정치와 학문에 큰 족적을 남긴 인물도 있으며 일부 경우에서는 평생에 걸쳐 처사의 삶으로 일관하며 학을 기른 인물도 있었다. 둘째, 전국 경향각지에 학을 기른 사람들이 있었다. 특히 한양 도성 내를 비롯해 인근에서도 여러 사례가 있다는 점이 주목된다. 셋째, 학을 기르는 행위는 혈연과 교우관계, 사승 등 학맥과 관련성이 컸음을 알 수 있었다. 이러한 점에서 학 기르기는 단순한 취향이나 풍류이기 보다 한 인물의 삶의 태도와 지향을 반영했다고 볼 수 있다. 넷째, 임포의 고사를 토대로 하는 성리학적 출처의식이 학을 기르는 행위에 큰 영향을 미친 것으로 보인다. 또 일부의 경우에서는 도가적 취향이 반영된 것으로 보이는 사례도 있었다. 다섯째, 학을 길들이는 것을 생업으로 삼은 사람도 있었다. 이러한 점은 학에 대한 수요가 꾸준히 존재했다는 것을 반증하고 있다. 본 연구의 한계는 번역 자료에 의존해 연구를 진행하였다는 점이다. 추후 자료의 발굴과 번역 성과가 더욱 축적된다면 보다 풍부한 사례를 확인할 있을 것이다.

대퇴신경 손상 환아의 보행분석 : 사례연구 (Gait Analysis of a Pediatric-Patient with Femoral Nerve Injury : A Case Study)

  • The femoral nerve innervates the quadriceps muscles and its dermatome supplies anteromedial thigh and medial foot. Paralysis of the quadriceps muscles due to the injury of the femoral nerve results in disability of the knee joint extension and loss of sensory of the thigh. A child could walk independently even though he had injured his femoral nerve severely due to the penetrating wound in the medial thigh. We measured and analyzed his gait performance in order to find the mechanisms that enabled him to walk independently. The child was eleven-year-old boy and he could not extend his knee voluntarily at all during a month after the injury. His gait analysis was performed five times (GA1~GA5) for sixteen months. His temporal-spatial parameters were not significantly different after the GA2 or GA3 test, and significant asymmetry was not observed except the single support time in GA1 results. The Lower limb joint angles in affected side had large differences in GA1 compared with the normal normative patterns. There were little knee joint flexion and extension motion during the stance phase in GA1 The maximum ankle plantar/dorsi flexion angles and the maximum knee extension angles were different from the normal values in the sound side. Asymmetries of the joint angles were analyzed by using the peak values. Significant asymmetries were found in GA1with seven parameters (ankle: peak planter flexion angle in stance phase, range of motion; ROM, knee: peak flexion angles during both stance and swing phase, ROM, hip: peak extension angle, ROM) while only two parameters (maximum hip extension angle and ROM of hip joint) had significant differences in GA5. The mid-stance valleys were not observed in both right and left sides of vertical ground reaction force (GRF) in the GA1, GA2. The loading response peak was far larger than the terminal stance peak of vertical ground reaction curve in the affected side of the GA3, GA4, GA5. The measured joint moment curves of the GA1, GA2, GA3 had large deviations and all of kinetic results had differences with the normal patterns. EMG signals described an absence of the rectus femoris muscle activity in the GA1 and GA2 (affected side). The EMG signals were detected in the GA3 and GA4 but their patterns were not normal yet, then their normal patterns were detected in the GA5. Through these following gait analysis of a child who had selective injuries on the knee extensor muscles, we could verify the actual functions of the knee extensor muscles during gait, and we also could observe his recovery and asymmetry with quantitative data during his rehabilitation.

황해 및 인접 지역 퇴적분지들의 구조적 진화에 따른 층서 (Stratigraphic response to tectonic evolution of sedimentary basins in the Yellow Sea and adjacent areas)

  • 황해 및 인접 지역에 위치하는 퇴적 분지들의 구조적 진화에 따른 층서를 이해하기 위한 비교 연구가 통합층서기술을 이용하여 수행되었다. 본 연구의 잠정적 결과로 우리는 각 분지별 퇴적층들의 시$\cdot$공간상의 대비가 가능한 층서틀을 제안한다. 본 연구의 결과로 제안된 층서틀은 향후 황해 및 인접 지역의 석유자원 탐사를 위한 새로운 층서적 사고의 틀로 사용될 수 있을 것이다. 생층서 자료와 결합시켜 수행한 통합층서해석 결과, 캠브로-오오도비스기, 석탄기-트라이아스기, 쥬라기 초기-중기, 쥬라기 말기-백악기 초기, 백악기 후기, 팔레오세-에오세, 올리고세, 마이오세 초기, 마이오세 중기-플라이오세 퇴적층 등 9개의 단위층들이 인지된다. 본 연구를 통해 인지된 9개 단위층들은 구조층서단위로 황해 및 인접 지역 퇴적 분지들의 퇴적 작용 및 분지 형성과 변형에 관련된 구조운동 등에 관한 정보를 제공해 준다 남황해 분지는 고생대 동안 남중국 지괴의 북쪽 연변부에 발달하는 대륙 연변부 분지로 시작되었다 쇄설성 및 탄산염 퇴적물들이 상대적 해수면의 변동에 따라 윤회성을 보이면서 분지 내에 퇴적되었다. 그러나, 데본기 동안의 칼레도니안 조산운동에 의해 분지는 융기되어 침식을 받았으며, 결과로 캠브로-오오도비스기 단위층과 석탄기-트라이아스기 단위층 사이에 부정합이 형성되었다. 북중국 지괴와 남중국 지괴가 충돌될 때인 페름기 말기로부터 트라이아스기 말기 사이에 인도시니안 조산운동이 일어났다. 북중국 지괴와 남중국 지괴의 충돌에 따라 친링-다비-수루-임진강 습곡대가 형성되었으며, 고생대 퇴적층들은 융기된 후 변형을 받게 되었다. 이 후 습곡대에 평행한 대륙전사면이 빠르게 침강하면서 발해 분지 및 서한만 분지와 같은 대륙전사면 분지가 형성되어 쥬라기 초기-중기의 후조산성 퇴적물들이 분지를 충진시켰으며, 지역적으로 피기백 형태의 소규모 분지들이 저각의 역단층을 따라 발달하게 되었다. 이들 대륙전사면 분지나 피기백 형태의 분지들은 쥬라기 말기 동안에 일어나는 앤샤니안 조산운동 (일차)에 의해 변형된다. 그러나, 남황해 분지는 쥬라기 초기 및 중기 동안에 대륙내 침강 분지였던 것으로 보인다. 남황해 분지의 쥬라기 초기 및 중기 단위층은 분급도와 원마도가 양호한 규암역을 포함하는 두꺼운 기저 역암층과 함께 하성 및 호성 환경 하에서 퇴적된 사암 및 셰일들로 구성되어 있다. 한편, 탄루 단층대는 트라이아스기 말기로부터 좌수향의 운동을 시작하였으며, 쥬라기와 백악기를 거쳐 제삼기 초까지 계속되었다. 쥬라기 말기에 들어와 탄루 탄층대를 따라 이차 및 삼차 순위의 주향이동 단층들이 발달되면서 소규모 열개 분지들이 형성되기 시작하였다. 에오세말까지 지속된 탄루 단층의 이동에 의해 남황해 분지는 대규모의 횡압력을 받게되어 소규모 열개 분지들은 인리형 분지로 확장되었다. 그러나 쥬라기 말기와 에오세 말기까지 발해 분지는 융기되어 심한 변형을 받게되었다. 발해 분지의 백악기 초기 이후 에오세 말기까지의 부정합이 앤샤니안 조산운동 (이차 및 삼차)에 의해 형성된 것으로 해석된다. 한편 에오세 말에 이르러 인도판과 유라시아판의 충돌에 의한 히말라얀 조산운동의 영향으로 탄루 단층의 이동방향이 좌수향에서 우수향으로 변환되기 시작하면서 남황해 분지는 구조역전의 현상이 일어났으며, 동시에 발해 분지는 인리형 분지로 발달하게 되었다. 따라서, 올리고세 동안 발해 분지에서는 퇴적작용이, 남황해 분지에서는 심한 구조역전에 의한 분지변형이 동시에 일어났다 올리고세 이후 현재까지, 남황해 분지와 발해 분지들은 간헐적인 해침과 함께 광역적 침강을 유지하면서 안정된 대륙 및 대륙붕 지역으로 전이되었다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure.
